The difficulties associated with the interpretation of the results of regional and intra-articular anaesthesia are discussed with reference to eight lame horses. The clinical and radiographic features of each horse are described, together with the results of anaesthesia. One horse had clinical and radiographic signs consistent with navicular disease but it was not possible to relieve the lameness. Two horses had fractures of bones within the foot but lameness was not improved by palmar (abaxial sesamoid) nerve blocks. One horse had more than one cause of lameness. Four horses had joint pathology but none responded to intra-articular anaesthesia.  相似文献

An eight-month-old female rottweiler was presented with a history of intermittent lameness of the right hindlimb of 4 weeks duration. Radiographs showed a fragmentation in two plantar sesamoid bones of the metatarsophalangeal joint. Based on these clinical and radiographic signs sesamoid disease of the 2nd and 7th sesamoid bone in the right hindpaw was diagnosed. The fragmented sesamoids were surgically removed and histologically examined. The treatment was successful and the dog became lamnessfree. Additional radiographs showed fragmentations of sesamoid bones VII of both the forepaws, but they did not cause any lameness.  相似文献

Destructive lesions of the axial region of the proximal sesamoid bones were identified by radiography in eight fetlocks and seven lame adult horses. Lameness ranged from 2 to 5 (mean 4; scale 1 to 5) at the time of examination, with a duration of 10 days to two years (mean 5.6 months). Destructive lesions involved both proximal sesamoid bones when examined radiographically and were situated primarily at the level of the mid-body and apical region of the axial borders. Some lesions were cystic, whereas others eroded the axial border more diffusely. Scintigraphy revealed markedly increased activity within the proximal sesamoid bones of the clinically lame limb of four of the five horses examined. In four horses, post mortem computed tomography revealed axial border bone destruction and cavitary lesions within cancellous bone of affected proximal sesamoid bones. Lesions seen by computed tomography were larger than those identified on radiographic examination. Cavitary lesions not seen radiographically were identified in the proximal sesamoid bones of two clinically unaffected fetlocks examined for comparison in two of the seven horses. Evidence of acute, subacute or chronic/reparative osteomyelitis of the axial region of the proximal sesamoid bones was seen in the 10 fetlocks identified as abnormal from radiography or computed tomography. Also, three horses had septic synovitis of the flexor sheath of the clinically affected limb; of these, two had septic arthritis of the fetlock joint.  相似文献

This paper describes the clinical and radiographic features, and response to treatment, of 45 horses which showed lameness that was improved by intra-articular anaesthesia of the distal interphalangeal (DIP) joint. Although many horses had poor conformation of the foot of the lame limb, the majority showed no localising clinical signs suggestive of involvement of the DIP joint. Lameness was usually unilateral. No horse with bilateral lameness responded to treatment. Palmar digital nerve blocks frequently improved or alleviated lameness, although in some horses palmar (abaxial sesamoid) nerve blocks were required to eliminate lameness. This difference in response did not affect response to treatment. Intra-articular anaesthesia of the DIP joint usually resulted in resolution of lameness within 5 mins; a partial improvement in lameness or a slow response were poor prognostic indicators. None of the horses had radiographic abnormalities compatible with navicular disease. Radiographic changes of the distal interphalangeal joint per se were generally detectable only in lateromedial views and were identified in less than one third of the horses. Success rates were low following treatment of cases with radiographic abnormalities. In those with no radiographic abnormalities the response to corrective trimming and shoeing and intra-articular administration of sodium hyaluronate, once or repeatedly, was variable and no parameters could be used to predict the likely outcome. Treatment was successful in approximately 30 per cent of cases. Subsequent treatment of horses which remained lame, by intra-articular administration of polysulphated glycosaminoglycans, was not helpful; only a small proportion became sound following prolonged (nine months) rest.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

A degenerative disease of the volar sesamoid bones of the metacarpophalangeal and metatarsophalangeal joints is described in young dogs of the larger breeds. It is characterized by degenerative changes in the articular cartilage of the sesamoid bones and by calcification within their ligamentous attachments. The lesion can be associated with lameness although it can also be asymptomatic. In many cases, other orthopaedic lesions are present in the dog which makes clinical assessment difficult. Treatment is by surgical removal of the affected sesamoid bone. This is the first report of this particular clinical entity which has been designated sesamoid disease.  相似文献

Objectives : To evaluate the diagnostic merits of scintigraphy in dogs with clinically obscure lameness origin. Methods : In 43 dogs, the scintigraphic findings were compared with the results of subsequent radiological, computed tomography and/or arthroscopic findings and/or additional neurological examination. Results : In all dogs, focal increased uptake was found in one or more joints or bones. The presence of associated pathology was confirmed by radiography, computed tomography and/or arthroscopy of the limb region of interest. Additional neurological examination was performed when indicated. In cases of elbow pathology, unilateral involvement of the elbow was confirmed with computed tomography/arthroscopy in 14 cases, while bilateral increased uptake could not be confirmed with radiography or computed tomography in three cases. Three dogs showed an increased activity in one joint on the bone scan, while computed tomography, arthroscopy or radiography revealed bilateral joint disease. Generalised mild increased radionuclide uptake in all joints and bones was observed in the lame limb in 15 forelimbs and five hindlimbs. Generalised prominent uptake of an entire limb was noted in three cases suffering from neurological disease. Clinical Significance : Scintigraphy may provide information on areas of increased radionuclide uptake, thereby aiding in the localisation of obscure lameness. Generalised mild increased uptake of radionuclide in all joints and bones in the lame limb may occur in the older dog. The generalised prominent uptake, seen in three cases with neurological disease, may resemble reflex sympathetic dystrophy syndrome seen in human beings.  相似文献

Our aims were to evaluate the pattern of radiopharmaceutical uptake in horses with lameness related to the metacarpophalangeal and/or metatarsophalangeal joint and compare the results with similar information from sound horses. It was hypothesized that there would be a difference in radiopharmaceutical uptake between the lame and contralateral limb in unilaterally lame horses and that there would be a difference between lame and sound horses. Nuclear scintigraphic images of 43 horses with forelimb lameness and 30 horses with hindlimb lameness were evaluated. In all horses lameness was improved by intraarticular analgesia of the joint, or by perineural analgesia of the palmar/plantar (at the junction of the proximal 3/4 and the distal 1/4 of the metacarpal/metatarsal regions) and palmar/plantar metacarpal/metatarsal nerves. All images were assessed subjectively and a quantitative image analysis was performed by drawing a vertical line profile through the center of each joint in the lateral images and using region of interest analysis in both lateral and dorsal/plantar images. Ratios of radiopharmaceutical uptake were compared for each region between lame and contralateral limbs in unilaterally lame horses and between lame and control horses. There was a significant difference in the ratio of radiopharmaceutical uptake in the proximal aspect of the proximal phalanx between lame and nonlame forelimbs in unilaterally lame horses and in both lame and contralateral limbs of lame horses compared with control horses for the regions of the distal aspect of the third metacarpal/metatarsal bone and the proximal sesamoid bones. Objective: To establish whether osteochondral autograft transfer (OAT) procedures for osteochondritis dissecans (OCD) of the canine caudocentral humeral head would restore articular contour, resurface osteochondral defects, and resolve lameness in dogs with OCD. Study Design: Case series. Animals: Dogs (n=14) with caudocentral humeral head OCD (16 shoulders). Methods: After arthroscopic assessment, the humeral head was exposed by arthrotomy. The OCD lesion was debrided and OATS? instrumentation used for resurfacing the defect with osteochondral core grafts collected from the stifle. Recipient sockets were created to maximally resurface articular lesions. Outcomes measures included clinical, radiographic, and arthroscopic examination at 12–18 weeks and clinical examination at 12–46 months (mean, 30.8 months) postoperatively. Results: Resurfacing of the humeral head was achieved in all dogs. Subjectively, lameness resolved in 9 of 16 limbs by 5–6 weeks postoperatively, and in 13 limbs by 12–18 weeks. Radiographically, all autografts were maintained. On second‐look arthroscopy, the grafted section was intact, resilient on probing, satisfactorily conformed to the adjacent articular contour, and grossly appeared consistent with hyaline or fibrocartilage depending on the graft source. Of 12 limbs available for 12–46 months reevaluation, none were considered lame and mild discomfort on shoulder manipulation was recorded for only 1 joint. Conclusions: OAT procedures are technically feasible in the caudocentral aspect of the canine humeral head. Articular contour reconstruction, resurfacing of osteochondral defects, and amelioration or resolution of short‐term lameness may be achieved.  相似文献

Few reports have been published regarding the use of scintigraphy in the diagnosis of elbow joint lameness in dogs. Some authors have speculated about the potential use of bone scintigraphy and its suspected high sensitivity for the early diagnosis of abnormalities of the medial coronoid process (MCP) in dogs. Scintigraphy is used routinely in our institution in dogs presented for thoracic limb lameness and/or suspected of abnormalities of the MCP when radiographic findings were equivocal. Radiographic, scintigraphic, and surgical findings of the elbow joints of 17 dogs with elbow joint lameness were compared with radiographic, scintigraphic, and necropsy findings of the elbow joints of 12 clinically healthy Labrador Retrievers. Quantitative evaluation of scintigraphic images was performed to determine relative radiopharmaceutical uptake in the region of the MCP. Maximum relative uptake of the coronoid process in the normal dogs was taken as a threshold value to classify elbows as positive or negative for an abnormal MCP after all 24 elbows of the 12 healthy dogs were confirmed as being normal at necropsy. All 17 elbows from lame dogs were positive on scintigraphy and confirmed as having chondromalacia, a fissure, or fragmentation of the MCP. Based on our results, bone scintigraphy may be a valuable diagnostic tool for the diagnosis of abnormalities of the MCP in dogs, and particularly in older dogs where clinical and radiographic changes may be ambiguous.  相似文献

The results of a combined retrospective and prospective study of 130 cases of fragmented coronoid process (FCP) in 109 dogs are presented. Sixty-eight cases were treated surgically by medial elbow arthrotomy and 62 were treated medically (rest and anti-inflammatories). Case records were searched with the emphasis on the severity of lameness, physical abnormalities and in surgical cases, the appearance of the coronoid process lesion. Long term follow-up was obtained by a telephone survey. Analysis of case histories and physical data revealed no correlation between severity of lameness before treatment, severity of radiographic changes in the elbow joint and the type of lesion found at surgery. Surgical treatment did not decrease the incidence of post treatment lameness, but the surgically treated dogs were more active and less severely lame than those treated without surgery. It was concluded that young dogs with mild lameness due to FCP in the elbow probably do not benefit from surgical treatment but dogs with chronic, moderate to severe lameness have a better prognosis if treated surgically.  相似文献

Skeletal radiographs of four dogs with confirmed visceral leishmaniasis were reviewed. The dogs had lived in the Mediterranean area for six to 36 months prior to returning to the United States, where they lived for an additional six to 41 months before clinical signs appeared. Clinical findings included lameness, fever, cutaneous lesions, muscle atrophy, lymphadenopathy, hepatosplenomegaly, and weight loss. The dogs exhibited two distinct radiographic patterns. Periosteal proliferation and increased intramedullary radiopacity of long and flat bones occurred in two dogs. Osteolysis of bones of the carpus, tarsus, and stifle was noted in two dogs. Differences in radiographic appearance were presumed to be due to different hematogenous routes of infection. Leishmaniasis should be considered in the differential diagnosis of dogs that have traveled in endemic areas and exhibit the described radiographic changes.  相似文献

Thirty-four dogs that had surgical correction of medial patellar luxation (MPL) in 52 stifle joints were examined after a minimum follow-up period of 1 year (median, 3.6 years). The dogs were divided into the following three groups depending on their age at the time of surgery: group 1, 3 to 6 months; group 2, 8 to 20 months; and group 3, 2.2 to 12 years. Two of the dogs in group 3 had ruptured their cranial cruciate ligament in addition to having MPL. The results were based on a clinical assessment of the animal's gait, and physical and radiographic examination of the stifle joints. Six of seven stifle joints evaluated in group 1 had radiographic evidence of moderate to severe degenerative joint disease of the patellofemoral joint, and in two of the joints recurrence of MPL was observed. Failure to maintain reduction of the patellofemoral joint was also observed in 11 of 22 (50%) and 12 of 23 (52%) of the stifle joints in groups 2 and 3, respectively. In the latter groups, mild degenerative joint disease was evident radiographically in stifle joint that had not maintained reduction. Four of the 34 dogs were consistently (n = 2) or intermittently (n = 2) lame; the two dogs that were consistently lame had cranial instability of the stifle consistent with rupture of the cranial cruciate ligament. In the latter two dogs, the cranial cruciate ligament had been intact at the time of surgery for correction of MPL.  相似文献

OBJECTIVE: To determine clinical, radiographic, and pathologic abnormalities in dogs with multiple epiphyseal dysplasia (MED). DESIGN: Retrospective case series. ANIMALS: 19 dogs with MED from 10 litters. PROCEDURES: The diagnosis was made on the basis of radiographs of the shoulder region and vertebral column. Ten dogs underwent necropsy. RESULTS: There were 11 Hygenhund, 6 Dunker, 1 Golden Retriever, and 1 English Pointer. Most dogs were examined because of lameness that developed at 5 to 8 months of age. The most common radiographic abnormality was a deficiency in ossification of the epiphyses, apophyses, and cuboidal bones of the appendicular skeleton and the epiphyses of the vertebrae; ossification of the metaphyses and the diaphyses typically were normal. Disease severity was consistent among littermates, but varied among dogs from different litters. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that MED is a rare condition in dogs characterized by a deficiency in ossification of the epiphyses of the long bones, the epiphyses of the vertebrae, the cuboidal bones, and the apophyses. Radiographic abnormalities were evident in dogs examined as young as 8 weeks of age, and most dogs had developed severe lameness by 5 to 8 months of age. The condition most likely had an autosomal recessive mode of inheritance, although genetic studies of mode of inheritance could not be performed.  相似文献

Supraspinatus and biceps brachii tendinopathy in dogs   总被引:1,自引:0,他引:1  
Calcifying tendinopathy was seen on radiographic views of the scapulohumeral joint in 12 of 183 (7 per cent) dogs reviewed retrospectively. Calcifying tendinopathy was identified only in large breed dogs, five being labradors. All the affected dogs, except one, were presented for unilateral thoracic limb lameness, which was typically of a chronic and intermittent pattern. Pain on manipulation of the scapulohumeral joint in the lame leg was present in eight of 12 dogs (67 per cent). Calcification of the supraspinatus tendon was seen in eight dogs and calcification of the biceps brachii tendon was seen in the other four dogs. Radiographic changes indicative of biceps brachii tendon avulsion were found in conjunction with calcification of the contralateral biceps brachii tendon in two dogs. Filling defects, indicative of bicipital tenosynovitis, were also seen in positive contrast arthrograms of two dogs with biceps brachii calcification. Of the eight dogs for which radiographic views of both scapulohumeral joints were available, six dogs had bilateral radiographic signs of calcifying tendinopathy. Overall, radiographic signs of calcifying tendinopathy were seen in 18 scapulohumeral joints, but lameness was apparent in only nine limbs, suggesting the lesion may at times be asymptomatic. Further prospective investigation of calcifying tendinopathy is needed to determine why lameness does not always appear to be associated with the presence of the lesion.  相似文献

The fore and hind feet of 50 unselected Rottweilers were examined clinically and radiographically for abnormalities of their volar and plantar sesamoid bones. Lesions were found in 22 (44 per cent), invariably affecting the No. 2 and/or No. 7 sesamoids. The majority occurred in the forelimb. Clinical signs were detected in only five dogs and local analgesia in these cases indicated that the sesamoid changes were not the cause of the presenting lameness. Radiographically the defect varied from small radiodense foci separated from an otherwise normal sesamoid, to gross distortion of the bone into a number of calcified bodies. In none of these dogs was the sesamoid abnormality demonstrated to cause lameness and it was not necessary, therefore, to treat the condition surgically. It is postulated that these lesions result from disordered ossification with subsequent degenerative changes.  相似文献

OBJECTIVE: To determine clinical, radiographic, and scintigraphic abnormalities in and outcome of horses with septic or nonseptic osteitis of the axial border of the proximal sesamoid bones. DESIGN: Retrospective study. ANIMALS: 8 horses. PROCEDURE: Data collected from medical records included signalment; history; horse use; severity and duration of lameness; results of perineural anesthesia, radiography, ultrasonography, and scintigraphy; and outcome following surgery. RESULTS: Five horses did not have any evidence of sepsis; the other 3 had sepsis of the metacarpophalangeal or metatarsophalangeal joint or the digital synovial sheath. All horses had a history of chronic unilateral lameness. Three of 5 horses improved after diagnostic anesthesia of the metacarpophalangeal or metatarsophalangeal joint; the other 2 improved only after diagnostic anesthesia of the digital synovial sheath. Nuclear scintigraphy was beneficial in localizing the source of the lameness to the proximal sesamoid bones in 4 horses. Arthroscopy of the palmar or plantar pouch of the joint or of the digital synovial sheath revealed intersesamoidean ligament damage and osteomalacia of the axial border of the proximal sesamoid bones in all horses. All 5 horses without sepsis and 1 horse with sepsis returned to their previous uses. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that osteitis of the axial border of the proximal sesamoid bones is a distinct entity in horses that typically is associated with inflammation of the associated metacarpointersesamoidean or metatarsointersesamoidean ligament and may be a result of sepsis or nonseptic inflammation. Arthroscopic debridement may allow horses without evidence of sepsis to return to their previous level of performance.  相似文献

Osseous cyst‐like lesions of the proximal sesamoid bones (PSBs) were diagnosed in 7 horses. The diagnosis was achieved radiographically prior to magnetic resonance imaging (MRI) in only one horse, and in the other 6 horses the diagnosis was made using low field MRI (retrospective evaluation of the radiographs after the MRI revealed ill‐defined radiolucencies of the PSBs in 4 of these horses). The horses ranged in age from 3 to 12 years, and the affected limbs included 3 forelimbs and 4 hindlimbs. The onset of lameness was reported to be sudden in 6 horses and insidious in one, and the duration of lameness at the time of MRI ranged from 0.3 to 11 months. The degree of lameness in the 6 horses with sudden‐onset lameness was moderate to severe. Pain on flexion of the affected metacarpo(tarso)phalangeal (fetlock) joint or exacerbation of the degree of lameness following fetlock flexion was recorded in 4 of the 7 horses. The MRI findings in all cases included a focal high signal intensity lesion (all magnetic resonance sequences) at various locations in one PSB. Both septic and nonseptic aetiologies were identified. Four of the 7 horses were subjected to euthanasia due to persistent lameness, one remained chronically lame and only 2 were able to return to their previous level of exercise.  相似文献

Radiographs of all 4 fetlocks of 71 Standardbred racehorses were obtained at 3-month intervals for 1 year. Radiographic findings in the abaxial surface of the proximal sesamoid bones were classified into 3 types according to the severity of lesions, and correlation was made with clinical findings at time of examination. Type-1 lesions (1 or 2 linear defects less than or equal to 1 mm wide) were detected in 55% of horses at the start of training; clinical signs of disease were not manifested, and lesions did not become clinically relevant. Type-2 lesions (3 or more linear defects less than or equal to 1 mm wide) were detected in horses not manifesting clinical signs of disease, and were more frequently observed after 3 months of training. However, 66% of horses affected with diseases of the suspensory apparatus, including superficial flexor tendinitis and suspensory desmitis, also manifested this type of lesion. When lameness was observed, it was associated with the soft tissue problem, and the sesamoid bone changes were considered secondary. Type-3 lesions (wide, abnormally shaped linear defects) were detected in 7 horses at the start of the study; lesions remained in horses throughout the study and were consistently associated with lameness during training. Type-3 lesions were considered clinically relevant and indicative of primary sesamoiditis.  相似文献

Objectives— To describe a surgical approach for the removal of nonarticular base sesamoid fragments in performance horses and to report the outcome. Study Design— Retrospective study. Animals— Horses (n=11), 7 months to 10 years of age, with lameness because of nonarticular base sesamoid fragments. Methods— Lameness was localized to the metacarpophalangeal/metatarsophalangeal region by clinical examination or response to diagnostic local anesthesia. Radiographs confirmed the diagnosis of a nonarticular base sesamoid fragment. Surgical removal was performed with an incision over the base of the affected sesamoid through the digital flexor tendon sheath. After identifying the fragment with the aid of needles, a small vertical incision was made in the straight distal sesamoidean ligament (SDSL) and the fragment was freed from its attachments and removed. Six months convalescence and rehabilitation was prescribed for all horses. Results— Eleven horses had 16 nonarticular fragments of the base of the proximal sesamoid bones. Eleven (69%) fragments occurred in the forelimbs with the right front (82%) and medial sesamoid (73%) more commonly affected. Horses were treated by surgical fragment removal. In horses with follow‐up, 9 of 10 returned to their intended use. Conclusions— Surgical removal of nonarticular base sesamoid fragments can be accomplished through a palmar/plantar approach through the digital flexor tendon sheath and SDSL. This “keyhole” approach minimizes damage to the distal sesamoidean ligament attachments to the sesamoid base and allows some horses to return to their intended use. Clinical Relevance— Surgical removal of nonarticular base sesamoid fragments should be considered in horses with performance‐limiting lameness as a result of the fragment.  相似文献
