OBJECTIVE: To develop a radiographic stress technique to quantify hip joint laxity in dogs. STUDY DESIGN: Prospective study on client-owned dogs presented for hip dysplasia screening. ANIMAL POPULATION: 302 nonselected dogs (63 breeds). METHODS: Dogs were sedated and placed in dorsal recumbency. During pelvic radiography, the femoral heads were displaced manually in a craniodorsal direction. On these stress radiographs, the degree of lateral displacement of the femoral heads was assessed in terms of a subluxation index (SI) and compared with the degree of femoral head subluxation and the severity of hip dysplasia found on conventional extended hindlimb radiographs. RESULTS: The degree of subluxation on stress radiographs was significantly greater than on conventional radiographs. Correlation between the severity of canine hip dysplasia (CHD), graded according to conventional techniques, and the degree of subluxation, induced by radiographic stress technique, was positive (r = .57; P<.0001). Two critical SI values were noted. Of the dogs with an SI value of 0.3 or less, 99% were classified as CHD grade normal, borderline, or mildly dysplastic. In dogs with an SI value between 0.3 and 0.5, CHD grading ranged from normal to severely dysplastic. Of the dogs with an SI value greater than 0.5, 95% had dysplastic hip joints. CONCLUSIONS: Hip joint laxity cannot be quantified precisely on standard position radiographs. The proposed stress technique yields significantly higher degrees of femoral head subluxation than the standard position. 相似文献

OBJECTIVE: Evaluation of the relationship between canine weight gain from 6 to 15 weeks of age and passive coxofemoral joint (CFJ) laxity at 16 weeks of age. STUDY DESIGN: Longitudinal cohort study. ANIMALS: Full- or half-sibling hounds (n = 56). METHODS: Hounds were weighed weekly from 6 to 15 weeks of age. Individual average daily gain (ADG) was calculated for each week (weekly) and for the study (overall). PennHIP distraction index (DI) was determined for each CFJ at 16 weeks. Mixed effects linear models were evaluated for associations of DI (highest and mean) with 15-week weight and ADGs (actual or normalized). Left and right DIs were compared with a Student's paired t-test. Significance was set at P<.05. Trends were considered at P<.10. RESULTS: Mean (+/-SD) 16-week DI score and 15-week weight was 0.67 +/- 0.16 and 12.5 +/- 1.8 kg, respectively. Within animal left and right DIs were not significantly different. There were no significant associations between DI and any of the weight gains evaluated. There was a trend for a negative relationship between normalized 14-week ADG and DI in one statistical model. CONCLUSIONS: Weight gain from 6 to 15 weeks of age was unrelated to 16-week PennHIP DI in a homogenous canine population with moderate-to-severe CFJ joint laxity. CLINICAL RELEVANCE: Based on our results, ad libitum feeding between 6 and 15 weeks of age does not appear to have an adverse impact on joint laxity at 16 weeks of age as measured by the PennHIP DI. 相似文献

From mechanical principles and postmortem observations of coxofemoral joints of dogs, a hydrostatic mechanism influencing hip joint stability was discovered. This discovery led to the development of a stress-radiographic positioning method to quantitate hip joint laxity in dogs. The method incorporated 2 views with the dog in supine position and hips at neutral flexion/extension angle: a compression view, with the femoral heads fully seated in the acetabula; and a distraction view, with the femoral heads at maximal lateral displacement. An index measurement method was formulated to quantitate the relative degree of joint laxity appearing in either the compression or distraction view. Clinical evaluation of 6 dogs was done to compare the compression/distraction method with the standard hip-extended radiographic method. Also, the stress-radiographic method was performed on 16-week-old Borzoi and German Shepherd Dogs to compare the characteristics of inherent hip joint laxity in these breeds. In all dogs tested, hip joint laxity was masked by the standard hip-extended view as indicated by a 2.5-fold improvement in sensitivity to hip joint laxity of the new method (P less than 0.00001). Moreover, the mean hip joint laxity of 16-week-old German Shepherd Dogs exceeded the mean hip joint laxity of Borzois by 79% (P less than 0.00001). Reports in the literature document the incontrovertible association of hip joint laxity to the development of hip dysplasia in dogs. We believe the ability to accurately quantitate hip joint laxity will provide key diagnostic and prognostic criteria for the selection of pet dogs, and more importantly, breeding stock. 相似文献

OBJECTIVE: To determine whether dorsolateral subluxation (DLS) of the femoral head reflects osseous conformation of the coxofemoral (hip) joint and represents a property distinct from maximum passive laxity of the hip joint in dogs. ANIMALS: 14 Labrador Retrievers, 16 Greyhounds, 58 Greyhound-Labrador Retriever mixed-breed dogs, and 1 Rottweiler. PROCEDURES: DLS of the femoral head (DLS score) and passive laxity of the hip joint (distraction index) were determined radiographically in 3 groups of dogs: not treated (167 joints of 84 dogs); before and after injecting 2 ml of hyaluronan into 25 hip joints of 13 dogs; and before and after unilateral triple pelvic osteotomy in 5 dogs. Results of the 2 methods were compared for each group. RESULTS: In untreated dogs, the correlation coefficient (r) of DLS score versus distraction index was -0.73 and -0.69 for 84 left and 83 right hip joints, respectively. Mean coefficient of determination (r2) for both hips was 0.5. Mean DLS score did not differ before and after intra-articular injection of hyaluronan into either hip joint, whereas mean distraction index increased significantly after intra-articular injection. Unilateral triple pelvic osteotomy resulted in a significant increase in DLS score, compared with values obtained before surgery. However, distraction index before and after surgery did not differ significantly. CONCLUSIONS AND CLINICAL RELEVANCE: The DLS test assesses the congruity of the acetabulum and the femoral head in a canine hip joint and thus represents a characteristic distinct from maximum passive laxity. The DLS score and the distraction index evaluate different components of hip joint stability. 相似文献

The relationship between puppy hip laxity at 6–8 weeks and radiographic hip dysplasia at 12–18 months was investigated. Two methods of hip laxity assessment were studied and compared. A significant positive relationship was found. Using simple statistical and decision theory principles an approach to the application of these results to a selection programme for guide dogs was discussed and performed. The economic value of early examination for hip dysplasia was demonstrated for the situation studied. 相似文献

The objective of this study was to describe seven cases of unilateral bacterial infective coxarthritis from a total of 19 cases of bacterial infective arthritis (BIA), presenting over a two year period. We recorded the history, clinical signs, diagnostic process, treatment and clinical outcome in these cases. The data were obtained from medical records, review of the radiographs, and telephone follow-up with the owners. All of the dogs in this study had severe chronic osteoarthritis secondary to hip dysplasia, which caused periodic hind limb lameness. They were all admitted with severe acute hind-limb lameness. Pelvic radiographs were performed under general anaesthesia shortly after presentation, followed immediately by arthrocentesis of the affected joint. The synovial fluid was evaluated microscopically by direct smear and a sample was sent for culture and sensitivity. An attempt was not made to drain or lavage the affected joint during the course of treatment. The initial choice of antibiotics was empiric and subsequently modified, as required, based on the sensitivity results. Four of the dogs showed a rapid return to weight-bearing after the initiation of antibiotic treatment, and all of the patients returned to their pre-BIA level of function. Neither recurrent infections nor any adverse sequela requiring further intervention were reported by the owners on telephone follow-up. 相似文献

Bilateral degenerative coxofemoral joint disease and noninflammatory osteonecrosis in the femoral heads were diagnosed in a 5-month-old Standardbred colt. Cytologic evaluation and bacterial cultures of coxofemoral synovial fluid, and radiographic and pathologic examination of the coxofemoral joints were conducted. The objective of this study was to investigate the effect of xylazine on the stress and pain response of lame cows undergoing claw treatment in lateral recumbency (LR). Twenty-four lame, German Holstein-Friesian cows were included in a prospective, blinded, placebo-controlled clinical case study. Cows were randomly allocated to two groups (n=12) and either treated with xylazine (0.05 mg/kg BW, i.m.) or an equal volume of saline (controls) 15 min before placing them in LR. Before surgical treatment, each cow received retrograde intravenous local anaesthesia (LA). Over a period of 6h, heart rate (HR), respiratory rate (RR), plasma concentrations of cortisol, glucose, lactate and non-esterified fatty acids (NEFA) were determined at preset time intervals and the animals' behaviour was recorded via video recording and pedometer. All xylazine-treated cows showed signs of mild sedation, a reduced pain response on insertion of the needle for LA, reduced ear flicking during surgery, reduced lameness score and longer standing periods as well as improved appetite in the first hour postoperatively. Mean HR and RR, as well as plasma concentrations of NEFA were significantly reduced after xylazine treatment. Cortisol concentrations were significantly lower in xylazine-treated cows after being placed in LR but were similar to controls during surgery. The use of a low dose of xylazine in the analgesic protocol for the treatment of lame cows in LR alleviates stress and potentiates the analgesic effects of LA. 相似文献

The authors are aware of only one previously described case of idiopathic infectious arthritis of the coxofemoral joint in a mature horse. The case described herein presented with clinical signs that localised pain to the proximal limb, subtle increased radiopharmaceutical uptake at the coxofemoral joint and an increase of the major acute phase protein, serum amyloid A in the horse's blood biochemistry. The latter finding led the authors to search initially for an infective process. Ultrasound guided aspiration of the coxofemoral joint yielded infected synovial fluid. Despite the duration of the condition, nonsurgical treatment allowed the horse to return to its previous, relatively modest, level of activity. This case is presented to supplement the single previously published case report of idiopathic adult coxofemoral infectious arthritis in the horse. 相似文献

A transarticular loop of autogenous fascia lata was used to stabilise the coxofemoral joint following open reduction of recurrent and long standing hip luxation in 10 dogs and two cats. The caudolateral approach used proved to be simple and allowed good visualisation. Good results were obtained and only one of 12 cases reluxated in a follow-up period varying from three months to one year. The loop technique required little sophisticated equipment and is within the scope of the average surgeon. 相似文献

Effect of anaesthesia and stress on the radiographic evaluation of the coxofemoral joint 总被引:1,自引:0,他引:1
A pelvic radiographic examination was performed on 15 large breed dogs without history and clinical sign of hip dysplasia. The effect of anaesthesia and of two stress-radiographic methods on the coxofemoral subluxation was evaluated. With anaesthesia a mild coxofemoral subluxation was seen in 31 per cent of the dogs, which all appeared radiographically normal when sedated. The Norberg angle was significantly decreased in anaesthetised dogs (P < 0–05). When dogs with a radiographically normal coxofemoral joint conformation were submitted to a stress-radiographic examination there was a shift towards an increased coxofemoral subluxation. Subluxation was seen in 70 per cent of the dogs when the knee fixation method of positioning was applied and in 100 per cent of the dogs when the wedge method was used. Correspondingly the Norberg angle decreased significantly when the coxofermoral joints were stressed (Pkneefix < 0–00003; Pwedge= 0–000008). The study concludes that the currently used procedure for evaluation of the hip joints does not necessarily disclose whether or not a dog has hip dysplasia. 相似文献

Canine hip dysplasia (CHD) is characterized by a malformation of the hip joint that leads to joint laxity and consequential degenerative joint disease. The most widely used method for diagnosis of CHD is the ventrodorsal hip-extended radiologic view, commonly referred to as the Orthopedic Foundation for Animals (OFA) method. The method of the University of Pennsylvania Hip Improvement Program (PennHIP), an alternative technique that is based on hip joint laxity, provides a quantitative assessment, the distraction index (DI), of the likelihood of the development of CHD because of increased laxity in the hip joint. Linear regression analysis showed that, across many breeds of dog, the incidence of CHD, as defined by the OFA, is positively correlated with the mean DI, the determination coefficient (r2) being 26%. We used families of Boykin spaniels (BSs) to determine the level of joint laxity in the breed and to conduct an initial whole-genome screening to identify markers that co-segregate with increased joint laxity. Although there was a positive correlation between the incidence of hip dysplasia and increased joint laxity, we did not find significant linkage in the 28 BSs that underwent genotyping, likely owing to the small size of the pedigree. 相似文献
