Laser is the acronym for light amplification by stimulated emission of radiation; the first laser beam was generated in 1960 and initially used for medical application in 1963. Today's modern surgical laser units produce an intensely focused light beam of uniform wavelength that interacts with a variety of biological cell types to vaporize, cut, coagulate, or shrink tissues. The resulting effect depends on the optical properties of the tissue and the wavelength, power output, spot size, and duration of laser beam application. Different lasers are suited to specific surgical tasks relative to their beam wavelength, power output, mode of operation (continuous, pulsed or superpulsed), and application (contact or noncontact) and instrumentation. The documented advantages of laser surgery over conventional surgical procedures are improved hemostasis, the ability to ablate or vaporize tissue, endoscopic access to body cavities, improved postoperative comfort, and shorter hospitalization and recovery times. By the mid-1980s, laser use in equine surgery was reported; the carbon dioxide and neodymium:yttrium-aluminum-garnet were the first models integrated into practice. The primary application of laser technology in equine surgery was for the correction of various upper-respiratory tract abnormalities and still remains the most prevalent use through endoscopy. Today, surgical lasers are used for the treatment of problems in the urinary, reproductive, and musculoskeletal systems, in addition to the skin and the eye. When we combine advances in laser technology and the practice of equine surgery, the result is clinicians better equipped to treat and manage the more difficult cases.  相似文献

Laparoscopy is a minimally invasive procedure that has applications as a diagnostic, therapeutic and prognostic technique. Specialized equipment is necessary to perform equine laparoscopy, and there is a large range of instruments, both disposable and non-disposable available. Laparoscopic procedures described include ovariectomy, cryptorchidectomy, adhesiolysis and herniorrhaphy. Laparoscopy can be performed in a standing or dorsally recumbent position, depending on surgeon preference, patient status and the procedure to be performed. Stapling equipment is frequently used in gastrointestinal surgery in horses. Advantages include decreased surgical time and a decrease in the risk of contamination. Stapling equipment is often used in creating anastomoses, both in the large and small intestines, as well as in vessel ligation. New surgical techniques intended to decrease adhesion formation include the use of carboxymethylcellulose and bioresorbable patches. Indwelling abdominal drains can be used for peritoneal lavage following surgery and also appear to decrease the risk of adhesion formation. Improvements in post-operative care, including the treatment of post-operative ileus and endotoxaemia can significantly improve the outcome of horses that have undergone surgery for abdominal disorders. Recommendations for the use of prokinetic agents in horses with ileus vary widely. Prokinetic agents include local anaesthetics, macrolide antimicrobials, cholinergic agonists and dopamine antagonists. Endotoxaemia is common in horses following surgery for gastrointestinal disorders. The antibiotic polymyxin B binds to the circulating endotoxin molecule, decreasing its half-life in the intra-vascular space and reducing associated inflammation. This drug appears to be an effective and affordable treatment option for horses with endotoxaemia. The use of specific cyclooxygenase inhibitors in veterinary medicine have been studied recently. Selective cyclooxygenase-2 inhibitors may provide comparable anti-inflammatory and analgesic properties to the non-selective non-steroidal anti-inflammatory drugs. These drugs appear to have similar clinical effectiveness and will hopefully minimize deleterious side effects. The optimal healing of ventral midline incisions in horses is related to many factors including appropriate suture patterns and bite size, in addition to appropriate post-operative exercise recommendations. Recent advances in surgical techniques and post-operative care should decrease the morbidity and mortality associated with abdominal surgery. Reasons for performing study: Medications are frequently employed to treat intra‐articular (IA) problems in the performance horse. Actual usage of the different IA medications in horses is not available. Objectives: To determine the most common usage of these medications, members of the American Association of Equine Practitioners (AAEP) were surveyed. Methods: An email link to an online survey was electronically sent to 6305 AAEP members and the responses tabulated and analysed with a logistic regression model. Results: A total of 831 survey responses were submitted and tabulated. Eighty per cent of the respondents indicated that they see 100% equine cases in their practice. The majority of respondents (77%) use triamcinolone acetonide (TA) to treat high motion joints and 73% use methylprednisolone acetate (MPA) to treat low motion joints. Veterinarians treating the Western performance and Sport horse were significantly more likely to use TA in high motion joints compared to MPA (P = 0.0201 and P<0.0001, respectively). Triamcinolone acetonide use compared to MPA in high motion joints by racehorse veterinarians was significantly lower compared to other veterinarians (P<0.0001). Polysulphated glycosaminoglycan (Adequan) and hyaluronate sodium (Legend) were the most commonly used disease modifying products (63 and 57% of respondents, respectively). Sport horse practitioners were significantly more likely than race or show horse veterinarians to utilise IRAP products (P = 0.0035 and P = 0.04, respectively). Respondents who had been in practice for more than 10 years were significantly less likely to use antimicrobials in their joint injections compared to those in practice for less than 10 years (P<0.0001). Conclusions: Significant differences existed in usage of medications related to primary discipline treated and years practicing. Potential relevance: The results of this study aid in defining the current usage of different joint therapy medications within equine practice. Automatic suturing instruments were tested in two ponies and two horses. The TA stapler was used for bowel closure (enterotomies and gut transection) and for end-to-end 'triangulation' anastomoses. The GIA stapler was used for making side-to-side anastomoses. In one horse several enterotomies were made at different stages and the results of suturing by hand and with a stapler were compared. The utility of both types of staplers is discussed in relation to clinical evaluations and macroscopical and microscopical findings post mortem.  相似文献

The advantages and disadvantages of various surgical lasers are discussed. Included are aspects of laser safety, anesthesia and analgesia considerations for laser surgery, and diagnostic considerations. Horses with lesions such as ethmoid hematomas, nasal polyps, and lymphoid masses are ideal candidates for laser treatment. Other conditions that are suitably treated with lasers, such as dorsal displacement of the soft palate and entrapment of the epiglottis, are described.  相似文献

Objective To determine the clinical and histologic effects of diode endoscopic cyclophotocoagulation (ECP) in the phakic equine eye. Animals studied Phase I: 10 equine cadaver eyes. Phase II: four normal adult horses. Procedures Phase I: ECP probe angle of reach (AR) was determined. Multiple ECP energy levels: 0.75, 0.90, 1.05, 1.20, 1.35, 1.50 J, and the resulting visible and histologic ciliary process changes were evaluated. Phase II: Ocular quadrants were treated with ECP at 0.90, 1.14, 1.38 J, and a control. The contralateral eye underwent a sham operation. Tissue changes (clinical and histologic) were evaluated. Results Phase I: Mean combined AR was 162 ± 29 degrees. Mean visible tissue scores: 2.60 ± 0.58 (0.75 J) to 5.04 ± 0.30 (1.50 J) from possible total of 6. Tissue ‘popping’ was observed at 1.50 J. Histologic ciliary tissue damage was present at all settings. Phase II: Mean visible tissue scores: 2.90 ± 0.48 (0.90 J), 3.61 ± 0.57 (1.14 J), and 4.52 ± 0.56 (1.38 J). Tissue ‘popping’ was observed at 1.38 J. Histologic ciliary tissue damage was present at all settings. Clinical effects included acute inflammation, intraocular pressure reduction, cataract formation, corneal edema, corneal ulceration, and postoperative ocular hypertension. Conclusions Diode ECP between 0.90 and 1.14 J is a potential treatment option for glaucoma in horses based on visible tissue effects and target ciliary epithelium damage. Iatrogenic cataract development may limit the use of an anterior chamber approach in phakic horses. Supported in part by an ACVO VAF grant.  相似文献

Serum amyloid A (SAA) has become an indispensable part of the management of equine patients in general practice and specialized hospital settings. Although several proteins possess acute phase properties in horses, the usefulness of SAA exceeds that of other acute phase proteins. This is due to the highly desirable kinetics of the equine SAA response. SAA concentrations exhibit a rapid and pronounced increase in response to inflammation and a rapid decline after the resolution of inflammation. This facilitates the detection of inflammatory disease and real-time monitoring of inflammatory activity. SAA may be used in all stages of patient management: (1) before diagnosis (to rule in/rule out inflammatory disease), (2) at the time of diagnosis (to assess the severity of inflammation and assist in prognostication), and (3) after diagnosis (to monitor changes in inflammatory activity in response to therapy, with relapse of disease, or with infectious/inflammatory complications). By assessing other acute phase reactants in addition to SAA, clinicians can succinctly stage inflammation. White blood cell counts and serum iron concentration change within hours of an inflammatory insult, SAA within a day, and fibrinogen within 2–3 days; the interrelationship of these markers thus indicates the duration and activity of the inflammatory condition. Much research on the equine SAA response and clinical use has been conducted in the last decade. This is the prerequisite for the evidence-based use of this analyte. However, still today, most published studies involve a fairly low number of horses. To obtain solid evidence for use of SAA, future studies should be designed with larger sample sizes.  相似文献

Standing female urogenital endoscopic surgery is facilitated by the more dorsal location of the organs of the female reproduction tract. The most common reason for laparoscopic surgery on the female urogenital system is ovariectomy; however, the technique has been used to diagnose periparturient or reproductive diseases and to perform surgical embryo transfer. Standing surgical approaches avoid the risk and expense of general anesthesia, but these techniques are limited by the temperament and size of the patient and the availability of facilities for restraint. Owner acceptance of laparoscopic procedures is excellent. It is anticipated that standing endoscopic surgery may become the preferred technique for applicable procedures.  相似文献

Standing laparoscopy in the horse was used initially as a diagnostic tool to assist in the diagnosis of a cryptorchid testicle. Now, this technique is also used for treatment of cryptorchid testicles. The focus of this article is the standing laparoscopic cryptorchidectomy, the most common laparoscopic procedure performed on the male equine urogenital tract. The ventral abdominal approach for laparoscopy is versatile. Reported techniques using a ventral approach in the mare include bilateral ovariectomy and granulosa cell tumor removal. Laparoscopic ovariectomy is performed by way of four instrument portals in the caudoventral aspect of the insufflated abdomen. The procedure has several advantages over the traditional approaches of colpotomy and celiotomy, including tension-free closure of all incisions and minimal invasiveness.  相似文献

Evaluation of the horse with colic has always been challenging since the patient's large size precludes many of the diagnostic imaging procedures commonly used in human medicine. Diagnostic methods such as radiography, laparoscopy, endoscopy, and peritoneal fluid analysis can serve to increase the accuracy of presurgical evaluation. Prognosis in individual cases can be best predicted by careful analysis of selected clinicopathological data, physical examination findings, and surgical biopsies. However, no predictive model is 100 per cent accurate, and clinicians must continue to rely on clinical evidence and instinct and to use these diagnostic and prognostic procedures only as guides for case management.  相似文献

Possibly no other development has revolutionized general surgery more dramatically than laparoscopic surgery in human patients. The development of instrumentation suitable for use in horses has allowed the veterinary surgeon to begin to introduce minimally invasive procedures. Clearly, in people, one of the greatest benefits is reduced patient morbidity and earlier return to function. The author's experience with laparoscopic procedures in the horse would tend to reinforce those observations. The most significant obstacle to widespread acceptance of minimally invasive surgery in the horse is possibly the veterinarian's reluctance to expend the funds necessary to purchase the necessary equipment as well as a hesitancy to abandon time-honored approaches to common surgical conditions. As a profession, veterinarians have been criticized for undervaluing their own services. Clearly, these new minimally invasive procedures can be successfully marketed if the advantages of the approaches in terms of patient well-being are impressed on the client.  相似文献

Two functionally important differences exist between the diode laser and the carbon dioxide (CO2) laser (used more commonly in small animal surgery). Diode laser energy is delivered through a quartz fiber instead of being reflected through an articulated arm or waveguide. Quartz fibers are generally more flexible and resilient than waveguides and can be inserted through an endoscope for minimally invasive procedures. Laser-tissue interaction is the other significant difference. The CO2 laser is completely absorbed by water, which limits the effect to visible tissue. The diode wavelength is minimally absorbed by water and may affect tissue as deep as 10 mm below the surface in the free-beam mode. With proper respect for the tissue effect, these differences can be used to the advantage of the patient.  相似文献

Survival rates and post-operative complications after equine colic surgery   总被引:2,自引:0,他引:2  
SUMMARY A retrospective analysis of 74 cases that underwent surgery for colic was undertaken to determine short and long term survival rates and the incidence of postoperative complications. In 28 cases colic was related to small intestinal lesions and in 46 cases to large intestinal lesions. Pre-operative packed cell volume and heart rate were found to be inversely related to short-term survival. Twenty-five horses (34%) recovered from surgery and were discharged. Of 18 of these cases with available histories, 6 subsequently had one or more episodes of colic since surgery of which 5 eventually died or were euthanased due to further colic; the remaining 12 have remained free from recurrence of colic for longer than 6 months.  相似文献
