Wick catheters were used to measure intracompartmental muscle pressures (ICMP) within the long heads of the triceps brachii and extensor carpi radialis muscles of 8 horses maintained under halothane anesthesia while their breathing was controlled by intermittent positive-pressure ventilation. Blood gas, cardiac output, and blood pressure determinations were monitored to maintain a stable plane of anesthesia. The horses were positioned in left lateral recumbency and were placed sequentially on each of 4 contact surfaces for 1 hour. The 4 surfaces used for each horse were concrete, foam rubber, air dunnage bag, and a water mattress. Hematologic and biochemical determinations were made before and 24 hours after anesthesia. All horses recovered from the anesthesia. One horse had forelimb lameness for 36 hours after anesthesia, which was clinically diagnosed as a myoneuropathy. The ICMP values were markedly elevated in the muscle bellies of the lower limb of all horses. Supporting the horse on a water mattress caused the least dramatic pressure elevation and foam caused the most. The triceps muscle and, to a lesser extent, the extensor carpi radialis muscle of the lower limb are at risk of ischemia in anesthetized horses because the ICMP may exceed the critical closing pressure of 30 mm of Hg required for capillary blood flow.  相似文献

Five horses that underwent prolonged anesthesia (greater than 3 hours) in dorsal recumbency for a surgical procedure were unable to stand after recovery and were euthanatized. A provisional diagnosis of postanesthetic myopathy was confirmed at necropsy in all 5 horses. However, distribution of affected muscles in these horses was atypical, because there was bilateral hind limb adductor muscle involvement.  相似文献

Desmitis of the proximal aspect of the suspensory ligament, or interosseus medius muscle, of the pelvic or thoracic limb is a commonly diagnosed cause of lameness of performance horses. Despite medical treatments available for horses with proximal suspensory desmitis (PSD), most horses treated medically for PSD of a pelvic limb remain persistently lame; this persistent lameness may be the result of a neuropathy caused by compression of nerves by an enlarged suspensory ligament. Few horses with PSD of a thoracic limb remain persistently lame. Based on the results of reports citing successful treatment of horses chronically lame because of PSD of a pelvic or thoracic limb, by excising a portion of the deep branch of the lateral plantar or palmar nerve (DBLPlN/DBLPaN), we theorized that persistent lameness of horses caused by PSD of a thoracic limb may also be due to compression of nerves that supply the ligament. The aim of this study was to determine if histological signs of compression neuropathy of the DBLPaN are present in horses with PSD in a thoracic limb. To test this hypothesis, we induced PSD by instilling collagenase into the ligament and then examined the DBLPaN after harvesting this nerve 2 months later. We found that the DBLPaNs of all treated limbs showed histologic changes suggestive of nerve compression. We conclude that studies examining the DBLPaN of horses with naturally occurring PSD for histological evidence of neuropathy are warranted.  相似文献

OBJECTIVE: To describe and evaluate subtotal ostectomy of dorsal spinous processes (DSP) performed in standing horses. STUDY DESIGN: Retrospective study. SAMPLE POPULATION: Nine adult horses. METHODS: The diseased portions of the DSP were removed with the horses standing and sedated after infiltrating a local anesthetic agent around the affected DSP. Medical records of affected horses were reviewed to determine history, findings of physical and radiographic examination, surgical technique, postoperative complications, and outcome. RESULTS: Five horses had subtotal ostectomy of DSP because of osteomyelitis, and 4 because of impingement and/or fracture of DSP. Affected portions of DSP were removed safely and effectively, with no apparent discomfort to the horses; hemorrhage was minimal. CONCLUSIONS: Subtotal ostectomy of DSP can be performed safely in standing, conscious horses and the risks associated with general anesthesia are eliminated. Hemorrhage is minimal compared to that which occurs when the procedure is performed with horses anesthetized and in lateral recumbency. CLINICAL RELEVANCE: Access and visibility are better when subtotal ostectomy of the DSP is performed with the horse standing rather than anesthetized and in lateral recumbency. Performing subtotal ostectomy of diseased DSP with the horse standing avoids the expense and risks of general anesthesia.  相似文献

Acute hemorrhagic myelopathy developed in the sixth cervical to the eighth thoracic spinal cord segments of a 1-year-old Quarter Horse colt that was castrated under general anesthesia while in dorsal recumbency. Clinical signs were consistent with severe transverse myelopathy caudal to the brachial enlargement and cranial to the lumbosacral enlargement of the spinal cord. Histologic examination of the spinal cord revealed hemorrhage in the gray matter, with multiple blood-filled clefts in otherwise normal neuropil. Hemodynamic changes in the spinal cord associated with anesthesia and dorsal recumbency may have led to hypoxic vessel damage, with massive hemorrhage after surgery, when the horse was returned to lateral recumbency. Postanesthetic hemorrhagic myelopathy is a possible complication of positioning in dorsal recumbency, during anesthesia, in rapidly growing, young horses.  相似文献

Objective— To describe anatomic considerations and arthroscopic technique in horses for arthroscopic removal of palmar/plantar osteochondral fragments from the proximal interphalangeal (PIP) joint. Study Design— Retrospective study. Animals— Adult horses (n=4) with osteochondral fragments of the palmar/plantar PIP joint. Methods— Arthroscopic removal of palmar/plantar osteochondral fragments within the PIP joint was performed with horses in dorsal recumbency under general anesthesia. Medical records of affected horses were reviewed to determine history; physical, lameness, and radiological findings; surgical technique; complications and outcome. Results— Two horses had lameness localized to the PIP joint. Two other horses had lameness suspected, but not confirmed to the pastern region. One of these horses had a history of intermittent lameness, but was not lame on admission. All horses had radiographic evidence of palmar/plantar osteochondral fragmentation within the PIP joint. Fragmentation was located abaxially in 2 horses in the hind limb and axially in 2 horses in the left forelimb. Osteochondral fragments were successfully removed via a palmar/plantar arthroscopic approach in all horses. Three horses returned to previous levels of athletic performance; 1 horse was used for trail riding instead of reining. Conclusions— Arthroscopy of the palmar/plantar pouch of the PIP joint allowed limited assessment of the joint and removal of osteochondral fragments. Clinical Relevance— Arthroscopy of the palmar/plantar PIP joint pouch for assessment and removal of osteochondral fragments is possible and should be considered when lameness is localized to this joint.  相似文献

Effects of 1.55 minimum alveolar concentration isoflurane O2 anesthesia (2.03% end-tidal isoflurane) on blood flow in the up-(nondependent) and down-(dependent) positioned skeletal muscles were studied at 60, 120, and 180 minutes in 6 healthy isocapnic ponies in right lateral recumbency on a nonpadded hardwood floor. Measurements were made, using 15-micron diameter radionuclide-labeled microspheres injected into the left ventricle, and comparisons were made with data obtained from ponies in the conscious state. Isoflurane administration caused a sharp reduction in cardiac output and systemic pressure (P less than 0.01), but total peripheral resistance did not change significantly. In the triceps brachii, gluteus medius, biceps femoris, and vastus lateralis of both sides, blood flow decreased significantly during 1.55 minimum alveolar concentration isoflurane anesthesia (P less than 0.01), and fluctuations did not occur with increasing duration of anesthesia. In masseter muscles, perfusion values during the 3 hours of anesthesia were not significantly different from values in awake ponies. Despite the fact that 4 ponies developed marked edema of the dependent masseter muscle, 1 pony without masseter edema developed postanesthetic forelimb lameness and 2 of the 4 ponies with masseter edema had generalized hind limb weakness after anesthesia; significant differences in blood flow between up- and down-positioned muscles were not observed. During isoflurane-O2 anesthesia in ponies, a sharp significant decrease in skeletal muscle blood flow was observed (P less than 0.01). Decreased equine skeletal muscle perfusion during isoflurane anesthesia also may be accompanied by accentuated O2 loss from the arterial blood via the countercurrent O2 exchange between large arterioles and venules.  相似文献

Objective

To compare the effects of two concentrations of oxygen delivered to the anaesthetic breathing circuit on oxygenation in mechanically ventilated horses anaesthetised with isoflurane and positioned in dorsal or lateral recumbency.

Methods

Selected respiratory parameters and blood lactate were measured and oxygenation indices calculated, before and during general anaesthesia, in 24 laterally or dorsally recumbent horses. Horses were randomly assigned to receive 100% or 60% oxygen during anaesthesia. All horses were anaesthetised using the same protocol and intermittent positive pressure ventilation (IPPV) was commenced immediately following anaesthetic induction and endotracheal intubation. Arterial blood gas analysis was performed and oxygenation indices calculated before premedication, immediately after induction, at 10 and 45 min after the commencement of mechanical ventilation, and in recovery.

Results

During anaesthesia, the arterial partial pressure of oxygen was adequate in all horses, regardless of position of recumbency or the concentration of oxygen provided. At 10 and 45 min after commencing IPPV, the arterial partial pressure of oxygen was lower in horses in dorsal recumbency compared with those in lateral recumbency, irrespective of the concentration of oxygen supplied. Based on oxygenation indices, pulmonary function during general anaesthesia in horses placed in dorsal recumbency was more compromised than in horses in lateral recumbency, irrespective of the concentration of oxygen provided.

Conclusion

During general anaesthesia, using oxygen at a concentration of 60% instead of 100% maintains adequate arterial oxygenation in horses in dorsal or lateral recumbency. However, it will not reduce pulmonary function abnormalities induced by anaesthesia and recumbency.  相似文献

One hundred sixty horses were anesthetized with xylazine, guaifenesin, thiamylal, and halothane for elective soft tissue and orthopedic procedures. Horses were randomly assigned to one of four groups. Group 1 (n = 40): Horses positioned in lateral (LRG1,; n = 20) or dorsal (DRG1,; n = 20) recumbency breathed spontaneously throughout anesthesia. Group 2 (n = 40): Intermittent positive pressure ventilation (IPPV) was instituted throughout anesthesia in horses positioned in lateral (LRG2; n = 20) or dorsal (DRG2; n = 20) recumbency. Group 3 (n = 40): Horses positioned in lateral (LRG3; n = 20) or dorsal (DRG3; n = 20) recumbency breathed spontaneously for the first half of anesthesia and intermittent positive pressure ventilation was instituted for the second half of anesthesia. Group 4 (n = 40): Intermittent positive pressure ventilation was instituted for the first half of anesthesia in horses positioned in lateral (LRG4; n = 20) or dorsal (DRG4; n = 20) recumbency. Spontaneous ventilation (SV) occured for the second half of anesthesia. The mean time of anesthesia was not significantly different within or between groups. The mean time of SV and IPPV was not significantly different in groups 3 and 4. Variables analyzed included pH, PaCO2, PaO2, and P(A-a)O2 (calculated). Spontaneous ventilation resulted in significantly higher PaCO2 and P(A-a)O2 values and significantly lower PaO2 values in LRG1, and DRG1, horses compared with LRG2 and DRG2 horses. Intermittent positive pressure ventilation resulted in normocarbia and significantly lower P(A-a)O2 values in LRG2 and DRG2 horses. In LRG2 the Pao2 values significantly increased from 20 minutes after induction to the end of anesthesia. The PaO2 and P(A-a)O2 values were not significantly different from the beginning of anesthesia after IPPV in DRG2 or DRG3. The PaO2 values significantly decreased and the P(A-a)O2 values significantly increased after return to SV in horses in LRG4, and DRG4. The PaO2 values were lowest and the P(A-a)O2 values were highest in all horses positioned in dorsal recumbency compared with lateral recumbency and in SV horses compared with IPPV horses. The pH changes paralleled the changes in PaCO2. Blood gas values during right versus left lateral recumbency in all groups were also evaluated. The PaO2 values were significantly lower and the P(A-a)O2 values were significantly higher during SV in horses positioned in left lateral (LRLG1) compared with right lateral (LRRG1) recumbency. No other significant changes were found comparing left and right lateral recumbency. Arterial hypoxemia (PaO2 < 60 mm Hg) developed in 35% of DRG1 horses and 20% of DRG2 horses at the end of anesthesia. Arterial hypercarbia (PaCO2= 50–60 mm Hg) developed in DRoi horses. Arterial hypoxemia that developed in 20% of DRG3 horses was not improved with IPPV. Arterial hypoxemia developed in 55% of DRG4 horses after return to SV. Some DRG4 horses with hypoxemia also developed hypercarbia, whereas some had PaCO2 values within normal limits. Arterial hypoxemia developed in one LRG1, and two LRG4, horses. Hypercarbia developed in onlv one LRG4 horse.  相似文献

The relationship between mixed venous O2 tension and cardiac output was studied in six anesthetized horses breathing 100% O2. Cardiac output, O2 consumption, mean arterial pressure, heart rate, and arterial and venous blood gases were measured after administration of xylazine or dobutamine to horses in lateral, sternal, and dorsal recumbencies. After approximately 3 hours, Escherichia coli endotoxin was administered while horses were in dorsal recumbency, and all measurements were repeated. Relationships between cardiac index (CI) and PVO2, heart rate, mean arterial pressure, jugular PVO2, and PVO2 of blood from a superficial limb vein were evaluated by linear regression analysis. Mean arterial pressure was significantly (P less than 0.05) correlated with CI in horses in all positions and after endotoxin administration. However, data points were poorly grouped. Heart rate and CI were significantly correlated in horses in all positions, but not after endotoxin administration. Correlations between jugular PVO2 and PVO2 of blood from a superficial limb vein were not significant in horses in sternal recumbency, and PVO2 of blood from a superficial limb vein was not significantly correlated with CI in horses in lateral recumbency. There was a significant and tight correlation between PVO2 and CI in horses in all positions and after endotoxin administration.  相似文献

Dantrolene, a drug used in the prevention and treatment of malignant hyperthermia, was believed responsible for prolonged postanesthetic recumbency in a horse. Prophylactically, dantrolene was given orally before induction of anesthesia. Dantrolene has been recommended for use in horses at risk of developing postanesthetic myopathy. OBJECTIVE: To determine whether heavy (> or = 680 kg [> or = 1,500 lb]) draft horses undergoing surgical treatment for acute signs of abdominal pain were at a greater risk for anesthetic and postoperative complications and lower postoperative survival rates than light (< 680 kg) draft horses. DESIGN: Retrospective case series. ANIMALS: 72 draft horses. PROCEDURES: Medical records of draft horses that underwent exploratory celiotomy for signs of acute abdominal pain from October 1983 to December 2002 were reviewed. Medical records of draft horses in which a celiotomy was performed for correction of reproductive abnormalities were not included in the study. RESULTS: When compared with light draft horses, heavy draft horses had longer durations of anesthesia, more postoperative complications, and lower survival rates. Seventy-six percent of horses that recovered from anesthesia had postoperative complications. Postoperative complications associated with low survival rates included myopathy and neuropathy, ileus, diarrhea, and endotoxemia. All horses with postoperative myopathy and neuropathy died or were euthanized. The short-term survival rate for horses that recovered from anesthesia was 60%. Horses undergoing small intestinal surgery had a worse prognosis for short-term survival than those undergoing large intestinal surgery. The survival rate for horses for which long-term (> 1 year) follow-up information was available was 50%. CONCLUSIONS AND CLINICAL RELEVANCE: Draft horses weighing > 680 kg that underwent surgery because of acute signs of abdominal pain had longer durations of anesthesia, more postoperative complications, and higher mortality rates than draft horses weighing < 680 kg.  相似文献

OBJECTIVE: To determine history, clinical and radiographic abnormalities, and outcome in horses with signs of navicular area pain unresponsive to corrective shoeing and systemic nonsteroidal anti-inflammatory drug administration that were treated with an injection of corticosteroids, sodium hyaluronate, and amikacin into the navicular bursa. DESIGN: Retrospective study. ANIMALS: 25 horses. PROCEDURE: Data collected from the medical records included signalment, history, horse use, severity and duration of lameness, shoeing regimen, results of diagnostic anesthesia, radiographic abnormalities, and outcome. RESULTS: 17 horses had bilateral forelimb lameness, 7 had unilateral forelimb lameness, and 1 had unilateral hind limb lameness. Mean duration of lameness was 9.2 months. All horses had been treated with corrective shoeing and nonsteroidal anti-inflammatory drugs for at least 6 months; 18 had previously been treated by injection of corticosteroids and sodium hyaluronate into the distal interphalangeal joint. Fourteen horses had mismatched front feet, and 21 horses had signs of pain in response to application of pressure over the central aspect of the frog. Palmar digital nerve anesthesia resulted in substantial improvement in or resolution of the lameness in all horses. Twenty horses (80%) were sound and returned to intended activities 2 weeks after navicular bursa treatment; mean duration of soundness was 4.6 months. Two horses that received numerous navicular bursa injections had a rupture of the deep digital flexor tendon at the level of the pastern region. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that navicular bursa treatment may provide temporary improvement in horses with signs of chronic navicular area pain that fail to respond to other treatments.  相似文献

The anesthetic sparring and cardiovascular effects produced by midazolam 0.8 mg/ml-ketamine 40 mg/ml-medetomidine 0.05 mg/ml (0.025 ml/kg/hr) drug infusion during sevoflurane in oxygen (MKM-OS) anesthesia was determined in healthy horses. The anesthetic sparring effects of MKM-OS were assessed in 6 healthy thoroughbred horses in which the right carotid artery was surgically relocated to a subcutaneous position. All horses were intubated and ventilated with oxygen using intermittent positive pressure ventilation (IPPV). The end-tidal concentration of sevoflurane (ET(SEV)) required to maintain surgical anesthesia was approximately 1.7%. Heart rate and mean arterial blood pressure averaged 23-41 beats/min and 70-112 mmHg, respectively. All horses stood between 23-44 min after the cessation of all anesthetic drugs. The cardiovascular effects of MKM-OS anesthesia were evaluated in 5 healthy thoroughbred horses ventilated using IPPV. Anesthesia was maintained for 4 hr at an ET(SEV) of 1.7%. Each horse was studied during left lateral (LR) and dorsal recumbency (DR) with a minimum interval between evaluations of 1 month. Cardiac output and cardiac index were maintained between 70-80% of baseline values during LR and 65-70% of baseline values during DR. Stroke volume was maintained between 75-85% of baseline values during LR and 60-70% of baseline values during DR. Systemic vascular resistance was not different from baseline values regardless of position. MKM-OS anesthesia may be useful for prolonged equine surgery because of its minimal cardiovascular depression in both of lateral and dorsal recumbency.  相似文献

Infrared thermography is an imaging modality gaining popularity as a diagnostic aid in the evaluation of equine lameness. Anecdotal reports of skin hyperthermia induced by local anesthesia, detected by thermography, have been made; however, no controlled studies have been reported. The purpose of this study was to examine the effects of perineural anesthesia on infrared thermographic images of the forelimb digits in normal horses. After environmental acclimation, infrared thermographs were made at intervals of 0, 5, 10, 15, 30, and 45 min from administration of mepivacaine hydrochloride or phosphate buffered saline in 6 adult horses with no clinical evidence of abnormality of the forelimb digits. The mean limb surface temperatures were compared by 2-factor ANOVA. Results indicated no significant difference between treatments, time after injection, or an interaction of time and treatment. Infrared thermographic imaging apparently can be performed within 45 min of perineural mepivacaine hydrochloride anesthesia without risk of artifactual changes in limb surface temperature.  相似文献

The effects of body position and postoperative oxygen supplementation on arterial blood gas tensions (PaO2 and PaCO2) and pH were examined in clinically normal adult horses during recovery from halothane anesthesia. Hypoxemia developed during recovery from anesthesia in spite of adequate alveolar ventilation in horses without postanesthetic oxygen supplementation. Hypoxemia developed in horses positioned in left lateral or right lateral recumbency, and in horses that were rolled to the opposite side during the recovery period. Arterial blood gas tensions were not significantly (P greater than 0.05) different between horses insufflated with 100% oxygen at the rate of 10 L/min during recovery and horses that did not receive oxygen supplementation during the recovery period. Horses that received 100% oxygen at the rate of 50 L/min through a demand valve had arterial blood gas tensions similar to those in standing awake horses.  相似文献

Objective — To determine the minimum alveolar concentration (MAC) of desflurane (DES) in the horse.
Study Design — Prospective study.
Animals — Six healthy adult horses (three males and three females) weighing 370 ±16 kg and aged 9 ±2 years old.
Methods — Anesthesia was induced with DES vaporized in oxygen via a face mask connected to a large-animal, semiclosed anesthetic circle system. The horses were endotracheally intubated and positioned in right lateral recumbency. Inspired and end-tidal DES were monitored using a calibrated Ohmeda RGM 5250 multigas analyzer (Ohmeda-BOC, Spain). The MAC of desflurane that prevented gross purposeful movement in response to 60 seconds of noxious electrical stimulation of oral mucous membranes was determined.
Results — The time from the start of DES administration to lateral recumbency was 6.1 ±0.9 min. The MAC of DES in these horses was 7.6 ±0.4%. Time required for the animal to regain sternal recumbency after 98 ±4 minutes of anesthesia was 6.6 ±0.5 minutes and the time to standing was 14.3 ±2.7 minutes.
Conclusions — The MAC of desflurane in these horses was 7.6 ±0.4%. DES provided a rapid induction to, and recovery from, anesthesia.
Clinical Relevance — Desflurane offers the potential for more precise control during anesthesia, and may allow a faster and uneventful recovery. It is important to know the MAC of an inhalant to use it clinically.  相似文献

OBJECTIVE: To evaluate the use of arthroscopy as the primary method for removal of large patellar fracture fragments. DESIGN: Retrospective study. ANIMALS: 5 performance horses of various breeds with patellar fractures. PROCEDURE: Clinical signs of lameness, external evidence of injury, and radiography were used to diagnose and determine fracture orientation. Arthroscopy of the stifle joint was performed on the affected limb with the horse positioned in dorsal recumbency and under general anesthesia. Progress after surgery was determined by evaluating medical records and via telephone conversations with owners. RESULTS: 4 of 5 horses had fractures of the medial aspect of the patella and 1 horse had a fracture of the lateral aspect of the patella. There were no postoperative complications with the joint or the arthroscopic portal incisions. Recovery periods ranged from 3 to 5 months. All horses recovered completely from surgery, and performed at the same or higher level of competition as before arthroscopy. CONCLUSIONS AND CLINICAL RELEVANCE: Femoropatellar joint arthroscopy is a favorable means by which evaluation of the stifle joint and removal of large fracture fragments can be achieved with negligible postoperative complications.  相似文献

Injury to the oblique and straight distal sesamoidean ligaments is becoming recognized as a more common cause of lameness in horses than was previously thought. The purpose of this study was to review the magnetic resonance (MR) imaging findings of 27 horses affected with desmitis of the oblique and/or straight distal sesamoidean ligament and determine long-term prognosis for horses with this diagnosis. Imaging was performed with horses in right lateral recumbency in a high-field 1 T magnet. All horses had lameness localized to the digit or metacarpophalangeal/metatarsophalangeal joint region with diagnostic local anesthetic blocks. Ten horses had forelimb lameness and 17 horses had hind limb lameness. MR imaging revealed abnormalities in the oblique distal sesamoidean ligaments in 18 horses, in the straight distal sesamoidean ligament in three horses, and in both the oblique and straight distal sesamoidean ligament in six horses. Treatment consisted of a 6-month rest and rehabilitation program in all horses. The digital flexor tendon sheath was injected with methylprednisolone acetate and hyaluronic acid in 22 horses to decrease inflammation in the injured ligaments before starting the rest and rehabilitation program. Two horses had ligament splitting performed, one in the oblique distal sesamoidean ligament and one in the straight distal sesamoidean ligament. MR imaging is an effective method for diagnosing injury to the oblique and straight distal sesamoidean ligaments in horses. Treatment, primarily a 6-month rest and rehabilitation program, allowed 76% of the horses to successfully resume performance.  相似文献
