Objective: To characterize the clinical findings in dogs and cats that sustained blunt trauma and to compare clinical respiratory examination results with post‐traumatic thoracic radiography findings. Design: Retrospective clinical study. Setting: University small animal teaching hospital. Animals, interventions and measurements: Case records of 63 dogs and 96 cats presenting with a history of blunt trauma and thoracic radiographs between September 2001 and May 2003 were examined. Clinical signs of respiratory distress (respiratory rate (RR), pulmonary auscultation) and outcome were compared with radiographic signs of blunt trauma. Results: Forty‐nine percent of dogs and 63.5% of cats had radiographic signs attributed to thoracic trauma. Twenty‐two percent of dogs and 28% of cats had normal radiographs. Abnormal auscultation results were significantly associated with radiographic signs of thoracic trauma, radiography score and presence and degree of contusions. Seventy‐two percent of animals with no other injuries showed signs of thoracic trauma on chest radiographs. No correlation was found between the radiographic findings and outcome, whereas the trauma score at presentation was significantly associated with outcome and with signs of chest trauma but not with the radiography score. Conclusion: Thoracic trauma is encountered in many blunt trauma patients. The RR of animals with blunt trauma is not useful in predicting thoracic injury, whereas abnormal chest auscultation results are indicative of chest abnormalities. Thorough chest auscultation is, therefore, mandatory in all trauma animals and might help in the assessment of necessity of chest radiographs.  相似文献

Twenty-six cases of mandibular fractures and luxations are reviewed. Mandibular trauma was due to car accidents (46%) and falls from great heights (23%). Cardiovascular shocks (40%), thoracic injuries (23%), epistaxis (30%), cleft palates (19%), subconjunctival hemorrhages (15%), broken canines (15%), concussions (15%), maxillary fractures (12%) were the most frequently encountered associated problems. Of the 26 cats, 22 had symphyseal fractures, 6 mandibular body fractures, 4 ramus fractures (3 of which were articular) and 6 had temporo-mandibular luxations. Open reduction and internal fixation was performed in 21 cases, surgery being indicated for unstable symphyseal and body fractures, the rostral body fractures being the most challenging to repair. Conservative therapy and ancillary treatment methods such as external coaptation and pharyngostomy were used for the fractures of the ramus. Six temporo-mandibular luxations associated with mandibular fractures in 5 cases were reduced under anesthesia. Good dental occlusion and mandibular function was obtained in the 24 treated cases.  相似文献

High-rise syndrome was diagnosed in 119 cats over a 4-year period. 59.6% of cats were younger than one year, and the average height of the fall was four stories. High-rise syndrome was more frequent during the warmer period of the year. 96.5% of the presented cats, survived after the fall. 46.2% of cats had fractured limbs; 38.5% of fractures were of the forelimb, 61.5% of the hindlimb. The tibia was fractured most often (36.4%), followed by the femur (23.6%). 78.6% of femoral fractures were distal. The mean age of patients with femoral fractures was 9.1 months, and with tibial fractures 29.2 months. Thoracic trauma was diagnosed in 33.6% of cats. Pneumothorax was diagnosed in 20% of cats, and pulmonary contusions in 13.4%. Falls from the seventh or higher stories, are associated with more severe injuries and with a higher incidence of thoracic trauma.  相似文献

A retrospective study was made of 75 mandibular fractures in 62 cats. Mandibular fractures comprised 14.5% of all fractures seen in 517 cats. Automobile trauma was the cause of injury in more than 50% of the cases. The mean age of patients was 29.5 months. Symphyseal fractures were most common (73.3%), followed by fractures of the body (16%), condyle (6.7%), and coronoid process (4%). Sixty-seven percent of the fractures were stabilized. Cerclage and interfragmentary wiring were the most common forms of fixation. Antibiotics were administered to 73.6% of the patients. Complications were reported in 24.5% of the cats. Malocclusion and soft tissue infections were the most frequent complications. Complications developed more commonly in cats with multiple or open fractures. Clinical union occurred by an average of 6 weeks (range, 3-12 weeks) for symphyseal fractures, 10 weeks (range, 8-16 weeks) for body fractures, 6 weeks for coronoid fractures, and 6 weeks (range, 4-8 weeks) for condylar fractures.  相似文献

Records of all cats that had undergone exploratory coeliotomy at the University of Edinburgh during the period November 1995 to July 2002 were reviewed. Seventy records were retrieved. There were 30 cats in which infection or inflammatory disorders predominated, 17 cats with neoplasia, three cats with trauma and 20 cats with other disorders. Exploratory coeliotomy was performed for diagnostic purposes in 28 cats (40 per cent), treatment in 34 cats (49 per cent) and for diagnosis and treatment in eight cats (11 per cent). Methods of intraoperative diagnosis included incisional biopsy of abdominal organs (52 cats), cytology (two cats), microbiology (17 cats) and gross appearance (17 cats). Fifty-eight cats (83 per cent) survived the hospitalisation period. Complications occurred in 18 cats (26 per cent) and were related to anaesthesia (four cats), the underlying disease process (15 cats), surgery (five cats) and were undetermined in one cat.  相似文献

The records of 267 dogs seen at the University of Minnesota Veterinary Teaching Hospital for fractures resulting from motor vehicle accidents were examined to determine the prevalence and types of thoracic wall and pulmonary trauma associated with such cases. Results were analyzed for type and prevalence of thoracic wall and pulmonary injury, and for the prevalence of such injury in dogs with and without extrathoracic injury, in dogs with fractures of single vs multiple bones, in dogs with single fractures of specific bones, in dogs with fractures in the cranial vs the caudal one half of the body, and in dogs with fractures ipsilateral vs contralateral to thoracic injury. The overall prevalence of thoracic wall and pulmonary trauma was 38.9%; pulmonary contusions, pneumothorax, and fractured ribs were the most common injuries. More than 1 type of thoracic wall or pulmonary injury was diagnosed in 57.7% of the cases. Of the dogs with thoracic injury, 24% also had extrathoracic injuries; 16.5% of dogs without thoracic injury had extrathoracic injuries, not including fractures. Of the dogs with fractures of 1 bone, 36.3% had thoracic injuries. Of the dogs with fractures of more than 1 bone, 42.3% had thoracic injuries. The prevalence of thoracic wall and pulmonary trauma was significantly associated with the site of the fracture (cranial vs caudal and ipsilateral vs contralateral); significant association with the specific bone fractured was also seen for some fractures.  相似文献

In a report describing life ending fractures (255 horses) from the Livestock Disease Diagnostic Center, Kentucky (1993 and 1994), 32 foals had rib fractures. The purpose of our study was to examine the incidence of rib fractures in newborn foals on a Thoroughbred studfarm by physical and radiographic examination, to determine factors which may contribute to the problem and to document any clinical consequences. All foals (263) included were examined within 3 days of birth. The thoracic cage was palpated externally for abnormalities and all foals were placed in dorsal recumbency to evaluate thoracic cage symmetry. Radiographs were used to diagnose foals with thoracic cage asymmetry (TCA) and rib fracture (RF). A diagnosis of costochondral dislocation (CD) was made when no radiographic evidence of fracture was present but there was severe TCA. Fifty-five foals (20.1%) had TCA (9 RF). One to 5 ribs were fractured on 9 of 40 radiographic studies. No consequences of the thoracic trauma was detected clinically, radiographically or ultrasonographically in this group of foals or at a 2- and 4-week follow-up examination. The percentage of foals with a history of abnormal parturition was higher in the TCA foals (15%) compared to the normal foals (6.8%). There were more primiparous dams in the TCA group than in the normal foal group. Fillies (56.6%) had a higher incidence of birth trauma than colts (43.4%). This study demonstrates that thoracic trauma is often present in newborn foals and may not always be of clinical significance. Dystocia foals and foals from primiparous mares should be considered high risk for thoracic trauma.  相似文献

An exaggerated "horizontal" alignment of the heart (increased sternal contact) and tortuous, redundant aorta are often seen on thoracic radiographs of older cats. Eighty-five cats, divided into 3 age groups [young (6–15 months), middle aged (3–7 years), old (10–17 years)], were determined to be normal on history, auscultation, ECG, and echocardiogram. Lateral and VD thoracic radiographs were examined and measured for evidence of these changes. Forty percent of cats 10 years or older were found to have the horizontal cardiac alignment, and 28% had the redundant aorta. These changes were not seen in Groups 1 and 2 of younger and middle aged cats. The angle between the heart and sternum in Group 3 (old) cats was significantly smaller than the same angle in younger cats. Systemic hypertension and hyperthyroidism, possible pathologic causes for this variation, were unlikely related to the cardiovascular changes due to the large number of older cats with this change, and lack of evidence of left ventricular hypertrophy.  相似文献

REASONS FOR PERFORMING STUDY: Previous studies have shown that in man ultrasonography is more accurate than radiography for detecting rib fractures. OBJECTIVES: To describe clinical, radiographic and ultrasonographic findings related with rib fractures in newborn foals in an equine critical care unit; and to compare diagnostic accuracy of ultrasonography to radiography. METHODS: A prospective ultrasonographic study was performed on 29 foals presented to the emergency unit. This study was performed at the Centre Hospitalier Universitaire Vétérinaire (CHUV), University of Montreal. Physical examination as well as radiographic and ultrasonographic examinations were performed. RESULTS: Thoracic radiographs revealed 10 rib fractures in 5 of 26 (19%) foals. Ultrasonography revealed 49 fractures in 19 of 29 (65%) foals of which fillies (n = 13; 68%) were significantly over represented as were fractures to the left thorax (n = 15; 78%). Seventeen of 19 foals (90%) had rib fractures located 3 cm or less from the costochondral junction, the distal part of the rib being displaced laterally in all cases. In 2 foals, where both thoracic radiographs and ultrasonography detected rib fractures, the site of fractures was located on the mid portion of the rib. Rib fractures were detected only by thoracic radiographs in one foal. Sixty-five percent (32/49) of fractured ribs had a moderate displacement (1-4 mm). CONCLUSIONS: Rib fractures are seen frequently in newborn foals in equine critical care units. Ultrasonography is more accurate than radiography and reveals fractures in most patients presented in emergency. The position (costochondral junction) of rib fractures and of the fragments suggest that most thoracic trauma probably occurs during parturition. POTENTIAL RELEVANCE: Ultrasound imaging increases awareness and improves the diagnosis of rib fractures in newborn foals.  相似文献

Medical records and histologic sections of 40 cats with acute pancreatitis were reviewed. Two distinct groups of cats with pancreatitis were established by histologic analysis of tissue. Group I (32 cats) had acute pancreatic necrosis (APN). Group 2 (8 cats) had suppurative pancreatitis. Ages of affected cats ranged from 3 weeks to 16 years. l h e majority consisted of indoor cats of the Domestic Short-Haired breed but Siamese cats were over-represented relative to the general population ( P > 0.05). Twenty-two percent of cats were obese and 57%were underweight. Thirty-eight percent of cats had acute disease. In the other cats, two stages in the progression of the disease were evident: (1) anorexia. weight loss, and lethargy, followed by (2) acute deterioration, development of shock, and a moribund state, despite fluid therapy. The most common clinical signs were severe lethargy (100%), reduced appetite (97%), dehydration (92%), and hypothermia (68%). The initial hemogram occasionally showed a neutrophilia (30%) and anemia (2670)but packed cell volume (PCV) decreased markedly to the extent that 555% of cats were anemic terminally. Serum biochemical abnormalities included increased activities of A1, T (68%) and ALP (50%). and increased conrentrations of bilirubin (64%) and cholesterol (64%).Cats with APN were hyperglycemic (64%)), glycosuric (60%) and ketonuric (20%), whereas cats with suppurative pancreatitis tended to be hypoglycemic (7%). Renal failure and electrolyte abnormalities were mild or infrequent escept for hypokalemia (56%)).This study characterizes a severe necrotizing pancreatitis in the cat similar to that reported in other species, and a histologically distinct suppurative pancreatitis.  相似文献

Computed tomography (CT) and thoracic radiography were performed in nonsedated, nonanesthetized, cats with thoracic disease. The final diagnosis was obtained with echocardiography, cytology, histopathology, necropsy, or response to therapy. For CT imaging, cats were in a positioning device using a 16 multislice helical CT system. Fifty‐four cats had CT imaging of which 50 had thoracic radiography. The most common diagnoses were lung neoplasia, lower airway disease, and cardiomyopathy (nine each). Other disease groups included mediastinal mass (eight), infection (seven), trauma (four), and hernia (three). CT provided additional correct diagnoses in 28% (14/50) and additional information in 74% (37/50) of the cats. Additional correct diagnoses achieved only with CT were most common for cats with lower airway disease. The most common additional findings with CT were lung nodules (n=4), lung masses (n=4), bronchiectasis (n=4), and mediastinal lymphadenopathy (n=3). Survey CT led to a significant different diagnosis or different prognosis in 20 of the 50 cats that were imaged both modalities. Contrast CT was performed in 19 cats, most commonly in cats with lung neoplasia (n=6), a mediastinal mass (n=4) or an infection (n=3), and provided additional correct diagnosis in two cats not achieved with survey CT. Thoracic CT using a positioning device in diseased awake cats is feasible, safe, and clinically useful.  相似文献

Objective – To characterize the clinical features and population differences among cats sustaining traumatic and nontraumatic rib fractures. Design – Retrospective clinical study. Setting – University small animal hospital. Animals – Thirty‐three cats with radiographic evidence of rib fractures. Interventions – None. Measurements and Main Results – Cats with rib fractures were identified by performing a computer search of the radiology database. Thirty‐three cats that sustained rib fractures were identified between January 2000 and September 2009. Seventeen cats had fractures due to trauma and 16 were deemed to occur from nontraumatic causes. A Mann‐Whitney rank‐sum test revealed statistically significant differences in the median ages between the 2 groups. Older cats were more likely to sustain rib fractures as a result of a presumed nontraumatic causes. A Chi‐square analysis showed that nontraumatic fractures occurred significantly more often in the midbody region and involved the 9th–13th ribs. The majority of cats with presumed nontraumatic rib fracture had respiratory disease; the remaining cats had chronic renal disease or neoplasia. Cats with traumatic rib fractures had external signs of trauma. Conclusion – Rib fractures in cats may be clearly associated with trauma, or may be an incidental finding in cats with comorbidities. Cats with diseases that cause prolonged respiratory effort or coughing, metabolic diseases, or certain neoplasms, are at increased risk of spontaneous nontraumatic rib fractures.  相似文献

Hospital records and radiographs of 211 dogs and cats with vertebral column fractures or luxations evaluated at the University of Tennessee Veterinary Teaching Hospital between April 1977 and September 1985 were reviewed. After neurologic examination, status of the animal was graded on a scale of 1 to 8. Decision to treat each animal either medically or surgically was based on the extent and type of injury, neurologic signs, veterinarian's experience, and owner's wishes. After treatment, neurologic status was evaluated on the aforementioned scale and differences in the outcome of treatment were determined between surgically and medically treated groups, relative to initial neurologic status and location of the fracture. Surgically treated animals had pretreatment mean (+/- SD) neurologic status (3.71 +/- 1.35) that was slightly worse (P = 0.0079) than that of medically managed cases (5.16 +/- 1.48). Animals of the surgically treated group improved significantly (P = 0.0122) more than did those of the medically treated group but after treatment, significant differences in neurologic status were not evident between surgically (6.67 +/- 1.49) and medically (7.07 +/- 1.24) treated animals. Medically treated animals required substantially longer to reach optimal neurologic status, but the average hospital stay was nearly twice as long for the surgically treated animals (13.5 days), compared with those treated medically (7.1 days). Animals with thoracic vertebral fractures had mean neurologic status that was worse than that in animals with cervical vertebral fractures (P = 0.0109). A retrospective evaluation was performed on 46 cats with arterial thromboembolism (ATE) that were treated with streptokinase (SK). Significant heart disease was diagnosed in 45/46 cats, and 21/46 cats had congestive heart failure. Variable dosing schemes of streptokinase were administered within 1–20 hours following the onset of clinical signs (median = 5.5 hours). There was no difference between survivors (S) and non-survivors (NS), based on time of administration of SK after onset of clinical signs. Twenty-five (54%) of the cats had return of pulses within 2–24 hours of treatment. Fourteen (30%) of the cats had return of motor function between 9 hours and 6 days. Fifteen of the cats (33%) were discharged from the hospital, 18 (39%) died in the hospital, and 13 (28%) cats were euthanized due to complications or poor response to treatment. Four of 5 cats (80%) with single limb dysfunction survived to hospital discharge. Life threatening hyperkalemia was diagnosed in 16 cats (35%) after SK administration. Hyperkalemia was more likely to occur with the longer duration of SK infusion. Eleven cats (24%) developed clinical signs of bleeding following SK administration and 3 of these cats required a blood transfusion. Laboratory testing documented coagulopathy following SK administration in 11 out of 17 cats tested. Hypothermia and azotemia prior to SK administration and the development of hyperkalemia were negatively associated with survival.  相似文献

OBJECTIVE: To describe clinical signs and results of treatment in cats with patellar luxation. DESIGN: Retrospective case series. ANIMALS: 42 cats in which patellar luxation had been diagnosed on the basis of results of palpation of the stifle joints. PROCEDURES: Degree of luxation was graded on a scale from 1 to 4, and severity of lameness was graded on a scale from 0 to 5. Radiographs of stifle joints were evaluated for signs of osteoarthritis. Long-term function was classified as poor, fair, good, or excellent. RESULTS: 34 cats had bilateral luxation and 8 had unilateral luxation. Only 7 (17%) cats had a history of trauma. Mean age of the cats was 3.3 years, and mean weight was 4.26 kg (9.4 lb); 26 (62%) were domestic shorthairs. Seventy-three of the 76 (95%) affected joints had medial patellar luxation. Luxation grades could be assigned to 65 joints, with grade 2 (30 joints) and 3 (22 joints) luxation being most common. Lameness grades could be assigned to 73 joints, with grade 1 lameness (27 joints) most common. Outcome was excellent for 8 of 17 joints treated without surgery and for 23 of 35 joints treated surgically. Complications attributable to surgery were reported in 8 cats. CONCLUSIONS AND CLINICAL RELEVANCE: Patellar luxation should be considered as a cause of hind limb lameness in cats. Low-grade luxation can be associated with lameness of the same severity as high-grade luxation. Surgical correction of patellar luxation in cats with grade 2 or 3 lameness can result in a favorable outcome.  相似文献

In this retrospective study of 22 cats with lower airway disease of either intermittent (23%) or persistent nature (77%), the Siamese breed (55%) was significantly over-represented. Females (68%) were slightly but not significantly over-represented. No significant association was found between the clinical stage of disease and the physical findings, thoracic radiographic changes or the response to treatment. Cough, the most common presenting complaint, was the only symptom detected in the cats with intermittent disease, yet the two most severely affected animals did not show it. Thoracic auscultation did not reveal any abnormality in 41% of the cats. Haematology revealed eosinophilia in 46% of the cats. A bronchial pattern was the most common radiographic abnormality (73%), followed by alveolar (32%) and interstitial patterns (23%). Interestingly, thoracic radiographs were normal in 23% of the cats. The combination of short-term corticosteroids and bronchodilators resulted in complete and long-term remission of symptoms in nine cats, while the other 11 required ongoing medication because of relatively frequent relapses. One of the remaining two cats died during an asthmatic crisis, while the other was lost to follow up.  相似文献

OBJECTIVE: To evaluate factors associated with response to treatment, remission duration, and survival in cats with low-grade lymphoma affecting various organ systems. DESIGN: Retrospective case series. Sample POPULATION: 41 cats with histologically confirmed low-grade lymphocytic lymphoma. PROCEDURES: Medical records and biopsy specimens of cats with histologically confirmed low-grade lymphocytic lymphoma of various organ systems treated with prednisone and chlorambucil between 1995 and 2005 were reviewed. The Kaplan-Meier method was used to estimate remission duration and survival. Factors potentially associated with prognosis were compared. RESULTS: Common clinical signs were weight loss (83%), vomiting (73%), anorexia (66%), and diarrhea (58%). Seventy-eight percent of cats tested had low serum cobalamin concentrations. Lymphoma was confined to the gastrointestinal tract in 68% of cats. Fifty-six percent of cats achieved a complete response to treatment, and 39% achieved a partial response. Five percent of cats had no response. No association was found between any risk factors (including anatomic site) and response to treatment. Partial response was associated with shorter remission duration, compared with complete response; median remission duration was 428 days for cats achieving a partial response, compared with 897 days for cats achieving a complete response. No other factors were associated with remission duration. Overall median survival time was 704 days. No factors were significantly associated with survival time. CONCLUSIONS AND CLINICAL RELEVANCE: Most cats with lymphocytic lymphoma responded to treatment with prednisone and chlorambucil, and most factors evaluated were not associated with outcome.  相似文献

Common historical findings in 26 cats with pyothorax were dyspnea (85%), lethargy (65%), and anorexia (62%), common clinical findings were dyspnea (89%), tachypnea (73%), fever (39%), and hypothermia (27%). Frequent laboratory abnormalities were leukocytosis (68%) with a left shift (100%), anemia (65%), hypoalbuminemia (91%), hyperglobulinemia (86%), hyperbilirubinemia (60%) as well as azotemia (52%). The thoracic effusion was purulent in all cats, the thoracic fluid evaluation (n = 19) met the criteria for an inflammatory exudate (protein 32-63 g/l, median 44; cell count 54.4-390 x 10(9)/l, median 100). Cytological analysis revealed bacteria in 21 of 24 cats. From 16 cats 13 different genera were isolated, 5 cultures were without bacterial growth. In 12.5% of the cats a single population of bacteria was identified, and in 87.5% a mixture of 2-4 different bacterial species were cultured. Obligate anaerobic bacteria were the most common isolates (70%) followed by facultative anaerob (22.5%) and aerob growing (7.5%) bacteria. Bacterial culture most often yielded Fusobacteria spp., Prevotella spp., Pasteurella spp., Porphyromonas spp. and Bacteroides spp. Three cats were euthanized at the day of presentation. A thoracic lavage was performed in 22 cats; mechanical complications with the chest tubes occured in 4 cats. Thoracotomy was performed in 2 cats. Of the 23 treated cats 17 survived (74%).  相似文献

Objective— To describe a simple technique of intramedullary (IM) pinning for open reduction and internal fixation of metacarpal and metatarsal fractures in cats and evaluate outcome.
Study Design— Retrospective study.
Animals— Cats (n=17).
Methods— Medical records of cats with metacarpal and metatarsal fractures treated by a specific IM pinning method with Kirschner wires were reviewed. Types of fractures and number of pins used were recorded. Pre- and postoperative radiographs were evaluated. Outcome was assessed based on clinical examination, evaluation of radiographs, and owner questionnaire.
Results— Seventeen questionnaires were evaluated. Thirteen cats had a final recheck and radiographic assessment. Eight cats were lost to follow-up. Sixteen owners (94%) reported no obvious change in their cat's gait after treatment. One owner reported excellent function in her cat despite a slightly abnormal gait because of bent implants.
Conclusions— IM pinning after distraction of bone fragments is a reliable technique for treatment of metacarpal and metatarsal fractures in cats. Complications associated with fracture healing may or may not be associated with lameness.
Clinical Relevance— IM pinning used in conjunction with appropriate bandaging for 4 weeks yields excellent functional outcome for treatment of metacarpal and metatarsal fractures in cats. Metacarpo- and metatarsophalangeal joint damage is prevented using this technique.  相似文献

The medical records of 74 dogs and 26 cats with Horner's syndrome (HS) that were admitted to the New York State College of Veterinary Medicine between January 1975 and October 1985 were reviewed. In dogs, but not cats, HS was associated significantly (P less than 0.01) with increasing age. Dogs with hypothyroidism (defined liberally but not rigorously), intracranial neoplasia, or thoracic neoplasia, cats with otitis media/interna (defined liberally), and dogs and cats with brachial plexus root avulsion were at greater risk for developing HS than were animals that were hit by a car. Dogs and cats with otitis externa were at less risk of developing HS than were animals that were hit by a car. The cause of HS could not be determined in 50% of dogs and 42.3% of cats. The results of topical adrenergic drug testing in dogs were inconclusive in localizing lesion site. In dogs and cats, HS appeared to be unassociated with gender, breed, or right vs left side. The important causes of HS in dogs and cats were trauma (hit by car), brachial plexus root avulsion, intracranial and thoracic neoplasia, and otitis media/interna.  相似文献
