Pneumothorax (45 ml of N/kg of body weight insufflated into the pleural space) in anesthetized dogs ventilated with air caused a significant (P less than 0.05) increase in pleural pressure, central venous pressure, capillary wedge pressure, and venous admixture. Cardiac index (CI) and arterial O2 tensions were decreased. Ventilation with 100% O2 increased arterial O2 tensions, but did not affect calculated intrapulmonary shunting of blood or CI. Application of 10 cm of H2O-positive end-expiratory pressure in the presence of pneumothorax during positive-pressure ventilation and high-frequency jet ventilation reduced intrapulmonary shunting of blood, which remained higher than control values, and caused a further decrease in CI. Cardiopulmonary function during pneumothorax in anesthetized dogs was more profoundly affected by the application of positive end-expiratory pressure than by the form of mechanical ventilation.  相似文献

Laser surgery and laparoscopy are two relatively new surgical techniques gaining popularity in veterinary medicine, which require special consideration when being performed on the anesthetized patient. For laser surgery, consideration must be given to the possibility of atmospheric contamination, inappropriate energy transfer, eye injury, perforation of a vessel or anatomic structure, perforation of the endotracheal tube, and fire. The primary concern with laparoscopy and thoracoscopy is the creation of a pneumoperitoneum or pneumothorax, which can result in (1) hypercarbia and inadequate ventilation, (2) poor cardiac output and systemic blood pressure, and (3) gas embolism. To minimize complications, patients should be placed on positive pressure ventilation, be well hydrated before and during the procedure, and be thoroughly monitored (ECG, capnography, pulse oximetry.  相似文献

ObjectiveCardiovascular responses to hypovolemia and hypotension are depressed during general anesthesia. A considerable number of anesthetized and critically ill animals may not benefit hemodynamically from a fluid bolus; therefore, it is important to have measures for accurate prediction of fluid responsiveness. Static measures of preload, such as central venous pressure, do not provide accurate prediction of fluid responsiveness, whereas dynamic measures of cardiovascular function, obtained during positive pressure ventilation, are highly predictive. This review describes key physiological concepts behind heart–lung interactions during positive pressure ventilation, factors that can modify this relationship and provides the basis for a rational interpretation of the information obtained from dynamic measurements, with a focus on pulse pressure variation (PPV).Database usedPubMed. Search items used were: heart–lung interaction, positive pressure ventilation, pulse pressure variation, dynamic index of fluid therapy, goal-directed hemodynamic therapy, dogs, cats, pigs, horses and rabbits.ConclusionsThe veterinary literature suggests that targeting specific PPV thresholds should guide fluid therapy in lieu of conventional assessments. Understanding the physiology of heart–lung interactions during intermittent positive pressure ventilation provides a rational basis for interpreting the literature on dynamic indices of fluid responsiveness, including PPV. Clinical trials are needed to evaluate whether goal-directed fluid therapy based on PPV results in improved outcomes in veterinary patient populations.  相似文献

OBJECTIVE: To describe an unusual response to evacuation of a chronic pneumothorax in a dog. CASE: Resolution of a pneumothorax is usually associated with lung expansion and a marked improvement in pulmonary function. In the case presented here, evacuation of a chronic pneumothorax improved oxygenation, but markedly impaired ventilation. CLINICAL RELEVANCE: This is a previously unreported complication associated with the treatment of pneumothorax in dogs. It suggests that in some cases of chronic pneumothorax, total evacuation of the pleural air pocket may improve oxygenation but ventilation deteriorates and increases PCO2, causing further complications.  相似文献

OBJECTIVE: Evaluation of the LMA-ProSeal for positive pressure ventilation (PPV) in the pig. STUDY DESIGN: Prospective observational study. ANIMALS: Twelve German country pigs, weighing 25-62 kg. METHOD: Lungs of pigs were mechanically ventilated under general anaesthesia using the LMA-ProSeal. The ease of insertion, number of attempts and total time until placement of the LMA-ProSeal and gastric tube were recorded. Bronchoscopy was performed to determine the position of the LMA-ProSeal and to detect signs of aspiration. Ventilation variables and the leak airway pressure (P(leak)) were measured. An arterial blood gas sample was taken to determine the adequacy of ventilation. RESULTS: The airway was secured in all pigs within 39 +/- 19 seconds (27-51). Different sizes of LMA-ProSeal were used; up to 30 kg: size 3, up to 43 kg: size 4; and above 43 kg: size 5. In all but one animal the P-LMA and gastric tube were inserted at the first attempt. In nine animals gastric fluid was drained through the gastric tube. There was no evidence of aspiration in any animal. The mean [+/-SD (95%CI)]P(leak) was 28.8 +/- 7.5 cm H(2)O (24.06-33.60) and normal ventilation was achieved in all animals. CONCLUSIONS: The results of this study indicate that the airway of pigs weighing 25-62 kg can be secured safely and reliably with the sizes 3, 4 and 5 LMA-ProSeal. CLINICAL RELEVANCE: Endotracheal intubation in pigs can be difficult so there is a risk of hypoxemia in the apnoeic animal. With the LMA-ProSeal the airway can be secured rapidly, safely and reliably. Use of the Standard-LMA under PPV can be associated with gas leakage into the stomach and the subsequent risk of gastric distension and regurgitation. Both the ability to drain the stomach and the high P(leak) of the LMA-ProSeal could contribute to improved protection against aspiration under PPV.  相似文献

Objective: This paper characterizes the clinical findings in 5 cats with feline asthma complicated by concurrent pneumothorax. Design: Retrospective study. Medical records of cats with concurrent diagnoses of asthma and pneumothorax that were presented to the Veterinary Hospital of the University of Pennsylvania from 1990 to 2000 were reviewed. Results: Of 421 cases of feline asthma, 5 cats fulfilled the inclusion criteria (1.2%). All 5 had respiratory distress at presentation. One cat was panting, and the other 4 cats had respiratory rates of 28, 52, 58 and 120 breaths per minute (bpm), respectively (mean RR 65±39 bpm). Historical findings included untreated chronic cough (n=3), previously treated asthma (n=1), and no previous illness (n=1). Thoracocentesis was performed in 4/5 cats, and 3 of those cats required thoracostomy tubes. Four cats required immediate oxygen supplementation, and 1 of those cats required ventilation. All 5 cats had evidence of pneumothorax on initial radiographs. Follow‐up radiographs revealed partial or complete resolution of pneumothorax in 4 cats which were discharged alive with total hospitalization of 2–7 days, but were then lost to follow‐up. One cat was euthanized because it could not be weaned off mechanical ventilation, and necropsy confirmed end‐stage feline asthma and emphysema. Conclusion: Small airway obstruction can predispose asthma patients to increased alveolar pressure, emphysema, and spontaneous pneumothorax, which can lead to dyspnea in affected cats. The short‐term outcome in these cats was good despite the severity of dyspnea at presentation.  相似文献

A total of 417 pigs with an estimated average liveweight of 91 kg were transported to slaughter in three journeys, carried out in late June, each consisting of a three-hour drive followed by a one-hour holding period at the slaughter plant before they were unloaded. The pigs were accommodated on the lower two decks of a three-deck articulated lorry, each deck having five equal-sized pens; half were kept in naturally ventilated pens and half in pens with fan-assisted ventilation. The pigs' body temperature was recorded as the temperature of the blood lost at exsanguination, and as the temperature of the inner surface of the ear measured by thermal imaging. The pigs carried on the upper deck were hotter than those carried on the lower deck, and the pigs carried at the front of the vehicle were hotter than those at the back. There were small differences between the temperatures of the pigs in the different pens, but overall the temperatures of the pigs kept in pens with fan-assisted ventilation were no lower than those of the pigs kept in pens with natural ventilation. The higher temperature of the pigs transported in the front of the vehicle was associated with significantly higher serum concentrations of cortisol and activities of creatine kinase, and a tendency to higher albumin concentrations and osmolality, suggesting that they may have been exposed to more physical and psychological stress, and were possibly slightly more dehydrated, than the pigs in the rearmost pens. The type of ventilation had no effect on the blood composition of the pigs.  相似文献

A mare with a thoracic laceration was anesthetized with halothane-O2 for surgical repair. Shortly after initiation of controlled ventilation, inspiratory plateau pressure began to increase. The increase in pressure continued despite decreasing the tidal volume. Tension pneumothorax was suspected, but because arterial PCO2 and PO2 were normal, the anesthetic apparatus was examined. Excess gas continued to exit through the bellows pop-off valve even when the fresh gas flow was turned off. It was determined that driving gas from the ventilator was entering the breathing circuit through a hole in the bellows. Testing the integrity of the bellows before use is suggested to avoid this potentially harmful complication.  相似文献

High-frequency ventilation (HFV) is a form of artificial ventilation that uses higher rates and smaller tidal volumes than those used with conventional mechanical ventilation (intermittent positive pressure ventilation [1PPV]). HFV is divided into three categories based on the rate and type of equipment used: 1) high-frequency positive pressure ventilation administered through a system with low internal compliance having an expiratory valve to maintain positive airway pressure during expiration and rates of 60 to 120/minute; 2) high-frequency jet ventilation delivered through an open system or a system with an expiratory valve at rates of 120 to 400/minute; and 3) high-frequency oscillation using open systems and rates of 400 to 2400/minute. All forms of HFV provide adequate ventilation in normal animals at lower peak and mean airway pressures, resulting in less cardiovascular depression than that associated with 1PPV. HFV provides adequate ventilation with less cardiovascular depression and risk of barotrauma than 1PPV in the presence of pulmonary disease. The exact mechanism of gas exchange during HFV is not clearly understood, but enhanced diffusion as a result of turbulent gas flow appears to be a major factor.  相似文献

Airborne transmission of Actinobacillus pleuropneumoniae was studied as the percentage of air needed to establish airborne transmission from an infected pig unit into a neighbouring non-infected pig unit. The experiment was carried out in two containers constructed as pig units, placed 1m apart and connected by pipes. By manipulating the air pressure in the two units, the amount of ventilation air transferred from the infected pigs (unit A) to the non-infected pigs (unit B) was controlled and measured. In three experiments, between 48 and 50 specific pathogen free-pigs were randomly assigned to each of the two units. In unit A, five pigs (experiment 1) or eight pigs (experiments 2 and 3) were inoculated with A. pleuropneumoniae serotype 2. In experiments 1 and 3, 10% of the air was transferred from unit A to B; in experiment 2, 70% of the air was transferred. In the non-infected unit (B), 36% of the pigs seroconverted during experiment 2 (70% air transfer), whereas none of the pigs seroconverted in experiments 1 and 3 (10% air transfer). As air transmission between closely located pig units has been estimated to be less than 2% under field conditions, these results indicate that airborne transmission of A. pleuropneumoniae serotype 2 between closely located pig units is rare.  相似文献

A series of three experiments, differing primarily in airflow volume, were performed to evaluate the likelihood of airborne transmission of Porcine Reproductive and Respiratory Syndrome Virus (PRRSV) from infected to non-infected pigs. Pigs were housed in two units (unit A and unit B) located 1m apart and connected by pipes. The air pressure and diameter of the pipes, depending on experiments, were strictly controlled to allow desired airflow volumes from unit A to unit B. Either 25 (experiment 1 and experiment 3) or 26 (experiment 2) pigs infected recently with PRRSV, and either 25 (experiment 1 and experiment 3) or 17 (experiment 2) pigs from a PRRSV-free herd, were housed in unit A. Either 50 pigs (experiment 1 and experiment 3) or 43 pigs (experiment 2) from a PRRSV-free herd were housed in unit B. The amount of air transmitted from unit A to unit B, expressed as a percentage of ventilation intake, was approximately 70, 10, and 1% for experiment 1, experiment 2 and experiment 3, respectively. Blood samples were collected from all pigs once per week and analyzed for antibodies against PRRSV. Based on these methods, airborne transmission of PRRSV from infected to non-infected pigs was confirmed in each of the three experiments.  相似文献

Acute respiratory distress syndrome (ARDS) was diagnosed in 2 dogs with acute dyspnea. Short-term positive pressure ventilation and intense critical and nursing care were provided. Both dogs improved and were discharged. Few reports describe successful recovery from ARDS. Due to advances in positive pressure ventilation and improvement in the supportive care of critically ill veterinary patients, the prognosis for ARDS may improve.  相似文献

The purpose of this study was to investigate the cardiopulmonary influences of sevoflurane in oxygen at two anaesthetic concentrations (1.5 and 2 MAC) during spontaneous and controlled ventilation in dogs. After premedication with fentany-droperidol (5 microg/kg and 0.25 mg/kg intramuscularly) and induction with propofol (6 mg/kg intravenously) six dogs were anaesthetized for 3 h. Three types of ventilation were compared: spontaneous ventilation (SpV), intermittent positive pressure ventilation (IPPV), and positive end expiratory pressure ventilation (PEEP, 5 cm H2O). Heart rate, haemoglobin oxygen saturation, arterial blood pressures, right atrial and pulmonary arterial pressures, pulmonary capillary wedge pressure and cardiac output were measured. End tidal CO2%, inspiratory oxygen fraction, respiration rate and tidal volume were recorded using a multi-gas analyser and a respirometer. Acid-base and blood gas analyses were performed. Cardiac index, stroke volume, stroke index, systemic and pulmonary vascular resistance, left and right ventricular stroke work index were calculated. Increasing the MAC value during sevoflurane anaesthesia with spontaneous ventilation induced a marked cardiopulmonary depression; on the other hand, heart rate increased significantly, but the increases were not clinically relevant. The influences of artificial respiration on cardiopulmonary parameters during 1.5 MAC sevoflurane anaesthesia were minimal. In contrast, PEEP ventilation during 2 MAC concentration had more pronounced negative influences, especially on right cardiac parameters. In conclusion, at 1.5 MAC, a surgical anaesthesia level, sevoflurane can be used safely in healthy dogs during spontaneous and controlled ventilation (IPPV and PEEP of 5 cm H2O).  相似文献

The tail length (docked, tipped or undocked) and tail status (bitten or unbitten) of 27,870 pigs from 450 units was recorded at six UK abattoirs. A farm survey of the final finishing stage was used to investigate the relationship between management practice and tail biting. This showed that docking was the most important factor influencing the probability of being not bitten, with 2.4% of docked and 8.5% of long-tailed pigs being tail-bitten. The following factors reduced the probability of long-tailed pigs being tail-bitten; light straw provision, use of natural ventilation or artificially controlled natural ventilation (ACNV), mixed sex grouping, meal or liquid feeding, and use of double or multi-space feeders. Docked and long-tailed pigs provided with light straw and natural ventilation/ACNV had levels of tail biting of 1.2% and 4.3% respectively; 3.9% of docked pigs with artificial ventilation and no straw were tail-bitten. Long-tailed pigs fed via double or multi-space feeders also had 3.9% of tails bitten.  相似文献

The effects of ketanserin on pulmonary hemodynamics, lung mechanics, and gas exchange were determined in anesthetized 10- to 14-week-old pigs after they were endotoxemic for 1 or 4.5 hours. Saline solution was given to controls (group 1). Escherichia coli endotoxin (055-B5) was infused IV at a dosage of 5 micrograms/kg for 1 hour (group 2). In group 3, endotoxin was infused at 5 micrograms/kg the first hour plus a continuous infusion of endotoxin at 2 micrograms/kg/hr. Ketanserin, a specific serotonin receptor antagonist, was infused IV (300 micrograms/kg) after pigs were endotoxemic for 1 or 4.5 hours (groups 2 and 3, respectively). At 1 hour of endotoxemia, mean pulmonary artery pressure and pulmonary vascular resistance were increased, and cardiac index was decreased. Ketanserin caused a small attenuation of the increases in mean pulmonary artery pressure and pulmonary vascular resistance, indicating that serotonin may have a small role in the endotoxin response at 1 hour. At 4.5 hours of endotoxemia, mean pulmonary artery pressure, pulmonary vascular resistance, alveolar dead space ventilation, and alveolar-arterial oxygen gradient were increased, and cardiac index and lung dynamic compliance were decreased; ketanserin significantly attenuated the endotoxin-induced changes in cardiac index, mean pulmonary artery pressure, pulmonary vascular resistance, and lung dynamic compliance. Ketanserin also decreased the blood temperature after pigs were endotoxemic for 4.5 hours. Five hysterectomy-derived colostrum-deprived pigs housed in individual cages with positive ventilation developed severe skin cyanosis and edema of the subcutaneous tissues in the submandibular, thoracic, and abdominal regions. The 5 pigs were killed 7 to 10 days after birth. Eosinophilic intranuclear inclusion bodies were found in endothelial cells of capillary and small blood vessels throughout the body. Ultrastructurally, nuclei of these affected endothelial cells contained small crystalline arrays of virus particles, which were considered to belong to the adenovirus-like group from their size and structure. The present results indicated that the porcine adenovirus-like agent might also have the ability to produce transplacental infection.  相似文献

Epidemiological aspects of Mycoplasma hyopneumoniae (Mh), influenza H1N1 and H3N2 viruses, and Aujeszky's disease virus (ADV) were investigated in slaughter pigs from 50 fattening pig herds. Herd factors as potential risk indicators for respiratory disease were obtained by means of a questionnaire. At slaughter, blood samples were collected from each herd, and the proportion of seropositive pigs per herd was assessed for each of these pathogens. The median herd-level seroprevalence of the agents were: Mh 88%, H1N1 100%, H3N2 60% and ADV 90%. The percentage of herds in which all investigated fattening pigs were seronegative for these agents was: Mh 0%, H1N1 0%, H3N2 12% and ADV 18%. The percentage of herds in which all investigated fattening pigs were seropositive for these agents was: Mh 8%, H1N1 71%, H3N2 22% and ADV 40%. A positive association was found between influenza H1N1 and H3N2 viruses, and a negative association between influenza H3N2 virus and ADV. There were no risk indicators for the seroprevalence of Mh. Three risk indicators were associated with the seroprevalence of influenza H1N1 virus: a fully slatted floor, an increasing number of pigs in the municipality and dry feeding. Three risk indicators were found for the seroprevalence of influenza H3N2 virus: purchase of pigs from > or = two herds, an increasing number of pigs in the municipality and natural ventilation. The seroprevalence of ADV was influenced by two risk indicators: an increasing number of pig herds in the municipality and an increasing number of pigs per pen.  相似文献

The influence of normal and exaggerated ventilation on the QRS was studied in 10 dogs. Peak-to-peak amplitudes of QRS in 4 circumferential unipolar and 2 limb lead ECG were obtained during eupnea and during hyperinflation produced by intermittent positive pressure ventilation or negative extrathoracic pressure. Marked changes were not observed in any lead during eupnea. During hyperinflation to a transpulmonary pressure of 10 mm of Hg (whether by intermittent positive pressure ventilation or negative extrathoracic pressure), voltages decreased in leads I, V10, rV3, and aVF and were variable in leads V3 and V5. Radiography revealed that during the hyperinflation the heart was displaced to the nondependent side of the thorax further from the dependent electrodes and that the heart became generally smaller. Changes in voltage could be attributed to alterations in relationships between the heart and electrodes on the torso surface or to changes in volume conductor properties of the heart and torso.  相似文献

目的比较两种免疫酶试剂盒recomWell HEV IgG(swine)ELISA(recom Well )和recomLine HEV IgG(swine)(recom Line )检测猪抗E型肝炎病毒(hepatitis E virus,HEV)IgG的准确性。方法分别采用基因3型和基因4型人HEV(human HEV,hHEV)人工感染无菌猪各2头,采用上述试剂盒检测猪感染后不同天数的血清样品中抗HEV IgG,同时对两头接种PBS液的无菌猪血清样品18份和4头未感染猪的18份血清进行了检测。结果 2头猪人工感染基因3型hHEV后,recomWell检测均于感染后21d出现阳性,并持续56d;recomLine检测,其中1头猪于感染后14d呈阳性,而另1头于感染后21d呈阳性,且均持续56d。2头猪人工感染基因4型hHEV后,两种试剂盒检测,其中1头猪于感染后21d呈阳性,另1头35d呈阳性,并均持续56d仍为阳性。18份对照血清两种试剂盒检测均为阴性。18份未人工感染猪血清中,只有1份两种试剂盒检测均为阳性。两种试剂盒同时检测的共72份样品中,recomWell检出阳性样品23份,阳性率约为32.0%(23/72),recomLine检出阳性样品24份,阳性率约为33.30%(24/72),检出阳性率两者差异不显著(P>0.05);在recomWell检出的23份阳性样品中,recomLine检测均为阳性,两者阳性检出符合率为100%(23/23);recomWell的漏检率约为4.1%(1/24)。结论 recomWellR和recomLine均可用于猪抗HEVIgG的检测,recomLine检测灵敏度略高于recomWell,且操作更简便,不需要特殊昂贵的检测设备,特别适合用于基层检测猪抗HEV IgG。  相似文献
