A 2-week-old Miniature Horse foal was referred for evaluation and treatment of a luxated right tarsometatarsal joint. Treatment consisted of closed reduction and internal fixation using two partially threaded Steinmann pins placed in normograde fashion through the tuber calcis into the proximal third metatarsus. Traumatic luxation has been reported to occur in the tarsocrural, proximal intertarsal and tarsometatarsal joints within the equine tarsus. Treatment for luxation of the distal intertarsal joint has not been documented. The treatment method most commonly suggested for tarsal luxation is closed reduction and cast immobilisation. Internal fixation using lag screws and plating has also been described. A combination of internal fixation and external coaptation is thought to achieve maximal stability and allow faster convalescence in cases of tarsal luxation. This case report describes for the first time a technique using two Steinmann pins to achieve successful internal fixation of a traumatic tarsometatarsal joint luxation in a 2-week-old Miniature Horse foal.

A 4-year-old castrated male Miniature Horse was evaluated because of severe right hind limb lameness of 5 days' duration. The diagnosis of craniodorsal luxation of the right coxofemoral joint was made by physical examination and radiographic imaging. Closed reduction was attempted but was unsuccessful. Surgical reduction was successfully performed, using toggle pin, synthetic capsular reconstruction, and trochanteric transposition techniques. No postoperative complications were observed. Follow-up 26 months after surgery revealed no recurrence of the luxation and no evidence of lameness. These surgical techniques are used successfully for repair of coxofemoral luxations in small animals. To our knowledge, there has been no report of these techniques attempted in horses. These surgical techniques may have merit for the treatment of coxofemoral luxations in small equine patients.

Shoulder luxation in a 3-year-old Thoroughbred filly was treated by closed reduction, using a calf jack to provide traction on the limb. Arthroscopic examination of the shoulder allowed removal of soft tissue and bony debris. After surgery, the filly was confined to a box stall for 6 months. Eight months after surgery, the filly was doing light work and was not lame. Muscular atrophy present before surgery had resolved. Use of a calf jack may allow reduction of shoulder luxation under field conditions.

This report describes the clinical, ultrasonographic and radiographic findings in an 8‐year‐old Icelandic mare with unilateral shoulder luxation after general anaesthesia. Reduction of the luxated humerus was performed in a closed fashion using a calving jack under general anaesthesia. The horse recovered well and ultimately returned to its previous level of performance.

A 19-year-old Thoroughbred gelding presented with sudden onset, non-weight bearing lameness in the right hindlimb. Radiography confirmed distal luxation of the patella, which was replaced into its normal anatomical location under general anaesthesia. There were no pathological sequelae noted on follow-up examination 9 months after the initial injury. To our knowledge, this is a rare manifestation of patellar luxation, only reported once previously in the equine literature.

This case report describes chronic sepsis of the right temporomandibular joint in a juvenile horse. Diagnostic work-up included physical examination, radiography, and computed tomography. Humane euthanasia was indicated due to the chronicity of the condition, prognosis, and financial constraints.

This paper reports on luxation of the elbow joint without concomitant fracture in a 1-month-old foal. Conservative treatment, with closed reduction and full-limb bandaging, including caudal and lateral splints, seemed successful initially, however, failed to provide enough stability and luxation recurred, and open reduction and surgical placement of prosthetic collateral ligaments was required. Luxation of the elbow joint should be considered when acute non-weight bearing forelimb lameness occurs associated with pain and swelling in the area of the elbow in young foals. Closed reduction failed to provide sufficient joint stability.

A 27-month-old Rocky Mountain Horse was examined because of a fracture of the proximal portion of the ulna and luxation of the humeroradial joint (Monteggia fracture). Open reduction was performed, using a mechanical distractor, and the ulnar fracture was stabilized by application of a bone plate and screws. After surgery, the horse developed an infection of the surgical site, and bacterial culture of fluid from the surgical site yielded a pure growth of methicillin-resistant Staphylococcus epidermidis susceptible to oxytetracycline, erythromycin, rifampin, and vancomycin. Treatment with oxytetracycline did not result in a favorable clinical response. Therefore, the horse was treated systemically with vancomycin and rifampin, and vancomycin-impregnated polymethyl methacrylate beads were implanted at the surgical site. Six months after surgery, the horse was sound at a walk or trot, and bony union was evident on radiographs of the elbow joint.

A 5‐year‐old Warmblood mare was presented to the clinic 8 h after being found by the owners with luxation of the second cervical vertebra. Clinically, the horse showed an extended posture and reduced movement of the head and neck. A defined lateral swelling of the neck at the level of the first cervical vertebrae was evident. Successful reduction was achieved with the horse under general anaesthesia in lateral recumbency using an electrically powered hand pallet truck. The mare showed no neurological disorders before or after reduction. Seven months after the incident the mare could be ridden and exhibited no functional abnormalities relating to the neck.

A method of assessment of cases of medial luxation of the patella for deciding the type of surgical intervention is described. The results of treatment of the cases after such an assessment indicate that, if there is no gross distortion and no deviation of the tibial tuberosity, simple lateral capsular overlap gives a satisfactory return of function, even in cases of irreducible patellar luxation.

Vacuum phenomenon was induced inadvertently during radiographic examination of a metatarsophalangeal joint of a lame horse. The phenomenon was recreated in a sound horse when a metacarpophalangeal joint was radiographed in a stress-flexed position. Distraction of apposing articular surfaces may induce the vacuum phenomenon, which could result in misdiagnosis of an osteochondral defect or fracture.

Arthroscopic treatment of temporomandibular joint sepsis in a horse
OBJECTIVE: To report arthroscopic debridement and lavage of a septic temporomandibular joint (TMJ) in a horse. DESIGN: Clinical case report. ANIMALS: A 12-year-old Thoroughbred mare. METHODS: Arthroscopic investigation of the dorsal joint pouch of the right TMJ was made through a stab incision into the dorsocaudal compartment. Mechanized resection of synovium and fibrinous debris combined with copious lavage, and intra-articular and systemic antibiotic administration was used to treat the septic TMJ. RESULTS: Arthroscopic debridement and lavage of the TMJ, in combination with intra-articular and systemic antimicrobials resulted in resolution of sepsis. Eight months post-operatively, there was no clinical evidence of degenerative joint disease or ankylosis of the TMJ. CONCLUSION: TMJ sepsis is rare in horses, however, standard arthroscopic equipment can be used in the management of this condition with minimal complications. CLINICAL RELEVANCE: Arthroscopic debridement and lavage should be considered for evaluation and initial treatment of TMJ sepsis in horses.
