OBJECTIVE: To examine the effects of juvenile pubic symphysiodesis (JPS) on hip joint conformation, hip laxity, gait, and the development of degenerative joint disease (DJD) in dysplastic puppies operated at 15 and 20 weeks of age. STUDY DESIGN: Randomized controlled prospective study. ANIMALS-Eighteen female hound puppies with increased hip laxity. METHODS: Puppies were randomized to 1 of 4 treatment groups: JPS at 15 weeks of age (n = 6), sham-operated control at 15 weeks of age (n = 3), JPS at 20 weeks of age (n = 6), and sham-operated control at 20 weeks of age (n = 3). Hip extension with pain scoring, Ortolani palpation, hip reduction angle measurement (HRA), PennHIP radiography (University of Pennsylvania) with measurement of distraction index, Norberg angle measurement, and transverse computed tomographic imaging to measure acetabular angle (AA) and dorsal acetabular rim angle (DARA), were tested preoperatively, and at 1 and 2 years of age. RESULTS: JPS resulted in significant changes in AA, HRA, DARA, and conversion to Ortolani negative status. Larger and more rapid changes in hip conformation were seen when surgery was performed at 15 weeks of age. No significant changes were identified in control dogs. Twenty-five percent of JPS dogs developed DJD whereas 83% of control dogs developed DJD. CONCLUSIONS: JPS resulted in significant improvements in hip joint conformation and hip laxity in dysplastic puppies treated at 15 and 20 weeks of age. Improvements in conformation were significantly greater when surgery was performed at 15 weeks of age. CLINICAL RELEVANCE: JPS appears to be a promising treatment for hip dysplasia and is a safe and technically simple procedure to perform.  相似文献

OBJECTIVES--To determine thermal necrosis zone and the maximal temperature achieved with various electrocautery doses applied to the pubic symphysis of immature dogs, and to establish the minimal electrocautery dose needed to affect thermal necrosis of the germinal chondrocytes of the immature canine pubis. STUDY DESIGN--A randomized, controlled, prospective study. ANIMALS--Twelve puppies, 15 to 17 weeks of age. METHODS--Each treated puppy was randomly selected to receive 3 of 33 possible doses of electrical current applied at uniform intervals along the pelvic symphysis. Three treatment sites were sham-operated controls. All treatment zones were biopsied 7 days after application of electrocautery. Confocal laser microscopy with calcein green and ethidium homodimer red stains were used to determine cell type and viability within treatment and control zones. The physeal diameter and thermal necrosis zone were measured. Thermal necrosis zone and dose, and temperature and dose were compared using regression analysis. Prediction equation analysis was used to establish dose recommendations. RESULTS--Doses of 40 W produced a highly significant linear relationship (R(2) =.88, P <.01) with thermal necrosis zone, and a significant linear relationship (R(2) =.76, P <.02) with maximal temperature. CONCLUSIONS--The thermal necrosis zone within the pelvic symphysis and maximal temperature achieved can be reliably predicted using electrocautery doses of 40 W. For application of electrocautery to induce pubic symphysiodesis, doses should be chosen based on measured or estimated physeal size. Using prediction equation analysis, the surgeon can plan mean thermal necrosis zone with acceptable variance. CLINICAL RELEVANCE--Unipolar needle electrocautery can be used to reliably achieve symphyseal necrosis. Prediction equation analysis can be used to determine 40-W doses.  相似文献

OBJECTIVES: To evaluate the efficacy of juvenile pubic symphysiodesis (JPS) in a clinical setting for the early treatment of canine hip dysplasia (CHD), and to identify its indications and contraindications. METHODS: The final degree of CHD using the FCI (Fédération Cynologique Internationale) CHD classification in 5 Grades (A, B, C, D, E) was assessed at skeletal maturity in two homogeneous groups of dogs assessed at the age of 14 to 22 weeks and selected according to their susceptibility to CHD; one group was treated with JPS and one group was conservatively managed. Two hundred seventeen puppies completed the study; 81 were treated with JPS (group 1) and 76 were conservatively managed (group 2). A third group of 60 puppies with normal hips was followed as a negative control group. RESULTS: In group 1, 43.2% of the puppies had regression or a lack of progression of the disease in the final evaluation (Grade A & B), 25.9% had mild CHD (Grade C) and 30.9% had moderate and severe CHD (Grade D & E). In group 2, 23.6% of the puppies did not show any development of the disease (Grade A & B), 21.1% had mild CHD (Grade C) and 55.3% developed moderate to severe CHD (Grade D & E). Further investigation was done by comparing the severity of early signs of susceptibility to CHD with the final FCI Grades at adulthood in both groups. CLINICAL SIGNIFICANCE: The JPS procedure increased the odds of arresting or limiting the progression of CHD in mild to moderate grades of CHD, while it was less effective or ineffective in more severe forms.  相似文献

OBJECTIVE: To compare long-term outcomes of juvenile pubic symphysiodesis (JPS) and triple pelvic osteotomy (TPO) in dogs with hip dysplasia. DESIGN: Prospective clinical trial. ANIMALS: 18 dogs with hip dysplasia (ie, distraction index > or = 0.5 in at least 1 hip joint and no, mild, or moderate radiographic evidence of degenerative joint disease [DJD]). PROCEDURES: Dogs between 4 and 5.5 months old at enrollment were assigned to undergo JPS, and dogs between 5 and 12 months old were assigned to undergo TPO. All dogs were reexamined at 2 years of age. RESULTS: At 2 years of age, there were no significant differences between groups in regard to lameness scores, angle of extension of the hip joints, distraction index, peak vertical force, acetabular angle, radiographic DJD score, or owner-assigned scores of clinical function. Dorsal acetabular rim angle was significantly higher in dogs that underwent JPS than in dogs that underwent TPO. For dogs that underwent TPO, dorsal acetabular rim angle was significantly decreased and acetabular angle was significantly increased at 2 years of age, compared with values obtained prior to surgery. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that JPS and TPO have similar effects on hip joint conformation in dogs with moderate to severe hip dysplasia but that neither procedure eliminates the hip joint laxity characteristic of hip dysplasia or the progression of degenerative changes.  相似文献

Eight out of a litter of 13 puppies were either born dead or died within 48 hours of birth. Three puppies that died shortly after birth were necropsied. Two puppies had hemorrhage in the thoracic and peritoneal cavities, intestinal serosa, and meninges. The third puppy was smaller than the other two puppies but did not have detectable hemorrhage. Brodifacoum, a second-generation coumarin anticoagulant, was detected in livers from the two puppies with hemorrhage. The dam did not have clinical signs of coagulopathy before or subsequent to whelping. The owners were confident that the dog had not been exposed to rodenticide for at least 4 weeks before whelping. A presumptive diagnosis of in utero brodifacoum toxicity was made. To the authors' knowledge this is the first time a second-generation coumarin anticoagulant has been detected in the liver of a newborn animal. This case is also unique because the dam was unaffected, suggesting that fetuses are more susceptible to brodifacoum toxicity than adult animals.  相似文献

OBJECTIVES: To document the histories, clinical findings, and management of seven puppies with laryngeal collapse occurring secondarily to brachycephalic airway syndrome. METHODS: Seven brachycephalic puppies aged between 4.5 and six months underwent surgery for management of brachycephalic airway syndrome following presentation for exercise intolerance and increased respiratory noise and effort. RESULTS: Stenotic nares of varying severity and an elongated soft palate were common to all dogs. All dogs had tracheal hypoplasia and this was severe in four dogs. Laryngeal collapse was present in all dogs. Two dogs had stage I, four dogs stage II, and one dog stage III laryngeal collapse. The dog with stage III laryngeal collapse and one dog with stage II laryngeal collapse died. There was no apparent association between the changes evident on thoracic radiographs or the degree of tracheal hypoplasia and postoperative outcome. CLINICAL SIGNIFICANCE: The development of severe secondary laryngeal changes in dogs aged six months or less supports the suggestion that immature brachycephalic dogs should undergo assessment and, if indicated, surgery as soon as any clinical signs of BAS are apparent.  相似文献

Two Scottish Deerhound puppies had clinical and pathological features consistent with the diagnosis of congenital non-goitrous hypothyroidism. They were from separate litters, but were the progeny of the same sire and dam. The puppies were smaller, had shorter limbs and shorter, broader heads than their littermates. They also had histories of weakness, difficulty in walking and somnolence. A characteristic radiographic feature was the absence of epiphyseal growth centres. Both had depressed serum thyroxine (T4) levels and one did not respond to exogenous thyroid stimulating hormone. On necropsy, the thyroid glands were small, the follicles varied in size and contained little or no colloid. The adenohypophysis contained many cells with markedly vacuolated cytoplasm. It is suggested that the clinicopathological pattern is the result of a primary thyroid abnormality. Possible mechanisms include either primary thyroid hypoplasia or an unresponsiveness to thyroid stimulating hormone.  相似文献

OBJECTIVE: To evaluate the effect of pubic symphysiodesis (PS) on pelvic development in skeletally immature dogs. STUDY DESIGN: Prospective randomized clinical trial. ANIMALS: Eight 4 month-old, sexually intact female Greyhounds. METHODS: Initial PS was performed at 4 months of age using a powered stapling device. Because of failure of the initial surgery, a second PS was performed 1 month later by resecting the pubic symphysis with a rongeur followed by placement of handmade bone staples in four dogs. Sham PS was performed in four control dogs at 4 months of age. Pubic growth rate and pelvic development were evaluated using standard plane radiography and computed tomography. Specific measurements included acetabular ventroversion, Norberg angle, lateral center-edge angle, and pelvic inlet dimensions. Hip distraction indices were determined as well. RESULTS: PS at 4 months of age using a stapling device failed. Pubic symphysiodesis using hand made staples was successful at 5 months of age and did not result in any clinically significant intraoperative or postoperative complications. Pubic symphysiodesis markedly decreased pubic symphysis growth in the treatment group. Hip distraction indices and pelvic inlet circumference, area, and width significantly decreased in treated dogs compared to those in the control group. Acetabular ventroversion was significantly increased in treated dogs compared to those in the control group. CONCLUSIONS: PS decreases pelvic canal size, increases acetabular ventroversion, and does not appear to have any clinically significant complications. CLINICAL RELEVANCE: PS performed in skeletally immature dogs with hip dysplasia may provide an effect similar to a triple pelvic osteotomy and warrants further investigation.  相似文献

A condition causing neonatal mortality in litters of puppies presents all the definitive features of the respiratory distress syndrome as seen in other species such as the horse and man. Histopathology of lung reveals alveolar collapse, distended respiratory bronchioles and in some instances the presence of eosinophilic hyaline membranes. Evidence is presented which indicates that massive phospholipid release from lung tissue is involved in the pathogenesis of the condition. This may be related to extensive damage to great alveolar cells. The exact aetiology of the condition is not known. Résumé. Les auteurs décrivent un syndrome occasionnant une mortalité néonatale des portées de chiots et qui présente toutes les caractéristiques du syndrome de détresse respiratoire observé dans d'autres espèces animales et chez l'homme. L'examen histopathologique des poumons montra un collapsus alvéolaire, une distension des bronchioles et parfois la présence de membranes hyalines éosinophiles. Certains faits montrent qu'une libération massive de phospholipides à partir du tissu pulmonaire est impliquée dans la pathogénie de cette affection, ce qui serait en rapport avec les lésions des grandes cellules alvéolaires; cependant, on ne sait rien de la pathogénie exacte de l'affection. Zusammenfassung. Ein Zustand, der neonatale Mortalität in Würfen von Hunden verursacht, zeigt alle die definitiven Eigenschaften des Atemnotsyndroms bei anderen Spezies wie dem Pferd und dem Menschen. Die Histopathologie der Lunge ergibt kollabierte Alveolen, erweiterte respiratorische Bronchiolen und in manchen Fällen die Anwesenheit eosinophiler hyaliner Membranen. Es werden Tatsachen genannt, die darauf schliessen lassen, dass massive Phospholipid-freisetzungen aus dem Lungengewebe mit der Pathogenese des Zustands zu tun haben. A flexural deformity affecting one or both forelimbs identified in 21 puppies of six to 12 weeks of age closely resembled a condition which has long been known in foals and farm animals at birth or shortly afterwards. In the puppies the characteristic feature was carpal hyperflexion which was associated with tautness of the tendons of the flexor carpi ulnaris muscle. The majority recovered spontaneously with conservative management but in two puppies it was necessary to section the shortened tendons to enable a normal limb posture to be regained. The fact that there was a preponderance of dobermann pinschers (17 out of 21) in the group opens speculation about an hereditary factor being involved, but further study is needed before such a correlation can be reached.  相似文献

There are many common issues that owners find problematic with their puppy's or kitten's behavior, such as eliminating in inappropriate locations, chewing, mouthing, growling, and biting. Many of these issues can be prevented or managed by helping owners understand normal canine and feline behavior and by teaching the puppy and kitten socially acceptable behaviors. The focus always should be on rewarding acceptable behaviors rather than punishing unacceptable ones. Puppy Preschool and Kitten Kindergarten classes are an ideal avenue to introduce pet owners to modern, humane ways to train and socialize their pets to be a valuable part of today's society.  相似文献
