Renal allograft transplantation was performed as treatment for terminal renal failure in six veterinary patients. Three patients failed to survive the postsurgical period, one patient died due to acute allograft rejection, one patient died subsequent to complications of previous parenteral hyperalimentation and infection, and one patient was euthanatized due to the inability of the clients to maintain the effort necessary to manage a transplant recipient. Further clinical studies are necessary to determine if renal transplantation with cyclosporine (cyclosporin A)/prednisolone immunosuppression has the potential to provide practical treatment of terminal renal disease in clinical veterinary medicine. Pharmacologic and immunologic monitoring of transplant recipients with individualized immunosuppression is imperative. Careful surveillance is necessary to identify and eliminate nosocomial sources of infection. Candidates for transplantation cannot be critically ill or malnourished prior to surgery, and pet owners must be prepared for the financial and time commitments associated with caring for a renal allograft recipient.  相似文献

Objective— To report the prevalence of hypophosphatemia after renal transplantation in a historical cohort of cats. Design— Case series. Animals— Cats (n=86) that received a renal allograft. Methods— Medical records (January 200–June 2006) were reviewed. Signalment, clinical signs, pre‐ and postoperative diet, pre‐ and postoperative clinicopathologic variables, renal histopathology, and outcome were retrieved. Prevalence, onset, duration, treatment and associated clinical signs of hypophosphatemia were recorded. A χ2 test was used to compare hemolysis frequency between cats with normal serum phosphorus concentration or a single spurious low serum phosphorus concentration for <24 hours duration (group 1) and confirmed hypophosphatemia for >24 hours (group 2). A Cox proportional hazards model was used to evaluate the effects of hypophosphatemia on survival while controlling for other potentially confounding variables (age, sex, weight, body condition score, and pre‐ and 24 hours postoperative clinicopathologic variables). Results— Eighty‐six cats (mean age, 7.7 years) were identified. Hypophosphatemia occurred in 32 cats (37%), with a median onset of 2 days and median duration of 4 days. Treatment was initiated in 48 (56%) of hypophosphatemic cats. Survival and hemolysis frequency was not significantly different between groups, and no risk factors were identified. Conclusion— Hypophosphatemia occurs in cats after renal transplantation and does not affect survival. Clinical Relevance— The clinical importance of hypophosphatemia in renal transplant recipients remains unknown.  相似文献

The medical records of 11 cats with histopathologic findings consistent with central nervous system (CNS) Cuterebra larvae myiasis were retrospectively examined to determine if clinical features could identify this disorder antemortem. Young to middleaged indoor-outdoor domestic shorthaired cats presenting with acute neurologic signs from July through September predominated. Many cats recently had clinical signs consistent with upper respiratory disease. Most cats presented for depression, lethargy, or seizures. Almost all cats had abnormal rectal temperatures, either hyperthermia or hypothermia. Peripheral leukocytosis and eosinophilia were not characteristic of cats with CNS cuterebriasis. Cerebrospinal fluid analysis did not consistently disclose evidence of inflammation. Common neurologic deficits included blindness, abnormal mentation, and signs of unilateral prosencephalic disease. No specific clinical or clinicopathologic test was diagnostic for CNS cuterebriasis.  相似文献

Background

Renal infarcts identified without definitive association with any specific disease process.

Objective

Determine diseases associated with diagnosis of renal infarcts in cats diagnosed by sonography or necropsy.

Animals

600 cats underwent abdominal ultrasonography, necropsy, or both at a veterinary medical teaching hospital.

Methods

Information obtained from electronic medical records. Cats classified as having renal infarct present based on results of sonographic evaluation or necropsy. Time‐matched case‐controls selected from cats that underwent the next scheduled diagnostic procedure.

Results

309 of 600 cats having diagnosis of renal infarct and 291 time‐matched controls. Cats 7–14 years old were 1.6 times (odds ratio, 95% CI: 1.03–2.05, P = .03) more likely to have renal infarct than younger cats but no more likely to have renal infarct than older cats (1.4, 0.89–2.25, P = .14). All P = .14 are statistically significant. Cats with renal infarcts were 4.5 times (odds ratio, 95% CI: 2.63–7.68, P < .001) more likely to have HCM compared to cats without renal infarcts. Cats with renal infarcts were 0.7 times (odds ratio, 95% CI: 0.51–0.99, P = .046) less likely to have diagnosis of neoplasia compared to cats without renal infarcts. Cats with diagnosis of hyperthyroidism did not have significant association with having renal infarct. Cats with renal infarcts were 8 times (odds ratio, 95% CI: 2.55–25.40, P ≤ .001) more likely to have diagnosis of distal aortic thromboembolism than cats without renal infarcts.

Conclusions and Clinical Importance

Cats with renal infarcts identified on antemortem examination should be screened for occult cardiomyopathy.  相似文献

OBJECTIVE: To identify preoperative diagnostic results that predict postoperative complications and survival in feline renal-transplant recipients. STUDY DESIGN: Retrospective clinical study. ANIMALS: Sixty-one feline renal allograft recipients. METHODS: Medical records for 61 consecutive cats that underwent renal allograft transplantation between January 1, 1996, and December 1, 1999, were reviewed. Age, diagnosis, body weight, body condition score, preoperative medical treatment, systolic blood pressure, packed cell volume, biochemical parameters at admission and at the time of surgery, postoperative complications, and postoperative survival were recorded. Associations of preoperative data with the occurrence of postoperative complications were determined using logistic regression. Postoperative survival was graphed using a Kaplan-Meier cumulative-survival plot. Associations of covariates with postoperative survival were analyzed using Cox proportional hazards analysis. RESULTS: Two parameters were significantly associated with occurrence of postoperative central nervous system (CNS) disorders: blood urea nitrogen concentration (odds ratio = 1.083; 95% CI = 1.018 to 1.148) and serum creatinine concentration (odds ratio = 1.8; 95% CI = 1.413 to 2.187) at the time of surgery. Postoperative survival 6 months after transplantation was 59%, though 3-year survival remained at 42%. Of all covariates investigated, only recipient age (relative hazard = 1.183; 95% CI = 1.039 to 1.334) was significantly associated with survival. CONCLUSION AND CLINICAL RELEVANCE: Standard measures of preoperative renal dysfunction do not predict postoperative survival in cats after renal transplantation, although an increase in the degree of preoperative azotemia is associated with an increased risk of CNS disorders after surgery. Increased recipient age is associated with decreased survival after renal transplantation.  相似文献

Hypertension in Cats With Chronic Renal Failure or Hyperthyroidism   总被引:6,自引:0,他引:6  
The Doppler ultrasonic recording technique was used to measure systolic and diastolic blood pressures indirectly in 28 cats with naturally occurring renal failure, 39 cats with hyperthyroidism, and 33 clinically normal cats. The mean systolic and diastolic blood pressures in the normal cats were 118.4 +/- 10.6 mm Hg and 83.8 +/- 12.2 mm Hg, respectively. In the cats with chronic renal failure, both the systolic (146.6 +/- 25.4 mm Hg) and diastolic (96.6 +/- 15.2 mm Hg) blood pressures were significantly higher (P less than 0.0001 and P less than 0.01, respectively) than in the normal cats. Elevations in systolic and/or diastolic blood pressure were recorded in 17 (61%) of the 28 cats with chronic renal failure. In the 39 untreated hyperthyroid cats, both the mean systolic (167.9 +/- 28.9 mm Hg) and diastolic (111.6 +/- 21.5 mm Hg) pressures also were significantly higher (P less than 0.0001) than normal. Increased systolic and/or diastolic blood pressure was recorded in 34 (87%) of the 39 hyperthyroid cats. In seven cats with hyperthyroidism that were reevaluated two to four months after successful treatment of the hyperthyroid state, there was a significant fall in mean systolic pressure (P less than 0.05) from a pretreatment value of 159.5 +/- 15.4 mm Hg to a posttreatment value of 132.0 +/- 1.62 mm Hg. Overall, the results of this study indicate that mild to moderate hypertension is common in cats with chronic renal failure and in cats with untreated hyperthyroidism. In addition, the hypertension appears to be reversible following successful treatment of the hyperthyroid state.  相似文献

Dialysis therapy is not a radical cure of chronic renal failure in cats,while renal transplantation is the cure method.Renal transplanta-tion requires rigorous examination of donor and receptor,and the procedure is strictly sterile.The main points of renal transplantation are:renal artery and vein sutured to abdominal aorta and posterior vena cava,respectively;ureter sutured to bladder;minimizing the time of thermal ischemia.Cats should be given immunosuppressant before the operation,and the drug should be administered throughout the life.Various indicators should be monitored regularly to prevent the occurrence of immunological rejection.  相似文献

Polyacrylamide hydrogel (PAHG) is being developed to treat osteoarthritis in horses. Prior to application in osteoarthritic joints, evaluation of PAHG in normal joints is indicated. The objectives of this study are to evaluate the clinical, cytologic, histologic, and metabolic effects of PAHG in normal fetlock joints. This study is an in vivo controlled study utilizing six horses that had each of the four fetlocks assigned to either a 7, 28, 56 days or control group subjected to arthrocentesis only. Synovial fluid was collected prior to administration of 2.5 mL of 4% PAHG and again at the completion of the study for macroscopic, cytological, and cartilage metabolism evaluation. The completion of the study included gross and histologic examination of the cartilage and synovial membrane. There was a small but significant (P = .0242) increase in cell count in the synovial fluid at 7 days. There were significant changes in the synovial membrane histology score (P = .0277) as a result of hypertrophic synoviocytes. Biomarkers indicated a small increase in cartilage turnover 7 days after PAHG administration. The PAHG was visible on the surface of the synovium at 7 days, and PAHG appeared in the interstitial spaces of the synovium and intracellular at days 28 and 56. Data from this study provide information as to the tolerance and disposition on an intraarticular injection of PAHG in a normal fetlock joint during a 56-day study. There were no major or permanent detrimental effects seen with the administration of PAHG in normal joints.  相似文献

Background: Oxidative stress is an important component in the progression of chronic renal failure (CRF) and neutrophil function may be impaired by oxidative stress. Hypothesis: Cats with CRF have increased oxidative stress and decreased neutrophil function compared with control cats. Animals: Twenty cats with previously diagnosed renal failure were compared with 10 age‐matched control cats. Methods: A biochemical profile, CBC, urinalysis, antioxidant capacity, superoxide dismutase (SOD) enzyme activity, reduced to oxidized glutathione ratio (GSH : GSSG), and neutrophil phagocytosis and oxidative burst were measured. Statistical comparisons (2‐tailed t‐test) were reported as mean ± standard deviation. Results: The CRF cats had significantly higher serum blood urea nitrogen, creatinine, and phosphorus concentrations than control cats, and significantly lower PCV and urine specific gravity than control cats. The GSH : GSSG ratio was significantly higher in the CRF group (177.6 ± 197, 61.7 ± 33; P < .02) whereas the antioxidant capacity was significantly less in the CRF group (0.56 ± 0.21, 0.81 ± 0.13 Trolox units; P < .005). SOD activity was the same in control and CRF cats. Neutrophil oxidative burst after Escherichia coli phagocytosis, measured as an increase in mean fluorescence intensity, was significantly higher in CRF cats than controls (732 ± 253, 524 ± 54; P < .05). Conclusions: The higher GSH : GSSG ratio and lower antioxidant capacity in CRF cats is consistent with activation of antioxidant defense mechanisms. It remains to be determined if supplementation with antioxidants such as SOD beyond the level of control cats would be of benefit in cats with CRF.  相似文献

Ultrasound is an excellent imaging tool in the evaluation of the biliary tract in cats. Extrahepatic obstruction could be diagnosed by the presence of dilated common bile duct (CBD), which was measured ventral to the portal vein in the porta hepatis. At this level, the diameter of the CBD in 6 healthy cats and in 22 cats with nonsurgical jaundice was 4 mm. Six of 7 cats with a CBD >5 mm had extrahepatic biliary obstruction.  相似文献

Abstract— Dermatophytes were cultured from seventy of 1824 (3.8 per cent) canine samples and 61 of 408 (14.9 per cent) feline samples submitted over ten years. Microsporum canis was the most common species isolated (86/131). M. gypseum (31/70) and M. canis (30/70) were isolated most frequently from dogs whereas M. canis was isolated most frequently from cats (56/61). Both male and female dogs and cats were equally affected by dermatophytosis. There was a higher incidence in both dogs and cats less than one year of age. Mixed breed dogs (19/70), domestic shorthaired cats (39/61) and Persian cats (15/61) were most often affected. M. gypseum had a greater incidence of infection in summer and most often caused localized disease in dogs. In dogs, localized dermatophytosis was more common. Localized infections were most often caused by M. canis in all animals, with the head (23/75), feet (10/75) and tail (7/75) affected most frequently. Résumé— Dos dermatophytes ont été isolés à partir de 70/1824 (3,8 pourcent) prélèvements canins et 61/408 (14,9 pourcent) prélèvements félins collectés sur une période de dix ans. L'espèce la plus souvent isolée était Microsporum canis (86/131). Microspomm gypseum (31/70) et M. canis (30/70) étainet les plus fréquemment isolés chez les chiens, alors qu'il s'agissait de M. canis (56/61) chez les chats. Les mâles et les femelles, chiens ou chats, étaient également représentés. Il y avait une nette incidence des animaux de moins d'un an, tant chez les chiens que chez les chats. Les chiens croisés (19/70), les chats européens (39/61) et les chats Persans (15/61) étaient les plus souvent atteints. L'incidence de M. gypseumétait plus importante l'été et provoquait les plus souvent des lésions localisées les chiens. Microsporum canis war die am häufigsten isolierte Spezies (86/131). Microsporum gypseum (31/70) und M. canis (30/70) wurden am häufigsten bei Hunden isoliert, M. canis dagegen wurde am häufigsten bei Katzen isoliert (56/61). Männliche wie weibliche Hunde und Katzen waren gleichermaßen von Dermatomykosen betroffen. Sowohl bei Hunden also auch bei Katzen war die Inzidenz bei Tieren unter einem Jahr höher. Am häufigsten erkrankten Mischlingshunde (19/70), Kurzhaarhauskatzen (39/61) und Perserkatzen (15/61). Die Inzidenz der Infektion mit M. gypseum war im Sommer größer, bei Hunden verursachte es mehr lokalisierte Erkrankungen. Bei Hunden überhaupt waren lokalisierte Dermatomykosen häufiger anzutreffen. Lokalisierte Infektionen wurden bei alien Tieren am häufigsten durch M. canis verursacht, am meisten betroffen waren Kopf (23/75), Pfoten (10/75) und Schwanz (7/75).
