A German shepherd dog was presented two months after surgery for correction of acute gastric dilatation volvulus. The dog had been diagnosed with exocrine pancreatic insufficiency. Radiographs revealed marked gaseous distension of one loop of intestine with a generalised increase in intestinal gas content. A 360 degrees anticlockwise rotation of the descending and transverse colon, around the longitudinal axis of the mesocolon, was diagnosed at exploratory coeliotomy. The transverse and descending colon appeared uniformly necrotic and an end-to-end colo-colic resection and anastomosis was performed. The dog initially made satisfactory postoperative progress but was euthanased on the third postoperative day after it developed an intestinal intussusception.  相似文献

Chronic gastric volvulus in dogs results in long-standing gastrointestinal signs unlike those of acute gastric dilatation and volvulus. This report describes chronic gastric volvulus in seven dogs. The majority of dogs presented with weight loss, chronic vomiting, lethargy and abdominal pain. A combination of radiographic, ultrasonographic and endoscopic imaging indicated altered positioning of gastric landmarks. Dynamic changes were identified in some cases. Exploratory coeliotomy and surgical gastropexy were performed in all dogs. Clinical signs improved or resolved in six of seven dogs postoperatively. Chronic gastric volvulus is an uncommon condition in dogs, but should be considered as a differential in cases presenting with the above clinical signs.  相似文献

Sixteen cases of hiatal hernia were diagnosed in cats and dogs at the University of Tennessee between 1978 and 1989. Three cases were found as incidental findings during thoracic or abdominal radiography and another two cases were found during post mortem examination. The remaining 11 cases were evaluated clinically and radiographically. The results of conservative or surgical therapy were reviewed. Not all animals with a hiatal hernia were symptomatic. Five animals with signs underwent surgery and another three had surgical intervention but were asymptomatic. Medical management was successful in controlling the signs in two animals showing signs consistent with reflux oesophagitis. One dog with ‘shortened oesophagus syndrome’ was treated inappropriately with conventional surgical techniques used in veterinary medicine for hiatal hernia. This study clearly emphasises the need to evaluate carefully each dog or cat with hiatal hernia before choosing the best therapeutic plan.  相似文献

A combined axial and paraoesophageal hiatal hernia complicated by splenic involvement was diagnosed in a two-year-old Chinese shar-pei. A circumcostal gastropexy was performed following reduction of the hernia and fundoplication of the cardia The dog recovered well and is asymptomatic six months postoperatively. The literature concerning hiatal hernia in the dog is reviewed and the various surgical techniques described for its management are discussed.  相似文献

A two‐year‐old Jack Russell terrier presented with a chronic history of weight loss and the recent development of a discharging wound in the left inguinal region that was confirmed by ultrasound and contrast radiography to be an enterocutaneous fistula. One day following admission the dog displayed signs of abdominal pain and the general condition deteriorated. At exploratory coeliotomy there was evidence of septic peritonitis and a segment of jejunum was found firmly adhered to the left inguinal canal. The affected jejunal segment was excised and an end‐to‐end anastomosis performed. A penrose drain was placed in the inguinal wound which was subsequently managed with open drainage. The inguinal wound healed successfully by second intention and the dog returned to normal body condition. The left testis became atrophic and castration was performed several weeks later. To the authors’ knowledge this is the first veterinary case report of an inguinal enterocutaneous fistula.  相似文献

Cecal dilatation and volvulus in dairy cows: 84 cases (1977-1983)   总被引:1,自引:0,他引:1  
The records of 84 cows with cecal dilatation or cecal volvulus were reviewed. The diagnosis was made by exploratory laparotomy or necropsy in 21 cows with cecal dilatation, and in 45 cows with cecal volvulus. Eighteen of the cows with cecal dilatation responded to medical management and the diagnosis was made by physical examination alone. The most useful criteria for determining that surgery was necessary were the presence of a high heart rate, apparent abdominal pain, scant or absence of feces, and the apex of the cecum rotated cranially on rectal examination. The prognosis for cattle with cecal dilatation and volvulus was good following medical therapy or decompression at surgery unless there was severe vascular compromise of the cecum. There was a 10% recurrence rate, and in these cows a partial typhlectomy was performed.  相似文献

Paraesophageal hiatal hernia and pyloric obstruction in a dog   总被引:1,自引:0,他引:1  
Paraesophageal hiatal herniation and pyloric obstruction were diagnosed in a pup with a history of vomiting. Findings of contrast radiography included esophageal reflux, delayed gastric emptying time, and paraesophageal herniation. Exploratory celiotomy revealed increased firmness of the pylorus and a primary defect in the esophageal hiatus, which allowed gastric herniation. Nissen fundoplication was performed following reconstruction of the esophageal hiatus, and pyloroplasty was performed to relieve the gastric outlet obstruction. Pyloric biopsy findings were consistent with a diagnosis of chronic gastritis. Recovery from surgery was initially unremarkable; however, the dog died suddenly 3 weeks after surgery. Necropsy revealed a large diaphragmatic hernia adjacent to the esophageal hiatus; the hernia had resulted in incarceration of the abdominal organs. The hiatal hernia reconstruction remained intact and was not the cause of the diaphragmatic disruption.  相似文献

Volvulus of the small intestine was diagnosed as a complication of acquired inguinal herniation in 2 horses. One of the horses continued to have signs of pain after reduction of the hernia. The volvulus was diagnosed at a second surgery, but the intestine was devitalized, and the horse was euthanatized. Ventral midline exploratory surgery was performed on the second horse, in conjunction with an inguinal approach. The small-intestinal volvulus was diagnosed and corrected at this time. It is suggested that ventral midline abdominal exploration be performed when acquired inguinal herniation causes acute small-intestinal obstruction in horses.  相似文献

The thoracic bellows mechanism consists of the rib cage and the diaphragm. The purpose of this study was to determine if nontraumatically acquired lesions of the bellows were secondary to underlying disease. Abnormalities of the bellows, specifically stress fractures of the ribs and hiatal hernia, were found in 21 dogs and cats with underlying cardiopulmonary disease, neuromuscular disease, or metabolic disease. A case-control study of Bulldogs demonstrated that hiatal hernia was associated with the more severe manifestations of brachycephalic syndrome. Stress fractures occurred mostly in females and in cats, and involved multiple ribs. Fractures were usually related to severe respiratory effort, but also occurred in association with metabolic disease. Hiatal hernia was also associated with severe respiratory effort, but may be exacerbated if a neuromuscular disorder affecting the diaphragm is present. Abnormalities of the thoracic bellows, such as rib stress fractures and hiatal hernia, may be signs of underlying disease, rather than being primary causes of disease.  相似文献

A case of nasopharyngeal stenosis with secondary hiatal hernia is described. An 8-year-old castrated male domestic shorthair cat was referred for a chronic upper respiratory problem and presumptive vomiting. Despite conservative management by the primary care veterinarian, the cat's condition progressed. The cat was presented to an emergency facility prior to referral to a specialty hospital. On presentation, inspiratory stridor was evident. Thoracic radiography revealed a hiatal hernia. Computed tomography indicated pharyngeal edema and probable nasopharyngeal stenosis. Endoscopy confirmed the presence of nasopharyngeal stenosis consistent with either stricture or choanal atresia. Balloon dilation of the choana was performed. The hiatal hernia regressed spontaneously post-resolution of the nasopharyngeal stenosis. The cat remained asymptomatic at recheck 3 months later.  相似文献

A six‐month‐old Labrador retriever presented for investigation of a colonic mass identified as an incidental finding during exploratory coeliotomy. Computed tomography identified a lesion in the colon which occupied part of its lumen and shared blood supply with the remainder of the colon. The lesion was suspected to be a colonic duplication and it was excised by segmental colectomy during exploratory coeliotomy. Histopathology from the excised colon confirmed the diagnosis of a colonic duplication. The dog recovered uneventfully and had no complications. To the authors’ knowledge, this is the first report of an asymptomatic, spherical, communicating colonic duplication and the first report to describe segmental colectomy for the management of this condition in veterinary patients.  相似文献

Cecal-colic volvulus was confirmed by surgery in two dogs. Both dogs were intact, male Great Danes. The clinical and diagnostic findings were not specific for large bowel volvulus. Clinical signs included peracute to acute onset of vomiting, mild abdominal distention and pain, lack of feces, and tenesmus. Abdominal radiographs demonstrated severe dilatation of bowel loops in both dogs. One dog survived.  相似文献

Primary splenic torsion in dogs is uncommon and can occur in acute or chronic form. The chronic form is difficult to diagnose because the clinical signs are vague and sometimes intermittent. A dog with a history of diaphragmatic hernia repair two years previously presented with chronic, vague clinical signs and an abdominal mass. The mass was revealed to be spleen on ultrasonography. On exploratory laparotomy, the dog was found to have a splenic torsion of approximately 180 degrees with mature, fibrous adhesions retaining the spleen in a torsed position. A splenectomy was performed, and the dog recovered uneventfully with complete resolution of prior clinical signs. Prognosis for dogs with splenic torsion is good, although complications are relatively common.  相似文献

A 2-year-old intact female Alaskan husky sled dog was presented with a history of chronic exercise-induced regurgitation refractory to medical management. Previous diagnostics were unremarkable except for an endoscopic examination and histopathologic evaluation of the upper gastrointestinal tract that revealed the presence of Helicobacter spp. and mild non-specific inflammation of the proximal duodenum. A laparoscopic hiatal herniorrhaphy, esophagopexy, fundopexy, and ovariectomy were performed without complications in anesthesia or surgery and clinical improvement was observed with continued follow-up for 8 months after surgery.Key clinical message:Surgical treatment for hiatal hernia may be considered in racing Alaskan sled dogs with regurgitation refractory to gastric protectant therapy.  相似文献

The records of nine dogs with intestinal volvulus were reviewed. Most were young adult (median 2.6 years) males (6/9) of medium to large breeds. Vigorous activity, dietary indiscretion, or trauma preceded volvulus. Signs included a peracute to acute onset of vomiting, hematochezia, a moderately distended and often painful abdomen, and shock. Normal packed cell volume, altered white blood cell count, hypoproteinemia, hypoalbuminemia, hypokalemia, pathologic bowel dilatation, and serosanguineous abdominal fluid were common clinical findings. Jejunum was most often affected. One dog survived with surgical management.  相似文献

A 7-years-old Clydesdale mare was presented with severe abdominal distension and acute colic. Dilated large intestine was palpated per rectum and a ventral midline exploratory laparotomy was performed. A 180 degrees volvulus of the pelvic flexure was present, associated with an inelastic band of tissue connecting the mesocolon to the umbilicus. The band was ligated and transected, and the volvulus reduced. Postoperative complications included hyponatraemia, metabolic acidosis and laminitis. The possible aetiology of the mesocolic-umbilical band is discussed.  相似文献

The case records of 4 cats and 1 dog with megacolon and constipation were reviewed to study the history, clinical signs, pathologic changes, and longterm results of surgical treatment. The 4 cats had idiopathic megacolon. The dog had megacolon secondary to dietary indiscretion. All animals had radiographic evidence of a large colon, which was confirmed by exploratory coeliotomy. Two of the cats and the dog had an intraoperative finding of megacecum as well. Subtotal colectomy was performed in all cases, with the ileocolic valve being removed in 3 of these cases. End-to-side anastomosis was used to restore the integrity of the bowel. All animals were treated successfully with this surgical technique, as judged by clinical response and owner satisfaction. Histologically, there were no consistent lesions to explain the cause of this disease in the cats.  相似文献

OBJECTIVES: To document the histories, clinical findings, and management of seven puppies with laryngeal collapse occurring secondarily to brachycephalic airway syndrome. METHODS: Seven brachycephalic puppies aged between 4.5 and six months underwent surgery for management of brachycephalic airway syndrome following presentation for exercise intolerance and increased respiratory noise and effort. RESULTS: Stenotic nares of varying severity and an elongated soft palate were common to all dogs. All dogs had tracheal hypoplasia and this was severe in four dogs. Laryngeal collapse was present in all dogs. Two dogs had stage I, four dogs stage II, and one dog stage III laryngeal collapse. The dog with stage III laryngeal collapse and one dog with stage II laryngeal collapse died. There was no apparent association between the changes evident on thoracic radiographs or the degree of tracheal hypoplasia and postoperative outcome. CLINICAL SIGNIFICANCE: The development of severe secondary laryngeal changes in dogs aged six months or less supports the suggestion that immature brachycephalic dogs should undergo assessment and, if indicated, surgery as soon as any clinical signs of BAS are apparent.  相似文献

Haemoglobinuria and periumbilical discoloration (also known as Cullen's sign) are clinical signs uncommonly reported in veterinary patients. This report describes a case of retroperitoneal haemorrhage in a dog, associated with haemoglobinuria and Cullen's sign. To the authors' knowledge, these clinical signs have not previously been reported singularly or in combination with retroperitoneal haemorrhage in dogs. A neutered male Shetland sheepdog, which was presented for haematuria, also had an abdominal mass, abdominal pain and a large area of periumbilical discoloration. Laboratory studies determined that haemoglobinuria was the cause of the red-coloured urine. Abdominal radiographs suggested a splenic mass and a coeliotomy was performed. During the induction and throughout the anaesthetic period the dog was hypertensive and a large haematoma originating from the right retroperitoneal space was identified at surgery. The cause of the haemorrhage was uncertain but a ruptured phaeochromocytoma was thought possible on the basis of the persistent hypertension and the location of the haemorrhage.  相似文献

A 45‐day‐old foal was presented for abdominal pain. Examination revealed the presence of haemoabdomen. An exploratory celiotomy was performed that revealed a large tear on the visceral surface of the spleen. A splenectomy was performed without rib resection. An abdominal wall hernia and leucocytosis had developed by 6 weeks post surgery. A second celiotomy to repair the hernia and explore the cause of the leucocytosis was performed 10 weeks after the first surgery. A portion of the caecum and colon had become adhered to the body wall at the site of the hernia; these areas were resected and the hernia repaired with a polypropylene mesh. The filly made a good recovery and is currently in training. Splenic rupture should be considered in any horse showing signs of abdominal pain with ultrasonographic findings consistent with haemoabdomen.  相似文献
