A 3-year-old Darreh-Shuri stallion was presented with left forelimb open dorsal fetlock luxation immediately after being hit by a motorized vehicle. The horse was in a non–weight-bearing position on the affected limb. General physical examination revealed normal vital parameters and no special concurrent abnormality. Radiographic examination in lateromedial and dorsopalmar views of the affected limb showed complete luxation of the metacarpophalangeal joint with no fractures. Ultrasonographic evaluation revealed intact collateral ligaments. Under general anesthesia and following routine preparations, forceful manual pressure was exerted on the distal part of the metacarpus and proximal part of the first phalanx to reduce the luxation. Joint capsule and skin laceration were sutured. Intraarticular amikacin (500 mg) was administered and repeated on the third and fifth days postoperatively. A palmar polyvinyl chloride, synthetic plastic polymer (PVC) splint was placed following adequate padding from the carpus to the toe. The higher and lower third of the splint was casted and the middle part left open to manage the joint and skin lesion. Intraarticular sodium hyaluronate (20 mg) was administered 10 days later. After cast removal following 5 weeks, the stallion revealed no sign of lameness. The owner was instructed to keep the stallion confined to a stall for at least 2 more months with a restricted daily short hand walk. Special casting methods for immobilization of a limb with an open joint luxation can tremendously improve the healing process.  相似文献

The accessory ligament of the hindlimb suspensory ligament arises from the plantar aspect of the calcaneus and fourth tarsal bones and blends with the suspensory ligament in the proximal aspect of the metatarsus. The accessory ligament of the suspensory ligament of both hindlimbs of 12 mature horses, with no history of hindlimb lameness, was assessed ultrasonographically. The ligament comprised linear parallel echoes which were consistently shorter than those of the lateral digital flexor tendon; the dorsal and plantar borders were parallel. Lameness associated with injury of the accessory ligament of the suspensory ligament was identified in 6 of 8 lame hindlimbs of 5 horses, unilaterally in 4 horses and bilaterally in one horse. In all horses there was concurrent proximal suspensory desmopathy. There was localised oedematous swelling on the distal medial aspect of the chestnut extending distally in 2 horses. Lameness was best identified when the horses were ridden. Perineural analgesia of the deep branch of the lateral plantar nerve abolished lameness in 3 horses but perineural analgesia of the tibial nerve was required to abolish lameness in the hindlimb with injury of the accessory ligament of the suspensory ligament in 2 horses. Injury of the accessory ligament of the suspensory ligament was characterised by marked decrease in echogenicity of the ligament and loss of parallel alignment of the linear echoes, which were shorter than normal in longitudinal images.  相似文献

Fractures involving the fetlock region are encountered commonly in Thoroughbred racehorses. Condylar fractures of the distal metacarpus/metatarsus and mid‐sagittal fractures of the proximal phalanx account for the majority of fetlock injuries during both racing and training and clinical features, management and outcome of these injuries have been well described. Transverse fractures of the distal diaphysis of the third metacarpus, however, are seldom observed and have received little attention in the literature. This paper describes clinical and radiological findings associated with 6 cases encountered in first opinion racehorse practice over a 3 year period. Transverse third metacarpal fractures vary in presentation but typically have clinical signs referable to the fetlock region and may, on occasion, be life‐threatening. They are therefore an important differential diagnosis when a fracture involving the fetlock is suspected.  相似文献

Bone-phase scintigraphy is sensitive to the dynamic process of bone modeling and remodeling, which may be adaptive or pathologic. Our knowledge of normal patterns of radiopharmaceutical uptake (RU) is limited, making interpretation of images problematic. It is therefore important to characterize normal patterns of RU at specific sites and relate these to age and exercise, to ensure valid interpretation of images in clinical cases with subtle lesions. This study aimed to characterize patterns of uptake of radiopharmaceutical in the distal tarsal region in clinically normal horses, and to investigate the effects of age and work discipline. Retrospective evaluation of nuclear scintigraphic images of the distal tarsal region of 30 clinically sound, high-level showjumpers and lower-level riding horses was performed. All images were acquired dynamically as a series of 35 two-second frames, which were then motion corrected and summated. The images were assessed using vertical and horizontal profile analysis across the distal tarsal region, and regions of interest comparisons between the distal tarsal region and tibia within each horse. There was a repeatable pattern of RU across the distal tarsal region in both horizontal and vertical directions. There was significantly greater RU on the dorsal compared with plantar aspect, and lateral compared with medial aspect of the distal tarsal region. RU in the right distal tarsal region was significantly greater than in the left. Elite showjumpers had significantly higher RU ratio compared with the lower-level riding horses, but no significant alterations in the pattern of RU were detected.  相似文献

Objective —To describe incomplete oblique sagittal dorsal cortical fractures of the equine third metacarpal bone, their surgical repair, and subsequent performance of the horses.
Study Design —Retrospective examination of medical records and racing performance.
Animal Population —Six Thoroughbred race horses, 2 to 4 years of age.
Methods —Radiographic confirmation of all fractures preceded general anesthesia and surgical correction. Three fractures were treated by intracortical compression using screws placed in lag fashion, and five fractures were treated by osteostixis. Race records were reviewed for each horse to determine performance after surgery.
Results —Fractures were best observed on palmarodorsal radiographic projections. Three horses treated by intracortical compression returned to racing, but fracture recurred in one horse and was treated by osteostixis. This horse and the other three horses treated by osteostixis raced after surgery.
Conclusions —Horses with incomplete oblique sagittal fractures of the dorsal cortex of the third metacarpal bone can race after surgical management of the fracture by screws placed in lag fashion or osteostixis. The authors' preferred surgical procedure for managing this fracture is osteostixis.
Clinical Relevance —Palmarodorsal radiographic projections of the third metacarpal bone are recommended in young Thoroughbred race horses suspected of having dorsal metacarpal stress fractures.  相似文献

Reasons for performing study: There is limited information on potential diffusion of local anaesthetic solution after various diagnostic analgesic techniques of the proximal metacarpal region. Objective: To document potential distribution of local anaesthetic solution following 4 techniques used for diagnostic analgesia of the proximal metacarpal region. Methods: Radiodense contrast medium was injected around the lateral palmar or medial and lateral palmar metacarpal nerves in 8 mature horses, using 4 different techniques. Radiographs were obtained 0, 10 and 20 min after injection and were analysed subjectively. A mixture of radiodense contrast medium and methylene blue was injected into 4 cadaver limbs; the location of the contrast medium and dye was determined by radiography and dissection. Results: Following perineural injection of the palmar metacarpal nerves, most of the contrast medium was distributed in an elongated pattern axial to the second and fourth metacarpal bones. The carpometacarpal joint was inadvertently penetrated in 4/8 limbs after injections of the palmar metacarpal nerves from medial and lateral approaches, and in 1/8 limbs when both injections were performed from the lateral approach. Following perineural injection of the lateral palmar nerve using a lateral approach, the contrast medium was diffusely distributed in all but one limb, in which the carpal sheath was inadvertently penetrated. In 5/8 limbs, following perineural injection of the lateral palmar nerve using a medial approach, the contrast medium diffused proximally to the distal third of the antebrachium. Conclusions and potential relevance: Inadvertent penetration of the carpometacarpal joint is common after perineural injection of the palmar metacarpal nerves, but less so if both palmar metacarpal nerves are injected using a lateral approach. Following injection of the lateral palmar nerve using a medial approach, the entire palmar aspect of the carpus may be desensitised.  相似文献

Reasons for performing the study: Lateral condylar (LC) fractures of the third metacarpus (McIII) are a common reason for euthanasia in racehorses, and may be the result of repetitive overloading or cumulative pathological change. Magnetic resonance imaging (MRI) allows monitoring of bone and cartilage to detect pathological and adaptive changes that may be precursors of fracture. Objectives: To describe bone and cartilage MRI features in the distal condyles of McIII of Thoroughbred racehorses, with and without condylar fracture. Hypotheses: 1) A greater degree of bone and cartilage adaptation or pathology will be seen in fractured McIIIs compared with their respective contralateral McIIIs. 2) Contralateral McIIIs will have a greater degree of bone and cartilage adaptation or pathology than McIIIs from control horses that did not sustain a LC fracture. Methods: The McIIIs from 96 horses subjected to euthanasia at racecourses were divided into 3 groups: Group 1: nonfractured bones from horses without LC fracture; Group 2: nonfractured bones from horses with unilateral LC fracture; and Group 3: fractured bones from horses with unilateral LC fracture. The MR images were examined and graded for bone and cartilage changes. Results: Nine percent of Group 1 (n = 9) and 11% of Group 2 bones (n = 5) had incomplete LC fractures. Focal palmar necrosis was most frequently detected in bones from Group 1 (12%) compared with Groups 2 (9%) and 3 (4%). The prevalence of bone and/or cartilage abnormalities tended to increase from Group 1 to Group 2 to Group 3. Conclusions: Magnetic resonance imaging is able to detect cartilage and bone changes that may be associated with LC fracture. There was no significant difference in bone/cartilage changes between bones from Groups 1 and 2, despite increased pathology in Group 2 bones. Potential relevance: Periodic monitoring of bone and/or cartilage changes in distal McIII of Thoroughbred racehorses may help to prevent catastrophic LC fractures.  相似文献

通过13个微卫星座位对蒙古马和纯血马两大品种的60匹马进行了遗传检测。用8%非变性聚丙烯酰胺凝胶电泳分离PCR扩增产物,并用银染法显色。通过软件计算了各微卫星座位的等位基因频率、有效等位基因数(Ne)、多态信息含量(PIC)和杂合度(H)。结果13个座位中UCDEQ440变异最大,TKY16变异最小;纯血马的Ne、PIC和H的平均值均稍高于蒙古马;总群体的平均遗传分化系数是0.0429。这些研究将为今后我国培育优良品种马提供思路。  相似文献

This case report describes the evaluation, surgical approach and outcome of an 8-month-old foal with severe angular limb deformities of both metatarsophalangeal joints (MTPJ). Radiography and computed tomography (CT) were used to diagnose a windswept deformity of the hindlimbs with associated bilateral physitis. The orthopaedic surgery was planned and rehearsed using three-dimensional (3D) bone models. A bilateral closing wedge ostectomy was performed on the distal MtIII, and 3.5 mm LCP® medial distal tibia plates were used bilaterally for osteosynthesis. Even though convalescence was prolonged and complicated by partial unilateral implant failure and osteoarthritis (OA), reduction of the deformities was successful and resulted in a sound foal.  相似文献

A 9‐year‐old Warmblood gelding underwent magnetic resonance (MR) imaging of the carpal and proximal metacarpal regions for evaluation of prolonged right forelimb lameness. Magnetic resonance findings were indicative of desmopathy of the right lateral carpometacarpal ligament and the interosseous ligament between the third and fourth metacarpal bones. Based on the MR findings and lack of response to conservative therapy, surgery using a drilling technique was performed to facilitate fusion of the right carpometacarpal joint. After surgery and a 6‐month rest and rehabilitation programme, the horse returned to soundness. This case report describes a unique combination of lateral carpal/proximal metacarpal soft tissue injuries that resulted in prolonged lameness and were treated with facilitated arthrodesis of the carpometacarpal joint. These soft tissue injuries should be considered as a differential diagnosis in horses with lameness localised to the proximal metacarpal/distal carpal region.  相似文献

A 7‐month‐old Thoroughbred filly was evaluated because of an enlarging mass present on the proximomedial aspect of the left metacarpus. The mass was first noticed at age 2 months and had continued to increase in size. The mass was firm, nonpainful to palpation, covered with normal haired skin and had never been associated with lameness. Radiographs obtained at ages 2 and 7 months revealed an enlarging and partially mineralised mass associated with the second metacarpal bone. Based on the clinical and radiographic progression, as well as the location of the mass, a poor prognosis for future athletic soundness was given and the filly was subjected to euthanasia. The mass had histological features of both an ossifying fibroma and osteoma.  相似文献

A 17-year-old Irish Draught cross gelding with a history of chronic recurrent right hindlimb cellulitis was evaluated due to acute onset of severe lameness affecting this limb. Scintigraphic examination identified marked radiopharmaceutical uptake within the central and third tarsal bones and the lameness improved following tibial and peroneal nerve blocks. Distal intertarsal joint collapse was observed but the radiographic changes were considered insufficient to explain the lameness severity. Magnetic resonance imaging revealed marked bone pathology affecting both the central and third tarsal bones. A subcutaneous abscess developed on the dorsolateral aspect of the limb, but clear evidence of joint infection was not established. Computed tomography showed lytic lesions affecting mainly the third tarsal bone, so the animal was treated surgically for suspected osteomyelitis involving this bone by implantation of gentamicin impregnated polymethylmethacrylate. Shortly following surgery the horse developed gastric rupture and was euthanased. Osteonecrosis of the third tarsal bone was identified, but sepsis was not demonstrated to be the cause. It is hypothesised that osteonecrosis developed as a sequel to chronic osteomyelitis which may have responded to previous antimicrobial treatment. The report also highlights the importance of gastric disorders as a differential for discomfort in animals undergoing treatment for orthopaedic disease.  相似文献

An 8‐year‐old Paint Horse gelding was evaluated for a persistent left forelimb lameness (grade 4/5), with a hard swelling at the dorsomedial aspect of the carpometacarpal joint, due to osteoarthritis. Previous systemic and local anti‐inflammatory therapy had only a temporary effect. Partial carpal arthrodesis was suggested, but the owner elected for conservative treatment. The horse was confined to a small paddock and received phytotherapeutic supplementation with Harpagophytum procumbens. The lameness gradually resolved but a similar hard swelling developed on the right carpus. Radiographic follow‐up 1.5 years later revealed a spontaneous bilateral ankylosis of the carpometacarpal joint.  相似文献

The anatomic variations and the degenerative changes in the collateral cartilages and middle and distal phalanges in 6 Ardenner colts were characterized by radiography, scintigraphy, and magnetic resonance imaging (MRI). The radiographic changes were assessed between the ages of 16.5 and 25 months. An anatomic variation of the middle and distal phalanges was demonstrated in some of the colts. MRI examination of the 6 colts revealed an association between the deep digital flexor tendon cross-sectional area and body weight as well as foot circumference. Also, a thin collateral sesamoidean (suspensory navicular) ligament was observed subjectively. The presence of an extensive ossification of the collateral cartilages of the foot was found in these young horses. The ossification was characterized by the existence of 2 separate ossification centers, which had a tendency to unite. The radiographic interphalangeal degenerative lesions seen appear as bone remodeling of the dorsal edges of the middle and distal phalanx, on the insertion sites of collateral ligaments of the distal interphalangeal joint, the digital extensor tendon, and the distal interphalangeal joint capsule.  相似文献
