OBJECTIVE: To describe a novel technique for blepharoplasty to cover a tissue defect involving >/=50% of the lower eyelid. STUDY DESIGN: Prospective clinical study. ANIMALS: Five cats with lower eyelid squamous cell carcinoma (SCC). METHODS: En bloc resection of SCC by removing >/=50% of the lower lid with either the medial or lateral canthus was performed without other adjunctive treatment for SCC. The lid defect was reconstructed with a transposition skin flap derived from the frontal (medial defect) or temporal (lateral defect) region. The third eyelid was advanced laterally without dissection from its insertion; its outer conjunctival layer was removed, and the skin flap was sutured with single interrupted sutures dorsally over the nictitating membrane, ventrally to the cutaneous edge of the surgical wound and medially or laterally (depending on the canthus removed) to the skin of the remaining lower lid. RESULTS: Satisfactory cosmetic and functional results were achieved and the Schirmer tear tests were normal. In 2 cats, the skin flap needed monthly hair trimming to avoid corneal lesions. CONCLUSIONS: After en bloc resection of SCC involving >/=50% of the lower eyelid, reconstruction can be achieved by relocation of the third eyelid and use of a cutaneous transposition flap sutured to the scarified external surface of the third eyelid. Eyelid apposition and lacrimal function were preserved. CLINICAL RELEVANCE: Blepharoplasty using a cutaneous transposition flap sutured to the scarified surface of a relocated third eyelid should be considered for reconstruction of lower eyelid defects with >/=50% tissue loss of the lid margin.  相似文献

A mature, neutered male domestic cat was presented with a chronic axillary wound of unknown origin and at least three years' duration. The diagnostic investigation included screening tests for feline leukaemia virus and feline immunodeficiency virus, tissue culture and histological examination. No underlying aetiology or perpetuating cause could be identified. An omental flap was created, passed via a subcutaneous tunnel, and packed into the wound site after excision of all chronic granulation tissue. The skin defect was closed using an omocervical axial pattern skin flap. A small area of the distal edge of the flap became necrotic but the defect healed by second intention. To the author's knowledge, this is the first clinical report of the use of an omocervical skin flap for repair of a chronic axillary wound in a cat. This flap offers a useful alternative where the use of a thoracodorsal axial pattern flap is not possible due to the extent of the lesion.  相似文献

A 6.5-year-old, castrated male Dalmatian was presented with a 3-month history of a chronic, nonhealing wound related to a previously excised lick granuloma. Reconstruction of the wound on the lateral metatarsal region was achieved using a phalangeal fillet technique, without digital pad transposition. The skin flap healed successfully with very good cosmetic and functional results.  相似文献

OBJECTIVE: To describe use of the scrotum as a pedicle flap to cover defects created by tumor excision in the perineum or caudal and medial aspect of the thigh. STUDY DESIGN: Clinical study. ANIMALS: Three dogs. METHODS: After tumor excision and prescrotal castration, an incision was made around the base of the scrotum, leaving a pedicle on the side opposite the skin defect. After removal of the tunica dartos and abdominal fascia, the scrotal skin was stretched and shaped to the defect, then sutured in position with a simple interrupted pattern. Bandages that were changed every 48 hours compressed the flaps, and healing was observed until sutures were removed. RESULTS: Two dogs had first intention healing in 15 days, whereas 1 dog developed necrosis of 10% of the flap, and this area healed by second intention. CONCLUSION: In male dogs, the scrotal skin can be used as a pedicle flap for reconstructive surgery of wounds in the perineum and the proximomedial and caudal aspect of the thigh. CLINICAL RELEVANCE: The perineal region remains a surgical challenge because of the lack of the available skin for reconstruction of surgical wounds. The scrotal skin should be considered for use as a transposition flap to cover skin defects in this region.  相似文献

Orbital exenteration accompanied by wide eyelid excision in the cat and dog may leave a defect that cannot be closed in a primary fashion. This report describes the use of a caudal auricular axial pattern flap to effect closure following orbital exenteration in three cats and one dog. The most common complication was distal flap necrosis, which necessitated a second surgery in two patients.  相似文献

A 13-year-old Morgan gelding was examined for right forelimb lameness and tenosynovitis of the right common carpal sheath of the digital flexor tendons. The horse had moderate right forelimb lameness at the trot and marked effusion of the right common carpal sheath of the digital flexor tendons. Ultrasonographic examination revealed a soft tissue mass within the proximal pouch of the affected tendon sheath, located adjacent to the distal physis of the radius. Cytology and culture of the fluid revealed a sterile, eosinophilic tenosynovitis. Tenoscopic exploration confirmed the presence of a capsulated soft tissue mass. Thecotomy was required to fully debride the mass, which histology revealed to be a mast cell tumour. At 22 months postoperatively, the horse developed mild right forelimb lameness and eosinophilic tenosynovitis because of recurrence of the mastocytoma. Mastocytosis is a possible differential diagnosis in any horse exhibiting lameness associated with tenosynovitis. Surgical excision combined with rest and postoperative intrasynovial and systemic corticosteroids may be palliative.  相似文献

A 5-year-old gelding used for showing was presented for surgical repair of a full-thickness 15 mm diameter defect in the right pinna, which had occurred as a delayed complication following laser excision of a sarcoid. The defect had resulted in progressive deformity of the ear, and the horse was considered at risk of further injury if the defect became entrapped on a fixed object. Two artificial dermis meshes were inserted to encourage the formation of granulation tissue across the defect, and a commercially available skin expander was implanted adjacent to the site. Eighteen days later, a second surgery was performed to remove the skin expander and to mobilise a local rotational skin flap to close the defect. Both surgeries were performed under standing sedation and local anaesthesia. The site healed well, avoiding further potential trauma to the ear and deformity due to tissue contracture at the site, although some deformation of the lateral aspect of the cartilage remained.  相似文献

A 16‐hour‐old Quarter Horse filly presented for evaluation of a soft mass in the right caudoventral lateral abdominal wall. Radiography and ultrasonography confirmed the presence of a nonreducible lateral abdominal wall hernia with small intestine located between the skin and body wall. The size of the defect itself could not be imaged and the filly was taken to surgery. A 4 cm vertical defect that appeared congenital was found in the transverse abdominal muscle and was repaired by primary apposition. This case report describes the repair and outcome of this unusual type of congenital hernia.  相似文献

A 6-wk-old tiger (Panthera tigris) was evaluated for severe skin lacerations from an adult tiger attack. A caudal superficial epigastric skin flap was surgically placed to cover a defect that could not be closed over the hind limb; however, the skin flap did not adhere well to the granulation tissue over a period of 1 mo. The granulation bed matured and deteriorated. A subatmospheric pressure technique (vacuum-assisted closure, V.A.C. Therapy, Kinetic Concepts Inc., San Antonio, Texas 78219, USA) was utilized, and flap adherence occurred after 4 wk. This technique should be considered when dealing with severe or chronic wounds in tractable animals.  相似文献

A 6-year-old Cleveland Bay mare was referred to the University of Liverpool Equine Hospital for repair of an iatrogenic nasocutaneous fistula. The fistula had developed following attempted ablation of an epidermal inclusion cyst located in the right nasal diverticulum with intra-lesional formalin 6 weeks previously. Subsequent tissue necrosis resulted in the formation of a full-thickness defect in the overlying tissues and the creation of a 5 × 5 cm nasocutaneous fistula. Surgical management consisted of initial debridement of the fistula and use of a commercial dermal regeneration mesh to bridge the defect, acting as a tissue scaffold. The levator labii superioris muscle was utilised to cover the scaffold by transecting it close to its proximal origin and reflecting it rostrally. Finally, a transposition skin flap was created adjacent to the site and was rotated to cover the defect. The mare made an excellent recovery, and follow-up confirmed complete healing of the surgical site with an excellent cosmetic outcome.  相似文献

OBJECTIVE: To evaluate the use of subdermal plexus skin flaps for closing defects after excision of cutaneous and subcutaneous tumors in dogs and to compare outcome of flaps secured with sutures and those secured with butyl-cyanoacrylate and intermittent sutures. STUDY DESIGN: Clinical study. ANIMALS: Fifteen dogs. METHODS: After excision of cutaneous or subcutaneous tumors the skin defect was reconstructed by random flaps based on the subdermal plexus. Flap skin edges were apposed with simple interrupted 4-0 monofilament nylon sutures (group 1; 5 dogs) or nylon sutures alternated with butyl-cyanoacrylate adhesive (group 2; 10 dogs). Flaps were evaluated every 48 hours when bandages were changed, until complete healing. RESULTS: Random flaps based on the subdermal plexus were effectively used to close wound defects; mean flap survival was 89%. Partial flap necrosis occurred in 4 dogs. Wound margins apposed with butyl-cyanoacrylate had thinner and more esthetic scars than sutured margins. CONCLUSION: Random flaps based on the subdermal plexus proved to be versatile for covering limb wounds after excision of cutaneous or subcutaneous tumors. Mean survival rate was comparable to that reported for axial pattern flaps. Butyl-cyanoacrylate adhesive was easy to apply, allowed accurate margin apposition with good cosmetic outcome and reduced sutures needed. CLINICAL RELEVANCE: Cyanoacrylate adhesive should be considered in lieu of suture closure to secure random skin flaps based on the subdermal plexus in dogs.  相似文献

A 7-year-old, neutered, male Labrador retriever was diagnosed with a tarsometatarsal grade II mast cell tumor. Metastasis was identified in the popliteal lymph node. Amputation was not an option. A reverse saphenous skin flap was used to cover the skin defect caused by excision of the tumor. Surgery was followed with adjunctive chemotherapy.  相似文献

Subluxation of the second carpal bone (C2) in two racing greyhounds, and luxation associated with other carpal injuries in a Staffordshire bull terrier, were diagnosed as causes of thoracic limb lameness. The clinical signs of subluxation were subtle, with local soft tissue swelling and a pain reaction on digital pressure over the dorsal aspect of C2. In contrast, the dog with a complete luxation was very lame, having marked soft tissue swelling with carpal hyperextension and valgus. Diagnosis was made by radiography. Subluxation of C2 was seen as a small, dorsally displaced opacity between the proximal and distal rows of carpal bones on the mediolateral view and, on the oblique view taken in only one case, as a marked dorsal displacement of one-third of the proximal joint surface of C2. In the case of luxation, C2 appeared on the mediolateral view lying dorsal to the radial carpal bone. There was also fracture of C4. Both greyhounds were treated by internal fixation and returned to racing. The Staffordshire bull terrier was treated by pancarpal arthrodesis with a successful outcome.  相似文献

A male, 10 month old llama with malformation of both front limbs was presented. Both front limbs had one more digit located medially. The distal phalanx of this additional digit at the left front limb reached the ground by the tip of the keratinzed pad and the toenail. The accessory digit at the right front limb was bent in a 90 degree angle caudolateraly. Beside the digital bones of the accessory digits the second metacarpal bone and the first carpal bone could be detected in both front limbs by radiological examination. Secondary a bilateral slight carpal valgus deformity could be seen.  相似文献

A neutered male German shorthaired pointer sustained severe bite wounds to the left caudal flank and thigh area. Thorough wound lavage and debridement was performed immediately and also three days after presentation. Daily wound dressing resulted in the production of a mature granulation tissue bed. Prior to wound closure, colour flow Doppler ultrasonography was used to confirm blood flow through the right and left caudal superficial epigastric arteries and veins. Sixteen days after presentation, right and left caudal superficial epigastric axial pattern flaps were simultaneously elevated to cover the defect. The right flap was elevated as an island flap, rotated 120 degrees and used to cover the caudodorsal aspect of the defect. The left flap was elevated and rotated dorsally to cover the cranioventral aspect of the defect. Ninety per cent wound coverage was achieved and flap survival was total. The donor site defect was closed primarily and no dehiscence occurred. Three months postsurgery, the entire defect was closed and limb function was normal.  相似文献

Sixteen normal 20 to 30 kg dogs had standard femoral head and neck excision (8 dogs) or femoral head and neck excision with interposition of a biceps muscle flap (8 dogs). Lameness lessened at week 6, and did not differ between groups; neither group returned to a completely normal gait. Weightbearing decreased on the operated pelvic limb compared to controls (forceplate analysis). Weightbearing force of the muscle flap dogs was significantly less at weeks 6 and 16 than that of the standard femoral head and neck excision dogs. The operated limb of both groups was shortened; shortening was significantly greater for the muscle flap group. Hip extension was limited in both groups. Both groups had decreased (more acute) hip, stifle, and hock angles measured in standing position. Thigh muscle atrophy was more pronounced in the muscle flap group at week 6. More soft tissue was interposed in the muscle flap group at necropsy compared to the group with standard femoral head and neck excision, but the difference was not statistically significant. Skeletal muscle fibers were present in the interposed fibrous connective tissue of the muscle flap group, and the healed surfaces were smoother than those of the standard femoral head and neck excision group.  相似文献

OBJECTIVE: To report the surgical management of a traumatic distal antebrachial wound using a medial saphenous fasciocutaneous free tissue flap and a type I external skeletal fixator (ESF). STUDY DESIGN: Case report. ANIMALS: A 7-year-old spayed, female Labrador retriever. RESULT: Wound coverage and pancarpal arthrodesis were accomplished during a single anesthetic episode using a medial saphenous fasciocutaneous free tissue flap and type I ESF. A second surgical procedure was performed to expedite healing of part of the original wound that was initially left to heal by second intention. CONCLUSIONS: Free tissue transfer can be used in combination with an ESF for repair of distal antebrachial wounds, allowing orthopedic repair and soft tissue coverage in a single anesthetic episode. CLINICAL RELEVANCE: Concurrent use of free tissue transfer and carpal arthrodesis with an ESF could be used for repair of complex antebrachial wounds without need for multiple procedures, extended hospitalization, and increased cost.  相似文献

Microneurovascular Free Digital Pad Transfer in the Dog   总被引:1,自引:0,他引:1  
By cadaver dissections, the fifth digit of the canine hind limb was determined to have a consistent neurovascular anatomy, and therefore be a suitable donor for an axial pattern digital pad flap. The defined digital pad flap was transferred to the region of an excised metacarpal pad by microneurovascular anastomoses in five operations on four dogs. One flap failure was considered to result from failure of the venous anastomosis. In all four successful transfers, cutaneous sensation was reestablished on average in 78 days. There was histologic evidence of nerve regeneration across the anastomosis in one dog at week 24. The transferred pads of three dogs monitored for 15 months underwent hypertrophic changes. There were no complications in two active dogs. In one dog, superficial ulceration of a region of the flap adjacent to the pad required surgical revision. This dog continued to show mild lameness after daily runs of 3 to 4 km.  相似文献

This report describes a novel technique for abdominal wall reconstruction using an internal abdominal oblique muscle flap in an Australian kelpie. En bloc resection of a chondrosarcoma and biopsy scar centred on the 13th rib was performed to include full thickness thoracic wall (12th rib, extending caudally) and lateral abdominal wall (including the vascular pedicle of the external abdominal oblique muscle). The diaphragm was advanced to close the thorax. A flap using the caudal internal abdominal oblique muscle with the base dorsally was elevated and rotated 90° to fill the dorsal defect. The ventral defect was closed using the composite ventral abdominal muscles. The skin was closed primarily. The dog developed a self-resolving seroma. Twelve months postoperatively, the dog was able to engage in agility competitions. A viable muscle flap using the internal abdominal oblique muscle provides a useful alternative to previously described techniques for autogenous closure of a large abdominal wall wound.  相似文献
