Introduction

We sought to determine the feasibility, measurement variability, and within-day repeatability of tricuspid annular plane systolic excursion (TAPSE) measured by two-dimensional echocardiography (2D TAPSE), generate reference intervals for 2D TAPSE, assess agreement and correlation between 2D TAPSE and the conventional TAPSE measured by M-mode echocardiography (MM TAPSE), and to assess the ability of 2D TAPSE to track a drug-induced decrease in right ventricular (RV) function compared with MM TAPSE.

Animals

Seventy healthy privately owned dogs of varying bodyweight.

Methods

All dogs underwent a single echocardiogram to quantify RV function by both TAPSE methods. Ten dogs underwent a second echocardiogram 2–3 h after the first to assess within-day repeatability, and 20 different dogs underwent a second echocardiogram 3-h after atenolol (1 mg/kg per os (PO)). Intraobserver and interobserver measurement variabilities were assessed in 12 randomly selected studies using coefficients of variation. Statistical relationships between 2D TAPSE and bodyweight, gender, heart rate, and age were explored.

Results

2D TAPSE could be measured in all dogs. Coefficients of variation for repeatability and measurement variability were low (≤12%). Bodyweight-dependent reference intervals for 2D TAPSE were generated using allometric scaling. TAPSE methods were strongly correlated (r = 0.72; p<0.0001) but 2D TAPSE measured consistently less than MM TAPSE (?1.6 [2.2] mm) when analyzed by Bland–Altman's method. Both TAPSE methods were significantly (p≤0.014) reduced after atenolol but percent decrease in 2D TAPSE (?16.2 [9.3]%) was significantly greater (p=0.03) than MM TAPSE (?7.5 [13.8]%).

Conclusions

Two-dimensional echocardiography TAPSE appears well suited for clinical assessment of RV function. The TAPSE methods should not be used interchangeably.  相似文献

This case report describes the clinical presentation, the necropsy findings, and genetic results of a 13-year-old Warmblood mare presented with colic and a bilaterally loud, holosystolic murmur. Echocardiographic examination revealed the presence of a thoracic aortic aneurysm, an aortic pseudoaneurysm, a periaortic hematoma (circumferential cuffing by perivascular hemorrhage), and aortopulmonary fistulation. A supravalvular aortic stenosis (SVAS) was visible during echocardiography. Necropsy confirmed that the thoracic aortic aneurysm had ruptured and connected to the pseudoaneurysm, which fistulated into the pulmonary artery. Histologically, the aneurysm wall revealed chronic lesions such as fibrosis, mucin depositions, mineralizations, and elastin fragmentation. The mid abdominal aorta showed lesions suggestive of a systemic elastin arteriopathy. Molecular analysis, however, could not attribute this disease to a variant in the elastin gene, the most common causative gene for SVAS. To the authors' knowledge, this case report describes a case of aortopulmonary fistulation in a Warmblood horse associated with the presence of SVAS and an aortic aneurysm.  相似文献

The objective of the current study was to describe the structures of the pastern and coffin joints in dromedary camel using x-ray, bone and soft tissue windows computed tomography (CT) and three-dimensional volume rendering (3DVR) of CT imaging. 3DVR of CT was obtained at the slight flexed dorsal view, plantar view, dorsolateral view and lateral view which explained all the surfaces and structures of the digit bony parts even the parts of the articular surface. The processed images of 3DVR of CT showed different patterns of the cortical, cancellous, subchondral bones and medullary cavity of the bones of the digits. The present study showed clearly all the hard and soft tissues in the pastern and coffin joints of the camel in CT images; however, the plantar ligaments of the pastern joint and ligaments of the navicular cartilage identified on CT images. The CT soft tissue window visualized the joint cavity and their pouches and tendon sheath of the flexor tendons better than the bone window CT. The radiographic, CT and 3D images could be used as a normal reference for the interpretation of some clinical diseases in the pastern and coffin joints of the camel.  相似文献

In the present study, the ‘acoustic windows’ for the measurement of the left ventricular systolic time intervals is studied by means of 2‐D echocardiography and cardiac Doppler ultrasonography, and the normal values in the horse (n = 112) are determined. The left ventricular isovolumetric contraction time, the pre‐ejection period (PEP), as well as the left ventricular ejection time (LVET) have been measured, and the values of the left ventricular total electromechanical systole (LVTES) and the PEP‐to‐LVET ratio have been calculated. It has been established that the most suitable window for the measurement of the aforementioned indices in 2‐D echocardiography is the right parasternal window in a view in short axis at the level of the cardiac base to measure the aortogram. In Doppler ultrasonography, the preferred window is the left parasternal using the five chambers apical view. The following values have been acquired: PEP = 0.071 ± 0.01 s; LVET = 0.532 ± 0.097 s; LVTES = 0.6 ± 0.1 s and PEP‐to‐LVET = 0.138 ± 0.025 measured by 2‐D echocardiography and PEP = 0.068 ± 0.009 s; LVET = 0.527 ± 0.076 s; LVTES = 0.598 ± 0.098 s and PEP‐to‐LVET = 0.131 ± 0.01 measured by Doppler ultrasonography.  相似文献

IntroductionScreening to assess likelihood of preclinical dilated cardiomyopathy (PC-DCM) prior to advanced diagnostic tests in Doberman Pinschers (DP) is desirable.ObjectiveTo investigate the combined value of physical examination (PE), N-terminal pro B-type natriuretic peptide (NTproBNP) and cardiac troponin I (cTnI) for identifying PC-DCM in DP.Animals, materials and methodsAll dogs underwent: PE, echocardiogram, 3-min ECG and cardiac biomarker measurement. Asymptomatic DP (414) were classified based on 3-min ECG and echocardiogram as: No-DCM/MMVD or myxomatous mitral valve disease (MMVD), PC-DCM based on echocardiogram (PC-DCM-Echo), PC-DCM based on arrhythmias with a normal echocardiogram (PC-DCM-ECG), equivocal DCM (EQ-DCM), and MMVD. Receiver operator characteristic curves and prediction models were derived.ResultsHeart murmurs and arrhythmias were rare and gallop sounds were absent in No-DCM/MMVD DP. Dogs ≥ four years old and males had higher probabilities of PC-DCM-Echo. Prediction models incorporating PE variables with NTproBNP had an area under the curve (AUC) of 0.940 for distinguishing between PC-DCM-Echo and all other groups, which was similar to the AUC for NTproBNP (0.939) or cTnI (0.932) alone. Discrimination between No-DCM/MMVD and all other groups was similar for NTproBNP (0.781) and cTnI (0.742) as individual tests, however, models combining PE variables and NTproBNP increased the AUC to 0.812. An NTproBNP cut-off of ≥548 pmol/L, was 100% sensitive and 77.3% specific for detecting PC-DCM-Echo.ConclusionsBoth NTproBNP and cTnI had good utility as sole tests to discriminate PC-DCM-Echo DP from all others. Models differentiating No-DCM/MMVD DP from all other DP were improved by using PE and NTproBNP together.  相似文献

Echocardiography     
Diagnostic ultrasound permits the clinician to image the beating heart, quantitate cardiac dimensions, identify specific congenital and acquired cardiac lesions, and estimate the degree of cardiac compensation and muscle failure that accompanies a specific lesion. The M-mode (motion) echocardiogram and two-dimensional echocardiogram are complementary studies that have proved useful for identification of cardiac septal defects, endocarditis, pericardial effusion, intracardiac shunting, cardiomegaly, and heart muscle failure. Appreciation of the technique and basic knowledge of the types of information that can be obtained from echocardiography are necessary to appropriately refer horses for these studies.  相似文献

Hyperthyroid cats were examined by electrocardiography, radiography, and echocardiography for the presence of cardiomegaly. These examinations were performed at the time of the initial diagnosis of hyperthyroidism and repeated during the course of antithyroid medication and after thyroidectomy was done. If the hyperthyroid cats had cardiomegaly, reduction in cardiac size was detected once the euthyroid state was reestablished. This decrease in size was appreciated electrocardiographically and radiographically. The echocardiogram demonstrated a decrease in left atrial size. A consistent decrease in left ventricular wall thickness was not observed by the echocardiogram, although 2 cats did have resolution of free wall hypertrophy.  相似文献

Pulmonary atresia and ventricular septal defect (PA-VSD) was diagnosed in a 2-year-old castrated male Terrier mix. Transthoracic echocardiography identified a large ventricular septal defect, overriding aorta and severe right ventricular hypertrophy. A main pulmonary artery could not be identified, consistent with pulmonary atresia or persistent truncus arteriosus. Transesophageal echocardiography and angiography confirmed PA-VSD with aortopulmonary collateral circulation arising from the descending thoracic aorta. This case report describes the antemortem diagnosis of the rare congenital defect PA-VSD in an adult dog.  相似文献

In healthy cats sedated with a combination of ketamine hydrochloride and acepromazine, the reproducibility of sequential echocardiographic examinations was studied. The effects of experimental alteration in heart rate, contractility, and preload on the echocardiogram also were studied. Ten cats had M-mode echocardiograms recorded once weekly for 5 weeks. There were no statistically significant differences in echocardiographic measurements over time, although measurements of certain cardiac structures of an individual cat did vary from time to time. The cardiovascular system was altered by a constant infusion of isoproterenol, followed by a rapid intravenous infusion of isotonic fluid equal to 25% of the calculated blood volume, and then by a rapid withdrawal of blood equal to the amount of fluid previously given. Cardiac output using indocyanine green dilution curves were determined for each treatment and compared to the cardiac output calculated from the echocardiogram. The correlation coefficient was 0.90 for all treatments. During the infusion of isoproterenol a significant increase in the percent fractional shortening and left ventricular wall amplitude occurred compared to the control echocardiogram. After the rapid infusion of fluid, none of the mean echocardiographic measurements significantly changed from control values although the left ventricular chamber diameter at systole significantly increased, and the percent fractional shortening significantly decreased, when compared to values obtained during isoproterenol infusion. Significant changes recorded between echocardiographic measurements made after blood withdrawal compared to those made after fluid loading included an increase in left ventricular wall thickness at diastole, a decrease in left ventricular diameter at diastole, and a decrease in the present left ventricular wall thickening. Cats were terminated and the average of measurements made during the repeatability study of the septal and left ventricular wall thickness during systole, rather than during diastole, more closely approximated the postmortem measurements.  相似文献

The aorta can rupture at the aortic root or aortic arch. In most breeds, the aortic root is the likely site and rupture leads to aortocardiac fistula with communication between the aorta and the right atrium, right ventricle and/or the interventricular septum. There is a high prevalence of aortic rupture in young Friesian horses and rupture occurs at the aortic arch with pseudoaneurysm and potentially aortopulmonary fistulation. Echocardiographic and post‐mortem techniques must be adapted to identify aortic arch rupture that is not generally identified with standard approaches. Given the narrow genetic base of the Friesian breed and the significant differences found in extracellular matrix composition and metabolism between Friesians and Warmbloods, genetic factors are likely to contribute to the condition in the Friesian breed.  相似文献

A 7 yr old castrated male Labrador retriever (35.6 kg) was evaluated for an acute onset of vomiting of 24 hr duration. On initial examination, the patient was febrile (103.8°F) and tachycardic (150 beats/min). Thoracic radiographs revealed left atrial enlargement with mild pulmonary infiltrates. The dog's condition worsened and repeat radiographs revealed worsening pulmonary infiltrates and pleural effusion. Treatment for heart failure was initiated. An echocardiogram showed a large 3 cm × 4 cm vegetation on the atrial surface of the posterior mitral valve. The patient was euthanized due to poor clinical appearance and infective endocarditis was suspected. Necropsy revealed an osteosarcoma of the posterior mitral valve, which cultured negative.  相似文献

Virtual otoscopy enables noninvasive 3D endoluminal imaging of the middle ear through postprocessing of computed tompgraphy (CT) data. A standardized imaging approach for the middle ear was established in six normal dogs in an attempt to optimize the clinical application and student education. High‐resolution CT data were obtained. Virtual otoscopic images of the middle ear cavity and ossicles were generated using commercially available software. The views of the four different directions (the ear canal, tympanic bulla, eustachian tube, and ossicular chain) were made for virtual otoscopy. The promontory, cochlea window, tympanic bulla, septum bulla, and auditory tube were distinguished easily and clearly. One of the ossicles, the malleus, was visualized accurately. However, small structures such as the incus and stapes always could not be seen. The main advantage of virtual otoscopy is not only to provide diagnostic information but also to enhance the quality and efficiency of student education, because it contributes to an understanding of the anatomy of the middle ear. We describe the normal topographical 3Dimages of the middle ear of the dog using virtual otoscopy.  相似文献

A one‐year‐old African pygmy hedgehog (Atelerix albiventris) was evaluated for severe respiratory distress. Physical examination findings included marked dyspnoea, cyanosis and tachypnoea. Radiographic findings included an enlarged heart and pulmonary oedema, and dilated cardiomyopathy was confirmed via echocardiogram. The patient was treated for congestive heart failure because of dilated cardiomyopathy with furosemide, enalapril, pimobendan and l‐carnitine. Within 24 hours of treatment, the pulmonary oedema and cyanosis had resolved. Following discharge, clinical improvement was noted by the owner and echocardiogram confirmed improved fractional shortening. Cardiomyopathy has been reported at post‐mortem examination in hedgehogs, but there are no reports of ante‐mortem diagnosis and treatment. Performing baseline cardiac assessment in hedgehogs is recommended, and treatment with l‐carnitine and pimobendan may improve outcome, as carnitine deficiency is a possible cause of cardiomyopathy in hedgehogs. Successful emergency treatment of congestive heart failure in the hedgehog of this report may be effective for other hedgehogs presented with similar clinical signs .  相似文献

A 4-year-old Boxer dog with a history of ventricular premature contractions was evaluated for anorexia. An electrocardiogram revealed atrial dissociation and an echocardiogram supported simultaneous mechanical atrial systole with the ectopic atrial electrical activity. Two domestic shorthair cats, a 6-month-old castrated male and a 7-month-old intact female, were diagnosed with complex congenital heart disease. Transthoracic echocardiography in both cats revealed a dilated arterial trunk overriding the interventricular septum with a large ventricular septal defect. The pulmonary trunk and branch pulmonary arteries were not visible using standard echocardiographic views in either cat. The differential diagnosis for both cats included truncus arteriosus communis vs. pulmonary atresia with ventricular septal defect. Each cat underwent computed tomography angiography to determine the origin and extent of the pulmonary blood supply and to better define extra-cardiac anatomy. Computed tomography angiography led to a diagnosis of truncus arteriosus communis with unrestricted pulmonary blood flow in one cat, whereas the other cat was diagnosed with pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals serving as the primary source of pulmonary blood flow. Computed tomography angiography allowed for the ante mortem differentiation of truncus arteriosus communis from pulmonary atresia with ventricular septal defect in these two cats, leading to an accurate diagnosis and providing valuable information to therapeutic decision-making for each case.  相似文献

A 12-week old, entire female Border terrier weighing 3·5 kg was presented for investigation of a continuous left heart base murmur. The clinical presentation and preoperative echocardiogram were consistent with a standard morphology of patent ductus arteriosus (PDA) but a discrete ductal vessel was not identified during surgical dissection. Surgery had to be abandoned due to deterioration of the patient's condition under general anaesthesia which led to cardiorespiratory arrest and death despite attempts at resuscitation. Necropsy identified a recess within the wall of the aorta communicating with the pulmonary artery via an ostium at the heart base which determined this structure as an intramural PDA. This morphology of PDA is previously unreported. This report demonstrates that an intramural PDA is not readily identifiable surgically because of the absence of a discrete ductal vessel and it is important to appreciate that unusual morphologies of PDA may occur.  相似文献

A 7-month-old female, spayed border collie was referred to the Ontario Veterinary College due to a continuous murmur noted by the referring veterinarian prior to ovariohysterectomy. Auscultation confirmed a grade VI/VI continuous murmur. An echocardiogram confirmed patent ductus arteriosus (PDA). OBJECTIVE: To evaluate a femoral window technique for retrieval of cemented total hip prostheses. STUDY DESIGN: Retrospective clinical study. ANIMALS: Twelve dogs with infection of a cemented modular total hip prosthesis. METHODS: Implant removal was performed by an extended craniolateral approach to the hip and proximal femur without trochanteric osteotomy. The femoral cement mantle was fragmented and removed with simple orthopedic instrumentation by a lateral femoral window that was repaired using cerclage wires. Surgical technique, intraoperative and postoperative complications, bacterial culture results, histopathologic findings, and completeness of cement removal were recorded. Follow-up radiographs were taken 5 to 9 weeks postoperatively. Long-term follow-up information was obtained by client questionnaire. Functional outcome was assessed by scoring ability to stand, sit, walk, run, play, climb stairs, and get into a car. RESULTS: Prosthesis retrieval was performed 2 to 41 months after implantation (median, 14 months). Complete removal of femoral cement was achieved in 10 dogs. A nondisplaced femoral fissure, extending proximally from the window, was an intraoperative complication in 2 dogs. Staphylococcus spp was most commonly isolated (6 dogs) from interfacial membrane samples. Systemic antibiotic therapy, dependent on susceptibility testing, was administered for 3 to 10 weeks postoperatively. There was radiographic evidence of osteotomy healing at 5- to 9-week reassessment. Recurrence of osteomyelitis was not observed. Long-term functional outcome was considered mildly abnormal. CONCLUSIONS: The lateral window was an effective technique for retrieval of retained femoral cement. CLINICAL RELEVANCE: Removal of an infected prosthesis using this technique generally resulted in a clinical outcome comparable to that with ab initio femoral head and neck excision.  相似文献

A 3-year-old castrated male domestic shorthair presented to the Cornell University Hospital for Animals for acute onset respiratory distress. Thoracic radiographs, echocardiogram, and electrocardiogram (ECG) revealed left-sided congestive heart failure, myocardial thickening with left atrial dilation, and sinus rhythm conducted with a left bundle branch block, respectively. Cardiac troponin I was elevated and continued to increase over 36 h (1.9 ng/mL, 3.1 ng/mL, and 3.5 ng/mL, sequentially every 12 h). The cat tested positive for Bartonella henselae and was treated with azithromycin (30 mg/kg by mouth (PO) every 24 h for 30 days), along with furosemide (1 mg/kg PO every 24 h), benazepril (0.4 mg/kg PO every 24 h), pimobendan (0.23 mg/kg PO every 12 h), and clopidogrel (18.75 mg PO every 24 h). Reevaluation at 6 weeks revealed normal respiratory rate on physical examination, normal cardiac structures and function on echocardiogram, resolution of left bundle branch block on ECG, and normal cardiac troponin I levels (0.06 ng/mL). All medications were discontinued at this time, and the cat continued to do well 5 months after reevaluation. Here, we report a case of transient myocardial thickening in a cat that was also positive for B. henselae.  相似文献
