ABSTRACT

Equine dentistry is a rapidly developing clinical specialty. It has benefitted from key advances in anatomical and physiological research, development of equipment and instrumentation, utilisation of standing sedation and anaesthesia protocols, a change towards minimally invasive surgical techniques, and the introduction of restorative and endodontic techniques translated from techniques used in human and canine dentistry. Anatomical research has provided further insight into the endodontic system of incisors and cheek teeth and how it changes throughout development with age. Studies of the periodontium have demonstrated a rich vascular supply and repair capacity. Routine dental visits are increasingly being performed utilising sedation and clinical instruments for routine examinations. Equipment has become more efficient, battery-operated and miniaturised giving benefits to both equine dental maintenance work and advanced techniques, assisting the transition to minimally invasive techniques, and the development of endodontic and restorative dentistry. Diagnosis has also benefitted from advances in equipment such as patient-side digital radiography systems, high definition oroscopy, and small diameter flexible fibrescopes that are capable of visualising inside a pulp canal. Dental units combining endodontic high- and low-speed drills, suction and air or water flush are becoming increasingly used and adapted for equine use. Sedative combinations and standing anaesthesia protocols have meant that revisions of traditional techniques, as well as novel techniques, can be performed with almost no requirement for general anaesthesia. Equine dentistry can only continue to advance in this way if there is early identification of dental disease through clinical oral examinations, leading to a system based on prophylaxis, as in human dentistry. This necessitates a change in attitude of the public and industry in general to a proactive approach, with early intervention based on examination and diagnostic findings of practitioners and not necessarily based on the clinical signs displayed by the patient.  相似文献

While the use of external skeletal fixation was once associated with substantial postoperative morbidity, clinical and experimental studies have led to technological advances and modifications in application techniques that have greatly improved the results obtained with this treatment modality. The past decade saw numerous advances in external skeletal fixator implants, components and instrumentation, including improvements in fixation pin design, and the development of new linear external skeletal fixation systems and economical circular external skeletal fixation systems specifically engineered for use in dogs and cats. In addition, a greater understanding of fixator biomechanics and the pathobiology of the bone-fixation pin interface have improved fixator application practices. This article reviews many of the more significant recent advances in external skeletal fixation.  相似文献

Safe, effective surgery can be performed in the standing, conscious horse using a combination of mechanical and chemical restraint. Clear indications for performing standing abdominal surgery exist, but many procedures are best performed under general anesthesia. The preferred approach involves a modified grid incision of the left paralumbar fossa. Flank celiotomy allows the surgeon to thoroughly explore the abdominal cavity, but few structures can be exteriorized, and visibility of abdominal contents is poor. Indications for standing abdominal surgery include diagnosis of abdominal masses, drainage and biopsy techniques, correction of left dorsal displacement of the large colon, and evaluation of rectal injuries, and performing loop colostomy techniques, laparoscopy, removal of retained testicles, correction of uterine torsions, surgical embryo transfer, ovariectomy in normal mares, and some experimental procedures. Standing surgical techniques are most useful and appropriate for surgical exploration, to correct uterine torsions, and to perform loop colostomy and surgical embryo transfer techniques. Perioperative antimicrobial and antiinflammatory therapy is recommended. Mild discomfort and ventral incisional swelling after surgery are expected.  相似文献

Laparoscopy is a minimally invasive procedure that has applications as a diagnostic, therapeutic and prognostic technique. Specialized equipment is necessary to perform equine laparoscopy, and there is a large range of instruments, both disposable and non-disposable available. Laparoscopic procedures described include ovariectomy, cryptorchidectomy, adhesiolysis and herniorrhaphy. Laparoscopy can be performed in a standing or dorsally recumbent position, depending on surgeon preference, patient status and the procedure to be performed. Stapling equipment is frequently used in gastrointestinal surgery in horses. Advantages include decreased surgical time and a decrease in the risk of contamination. Stapling equipment is often used in creating anastomoses, both in the large and small intestines, as well as in vessel ligation. New surgical techniques intended to decrease adhesion formation include the use of carboxymethylcellulose and bioresorbable patches. Indwelling abdominal drains can be used for peritoneal lavage following surgery and also appear to decrease the risk of adhesion formation. Improvements in post-operative care, including the treatment of post-operative ileus and endotoxaemia can significantly improve the outcome of horses that have undergone surgery for abdominal disorders. Recommendations for the use of prokinetic agents in horses with ileus vary widely. Prokinetic agents include local anaesthetics, macrolide antimicrobials, cholinergic agonists and dopamine antagonists. Endotoxaemia is common in horses following surgery for gastrointestinal disorders. The antibiotic polymyxin B binds to the circulating endotoxin molecule, decreasing its half-life in the intra-vascular space and reducing associated inflammation. This drug appears to be an effective and affordable treatment option for horses with endotoxaemia. The use of specific cyclooxygenase inhibitors in veterinary medicine have been studied recently. Selective cyclooxygenase-2 inhibitors may provide comparable anti-inflammatory and analgesic properties to the non-selective non-steroidal anti-inflammatory drugs. These drugs appear to have similar clinical effectiveness and will hopefully minimize deleterious side effects. The optimal healing of ventral midline incisions in horses is related to many factors including appropriate suture patterns and bite size, in addition to appropriate post-operative exercise recommendations. Recent advances in surgical techniques and post-operative care should decrease the morbidity and mortality associated with abdominal surgery. Familiarity with instruments covered in this article and with advances in veterinary endodontics has become necessary for the veterinary dental practitioner who wishes to practice dentistry using current techniques. Veterinary endodontic techniques that help to retain the teeth longer are increasing in demand as an option to extracting damaged teeth. Endodontic treatment involves removal of the irreversibly damaged pulp, followed by cleaning and shaping of the root canal space using endodontic instruments and subsequent filling, or obturation, with a semisolid material and a sealer.  相似文献

REASONS FOR PERFORMING STUDY: Previous studies of sinus surgery on standing horses have been based on techniques which utilise powered hand bone saws and preserve the bone flap, which was thought to be essential for a good cosmetic result. This report describes a simplified technique applied to the standing horse where the sinus flap construction used a large skull trephine and where the bone flap is necessarily discarded. OBJECTIVES: To assess whether the modified standing frontonasal flap (SFF) surgery offers an effective method to investigate and/or treat sinunasal disorders in the horse. METHODS: The case records of 60 horses subjected to modified SFF surgery were reviewed to analyse the efficacy of the technique when applied to a range of sinunasal disorders in terms of diagnosis, surgical findings, complications and long-term outcomes, including cosmetic effect. RESULTS: Resolution of clinical signs was achieved in 54 out of 60 horses, and for the remainder the SFF technique proved useful on a diagnostic basis. The cosmetic result was, in the opinion of the owners, excellent or satisfactory in 48 of 56 (86%) horses. CONCLUSIONS: The modified SFF technique was found to be practicable and provided satisfactory exposure of the sinus contents for diagnosis and removal of diseased tissue in a range of disorders. An acceptable cosmetic result can be obtained in the majority of horses despite rejection of the disc of overlying bone. Apart from avoiding the risks associated with general anaesthesia, the advantages of the modified SFF sugery are: a reduction of haemorrhage in the standing horse; reduced mucosal engorgement; straightforward anatomical orientation; a comfortable working height; and reduced surgery time. A sound knowledge of the functional anatomy of the area is a prerequisite before embarking on any form of sinunasal surgery. Complications are infrequent and usually transient. POTENTIAL RELEVANCE: The modified SFF technique is a valuable alternative to other invasive methods of sinunasal surgery.  相似文献

OBJECTIVE: To describe a technique for laparoscopic cryptorchidectomy in standing horses using electrosurgical instrumentation. STUDY DESIGN: Retrospective clinical study. ANIMALS OR SAMPLE POPULATION: Ten horses, 1 to 7 years of age, with unilaterally or bilaterally retained testes. METHODS: Food was withheld for a minimum of 12 to 24 hours. Horses were sedated using xylazine hydrochloride (0.5 to 1 mg/kg) and butorphanol tartrate (0.02 mg/kg) or detomidine hydrochloride (0.02 to 0.03 mg/kg) and restrained in standing stocks. Three portal sites in the paralumbar fossae were locally desensitized using 2% mepivacaine. After trocar and laparoscope insertion, the ipsilateral testicle, mesorchium, and ductus deferens were identified. The cranial mesorchium was coagulated with either monopolar (one horse) or bipolar (nine horses) electrosurgical forceps, and then the mesorchium, ductus deferens, and ligament of the tail of the epididymis were transected from cranial to caudal using laparoscopic scissors. Once the testis was freed, the transected mesorchium was inspected for hemorrhage and the testis was removed by connecting the two instrument portals (eight horses). In two horses, the testis was placed within a laparoscopic retrieval bag and then removed without enlarging the portal incision. If the testes were retained bilaterally, the retained contralateral testis was removed similarly through the opposite paralumbar fossa. If the contralateral testis was descended, it was removed by a standard, standing castration technique. RESULTS: Vessels of the mesorchium were adequately coagulated using bipolar and monopolar electrosurgical forceps. No immediate or short-term complications occurred in 10 horses at 3 to 11 months after surgery. CONCLUSION: Standing laparoscopic cryptorchidectomy can be performed easily and safely using electrosurgical instrumentation as the sole means of providing hemostasis of the equine mesorchium. CLINICAL RELEVANCE: Standing laparoscopic cryptorchidectomy using electrosurgical instrumentation provides a safe, reliable, and efficient alternative to achieve hemostasis of the equine mesorchium.  相似文献

Because many exotic pets are so small, the instrumentation and equipment used for surgery are different from in larger animals. Magnification greatly improves the surgeon's ability to accomplish surgeries in small patients and helps to provide hemostasis because small amounts of blood are significant in these tiny patients. Microsurgical instrumentation is important to allow for proper tissue handling and suture placement. This article reviews recent advances in both technology and technique that have greatly increased the success rate of retinal reattachment surgery. Indications for performing orthopedic surgery on the standing horse include inability to tolerate general anesthesia, risk of worsening an injury during recovery from anesthesia, and cost. The surgeon should be aware that performing surgery in the standing horse can be more demanding and require more experience than the same procedures when the time and convenience of general anesthesia are available. Improved sedatives and analgesics have allowed more latitude because the horses now are more tolerant than when older agents were used. Common sense should be applied to each situation before the decision is made to do a procedure, because every horse and problem is unique.  相似文献

Standing female urogenital endoscopic surgery is facilitated by the more dorsal location of the organs of the female reproduction tract. The most common reason for laparoscopic surgery on the female urogenital system is ovariectomy; however, the technique has been used to diagnose periparturient or reproductive diseases and to perform surgical embryo transfer. Standing surgical approaches avoid the risk and expense of general anesthesia, but these techniques are limited by the temperament and size of the patient and the availability of facilities for restraint. Owner acceptance of laparoscopic procedures is excellent. It is anticipated that standing endoscopic surgery may become the preferred technique for applicable procedures.  相似文献

This case report describes a 17‐year‐old horse that presented with a history of oral dysphagia secondary to buccal slab fractures and apical infection in two cheek teeth. During standing surgery to remove the diseased cheek teeth, the horse sustained a bilateral comminuted fracture of the premaxilla. An audible crack was followed by bilateral epistaxis. Subsequent radiography revealed minimal displacement at the fracture sites and the horse responded well to conservative management without the need for additional fixation techniques. One year later, the fracture appeared completely healed on repeat radiography. This is an uncommon complication of standing dental procedures necessitating the use of a speculum but is noteworthy and may be important to consider and inform owners in such aged cases.  相似文献

Rupture of the urinary bladder is rare in adult horses with various standing or recumbent surgical repair techniques historically reported as the treatments of choice. This case series describes the successful conservative management of cystorrhexis in 4 adult horses. Conservative management is therefore a potentially viable treatment option which forgoes the risks of general anaesthesia and may reduce the post operative complications associated with surgery of the bladder.  相似文献

Intrasinus neoplasia remains a rare but difficult condition to diagnose and treat in the horse, comprising approximately 8–19% of sinonasal disorders. There are, however, only a few case series upon which to base an approach to diagnosis and management ( Cotchin 1967, 1977 ; Madewell et al. 1976 ; Stunzi and Hauser 1976 ; Sundberg et al. 1977 ; Priester and McKay 1980 ; Boulton 1985 ; Hilbert et al. 1988 ; Dixon and Head 1999 ; Head and Dixon 1999 ; Tremaine and Dixon 2001a,b ). Squamous cell carcinoma (SCC) is the most common neoplasm observed in the equine paranasal sinuses. Evidence from other species would indicate that early recognition of SCC is crucial to the success of treatment and the ideal treatment remains complete excision with margins. Sinus involvement generally precludes this and we must often settle for surgical debulking, with or without adjunctive radio‐ or chemotherapy. In horses, as in other species, early recognition is difficult because clinical signs are nonspecific. Treatment is, therefore, often not attempted due to the extensive nature of lesions at presentation and the limited surgical access. The accompanying article by Kowalczyk et al. (2011 ) showed how 3‐dimensional (3D) imaging can identify the hallmark changes associated with aggressive neoplasia in the equine sinuses ( Kowalczyk et al. 2011 ). The value of computed tomography (CT) and magnetic resonance imaging (MRI) lies in noninvasive early diagnosis as well as lesion monitoring post intervention. Where CT can be performed with the horse in the standing position, avoidance of general anaesthesia offers further value, especially as standing surgical techniques now allow thorough, minimally invasive evaluation and biopsy of the equine sinuses. In combination, standing CT and minimally invasive sinus surgery allow accurate and early diagnosis and monitoring of disease progression, opening the door for advances in surgical and adjunctive treatments for this complex condition.  相似文献

Eyelid neoplasms are common in the older dog and typically benign. Eyelid neoplasms in the cat are less common and more often malignant. Resection of eyelid masses may be curative; however, restoration of the eyelid structure after mass excision is essential for maintaining long-term ocular surface health. Surgical techniques and instrumentation for eyelid surgery are reviewed. Indications, benefits. and limitations of sharp excision, cryotherapy, and laser excision and ablation are discussed. Accurate estimation of standing crop and herbaceous fuel moisture content (FMC) are important for grazing management and wildfire preparedness. Destructive sampling techniques have been used to accurately estimate standing crop and FMC, but those techniques are laborious and time consuming. Existing nondestructive methods for estimating standing crop in tallgrass prairie have limitations, and few studies have examined nondestructive estimation techniques for FMC in this environment. Therefore, our objective was to develop robust models for nondestructive estimation of standing crop and FMC in tallgrass prairie. We calibrated and validated stepwise multiple linear regression (SMLR) and artificial neural network (ANN) models for standing crop and FMC using data collected in tallgrass prairies near Stillwater, Oklahoma. Day of year (DOY), canopy height (CH), Normalized Difference Vegetation Index (NDVI), and percent reflectance in five wavelength bands were candidate input variables for the models. The study spanned two growing seasons and nine patches located within three pastures under patch burn management, and the resulting data set with > 3 000 observations was split randomly with 85% for model calibration and 15% withheld for validation. Standing crop ranged from 0 to 852 g m? 2, and FMC ranged from 0% to 204%. With DOY, CH, and NDVI as predictors, the SMLR model for standing crop produced a root mean squared error (RMSE) of 119 g m? 2 on the validation data, while the RMSE of the corresponding ANN model was 116 g m? 2. With the same predictors, the SMLR model for FMC produced an RMSE of 26.7% compared with 23.8% for the corresponding ANN model. Thus, the ANN models provided better prediction accuracy but at the cost of added computational complexity. Given the large variability in the underlying datasets, the models developed here may prove useful for nondestructive estimation of standing crop and FMC in other similar grassland environments.  相似文献

Standing laparoscopy in the horse was used initially as a diagnostic tool to assist in the diagnosis of a cryptorchid testicle. Now, this technique is also used for treatment of cryptorchid testicles. The focus of this article is the standing laparoscopic cryptorchidectomy, the most common laparoscopic procedure performed on the male equine urogenital tract.
