A 3-month-old Quarter Horse colt sustained a penetrating injury over the coronary band on the right forelimb resulting in an articular bone sequestrum and septic arthritis of the distal interphalangeal joint. Despite aggressive treatment for septic arthritis, severe osteoarthritis and ongoing sepsis resulted in persistent lameness. Facilitated ankylosis of the distal interphalangeal joint was performed using a combination of open surgical debridement, placement of an autogenous cancellous bone graft and stabilisation within a transfixation cast. Bony ankylosis of the joint was observed radiographically 7 weeks following surgery. Evaluation of the horse 30 months following surgery revealed complete fusion of the middle and distal phalanges, and the distal sesamoid bone. The horse is pasture sound and is used as a breeding stallion. This report describes the surgical approach and case management employed to achieve facilitated ankylosis as a treatment for unresolved septic arthritis of the distal interphalangeal joint. This is the first report of successful facilitated ankylosis of the distal interphalangeal joint in a foal.  相似文献

An 8-month-old Holstein heifer was evaluated for right hind limb lameness of 3 weeks' duration. Diagnoses were osteomyelitis and fracture of the distal sesamoid bone, septic arthritis of the distal interphalangeal joint, and osteomyelitis of the third phalanx. After excision of a section of tissue from the plantar aspect of the digit and partial section of the deep digital flexor tendon, the distal sesamoid bone was excised. The third phalanx was curetted, and the surgical wound was lavaged. Twenty months after surgery, the heifer was fully weightbearing on the affected digit. Surgical exploration of the digit should be considered an alternative to claw amputation in cattle that have severe digital infections.  相似文献

Objective— To determine in cattle with septic arthritis of the distal interphalangeal joint (SADIJ) the efficacy of resection of the distal interphalangeal joint (JRES) as a digit salvage technique compared with digital amputation (DAMP).
Study Design— Prospective, randomized clinical study.
Animals— German Holstein–Friesian dairy cattle with SADIJ of 1 hind limb (n=52).
Methods— SADIJ diagnosis was based on clinical examination and radiography. Cows were randomly assigned with owner consent to DAMP (n=26) or JRES (n=26). After JRES, a wooden block was fixed to the partner claw in combination with a tipping claw prophylaxis.
Results— After surgery, degree of lameness improved significantly faster after DAMP than after JRES. New claw diseases in the opposite limb occurred more frequently after JRES (n=6) than after DAMP (n=1). New claw defects developed in the partner claw on the operated limb in 6 cows after DAMP compared with 1 after JRES. Tipping claw was observed in 50% of JRES cows at day 180. Mean life span between groups was not significantly different (DAMP=13.5 months, JRES=10.9 months).
Conclusion— Higher surgical and postsurgical expenditures for JRES were not counterbalanced by a longer productive life; however, frequent disease of the partner claw of cows after DAMP should be considered a critical point, as this generally leads to culling.
Clinical Relevance— The higher expenditure for JRES can be justified only for young, valuable cattle.  相似文献

This review describes the indications for surgical treatment of distal interphalangeal joint (DIJ) infection in cattle and presents an overview of former surgical methods that have been modified for current use, including arthrotomy, resection of the distal sesamoid bone, resection of the DIJ and digital amputation. The post-operative life span, reasons for culling, pros and cons of claw-preserving techniques versus digital amputation, and pain management in general are also discussed. It would appear that severe claw diseases that result in septic arthritis of the DIJ do not necessarily lead to a reduced life span if cows are adequately treated and thorough aftercare is provided.  相似文献

A 4-year-old beefmaster cow was examined for a left hind leg lateral claw lameness due to septic arthritis of the distal interphalangeal joint and associated osteomyelitis of the second and third phalanges. Actinomyces pyogenes and Fusobacterium necrophorum, which have been demonstrated previously to act synergistically in ovine heel abscesses, were isolated from the affected digit. A claw amputation was performed because of the advanced destructive nature of the lesion.  相似文献

A 6-month-old 300-kg Quarter Horse filly was treated for septic arthritis of the distal interphalangeal joint and septic navicular bursitis that developed as a result of a deep puncture to the foot. Initial treatment consisted of establishing ventral drainage for the navicular bursa, lavage of the distal interphalangeal joint, and administration of broad-spectrum antimicrobial drugs and non-steroidal anti-inflammatory drugs. Because of continuing sepsis in the distal interphalangeal joint, subsequent treatment included packing the defect in the bottom of the foot with cancellous bone in an attempt to prevent ascending contamination of the joint, placing the limb in a short limb cast, and inserting a Penrose drain into the joint for passive drainage of septic exudate. The goal of treatment was to encourage ankylosis of the distal interphalangeal joint. Because of the filly's persistent lameness and laxity of the lateral collateral ligament in the contralateral carpus, the palmar nerves of the affected foot were injected with a long-acting local anesthetic at the level of the proximal sesamoid bones to encourage weight-bearing. Ankylosis of the distal interphalangeal joint was complete 9 months after the puncture, but a grade-2 lameness remained and the horse had a varus deformity resulting from ligamentous laxity of the lateral collateral ligament in the contralateral carpus.  相似文献

A modified technique for the resection of the distal interphalangeal joint and the proximal resection of the deep digital flexor tendon in cows is described. Septic arthritis of the joint was diagnosed in eight Holstein cows and treated in the field. Four of the cows were diagnosed with ascending tendonitis during the resection of the joint and their tendons were also resected. All the animals remained moderately to severely lame for two weeks postoperatively but quickly recovered and were sound within five months. Eight months after the last surgery only one cow had been culled, 321 days after its surgery, for reproductive failure. The other seven cows had survived for a mean period of 308 days, with a range from 235 to 392 days.  相似文献

A technique for the resection of the distal interphalangeal joint of cattle with infectious arthritis of the joint is described. The procedure has advantages over amputation of the digit because a weight-bearing claw is retained so that the animal can move on slatted floors and at pasture, and its productive life can be extended.  相似文献

A novel surgical approach to the distal interphalangeal joint of cattle, through the abaxial hoof wall, for the treatment of septic arthritis is described. In seven cattle a rectangular piece of horn (15 x 40 mm) was excised from the abaxial hoof wall and the joint was lavaged with Ringer's solution. Infected and/or necrotic tissue was removed from the joint and gentamicin-impregnated collagen sponges were introduced. The piece of horn was reattached, a rubber block was glued to the contralateral claw, and a bandage applied. Aftercare consisted of changes of bandage, systemic administration of antimicrobial drugs and box rest. At follow-up, six of the cattle were sound but one cow which had been slaughtered five months after surgery had remained slightly lame.  相似文献

The medical records of 12 horses with septic arthritis of a distal interphalangeal joint were reviewed to determine clinical features and response to treatment. Sepsis was caused by trauma or an injection that resulted in an open or contaminated distal interphalangeal joint. All horses were severely lame. Treatment included broad-spectrum parenterally administered antimicrobial drugs (ten horses), percutaneous through-and-through joint lavage (eight horses), indwelling drains (three horses), immobilization of the limb in a cast (three horses), intraarticular injection of sodium hyaluronate (one horse), intraarticular injection of antimicrobial drugs (five horses), curettage of the distal phalanx (one horse), and cancellous bone grafting to promote fusion (one horse). Five horses were euthanatized. Ankylosis of the affected joint developed in five horses, four of which are pasture sound. Two horses treated medically are sound although one underwent subsequent palmar digital neurectomy for treatment of navicular syndrome.  相似文献

Diagnosis of septic arthritis in the distal interphalangeal joint (SADIJ) is often difficult, particularly when arthrocentesis or probing of the joint space are inconclusive. The aim of this prospective study was to investigate the usefulness of ultrasonography in reinforcing a tentative clinical diagnosis of SADIJ in 60 Holstein–Friesian cows with septic arthritis in one distal interphalangeal joint (DIJ). The diagnosis of septic arthritis was based on the results of clinical examination and radiographic images, as well as surgical findings. The DIJ of the partner claw of the same foot and the normal DIJs of 18 healthy Holstein–Friesian cows were used as controls. Visualisation, echogenicity and flow phenomena of the synovial fluid were assessed ultrasonographically. The size of the dorsal pouch of the distal interphalangeal joint (DPDIJ) was determined by measuring two defined distances.The DPDIJ could be imaged in all cows with SADIJ, although the quality of the image varied from good (63%) or moderate (30%) to poor (7%). The echogenicity of the synovial fluid in joints with SADIJ did not differ from those of the partner claws. Flow phenomena were seen in only 30% of the DPDIJs with septic arthritis and in none of the DPDIJs of the partner claws. The DPDIJ was larger in DIJs with septic arthritis than in those of the partner claws and controls (P < 0.05). The dorsopalmar/plantar distance of the DPDIJ indicated a suitable sensitivity (0.97) and specificity (1.0) for the diagnosis of septic arthritis when a threshold value of 6 mm was used. Ultrasonographic examination of the DIJ appeared to be a reliable method for confirming a tentative clinical diagnosis of septic arthritis in cattle.  相似文献

The medical records of 16 horses with necrosis of the collateral cartilage of the distal phalanx were reviewed. Typical history included a laceration or puncture wound over the affected cartilage, resulting in persistent drainage proximal to the coronary band. All horses had signs of lameness. The duration of drainage prior to admission to the hospital ranged from 5 days to 5 months. The involved cartilages were left front lateral (n = 4), right front lateral (n = 5), left rear lateral (n = 1), right rear lateral (n = 4), left front medial (n = 1), and left rear medial (n = 1). The affected cartilage was curetted or resected in 13 horses, and conservative treatment was used in 3 horses. Follow-up information on 15 horses (2 to 72 months after discharge) revealed that 66% were sound and able to perform at their previous level. One horse was euthanatized 2 months after discharge from the hospital because it had septic arthritis of the distal interphalangeal joint. The outcome of treatment for each horse was evaluated with respect to the duration of drainage and the method of treatment. Horses with drainage of less than 1 month's duration had a better prognosis for eventual soundness, compared with horses with drainage of greater than or equal to 1 month's duration prior to initiation of treatment. Nine of the 13 horses managed surgically and 1 of the 3 horses managed conservatively were sound at follow-up evaluation.  相似文献

The medical records of 38 horses with puncture wounds of the navicular bursa were reviewed. Only 12 horses had a satisfactory outcome (breeding or riding). Of the remaining 26 horses, 19 were euthanized, five were sold due to persistent severe lameness, one died, and one was lost to long-term follow-up. Different combinations of conservative management prior to surgical debridement and drainage of the navicular bursa were unsuccessful in resolving the condition. Horses that were treated surgically within 1 week of the injury and had a hind leg affected had the best chance of a satisfactory outcome. Additional wound debridement was necessary in 15 horses after initial surgical treatment. The most common complications encountered were navicular bone osteomyelitis and sepsis of the deep digital flexor tendon. Thirteen of 14 horses that had rupture of the deep digital flexor tendon and subluxation of the distal interphalangeal joint had an unsatisfactory outcome. One mare subsequently developed ankylosis of the distal interphalangeal joint and was a useful brood mare. Two horses that had biaxial palmar digital neurectomy because of persistent lameness were later euthanized because of navicular bone fracture and rupture of the deep digital flexor tendon. Results from limited numbers of bacterial cultures and antibiotic sensitivities suggest that penicillin and an aminoglycoside antibiotic should be used as initial antibiotic therapy. Immediate surgical debridement and appropriate antibiotic treatment are recommended as the minimum therapy for penetrating wounds of the navicular bursa in horses.  相似文献

Total, full thickness hoof wall avulsion is rare in horses. Sometimes complications such as fracture of the distal phalanx, osteomyelitis, septic arthritis and degenerative joint disease have been reported. Limiting motion at the affected site and hoof stabilisation are essential to obtain a good hoof regeneration. This case report reviews the clinical features including diagnostic techniques (radiographic examination and venography) of a Quarter Horse filly presented with a complete and full‐thickness traumatic hoof capsule avulsion complicated by an open fracture of the third phalanx. A transfixation casting technique was performed followed after one month by 2 short limb casts applied for 2 months each. Twenty‐four months after trauma the foot had completely regrown. The dorsal aspect of the hoof wall appears to be remarkably shorter compared to the heels. From a radiographic viewpoint, the lateral aspect of the left hind foot shows diffuse bone remodelling and a deformed distal phalanx.  相似文献

OBJECTIVE: To determine the clinical characteristics and outcome of foals with septic osteitis of the distal phalanx. DESIGN: Retrospective case series. ANIMALS: 22 foals. PROCEDURES: Information obtained from medical records included signalment; clinical, laboratory, and radiographic findings; treatment method; and outcome. Foals included in the study had lameness referable to the foot, radiographic evidence of localized lysis or focal loss of bone density of the distal phalanx, and suppurative discharge or necrosis of the affected bone evident at surgery. Foals with a history or evidence of penetrating wounds or subsolar abscessation were excluded. RESULTS: Mean age of foals at initial evaluation was 40.8 days (range, 3 to 122 days). Twenty-one (95%) foals had lameness as the primary complaint. Lesions consistent with septic osteitis of the distal phalanx localized to specific areas of the bone on the basis of radiographic and surgical findings were located on the solar margin or toe (14/22 [64%]), extensor process (5/22 [23%]), and palmar or plantar process (3/22 [13%]). Hind limbs (18/26 [69%] affected limbs) were more frequently affected. Two foals had > 1 affected limb, 2 had additional sites of osteomyelitis, and 4 had concurrent septic arthritis. Surgical debridement and regional antimicrobial perfusion were performed during general anesthesia. Extensor process lesions were not debrided. Nineteen of 22 (86%) foals survived to be discharged from hospital, and 16 horses reached racing age. Eleven of 16 had race starts, of which 8 had official race starts and 3 had unofficial race starts. CONCLUSIONS AND CLINICAL RELEVANCE: Septic osteitis of the distal phalanx should be considered as a source of lameness in foals with signs referable to the foot and does not necessarily preclude a career in racing. Although infection may occur secondary to bacterial penetration of the hoof or sole, the distal phalanx should also be considered as a potential site for hematogenous septic arthritis or osteomyelitis in foals.  相似文献

Objectives— To report a technique for arthrodesis of the distal interphalangeal joint (DIPJ) with two 6.5 mm Acutrak Plus (AP) compression screws from a solar approach and outcome in 7 cattle.
Study Design— Case series.
Animals— Cattle (n=7) with DIPJ arthritis.
Methods— Retrieved data from medical records of cattle that had undergone DIPJ arthrodesis using 2 AP screws via a solar approach were signalment, history, clinical signs, preoperative blood work, preoperative radiographs, complications, postoperative radiographs, and postoperative lameness evaluation. Outcomes, assessed by owner interview 6–26 months after surgery, were classified as excellent, good, or poor.
Results— Fusion of the DIPJ was evident by 6 months. All cattle returned to full function with minimal lameness and normal appearance to the distal aspect of the limb. Four thoracic limbs and 3 pelvic limbs were affected; 3 medial and 4 lateral claws. Complications included moderate lameness associated with implants (n=2) that resolved after screw removal.
Clinical Relevance— Use of the AP screw system in cattle is an excellent option for DIPJ arthrodesis with minimal postoperative morbidity and excellent return to function.  相似文献

Arthrodesis was performed to treat septic arthritis of the proximal interphalangeal joint of 8 horses. Records of the horses were reviewed to determine outcome and possible factors that influenced success or failure. All horses were female. Seven horses had 1 joint treated and 1 horse was treated for bilateral pelvic limb involvement. The duration of sepsis before surgery ranged from 1 to 66 days. Bone lysis and production was radiographically apparent in 7 horses before surgery. Six horses had multiple bacterial organisms cultured from bone or synovial tissues; 2 horses had single isolates identified. After aggressive curettage, arthrodesis was accomplished with 3 parallel screws in 1 horse, 2 divergent narrow dynamic compression plates in 3 horses, and a single broad dynamic compression plate in 4 horses. Casts were applied to all horses for 1 to 6 weeks. Four horses survived to successful brood mare status. Four horses were euthanized during hospitalization because of continued discomfort or complications of sepsis. Arthrodesis of the proximal interphalangeal joint affected with septic arthritis appears to be an acceptable alternative to euthanasia for some horses.  相似文献

Scedosporium prolificans was associated with arthritis and degenerative osteomyelitis in a 6-year-old Thoroughbred racehorse. The horse was suspected to have an inflammatory lesion of the interosseous tendon, but treatment had resulted in only a minimal response. Shortly after diagnostic arthrocentesis of the left metacarpophalangeal joint was performed, the joint became severely swollen, and radiography of the area revealed lysis of the distal end of the third metacarpal bone, the proximal sesamoid bones, and the proximal end of the proximal phalanx. The horse did not respond to treatment and was euthanatized. At necropsy, severe erosive arthritis and degenerative osteomyelitis of the left metacarpophalangeal joint were seen. Swab specimens of the ulcerated lesions of the articular cartilage were submitted for microbial culture, and Scedosporium prolificans was isolated. Scedosporium prolificans is a newly recognized opportunistic fungal pathogen of humans and animals. In humans, S prolificans typically causes focal locally invasive infections that primarily involve musculoskeletal tissues; most often, infection is a result of penetrating trauma or surgical incision. In immunocompromised patients, fatal disseminated infection can occur. The fungus is resistant to almost all currently available antimycotic agents.  相似文献

Breeding soundness of beef bulls after circumcision: 33 cases (1980-1986)   总被引:1,自引:0,他引:1  
Case records of 33 beef bulls that had preputial prolapses and underwent circumcisions were reviewed. Data retrieved included age, breed, duration of preoperative medical treatment, complications of surgery, length of time from surgery to first breeding, ability of bull to copulate, duration of active breeding, and complications resulting in breeding unsoundness. The mean age of all bulls was 3.5 years (1 to 7 years). Breeds represented included Santa Gertrudis (n = 13), Beefmaster (n = 8), Brangus (n = 7), Brahman (n = 4), and Angus (n = 1). The mean duration of medical treatment before surgery for all bulls was 16.4 days (2 to 38 days). There was no statistically significant difference between the mean duration of medical treatment before surgery between bulls that became breeding sound (16 days), compared with those that were breeding unsound (19 days). However, bulls requiring more than 14 days of medical treatment before surgery were approximately 3 times (relative risk, 2.8) more likely to develop postoperative complications than those bulls requiring 14 days of treatment or less. Eleven bulls (33%) developed one or more postoperative complications. The complications resolved in 4 bulls, but 7 were never able to breed. Of these 7 bulls, 4 had problems directly related to the circumcision, 2 had extensive fibrosis of the prepuce already present at surgery, and 1 was discharged from the hospital with no apparent complications but was never able to breed. Twenty-five of the 33 bulls (76%) were breeding sound for 1 or more years after surgery.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Proximal open comminuted fractures of the fourth metatarsal bone (Mt IV) in eight horses were treated by complete removal of the affected bone and antimicrobial therapy. Two horses had concurrent septic arthritis of the tarsocrural or distal tarsal articulations, and five horses had radiographic evidence of osteomyelitis and sequestration of the affected bone. Five horses became athletically sound for their intended use, two horses with septic arthritis had residual lameness but were pasture sound, and one horse was lost to follow-up. Excision of the entire bone appears to be an acceptable treatment of open comminuted fractures of the proximal one-third of Mt IV that do not respond to more conservative modes of therapy.  相似文献
