In 1998, bluetongue (BT) was introduced in northern Africa and then extended to northern latitudes including the French island of Corsica. Following the outbreaks in Corsica in 2000 and 2001, cross-sectional studies and surveillances have been set up in Corsica and also in the southern part of mainland France, a disease-free area but considered at high risk because of its proximity. The surveillance was based on regular blood sampling of susceptible species and antibody detection by a commercial competitive ELISA kit (cELISA). The performance of this cELISA was evaluated on both field results obtained during the 2001 surveillance campaigns and experimental results. ROC analyses were carried out using RT-PCR results as gold standard for determining the infection status of animals. From all these sets of data, cut-off values optimising the diagnostic accuracy of the test were computed. Their values ranged around the manufacturer's 50% threshold from 41% to 63%. The area under the ROC curve obtained from field data was 0.843 (95% CI: 0.762-0.923). In all our results, it appeared also that the specificity of the cELISA test was always perfect if the cut-off was at least at 80%. This cELISA test does not seem sufficient to diagnose BT disease in animals with BT-like symptoms. However, complementary data are needed to better estimate sensitivity and specificity values of this BT test for its use either as a diagnostic tool in infected areas or as a screening test in BT-free areas. The use and validity of RT-PCR results as gold standard are discussed. As the lack of suitable data strongly limited the applicable analyses, a discussion based on the OIE recommendations about test evaluation is initiated.  相似文献

Bovine Spongiform Encephalopathy (BSE) rapidly evolved into an issue of major public concern particularly when, in 1996, evidence was provided that this disease had crossed the species barrier and infected humans in the UK with what has become known as "variant Creutzfeldt Jakob Disease" (vCJD). The aim of this paper is to describe the European Geographical BSE risk assessment (GBR) that was successfully used for assessing the qualitative likelihood that BSE could be present in a country where it was not yet officially recognized. It also discusses how this can lead to risk-based and therefore preventive management of BSE at national and international levels. The basic assumption of the GBR method is that the BSE agent is initially introduced into a country's domestic cattle production system through the importation of contaminated feedstuffs or live cattle. This is referred to as an "external challenge". The ability of the system to cope with such a challenge is, in turn, referred to as its "stability": a stable system will not allow the BSE agent to propagate and amplify following its introduction, while an unstable system will. The BSE-status of a country assessed by this system was used by the European Commission as the basis for trade legislation rules for cattle and their products. The GBR was an invaluable tool in evaluating the potential global spread of BSE as it demonstrated how a disease could be transferred through international trade. This was shown to be a critical factor to address in reducing the spread and amplification of BSE throughout the world. Furthermore, GBR resulted in the implementation of additional measures and management activities both to improve surveillance and to prevent transmission within the cattle population.  相似文献

The paper describes the specifics of the epidemiology of the Porcine Reproductive and Respiratory Syndrome (PRRS), that is its "behavior" as a communicable disease in porcine populations, and compares them to the general epidemiological characteristics of communicable diseases. This analysis shows that infection with the PRRS virus "behaves" epidemiologically both as an epidemic and as an endemic disease: on the one hand it can spread like an epidemic in na?ve populations, and on the other it seems to linger on infinitely in an affected population with its clinical expression varying from farm to farm like an endemic disease. The paper tries to draw "epidemiological" conclusions on the general methods for controlling and/or eradicating the disease, and to identify areas of further research.  相似文献

An epidemiological characterization of animal sporotrichosis was carried out between 2017 and 2018, in a highly urbanized area in Brazil, including outcomes and the spatial distribution of the cases according to the health vulnerability index (HVI) of the study territory. One hundred and sixty-five cats and four dogs suspected of sporotrichosis were identified by a surveillance and control program previously implemented in the study area. One hundred and five of these animals (62.1 %) were considered positive for Sporothrix spp., of which 103 were cats and two were dogs. Cases predominated among male cats (53.4 %), although gender did not present a statistically significant association (p > 0.05) with sporotrichosis cases. Considering the positive cats, 55.4 % were not neutered and free access to the street was reported as a habit for 86.4 % of them. Roaming free in the streets increased the chance of infection by 2.54 times. We observed a high frequency of euthanasia or death (48.8 %) among 86 infected cats available to be included in the follow-up stage of the study, even when they were treated (46.5 %) and a low cure rate (31 %). The disease spread, unrelated to the HVI in the territory. Data produced suggested that avoiding access to the street seems to have greater importance to sporotrichosis control in cats than neutering and reinforces the importance of health education, especially in relation to responsible feline ownership. The free offer of diagnosis and treatment also should be taken into consideration as important measures to control the disease.  相似文献

The immunological response of foals to Rhodococcus equi: a review   总被引:3,自引:0,他引:3  
Normal horses of all ages regularly show evidence of having responded immunologically to R. equi, thus adding serological support to epidemiological evidence that this organism is a normal intestinal inhabitant. More animals from "diseased" farms show a stronger antibody response when compared with foals from "healthy" farms. Various serological tests have been used to detect evidence of infection and to relate antibody level to severity of disease. Anti-R. equi IgG antibody levels, as measured by ELISA, are raised significantly during natural infection. Clinical severity of pneumonia can be correlated with lower specific antibody responses. Following experimental infection, immunological responses can be detected by complement fixation, indirect immunofluorescence, ELISA, lymphocyte blastogenesis and skin testing. Very little work has been carried out to evaluate vaccines against R. equi infection and results have not been encouraging. Success in treatment has been reported following passive immunisation. Administration of immune leucocyte extracts has had no effect on morbidity or mortality rates. The widespread distribution of this organism, together with the relative infrequency of disease caused by it, suggest that R. equi may initiate infection only in such circumstances as a very high infectious challenge, immunological immaturity or deficiency in the host and genetic predisposition.  相似文献

In a four‐year survey, the course of pasture infestation with infective larvae of Strongylidae and the pattern of development of the Strongylidae infection in the host were investigated. Bovine spongiform encephalopathy (BSE), a member of the transmissible spongiform encepahlopathies, has been a notifiable disease in Turkey since 1997. In 2002, the BSE status of Turkey was assessed by the EU Scientific Steering Committee as "it is likely but not confirmed".This study presents the results of a targeted surveillance study to assess the presence of BSE in the age risk population of Bursa, Turkey. In the assessment procedure, the immunohistochemical detection of protease-resistant prion protein (PrP-Sc) was aimed at and applied to 420 brain tissues of cattle slaughtered in Bursa at an age of 30-months and older. Ovine protozoan myeloencephalitis is a disease of sheep associated with the apicomplexan protozoan Sarcocystis tenella. This paper describes the clinical, pathological and epidemiological findings in three affected flocks from a well-defined geographical area. Clinical signs were restricted to sheep under one year old in their first winter and were first observed at least 42 days after they had been moved off the hill grazing areas to low ground pasture, or after they were given conserved forage. The findings are discussed in relation to the timescale of the life cycle of S. tenella and it is suggested that clinical disease was precipitated by the change from a low level of exposure to infection on the hill to a high level of exposure through contaminated pasture or forage. Possible preventive strategies are discussed.  相似文献

We constructed a stochastic bio-economic model to determine the optimal cost-efficient surveillance program for bovine tuberculosis. The surveillance programs differed in combinations of one or more detection methods and/or sampling frequency. Stochastic input variables in the epidemiological module described the dynamics of infection and the probability of detection. By means of an efficiency frontier, the trade-off between the expected cost and the epidemiological risk parameter relating to the outbreak size was evaluated. The surveillance scheme based on visual inspection of lesions on carcasses at slaughter was optimal given the current prevalence of the disease in the Netherlands if the objective was to minimise the expected costs. However, the efficient set also included two other schemes: slaughterhouse inspection in combination with GAMMA-interferon testing of blood samples and slaughterhouse inspection in combination with two-stage tuberculin testing. The choice ultimately will depend on the risk attitude of the decision-maker; a more-stringent surveillance scheme will be enforced if the expected outbreak size is to be constrained. In future scenarios, ELISA testing of bulk-tank milk in combination with the current slaughterhouse inspection procedure would outperform the surveillance scheme of solely slaughterhouse inspection if ELISA testing of bulk-tank milk becomes feasible.  相似文献

OBJECTIVE: The objective of this study was to determine the sensitivity of abattoir surveillance of intestinal tract lesions for detecting ovine Johne's disease (OJD) under normal meatwork conditions. DESIGN: The design of this study was a diagnostic test validation. The three OJD inspectors were the diagnostic test and follow-up histopathological examination was used for test validation. PROCEDURE: Approximately 1200 sheep were procured from known high prevalence OJD infected farms. The sheep viscera were tagged (numbered) and then examined as they were processed on the abattoir line by three experienced meat inspectors. Their observations were independently recorded on a cassette tape. Specified sections of viscera were prepared and subjected to histopathological examination and these results were compared with the inspector diagnoses. RESULTS: The sensitivity of abattoir inspection for OJD varied between inspectors from 53 percent to 87 percent. The specificity varied from 97 to 100 percent. It appeared that the level of sensitivity for detecting disease was higher in lines of sheep where the disease was more prevalent. It also appeared that formal training was an important aspect in ensuring a high level CONCLUSION: Abattoir surveillance is a very economical and rapid method of assessing the OJD status of sheep. On the basis of these results it is reasonable to suggest that abattoir surveillance has a sensitivity of approximately 70 percent. This technique is useful as an ancillary to other testing regimes for negative assurance programs where a sheep identification system is used.  相似文献

The possibility of the airborne spread of foot-and-mouth disease during the 2001 epidemic in the uk has been investigated in three epidemiological case studies. On the basis of evidence from field investigations, and a simple meteorological analysis, it is concluded that the spread of disease was consistent with the airborne transport of virus. The distances ranged from less than 1 km to 16 km; six of the farms were over 6 km from the source and involved the passage of virus over the sea combined with meteorological conditions which strongly favoured airborne disease transmission. The results of detailed atmospheric modelling demonstrated that airborne virus could have challenged livestock on all the farms studied. However, with one exception the 24-hour average daily concentrations of the virus were significantly below the experimentally estimated threshold for infection. A detailed model intercomparison established that, under stable atmospheric conditions, peak concentrations of virus up to two orders of magnitude higher might have been experienced for short periods, owing to fluctuations within the plume of virus, and model limitations. This finding would significantly reduce the apparent discrepancy between the experimentally estimated threshold for infection and the modelling results.  相似文献

Brucellosis is a zoonotic disease of great animal welfare and economic implications worldwide known since ancient times. The emergence of brucellosis in new areas as well as transmission of brucellosis from wild and domestic animals is of great significance in terms of new epidemiological dimensions. Brucellosis poses a major public health threat by the consumption of non-pasteurized milk and milk products produced by unhygienic dairy farms in endemic areas. Regular and meticulous surveillance is essentially required to determine the true picture of brucellosis especially in areas with continuous high prevalence. Additionally, international migration of humans, animals and trade of animal products has created a challenge for disease spread and diagnosis in non-endemic areas. Isolation and identification remain the gold standard test, which requires expertise. The advancement in diagnostic strategies coupled with screening of newly introduced animals is warranted to control the disease. Of note, the diagnostic value of miRNAs for appropriate detection of B. abortus infection has been shown. The most widely used vaccine strains to protect against Brucella infection and related abortions in cattle are strain 19 and RB51. Moreover, it is very important to note that no vaccine, which is highly protective, safe and effective is available either for bovines or human beings. Research results encourage the use of bacteriophage lysates in treatment of bovine brucellosis. One Health approach can aid in control of this disease, both in animals and man.  相似文献

In France, implementation of systematic screening programs in 2000, as a complement to the mandatory reporting of animals with clinical signs of BSE (passive surveillance), revealed certain limitations of the mandatory system. Indeed, systematic screening showed that some BSE cases were not detected by the clinical surveillance system, implying considerable BSE case under-reporting throughout the epidemic. As the most likely explanation for variant Creutzfeldt-Jakob disease (vCJD) is exposure to the aetiologic agent of BSE, it is essential to reconstruct the French BSE epidemic pattern accounting for this under-reporting. We estimated age- and year-specific incidence rates of BSE by using a back-calculation method. This approach relies on the principle that the number of clinical BSE cases is the consequence of the number of BSE-infected animals after a known incubation time, defined as the time between infection and clinical onset. We generalized this model to take into account epidemiological characteristics of BSE, such as French cattle mortality, BSE case reporting probability, and age-dependent susceptibility and/or exposure to the BSE agent. We confirmed that the average BSE incubation period is five years and that the peak risk of bovine infection occurs between 6 and 12 months of age. The results also showed that the proportion of underreporting is the most influential parameter in the model, and that BSE was substantially underreported until rapid tests were introduced. Indeed, only 103 BSE cases were detected by passive surveillance up to June 2000, while we estimated that there was 301 200 (95% confidence interval (CI) [27 600-837 600]) cattle infected by the BSE agent. Despite uncertainty over the beginning of the epidemic, we showed that the French BSE epidemic in the late 1980s was completely undetected, and only the second wave, after 1990, was observed.  相似文献

Histofarcin, a skin test antigen for the diagnosis of epizootic lymphangitis, was locally produced from the mycelial form of Histoplasma capsulatum var. farciminosum (HCF) in disease-endemic districts of Ethiopia and tested for its application in the field between April 2002 and May 2003. The test was evaluated using 108 mules, 84 in endemic and 24 in disease-free districts. Microscopic and mycological examinations of clinical lesions were used as the "gold standard" for the validation of the test. The concentration of histofarcin that caused an optimum reaction was 0.2-0.4 mg/mL in a 0.1 mL dose and this was attained 24-48 h post-injection. The sensitivity and specificity of the histofarcin test were 90.3% (CI = 73.1, 97.5) and 69% (95%, CI = 48.1, 84.9%) in disease-endemic districts. On the other hand, specificity was 100% (CI=94.8, 100) in disease-free districts. Positive and negative predictive values of the histofarcin test were 77.78% (95% CI = 60.4, 89.3) and 85.71% (95% CI = 62.6, 96.2), respectively. A marginal substantial agreement (kappa = 0.61, P = 0.0000) was observed between the clinical status and the result of the histofarcin test. A large proportion (31%) of 'false positives' was recorded in endemic districts, which could be due to the pre-clinical stage of the disease. The latter ended in lower specificity and positive predictive value of the test since the true infection status of a 'false positive' could not be known on the basis of clinical features. Therefore, standard test validation procedures including slaughtering and isolation of HCF is required. After proper validation, we conclude that the histofarcin test could play a significant role in detecting early infection, and differentiating of EL from glanders, ulcerative lymphangitis, and sporotrichosis.  相似文献

Serological tests for the examination of individual samples from single animals are evaluated based on their ability to detect true positives above a defined threshold value. If results are obtained not from an individual but from a bulk sample this concept usually is adopted such that the threshold is set to allow the detection of a single positive sample within the pool. In conjunction with the development of a diagnostic paratuberculosis ELISA for the examination of bulk milk samples it is discussed which interpolations of this concept are justified when defining the true status of a herd based on the test parameters and the seroprevalence within the herd. Here, bulk milk from up to 50 animals each and the corresponding individual samples of 4241 dairy cows from 28 herds in the state of Brandenburg are investigated, and results are subjected to different evaluation approaches. Based on epidemiological considerations and test parameters a "critical prevalence" is defined which then serves as basis for the deduction of a cut-off value to be used for bulk milk samples. Finally, the practical relevance of this approach is demonstrated by suggesting an initial scheme for paratuberculosis classification of dairy herds with respect to possible control measures.  相似文献

Risk-based surveillance systems reveal occurrence of disease or infection in a sample of population units, which are selected on the basis of risk factors for the condition under study. The purpose of such systems for supporting practical animal disease policy formulations and management decisions are: A: to detect an emerging disease or infection, if it becomes introduced into a population; or B: to substantiate freedom from a condition in a population; or C: to detect cases and estimate the prevalence of an endemic condition in a population. In risk-based surveillance these aims should be met with prudent use of resources while maintaining acceptable system performance. High-risk category units are selected for testing by identification of the presence of specific high-risk factor(s), while disregarding other factors that might also influence the risk. On this basis we argue that the most applicable risk estimate for use in designing and evaluating a risk-based surveillance system would be a crude (unadjusted) relative risk, odds ratio or apparent prevalence. Risk estimates found in the published literature, however, are often the results of multivariable analyses implicitly adjusting the estimates for confounding from other risk factors. We describe some potential unintentional effects when using adjusted risk estimates in evaluating the efficacy and sensitivity of risk-based surveillance systems (SSe). In two examples, we quantify and compare the efficacy and SSe using adjusted and crude risk estimates. The examples use Danish surveillance data from previously published studies to evaluate systems aimed at risk-based detection of new cases of an endemic infection, i.e. Salmonella in dairy cattle herds (Example 1), and for substantiating the absence of a specific infection, i.e. Trichinella in the national slaughter pig population (Example 2), respectively.  相似文献

Fortunately, MRSA infection and colonization are currently uncommon in veterinary medicine. Nevertheless, the increasing reports of the occurrence of MRSA infection in horses, veterinarians, and equine personnel dictate that serious consideration be given to the control of this pathogen in veterinary hospitals as well as in the equine community. It is unclear whether extrapolation from human hospitals and people in the community is appropriate; however, given the rapid increase in nosocomial MRSA in human hospitals and the recent shift of certain clones of MRSA into the community, it would be unwise to ignore this potential pathogen. If equine MRSA did, indeed, originate in the human population, complete eradication in the equine population is unlikely, regardless of the prevalence of infection in horses and the intensity of infection control measures, without concurrent eradication of MRSA in the human population, which is surely an impossible feat. Early institution of appropriate surveillance and other infection control measures should be used to attempt to limit the impact of MRSA in veterinary medicine, however. It has been stated, "The time to act is now, before the prevalence of MRSA in the community begins to rise and we end up with 50% of the community strains becoming methicillin-resistant". This statement was directed at control of MRSA in people; however, it is equally relevant in the veterinary context and should receive strong consideration.  相似文献

There is a considerable number of methods available for the development, maintenance and surveillance of specific-pathogen-free herds. The selection of a method for a specific remediation project is primarily dependent on the goal, the epidemiology of the agent to be eradicated, the feasibility and the available resources. At the beginning of the remediation programme, methods are required for the classification of the herd status. Clinical and analytical diagnostic tests may be used for individual herds or in an entire area. The interpretation of test results applied within a population deserves special attention. A series of techniques are available - alone or in combination - to remediate a herd that was classified as being infected: systematic treatment, vaccination, elimination of infected individual animals ("test and remove"), partial depopulation or total population (stamping out). These strategies are typically accompanied by cleaning and disinfection. After the remediation, the status of the herd needs to be certified and maintained. For the documentation of the status, monitoring and surveillance strategies such as clinical follow-up visits, notification systems and random sampling can be applied. When planning random surveys, statistical and epidemiological considerations are relevant. For the sustainable maintenance of the pathogen-free status, biosecurity measures at the individual farm level, but also broader approaches such as import restrictions, early warning systems and surveillance programmes are needed. General awareness regarding unusual clinical signs needs to be promoted among farmers and veterinarians. The longer a disease or an agent is absent from a herd or region, the lower becomes the risk awareness. In such a situation, mistakes can occur leading to an increased risk of re-infection.  相似文献
