Fractures of the distal phalanx are an important cause of lameness referrable to the foot. Depending on the fracture configuration and articular involvement, conservative or surgical treatment may be required. Fractures of the distal phalanx have been divided into six categories based on fracture configuration. Discussion of clinical features, management, and prognosis for horses with distal phalangeal fractures is presented for each fracture type.  相似文献

A yearling horse was presented with a septic, transverse fracture in the frontal plane of the distal phalanx. The fracture fragment was surgically removed from the parent bone via the sole. The foot was managed by corrective farriery and the horse was able to be ridden for pleasure.  相似文献

Evaluation of foot pain in the standing horse by magnetic resonance imaging   总被引:1,自引:0,他引:1  
The records of 41 horses with previously undiagnosed foot pain that had been examined by standing magnetic resonance imaging were reviewed and follow-up information was obtained from their owners two years after the examination. A range of soft tissue and osseous abnormalities were identified, with multiple lesions frequently occurring. Deep digital flexor tendonitis was recorded in 12 of the horses, and distension of the distal interphalangeal joint was identified in 15, but it was not always associated with lameness; in contrast, distension of the navicular bursa was always associated with lameness in the seven affected horses. Navicular bone lesions were identified in 13 of the horses, often in the absence of radiographic changes. Follow-up information was obtained for 35 of the horses, 27 of which were alive; of these, 16 had returned to their previous level of performance. Of the five horses with navicular bursal changes, four had been euthanased owing to lameness and the other had returned to work at a reduced level.  相似文献

This paper describes some anatomical findings relatedto ossification of the distal phalanx cartilages by anatomical and computerized tomographic sections. Radiographic findings of ossification were analyzed and correlated to age and breed. Ossification was found in 10% of warmblood horses and in 80% of draft horses and was much more severe in draft horses than in warmblood horses. The expansion of the hoof was measured in the laboratory in several feet of warmblood and draft horses and was found to be minimal with no differences whether ossification was present or not. Computerized tomography was performed on a foot of a slaughtered horse before and after up to 7000N loading in the laboratory in order to study the eventual changes of the cartilages and the hoof. These structures seemed quite unaffected by the load and expansion of the horny capsule was minimal. Minor expansion seems an inevitable structural behavior of the hoof and the classical hoof mechanism is questioned. The clinical significance of ossification in its relation to lameness was individually determined during clinical examination and by comparison of groups of horses with and without ossification with groups of horses with lameness from the foot and proximal to the foot. The severity of the ossification was related to the onset of lameness and no correlation was found. It is concluded that the clinical significance of ossification is almost nil. Ossification is not painful when developing and not thereafter. There is no correlation between navicular disease and ossification of the cartilages either. Orthopedic shoeing is of no significance at all. An unfavorable advice in purchase examinations in sporting horses due to cartilage ossification is not justified in the author's opinion.  相似文献

OBJECTIVE: To describe and compare the distribution of technetium Tc 99m (99mTc) pertechnate following intraosseous or i.v. injection (with or without use of a tourniquet) in the distal portion of the forelimb in standing horses. ANIMALS: 4 horses. PROCEDURE: Each horse received 4 forelimb treatments in random sequence: intraosseous infusion with tourniquet application (IOT), intraosseous infusion without tourniquet application, i.v infusion with tourniquet application (IVT), and i.v. infusion without tourniquet application. Dynamic nuclear scintigraphic imaging of the third metacarpal bone, proximal and middle phalanges, and distal phalanx was performed from the start of each treatment until 1 hour after infusion was completed. Radionuclide activity was compared within and between treatment groups. RESULTS: Tourniquet application was necessary to maintain high levels of radionuclide activity in the distal portion of the forelimb after intraosseous or i.v. infusion with 99mTc pertechnate; IVT and IOT treatments resulted in similar radionuclide activity in the proximal and middle phalanges and distal phalanx. Of the 4 treatments, there was significantly higher radionuclide activity in the distal aspect of the third metacarpal bone after the IOT treatment. CONCLUSIONS AND CLINICAL RELEVANCE: By use of a tourniquet, radionuclide administration via the intraosseous or i.v. routes resulted in effective perfusion of the distal portion of the forelimb and similar distribution of the agent in the phalanges of horses. Further studies are required to ascertain whether these findings apply to delivery of therapeutic agents in infected tissues via IOT or IVT.  相似文献

Computed tomography (CT) in equine orthopaedics is currently limited because of the price, availability, impossibility to transport the scanner into surgical theatre, and the contraindications of general anaesthesia in some patients. A pQCT (peripheral quantitative computerised tomography) scanner was designed by the authors to image the limbs of the horse, both in standing or recumbent position. Standing computed tomography of the foot with a pQCT scanner is feasible and well tolerated by the horse. It enables good visualisation of bony structures but is not suitable to evaluate soft tissues. The technique can also assist surgery by assessing the 3D configuration of bone lesions.  相似文献

An 11‐year‐old Thoroughbred gelding was presented for lameness investigation following acute onset hindlimb lameness, which developed during a period of paddock turnout. Clinical examination revealed a swelling over the dorsolateral aspect of the pastern that was painful on digital palpation. Radiography of this area was consistent with an aggressive bone lesion involving both joint surfaces of the middle phalanx with pathological fracture and distal collapse of this bone. On this basis, the horse was subjected to euthanasia. Post mortem histopathological examination revealed presence of an osteosarcoma without involvement of the articular cartilage. This bone tumour is extremely rare in horses and has not been reported in this anatomical location.  相似文献

Summary. Nineteen cases of fracture of the distal phalanx in horses are reviewed. This represented 1.1% of the total number of horses seen at Murdoch University Veterinary Hospital (MUVH) over a 32-month period. All horses were managed conservatively. Eight horses made a complete recovery. Three are being trained again and are not lame. Four horses made a limited recovery. Two horses are still resting and one horse was killed humanely when it still showed lameness after 2 years. One horse was killed humanely after the fracture was first diagnosed.  相似文献

Reasons for performing study: Hyperintense signal is sometimes observed in ligaments and tendons of the equine foot on standing magnetic resonance examination without associated changes in size and shape. In such cases, the presence of a true lesion or an artifact should be considered. A change in position of a ligament or tendon relative to the magnetic field can induce increased signal intensity due to the magic angle effect. Objectives: To assess if positional rotation of the foot in the solar plane could be responsible for artifactual changes in signal intensity in the collateral ligaments of the distal interphalangeal joint and in the deep digital flexor tendon. Methods: Six isolated equine feet were imaged with a standing equine magnetic resonance system in 9 different positions with different degrees of rotation in the solar plane. Results: Rotation of the limb induced a linear hyperintense signal on all feet at the palmar aspect of one of the lobes of the deep digital flexor tendon and at the dorsal aspect of the other lobe. Changes in signal intensity in the collateral ligaments of the distal interphalangeal joint occurred with rotation of the limb only in those feet where mediolateral hoof imbalance was present. Conclusions: The position and conformation of the foot influence the signal intensity in the deep digital flexor tendon and in the collateral ligaments of the distal interphalangeal joint. Potential relevance: The significance of increased signal intensity in the deep digital flexor tendon and in the collateral ligaments of the distal interphalangeal joint should be interpreted with regard to the position and the conformation of the foot.  相似文献

In several veterinary institutions, adjustments of CT machines have been made that allow for imaging of the standing horse. The risk of general anesthesia is eliminated and the shorter scan completion time reduces cost to clients. The objective of this retrospective, analytical study was to evaluate the technique, imaging diagnoses, feasibility, and image artifacts of multi-slice helical CT of horses’ distal limbs acquired under standing sedation. The CT images of 250 horses of various breeds, aged 3–23 years, that underwent standing distal limb CT were evaluated. Three observers assessed the CT images for artifacts and inter-observer agreement was calculated. Eighty-six percent (95% confidence interval (CI), 81–90) of the scans were carried out on the forelimbs, while 14% (95% CI, 10–19) were of the hindlimbs. A total of 65% (95% CI, 59–71) of horses that underwent standing sedated CT had single imaging diagnoses. Seventy-one percent (95% CI, 65–77) of the cases had unilateral lesions, 27% (95% CI, 22–33) had bilateral lesions and 2% (95% CI, 1–4) had no diagnosed lesions. The average CT acquisition time was 17.5 minutes (range = 15–20). The average number of acquisitions per horse was 1.7 (median = 1; range = 1–4). There was good to excellent agreement between all three observers for the presence of motion artifact in the metacarpo/metatarsophalangeal joints, identification of marked beam hardening artifact, mild solar/ skin dirt, and photon starvation artifact (kappa 0.61-0.80). No complications were encountered. Standing examination of the distal limb achieved diagnostic image quality that was obtained with minimal acquisition attempts and in a timely manner.  相似文献

This paper describes a rare malformation of the distal portion of the left foredigit of a 15-month-old half-bred colt which was severely lame. Radiological, pathological and tomographic studies revealed hypoplasia of the metacarpophalangeal skeleton, absence of the distal sesamoid and deformation of the distal phalanx. Morphological analysis of the lesion and computerised measurement of the width of the articular surface of the distal phalanx suggested an early fusion between the distal sesamoid and distal phalanx.  相似文献

A primary ossifying fibroma was found at necropsy in a 5-year-old Welsh pony cross mare that was humanely subjected to euthanasia for an intractable severe left hindlimb lameness. Antemortem radiographs of the left hind pastern were characterised by articular, subchondral and cortical areas of lysis. Primary ossifying fibroma is a rare lesion of the distal limb but should be considered when there is extensive lysis extending into the medulla and proximal cortical bone.  相似文献

The cardiovascular effects of romifidine, an alpha-2 adrenoreceptor agonist, were investigated in six horses using two doses (80 and 120 microg kg(-1)) in a cross-over study design. Cardiac index and mixed venous oxygenation were significantly decreased at 15 and 30 minutes after both doses of romifidine. Systemic vascular resistance was significantly increased with romifidine (120 microg kg(-1)). Arterial blood pressure increased initially and then gradually decreased; the doses of decrease was significant at 90 and 120 minutes with romifidine 80 and 120 microg kg(-1). There were minimal differences between the two doses of romifidine, and both should be used with care especially in horses with cardiovascular compromise, or when used in combination with other cardiovascular depressant drugs.  相似文献

REASONS FOR PERFORMING STUDY: The recommended duration of cryotherapy in horses is currently extrapolated from human medicine. Prolonged, continuous cryotherapy (days rather than minutes) may be of therapeutic value if it is safe and well tolerated. OBJECTIVES: To evaluate the effect of prolonged, continuous application of ice and water to the equine distal limb. METHODS: A slurry of ice and water was applied to the right forelimb of 4 Standardbred horses for 48 h. Hoof temperature, ambient temperature and ice boot temperature were logged continuously and clinical observations recorded every 2 h. Lameness examinations were performed prior to application and 1 week, 6 months and 1 year after removal of the ice boot. RESULTS: Continuous cryotherapy was well tolerated and resulted in marked cooling of the treated foot. No significant variation in clinical parameters was noted, and no lameness or gross pathology noted in the treated limbs at any examination period. CONCLUSIONS: The continuous application of ice and water is a safe and effective means of cooling the equine distal limb. The extremely low, constant temperatures achieved may be of benefit in the management of various distal limb conditions. POTENTIAL RELEVANCE: Providing clinicians and research workers with a potent therapeutic/prophylactic modality for disorders of the equine distal limb.  相似文献

Fractures of the distal phalanx of the forelimb were diagnosed in 5 colts and 3 fillies ranging in age from 2 weeks to 5 1/2 months at the time of fracture. Three fractures entered the distal interphalangeal joint and 6 fractures (one foal had bilateral fractures) were nonarticular. All foals with articular fractures became sound with conservative treatment. Four of 5 foals with nonarticular fractures became sound with conservative treatment and 1 foal developed separation of the hoof at the coronary band after application of acrylic around the hoof. On the basis of our observations in these foals, sagittal articular and nonarticular distal phalangeal fractures in foals should be treated conservatively. Foals so treated would have an excellent prognosis for return to soundness.  相似文献

OBJECTIVE: To report a technique for, and outcome after, arthroscopic removal of dorsoproximal chip fractures of a proximal phalanx in standing horses. STUDY DESIGN: Retrospective study. ANIMALS OR SAMPLE POPULATION: A total of 104 horses, 1 to 13 years of age, with a dorsoproximal chip fracture of a proximal phalanx. METHODS: Horses were restrained in standing stocks and sedated with intravenous xylazine, detomidine, or a combination of both drugs. Local analgesia was achieved with 2% mepivacaine administered intra-articularly and by subcutaneous infiltration in a crescent-shaped block dorsal to the fetlock. Sterile drapes were placed on the surgical field, and impervious drapes were used on the hoof and floor. Arthroscopic portals were created in the dorsal pouch of the metacarpophalangeal or metatarsophalangeal joints to remove chip fractures. Statistical analysis was performed on race records for all Thoroughbred racehorses and compared with previously published studies. Operative and hospitalization times were compared with those of the general hospital population, and risks associated with general anesthesia were examined. RESULTS: No major operative or postoperative complications occurred. Ninety-one percent of racehorses raced after surgery with 78% returning to race at the same or higher level. CONCLUSION: Standing arthroscopic surgery can be performed successfully to remove dorsoproximal chip fractures of the proximal phalanx. CLINICAL RELEVANCE: Standing arthroscopic surgery is a valid alternative treatment for experienced surgeons to avoid the expense and potential risks associated with general anesthesia.  相似文献

A review of third phalanx fractures in the horse: sixty-five cases   总被引:1,自引:0,他引:1  
Sixty-five cases of third phalanx (P3) fracture were retrieved from 20,638 case records at Michigan State University's Veterinary Clinical Center between Feb 1, 1964 and July 1, 1977. The fractures were classified by anatomic location, using a numbering system. Data from case records indicated the most common P3 fracture involved the articular surface of the coffin joint (53 of 65 cases; 81.5%). The greatest occurrence of P3 fracture was in Standardbreds (31 of 65 cases). Thirty-three of the 65 fractures were in geldings of all breeds; 57 of the cases were distributed among racing horses of four breeds; and the mean ages at time of fracture for these groups were 4.9 to 5.3 years. The most common cause of P3 fracture was racing injury. Of 57 P3 fractures of the forelimb, 51 (89.5%) involved the lateral aspect of the left limb or medial aspect of the right limb--the part of each foot supporting most of the horse's weight in turns, while racing counterclockwise. Methods and response of treatment were evaluated by questionnaires obtained from 31 of the 65 owners. This questionnaire solicited information from owners and trainers as to: (1) cause, (2) treatment, (3) outcome, and (4) the use of the horse. Results were tabulated for: (1) questionnaire information obtained, (2) a classification system for P3 fractures, based on the anatomic location of the fracture, and (3) distribution of fractures between limbs and position (medial vs lateral) within limbs.  相似文献
