老年肺心病患者血清甲状腺激素和IL-6水平的检测及意义

目的 :观察老年肺心病患者血清甲状腺激素和 IL- 6水平的变化及其相互关系。方法 :测定 6 4例老年肺心病患者急性加重期和缓解期动脉血气分析、血清总三碘甲状腺原氨酸 (TT3)、四碘甲状腺原氨酸 (TT4 )、促甲状腺素 (TSH)(RIA法 )及 IL- 6 (EL ISA法 )水平 ,并与 30例心、肺功能正常人作对照。结果 :老年肺心病患者急性加重期血清 TT3、TT4水平均显著低于缓解期及对照组 (P<0 .0 1 ) ,IL- 6水平显著高于缓解期及对照组 (P<0 .0 1 ) ;TT3、TT4 水平与 Pa O2 呈显著正相关 (r=0 .5 76 ,P<0 .0 1 ;r=0 .5 98,P<0 .0 1 ) ;IL- 6水平与 Pa O2 呈负相关 (r=- 0 .6 2 5 ,P<0 .0 1 ) ,与 TT3、TT4 水平呈负相关 (r=- 0 .30 1 ,P<0 .0 1 ;r=- 0 .374 ,P<0 .0 1 )。结论 :血清 TT3、TT4 和 IL- 6水平反映了老年人肺心病患者病情的严重性 ,检测血清中 TT3、TT4 和 IL - 6的浓度对判断病情及预后有一定意义

Serum levels of thyroid hormone and IL-6 in elderly patients with chronic pulmonary heart disease

Objective: To investigate the change of serum levels of thyroid hormone and IL-6 and the relationship between them in elderly patients with chronic pulmonary heart disease (CPHD). Methods: Serum levels of TT 3, TT 4, TSH, and IL-6 in 64 elderly patients with CPHD and 30 healthy controls were determined by RIA and ELISA, respectively, and the artery blood gas analysis was also performed. Results: Compared with those in the stable stage of CPHD and in the controls, serum levels of TT 3 and TT 4 were decreased, and IL-6 content was increased in the acute stage of CPHD (P<0.01). PaO 2 in patients showed positive correlations with TT 3 and TT 4 levels (r=0.576, P<0.01;r=0.598, P<0.01). IL-6 level was negatively correlated with PaO 2 (r=-0.625, P<0.01), and TT 3 and TT 4 levels (r=-0.301, P<0.01; r=-0.374, P<0.01). Conclusion: Serum levels of TT 3, TT 4 and IL-6 could reflect the severity of the elderly patients with CPHD, and the detection of these parameters is helpful in evaluating the illness state and the prognosis.
