经会阴三维超声对女性盆底功能障碍患者盆底变化的检测

目的 了解不同年龄女性盆底功能障碍(FPFD)患者的盆底变化.方法 对62例FPFD患者(≥60岁25例、30 ～ 40岁37例)进行经会阴三维超声检查,观察Valsalva动作时前、中、后盆腔脱垂情况,静息、Valsalva动作时肛提肌裂孔变化.结果 在Valsalva动作时,≥60岁组的子宫脱垂和直肠膨出发生率、尿道转移角度、膀胱膨出距离及子宫脱垂距离明显高于30～ 40岁组(P＜0.01或0.05).在静息和Valsalva动作时,≥60岁组的肛提肌裂孔前后经、左右径、面积及周长均明显大于30～40岁组(P＜0.01或0.05).结论 ≥60岁FPFD患者的盆底病变程度明显大于30～ 40岁者.经会阴超声检查是评价FPFD严重程度的简单、客观方法.

Perineum three-dimensional ultrasonography for detection of pelvic floor change in female pelvic floor dysfunction

Objective To investigate the change of pelvic floor in female pelvic floor dysfunction （FPFD） with different age. Methods Anterior, middle, posterior pelric prolapse during Valsalva maneurer, and anal levator hiatus during resting and Valsalva maneuver were observed by perineum three-dimensional ultrasonography in 62 FPFD patients （25 cases, ？Y 60 years; 37 cases, 30？？40 years）. Results During Valsalva maneuver, the incidence of uterine prolapse and rectal bulging, urethral transfer angle, and bladder bulging and uterine prolapse distance were significantly higher in patients aged over 60 years than those in 30~40 years （P0.01 or 0.05）. During resting and Valsalva maneuver, the vertical and transverse diameter, area and perimeter of anal levator hiatus increased remarkably in patients aged over 60 years compared with patients aged 30？？40 years （P0.01 or 0.05）. Conclusion The pelvic lesion is more severe in FPFD patients aged over 60 years than that in 30？？40 years. Perineum ultrasound examination is a simple and objective method for evaluating the severity of FPFD.
