绝经后阴道出血495例临床与病理分析

目的 :探讨妇女绝经后阴道出血的原因 ,绝经后阴道出血的年龄、绝经年限与病理类型的关系。方法 :分析 4 95例绝经后阴道出血并行诊断性刮宫、宫颈或病灶活检及 (或 )子宫切除患者的临床和病理资料。结果 :功能性病变所致绝经后出血占 37.6 % ,良性病变占 35 .8% ,恶性肿瘤占 2 6 .7% ;在恶性肿瘤中 ,子宫内膜癌占 4 6 .2 % ,宫颈癌占 35 .6 % ,卵巢癌占 8.3% ;宫体病变引起的绝经后出血最多 ,占 6 2 .6 % ;≤ 6 0岁者 ,功能性病变占多数 ;>70岁者 ,恶性肿瘤占多数。结论 :绝经后阴道出血的年龄越大 ,绝经年限愈长 ,恶性肿瘤发生率愈高。

Clinicopathologic analysis of 495 cases with postmenopausal vaginal bleeding

Objective:To investigate the cause of postmenopausal vaginal bleeding and the relationship of the patients' age and menopausal duration to the pathologic types.Methods:The clinicopathologic data including curettage,biopsy of cervical or focus and/or hysterectomy in 495 cases with postmenopausal vaginal bleeding were analyzed retrospectively.Results:The causes of postmenopausal vaginal bleeding were functional endometrial diseases(37.6%),benign lesions (35.8%),and malignant tumors(26.7%),and the last was primarily composed of endometrial carcinoma(46.2%),cervical cancer(35.6%)and ovarian carcinoma(8.3%).The disorders of uterine body were its main causes(62.6%).The functional endometrial bleeding was commonly seen in patients aged 60 or less,but the malignant tumors in those at the age of over 70.Conclusion:There is a high correlation between the incidence of malignant lesions and the patient's age and menopausal duration in postmenopausal vaginal bleeding.
