糖化血红蛋白、血清胱抑素C和尿微量白蛋白排泄率联合检测对早期糖尿病肾损伤诊断的临床价值

摘    要:目的探讨糖化血红蛋白(HbA1c)、血清胱抑素C(Cys C)和尿微量白蛋白排泄率(UAER)联合检测对糖尿病早期肾脏损伤诊断的价值。方法选择2型单纯糖尿病(DM)患者40例、2型糖尿病肾病(DN)患者35例及40例健康体检者,分别检测其糖化血红蛋白、血清胱抑素C和尿微量白蛋白排泄率,评价各项测定结果的关系。结果 DN组的Cys C、UAER检测结果显著高于另两组,差异均有统计学意义(P<0.01),DN组的HbAlc检测结果也比另两组高,差异有统计学意义(P<0.05);DM组HbA1c、Cys C和UAER均高于对照组,且差异有统计学意义(P<0.05);HbAlc与Cys C、UAER分别进行直线相关回归,均呈正相关(P<0.05)。结论 HbA1c、Cys C和UAER与2型糖尿病患者的早期肾损伤程度密切相关,联合检测此3项指标对于糖尿病肾损伤的早期诊断、治疗和监测意义重大。

Clinical Application of United Detection of HbAlc,Cys C and UAER in Diagnosis of Early Diabetic Nephropathy

Abstract:Objective To study the application of detection of glycosylated hemoglobin(HbA1c)、serum Cystatin C(Cys C)and urinary albumin excretion rate(UAER)in diagnosis for early diabetic nephropathy.Methods In endocrine Department,40 inpatients with type 2 diabetes mellitus(DM) 35 case with nephropathy(DN)pants,and 40 healthy volunteers were chose to undergo detection of glycosylated hemoglobin,Cystatin C and and urinary albumin excretion rate,and the relationships among the detected results were analyzed and evaluated.Results Cys C、UAER in DN group were significantly higher than that of the other two groups with statistically significant difference(P<0.01),and HbALc was higher than that of the other two groups,with statistically significant difference as well(P<0.05).All index of DM group were higher than that of the control group,with statistically significant difference(P<0.05).After making linear regresion for HbAlc with Cys C、UAER respectively,positive correlation appeared.Conclusion The levels of HbA1c、Cys C and UAER are closely related to the early injury of kidey with DM,while combined detection will have great sighificance for early dianosis,treatment and control of DN.
