The response of dogs with insulinoma to surgical and medical management is variable, with the majority developing intractable hypoglycaemia. A long-acting somatostatin analogue, Octreotide (SMS 201–995; Sandostatin) has been useful in the management of hypoglycaemia in humans with insulinoma, and preliminary reports suggest a beneficial clinical response in dogs with insulinoma. The present study objectively evaluated Octreotide in the management of three dogs with immunohistochemically confirmed insulinoma. Octreotide had no benefit over placebo, and little effect on circulating glucose and insulin concentrations despite clearly detectable plasma concentrations of Octreotide. No clinical improvement was apparent in two dogs given Octreotide over a period of two and three weeks. These results contrast with the positive clinical responses noted previously and indicate that further placebo controlled, objective studies are necessary before clear statements on the treatment of insulinoma with Octreotide are made.  相似文献

The purpose of this study was to determine the prevalence of positive allergen reactions in cats with small-airway disease (i.e. 'feline asthma', 'feline allergic bronchitis', 'feline bronchial disease'). Intradermal skin tests (IDT) and serum immunoglobulin E (IgE) tests were performed in 10 cats with idiopathic small-airway disease and in 10 normal cats without a history of respiratory disease. None of the cats had a history of skin disease or clinical signs of skin disease at the time of testing. Significantly more individual positive allergen reactions were found on serum IgE tests than on IDT in both groups of cats. Affected cats had significantly more individual positive allergen reactions on both tests than unaffected cats. Both IDT and serum IgE tests resulted in more individual positive allergen reactions to weeds, trees, grasses, and/or moulds in affected cats than in normal cats. Significantly more positive allergen reactions to house dust mites were found in affected compared to non-affected cats by IDT but not by serum IgE testing. One unexpected obstacle to inclusion of more affected cats in the study was the concurrent presence or history of suspect or known allergic skin disease. Concurrent allergic skin disease has not been reported in association with small-airway disease in cats. The increased prevalence of individual positive allergen reactions in affected cats may be due to increased immunological reactivity in these cats. Further studies are needed to answer this question and to determine what role, if any, aeroallergens have in the pathogenesis of this complex feline disease.  相似文献

Comparison of test characteristics allows a clinician to choose the optimal diagnostic test method for an individual patient. This study assessed the comparative test characteristics of noninvasive (NI) blood pressure measurement methods (oscillometric and Doppler) and used this information to develop optimal cutoff values for diagnosis of systolic hypertension in dogs by these NI methods. Simultaneous NI (oscillometric or Doppler methods) and invasive (arterial puncture [AP]) systolic blood pressure (SBP) measurements were obtained prospectively from normal dogs and dogs suspected of having systemic hypertension based on clinical signs. Oscillometric SBP readings were obtained from the distal hind limb (Osc-L, n = 54) or the proximal tail (T. n = 27). Doppler BP measurements were obtained using a forelimb cuff (n = 57). AP-SBP was categorized as hypertensive if > or = 160 mmHg, and sensitivity (Se). specificity (Sp), and likelihood ratios (LR) were calculated for diagnostic cutoff values ranging from 130 to 220 mmHg. Receiver operator characteristic (ROC) curves were analyzed to determine optimal cutoff values for diagnosis of AP-SBP > or = 160 mmHg. Optimal NI SBP cutoff values considered to reflect AP values > or = 160 mmHg were: Osc-L = 160 mmHg (Se: 65%, Sp: 85%. OBJECTIVE: To determine the effect of endurance training on QRS duration, QRS-wave amplitude, and QT interval. ANIMALS: 100 sled dogs in Alaska. PROCEDURE: Dogs were examined in early September (before training) and late March (after training). During the interim, dogs trained by pulling a sled with a musher (mean, 20 km/d). Standard and signal-averaged ECG were obtained before and after training. RESULTS: Endurance training significantly increased mean QRS duration by 4.4 milliseconds for standard ECG (mean +/- SEM; 62.3 +/- 0.7 to 66.7 +/- 0.6 milliseconds) and 4.3 milliseconds for signal-averaged ECG (51.5 +/- 0.7 to 55.8 +/- 0.6 milliseconds) without changing body weight. Increase in QRS duration corresponded to a calculated increase in heart weight (standard ECG, 23%; signal-averaged ECG, 27%). Signal-averaged QRS duration was correlated with echocardiographically determined left ventricular diastolic diameter for the X orthogonal lead (r = +0.41), Y orthogonal lead (r = +0.33), and vector (r = +0.35). Training also increased QT interval (234 +/- 2 to 249 +/- 2 milliseconds) and R-wave amplitude in leads II and rV2, increased peak-to-peak voltage and S-wave amplitude in the Y orthogonal lead, and decreased Q-wave amplitude in the Y orthogonal lead. CONCLUSIONS AND CLINICAL RELEVANCE: Electrocardiographic changes reflected physiologic cardiac hypertrophy in these canine athletes in response to repetitive endurance exercise. The QRS duration increases in response to endurance exercise training and, therefore, may be of use in predicting performance in endurance activities.  相似文献

Congenital aortic stenosis was diagnosed in six cats. Clinical findings included dyspnoea, systolic murmurs with maximal intensity at the left apex or right sternal border, and cardiomegaly with congestive heart failure. Some clinical features of feline aortic stenosis were similar to those observed in cats with hypertrophic cardiomyopathy. Definitive diagnosis was obtained by angiographic imaging of a discrete, consistent subvalvular obstruction, Doppler-echocardio-graphy, or by necropsy. Although, two of the cats were diagnosed at ages older than usually reported, the overall prognosis for cats with aortic stenosis was poor. Four of the six cats died or were euthanased within one year of the onset of clinical signs.  相似文献

Ten cases of bacterial endocarditis in the dog were studied by retrospective analysis of case records. Diagnosis of bacterial endocarditis was based on either strict clinical criteria or post mortem examination. A wide spectrum of presenting signs was seen including pyrexia, lameness, epistaxis, haemoptysis and dyspnoea. Haematological and biochemical abnormalities were non-specific and were attributed to the effects of septicaemia, embolism or chronic inflammation. Haemostatic abnormalities and, or, severe dysrhythmias appeared to be associated with a significantly poorer prognosis. Blood cultures were positive in four cases of eight that were cultured. Lesions compatible with bacterial endocarditis were detected in all eight cases that underwent echocardiographic examination. Right-sided vegetations present in two dogs were not detected by echocardiography. Two-dimensional echocardiography is a useful technique for the detection of vegetative endocarditis in the dog and allows rapid therapeutic intervention while blood culture results are pending.  相似文献
