Benign and malignant splenic masses can appear similar on B-mode imaging, making ultrasound sensitive but not specific in their diagnosis. Our goal was to characterize color and/or power Doppler characteristics of vasculature within and adjacent to a splenic mass, which would distinguish benign vs. malignant lesions. The hypothesis was that malignant splenic masses will have altered vascular patterns compared with benign masses. Color and power Doppler cineloops evaluating the vasculature within the mass and normal splenic parenchyma were obtained in sagittal and transverse planes using a standardized protocol. Categories of evaluation included presence of peritoneal effusion, a large aberrant or tortuous vessel within the mass, relative blood flow within the mass compared with normal parenchyma, and path of vessels in the adjacent parenchyma entering into the mass. All patients had histopathologic or definitive cytologic diagnosis. Thirty-one dogs were included. There were 13 malignant masses and 18 were benign. Peritoneal effusion was significantly associated with malignancy (P = 0.0007). Presence of an aberrant or tortuous vessel within the mass was nearly significant (P = 0.059). There was no significant difference in any of the color or power Doppler blood flow evaluations. Ultrasonographic findings of a splenic mass and peritoneal effusion may indicate malignancy. The presence of an aberrant vessel within a splenic mass could suggest malignancy; however more data are needed.  相似文献

Objective

To describe an ultrasound-guided approach for venous and arterial vascular access and catheterization in anesthetized adult Yorkshire cross-bred pigs.

Study design

Prospective experimental study.

Animals

Ten adult female Yorkshire cross-bred pigs, weighing 78.4 ± 5.6 kg (mean ± standard deviation).

Methods

Using ultrasound guidance and the Seldinger technique, a 7 Fr, 20 cm triple-lumen central venous catheter was placed in the external jugular vein and an 18 gauge, 16 cm catheter was placed in the femoral artery. The success rate of catheterization and the incidence of catheter patency over 24 hours of general anesthesia were recorded.

Results

Catheterization of the external jugular vein was successful in 10 out of 10 pigs and catheterization of the femoral artery was successful in eight out of 10 pigs. A surgical dissection technique on the femoral artery was performed in two pigs. Venous and arterial catheter patency was maintained in all pigs over the 24 hour study period.

Conclusions and clinical relevance

Ultrasound guidance resulted in success rates of 100% for catheterization of the external jugular vein and 80% for catheterization of the femoral artery in anesthetized adult Yorkshire cross-bred pigs. This technique is a noninvasive, easily performed alternative to surgical exposure of the vessels in large pigs undergoing surgical instrumentation for biomedical device testing.  相似文献

Objective

To determine the degree of agreement between arterial and venous blood gases in anesthetized lambs using a point-of-care analyzer.

Study design

Prospective experimental study.

Animals

A group of 12 female Dorset cross-bred lambs, weighing 37.3 ± 7.4 kg (mean ± standard deviation).

Methods

Lambs were anesthetized with isoflurane for catheterization of the jugular vein and femoral artery, and blood samples were collected simultaneously for analysis of pH, PCO2, PO2, base excess (BE), bicarbonate (HCO3?), total carbon dioxide (tCO2), oxygen saturation (SO2), ionized calcium (iCa) and potassium (K+) using the Vet Scan i-STAT handheld analyzer. The Bland–Altman method was used to calculate agreement between arterial and venous measurements.

Results

Strong agreement was identified between arterial and venous pH (bias = 0.04; 95% limits of agreement = 0.02–0.06), K+ (bias = –0.1 and 95% limits of agreement = –0.8 to 0.5) and iCa (bias = 0.04; 95% limits of agreement = –0.08 to 0.0003). There was poor agreement for PO2 (bias = 168.0; 95% limits of agreement = 77.4–258.7) and SO2 (bias = 9.1; 95% limits of agreement = 2.8–15.3), as these values were higher in arterial blood than in venous blood. Moderate agreement was present for BE (bias = 1.7; 95% limits of agreement = –2.7 to 6.1), PCO2 (bias = –2.1; 95% limits of agreement = –7.5 to 3.4), HCO3? (bias = 1.1; 95% limits of agreement = –3.1 to 5.3) and tCO2 (bias = 1.0; 95% limits of agreement = –3.3 to 5.3).

Conclusions and clinical relevance

Venous blood can be used for the measurement of pH, K+ and iCa in anesthetized Dorset cross-bred lambs. Arterial blood is required for accurate measurement of PO2, SO2, PCO2, HCO3?, tCO2 and BE to assess systemic blood oxygenation and pulmonary function in anesthetized Dorset cross-bred lambs.  相似文献

Pigs with complete resistance to porcine reproductive and respiratory syndrome (PRRS) virus (PRRSV) have been produced by genetically knocking out the CD163 gene that encodes a receptor of the PRRSV for entry into macrophages. The objectives of this study were to evaluate associations of naturally occurring single nucleotide polymorphisms (SNPs) in the CD163 gene and in three other candidate genes (CD169, RGS16, and TRAF1) with host response to PRRSV-only infection and to PRRS vaccination and PRRSV/porcine circovirus 2b (PCV2b) coinfection. SNPs in the CD163 gene were not included on SNP genotyping panels that were used for previous genome-wide association analyses of these data. An additional objective was to identify the potential genetic interaction of variants at these four candidate genes with a mutation in the GBP5 gene that was previously identified to be associated with host response to PRRSV infection. Finally, the association of SNPs with expression level of the nearby gene was tested. Several SNPs in the CD163, CD169, and RGS16 genes were significantly associated with host response under PRRSV-only and/or PRRSV/PCV2b coinfection. The effects of all SNPs that were significant in the PRRSV-only infection trials depend on genetic background. The effects of some SNPs in the CD163, CD169, and RGS16 genes depend on genotype at the putative causative mutation in the GBP5 gene, which indicates a potential biological interaction of these genes with GBP5. In addition, genome-wide association results for the PRRSV-only infection trials revealed that SNPs located in the CDK5RAP2 or MEGF9 genes, near the TRAF1 gene, had suggestive effects on PRRS viral load, which indicates that these SNPs might contribute to PRRSV neuropathogenesis. In conclusion, natural genetic variants in the CD163, CD169, and RGS16 genes are associated with resistance to PRRSV and/or PCV2b infection and appear to interact with the resistance quantitative trait locus in the GBP5 gene. The identified SNPs can be used to select for increased natural resistance to PRRSV and/or PRRSV-PCV2b coinfection.  相似文献
