No case series exists in the literature describing scapula stress fractures. The purpose of this study was to review the clinical findings, diagnostic images and outcome of 8 horses diagnosed with scapula stress fractures. Scapula stress fractures were seen in Thoroughbred racehorses displaying acute moderate forelimb lameness. Nuclear scintigraphy identified the mid‐distal scapula spine and/or scapula supraspinous fossa as predilection sites. Conservative management resulted in a good prognosis for return to racing, but stress fracture recurrence may be seen.  相似文献

Shoulder injury is an uncommon cause of lameness in horses. Trauma to the shoulder region may produce bone and soft-tissue damage. The present study describes the case of a severe left shoulder injury caused by blunt trauma in a 6-year-old gelding. Clinical and ultrasonographic examination revealed fracture of the supraglenoid tuberosity of the scapula and of the lesser tubercle of the humerus with fiber disruption and hematoma of the biceps brachii, bicipital bursitis, and suprascapular nerve damage. The gelding was successfully treated with three intralesional injections of autologous platelet concentrates administered at 2-week intervals. Although all these combined injuries took a minimum of 18 to 24 months for full recovery, the gelding reached full recuperation of the affected limb in 10 months. These results suggest that injections of autologous platelet concentrate could provide a therapeutic benefit in the treatment of shoulder bone fractures and soft-tissue injuries in horses.  相似文献

This clinical report describes two horses with bilateral maxillary fractures following dental treatment. The fractures occurred during dental treatment by a veterinarian, and both had rostral, transverse, and complete bilateral maxillary fractures with instability and minimal displacement. The fractures were repaired using bilateral intraoral wiring with the patients under general anesthesia. The postoperative period was without complications and the fractures healed as expected. Maxillary fractures during or after routine dental treatment are rare, but can occur, especially in older horses. Appropriate sedation and, if necessary, regional nerve blocks might reduce the risk of iatrogenic fractures of the maxilla. General anesthesia must be considered for tooth extraction in horses that are difficult to restrain.  相似文献

Seventy physeal fractures in horses were initially managed by euthanasia (18), stall confinement (25), application of a cast (7), or internal fixation (20). Of the 52 physeal fractures initially managed with stall confinement, a cast, or internal fixation, 23 (44%) healed and 13 (25%) of these horses became sound. The number of horses less than 4.5 months of age with pressure physeal fractures that became sound was significantly higher (p < 0.05) than the number of horses greater than 4.5 months of age. The number of horses with functional, pain-free limbs (sound horses) or functional limbs (lame horses) was not significantly different (p > 0.05) for Salter-Harris Type I, II, III, or IV pressure physeal fractures; however, critical examination for growth disturbances was not performed. The number of horses with pressure or traction physeal fractures of less than 5 days duration prior to presentation that healed or became sound was not significantly different (p > 0.05) when compared with those horses with fractures of greater than or equal to 5 days duration.  相似文献

SUMMARY: Case records of 450 horses with signs of neurological disease are reviewed. One hundred and nineteen horses with neurological disease due to trauma were examined, of which 60 were due to spinal cord trauma, 47 to brain or cranial nerve trauma and 12 to peripheral nerve trauma. Cervical vertebral fractures/trauma were the most common injury. Basisphenold/basloccipital bone fractures were the most common form of cranial trauma and facial nerve paralysis the most common cranial nerve injury. Eighty-nine horses with neurological disease due to malformation were examined. Cervical vertebral malformation occurred in 83 horses and congenital defects in 6 foals. Neurological disease due to inflammation or infection occurred in 30 horses. The most common disease of this type was meningitis, which occurred in 11 horses and foals. Neoplasms in the CNS caused neurological disease in 8 horses. The final category was miscellaneous neurological disease, which was diagnosed in 204 horses. Diseases in this category included neonatal (28 cases), toxic/metabolic (27 cases), idiopathic (133 cases), degenerative (3 cases) and other neurological diseases (13 cases). The most common condition was idiopathic laryngeal hemiplegia (116 cases).
Where possible, diagnosis relied on a thorough neurological examination with use of ancillary tests in selected cases including rhinolaryngoscopy, radiography, myelography, ophthalmoscopy and cerebrospinal fluid analysis when indicated. In many cases necropsy and histopathological confirmation or diagnosis was necessary.  相似文献

Objective —To describe incomplete oblique sagittal dorsal cortical fractures of the equine third metacarpal bone, their surgical repair, and subsequent performance of the horses.
Study Design —Retrospective examination of medical records and racing performance.
Animal Population —Six Thoroughbred race horses, 2 to 4 years of age.
Methods —Radiographic confirmation of all fractures preceded general anesthesia and surgical correction. Three fractures were treated by intracortical compression using screws placed in lag fashion, and five fractures were treated by osteostixis. Race records were reviewed for each horse to determine performance after surgery.
Results —Fractures were best observed on palmarodorsal radiographic projections. Three horses treated by intracortical compression returned to racing, but fracture recurred in one horse and was treated by osteostixis. This horse and the other three horses treated by osteostixis raced after surgery.
Conclusions —Horses with incomplete oblique sagittal fractures of the dorsal cortex of the third metacarpal bone can race after surgical management of the fracture by screws placed in lag fashion or osteostixis. The authors' preferred surgical procedure for managing this fracture is osteostixis.
Clinical Relevance —Palmarodorsal radiographic projections of the third metacarpal bone are recommended in young Thoroughbred race horses suspected of having dorsal metacarpal stress fractures.  相似文献

Reasons for performing study: To enhance understanding of the nature and pathogenesis of scapular fractures in racehorses. Hypothesis: Scapular fractures in racehorses have a consistent configuration related to sites of pre‐existing stress modelling and remodelling. Methods: Fractured and intact scapulae collected post mortem were examined visually and with computed tomography (CT). Scapular fracture configuration, bone modelling changes and standardised CT morphometry and density measurements were recorded. Statistical comparisons were made between fractured, nonfractured contralateral and control scapulae. Results: Thirty‐nine scapulae from 10 Thoroughbred (TB) and 10 Quarter Horse (QH) racehorses were obtained. All 14 fractured scapulae (from 12 horses) had a consistent comminuted fracture configuration. A complete fracture coursed transversely through the neck of the scapula at the level of the distal aspect of the spine (8.9 ± 0.9 cm proximal to the lateral articular margin of the glenoid cavity). The distal fragment of 13 fractured scapulae was split into 2 major fragments by a fracture in the frontal plane that entered the glenoid cavity (2.8 ± 0.4 cm caudal to the cranial articular margin). Focal areas of periosteal proliferation and/or radiolucency were present in the distal aspect of the scapular spine of all fractured and intact contralateral scapulae, but less commonly (P<0.01) in intact scapula from horses without a scapular fracture. Fractured scapulae had 7–10% lower mean density and 46–104% greater density heterogeneity in the spine adjacent to the transverse fracture compared to control scapulae (P<0.03). Conclusions and clinical relevance: Thoroughbred and QH racehorses have a characteristic scapular fracture configuration that is associated with pre‐existing pathology of the distal aspect of the spine. This location is consistent with scapular stress fractures diagnosed in lame TB racehorses. Catastrophic fracture is the acute manifestation of a more chronic process. Consequently, there are opportunities for early detection and prevention of fatalities.  相似文献

Supraspinatus and infraspinatus muscle paralysis with atrophy was treated by partial osteotomy of the scapula, deep to the suprascapular nerve. The horses had various gait abnormalities, which were corrected by the surgery, but regeneration of the muscles varied from partial to complete, depending on the duration of the condition and the degree of atrophy before surgery.  相似文献

OBJECTIVE: To evaluate nonsurgical management of type II fractures of the distal phalanx in Standardbred horses. DESIGN: Retrospective study of 48 affected horses. RESULTS: Most fractures occurred on the lateral palmar process of the left forelimb or the medial palmar process of the right forelimb; 81% of horses were considered sound enough to return to training and 63% raced. Of those returning to racing, 41% competed in > 10 races, 37% in 2 to 10 races and 22% in only 1 race. There was no difference in performance before and after fracture. Twenty-four of 25 horses had a bar shoe fitted for > 50% of the treatment phase. Of those horses returning to training without a bar shoe, 89% refractured at the same site. Sixty percent of horses returning to training with a bar shoe raced successfully. The total convalescent time, the time rested in a box and the time spelled in a paddock were similar for horses returning to racing and those that did not. The age of the horse had no effect on the ability to return to racing. CONCLUSION: The prognosis for type II fractures of the distal phalanx is guarded. It is advisable to fit a bar shoe on the horse during convalescence. Horses returning to training and racing with a bar shoe appear less likely to refracture the distal phalanx. Those horses that return to racing can perform at a level similar to that prior to fracture.  相似文献

The purpose in this study was to determine whether there was a significance difference between conservative management and plating of scapular body fractures. Bilateral scapular body fractures were created in 10 dogs. The left scapula in each dog was treated by open reduction and plating of the scapular spine. The right scapula was not treated. Plating enhanced healing and early return of function, but there was no significant difference in the long-term results.  相似文献

The clinical signs, diagnosis, surgical treatment, and postoperative results in 8 horses with comminuted or open fractures of the splint bone are described. Surgical therapy was chosen to hasten the convalescent period, prevent large callus formation, improve cosmetic appearance, and, in open fractures, for aggressive wound management. Four horses used for nonracing purposes had full recovery following surgery. Two of four racehorses had residual lameness after surgery. Three of four racehorses eventually raced with decreased performance.  相似文献

Stress fracture of the scapula in two horses is described. Both horses had a history of acute lameness after strenuous exercise. Using elective nuclear scintigraphy focal increased radiopharmaceutical uptake in the affected scapula consistent with a fracture was identified. Standing radiography was nondiagnostic in both horses. Ultrasonographic findings in one horse were consistent with a minimally displaced incomplete fracture. Dorsal frontal fractures of the first phalanx in nine horses are described. All fractures were in hindlimbs, and two horses were affected bilaterally. Of the eleven fractures, seven were incomplete and nondisplaced, one was complete and nondisplaced, and three were complete and displaced. Two horses with complete displaced fractures had their fractures repaired with lag screw fixation combined with open reduction, and the other seven horses were treated nonsurgically.
All horses were sound on the injured limb within 3 to 6 months of the injury. Lag screw fixation was recommended for horses with complete, displaced dorsal frontal fractures in order to reduce the articular defect and help prevent secondary joint changes. Nonsurgical therapy in the form of stall rest with a heavy support wrap resulted in bony healing of the fracture in 4 to 6 months.
Follow-up was obtained on all nine horses: two were retired for breeding; five performed equal to or better than their previous level of racing performance; one was performing successfully as a combined training horse; and one returned to a lower level of performance due to an unrelated forelimb lameness.  相似文献

REASONS FOR PERFORMING STUDY: There is limited information on the prevalence of idiopathic cheek teeth (CT) fractures in the general equine population and on which CT are most commonly affected. OBJECTIVES: To obtain information on the prevalence of fracture patterns and clinical details of idiopathic CT fractures. METHODS: Details of cases with idiopathic CT fractures encountered were obtained via a questionnaire sent to suitably experienced veterinary practitioners and equine dental technicians (EDTs). RESULTS: Details of 147 horses that suffered a total of 182 idiopathic CT fractures; and median 0.4% (range 0.07-5.9%) of horses examined were diagnosed with such fractures; 133 maxillary CT and 49 mandibular CT fractures were found; maxillary Triadan 09s and 10s were preferentially fractured (46% of all fractured CT); fracture patterns included maxillary CT slab fractures (through 1st and 2nd pulp chambers) in 87 teeth (48% of all fractured CT), maxillary CT midline sagittal fractures (through the infundibula) in 31 CT; and various other types of maxillary CT fractures in 15 CT. In the mandibular CT, lateral slab fractures (through the 4th and 5th pulp chambers) were found in 28 CT and a variety of other fracture patterns in the other 21 mandibular CT. Clinical signs included quidding in 33% of cases, bitting and behavioural problems (29%) and halitosis (12%), however 39% of horses with idiopathic CT fractures were asymptomatic. Treatments included oral extraction of the smaller dental fragment in 37% of cases, extraction of the entire fractured tooth (9%); removal of sharp edges on the remaining part of fractured tooth (14%); reduction of height of the opposite tooth (4%); other surgical or endodontic treatments, or referral of case for further investigation and treatment (10%); or no specific treatment (39%). Following treatment, 81% of cases were reported to be asymptomatic, 6% had ongoing clinical problems and the outcome was unclear in 13% of cases. CONCLUSIONS: Idiopathic CT fractures are present in approximately 0.4% of horses and may variably cause none to severe clinical signs. Most idiopathic CT fractures respond well to treatment. POTENTIAL RELEVANCE: Understanding of the prevalence, clinical findings and treatment of idiopathic CT fractures will improve management of these disorders.  相似文献

Fifteen cases of radial fractures in adult horses weighing more than 300 kg are discussed. Four of the horses were destroyed on humane grounds immediately because of a poor prognosis and expense of internal fixation; and two horses at five days and five weeks, respectively after treatment by cast application was initiated. Internal fixation was used in nine horses but of these only two horses recovered completely and resumed their former activities. In eight cases, two plates were applied, one lateral or medial and the other cranial. The internal fixation techniques of all nine horses were scrutinised and suggestions made for the future treatment of radial fractures. These suggestions are (1) the use of ASIF 5.5 mm cortical bone screws using the total width or thickness of the bone in each case. (2) Application of the dynamic condylar screw with its plate in distal or proximal fractures to allow more support. (3) Incorporation of a cancellous bone graft to the fracture. (4) Possible table recovery to prevent breakdown of the fixation during recovery. It is important that bone plates are applied over the total length of the bone. The fact that only two out of 15 horses survived and recovered completely underlines the problems associated with treatment of radial fractures in the adult horse.  相似文献

Objective: To describe the diagnosis and treatment of fractures of the deltoid tuberosity. Study Design: Case series. Methods: Medical records (1992–2009) of 19 horses with radiographic confirmation of deltoid tuberosity fractures were reviewed. Data retrieved included signalment, clinical and diagnostic imaging findings, and treatment. Outcome was determined by telephone questionnaire of owners and referring veterinarians. Results: Most horses were markedly lame on admission and 53% had reduced protraction of the affected limb. All fractures were identified on a cranio45° medial‐caudolateral oblique projection; however, only 32% (6 horses) were detected on a mediolateral projection whereas 86% were evident ultrasonographically. Treatment by local wound care and stall rest resulted in return to athletic function without lameness for 13 of 14 horses that had follow‐up. Conclusions: A cranio45° medial‐caudolateral oblique radiographic view was better than a mediolateral projection for identification of deltoid tuberosity fractures. Ultrasonographic detection of fractures was similar except when gas accumulation obscured the fracture site. Deltoid tuberosity fractures can cause severe lameness but can be treated successfully with conservative management.  相似文献

Fifteen fractures of the palmar or plantar process of the proximal phalanx in 15 horses are described. Ten were articular and five were nonarticular. Two modes of therapy were used depending on the location of the fracture. Nonarticular fractures were treated with stall rest or reduction in exercise, and articular fractures were treated with either surgical removal or internal fixation of the fragment. All horses that were operated on were sound within 6 months after surgery. Follow-up information was obtained on all horses through client questionnaire and race records in the racing breeds (Standardbred and Thoroughbred) and client questionnaire alone is the nonracing breeds (Quarterhorse and Arabian). Fourteen horses returned to an equal or better level of performance than before injury; one horse returned to a lower level of performance.  相似文献

REASONS FOR PERFORMING STUDY: Studies on arthroscopic removal of apical proximal sesamoid fracture fragments in Thoroughbred (TB) horses age > or = 2 years have reported a high success rate. However, there are no reports documenting the racing prognosis of TB horses that undergo such surgery as weanlings or yearlings. OBJECTIVES: To describe the incidence of apical proximal sesamoid fractures in immature TB horses, age < 2 years and determine probability and quality of racing performance after arthroscopic removal of such fractures in 151 TB weanlings and yearlings. METHODS: The medical records of TB horses age < 2 years that underwent arthroscopic surgery for removal of apical proximal sesamoid fracture fragments were reviewed. Follow-up information was obtained from race records. Student's t tests were used to compare performance variables of operated racehorses to that of their maternal siblings. RESULTS: Ninety-two percent (139/151) of fractures occurred in the hindlimbs and 8% (11/151) in the forelimbs (fracture of both fore- and hindlimb, n = 1). Horses with forelimb fractures had a greatly reduced probability of racing (55%) compared to those with hindlimb fractures (86%). Overall, 84% of the horses raced post operatively and had performance records similar to that of their maternal siblings, 78% (787/1006) of which raced. CONCLUSIONS: Arthroscopic removal of apical proximal sesamoid fracture fragments in TB weanlings and yearlings carries an excellent prognosis for racing in horses with hindlimb fractures and a reduced prognosis in those with forelimb fractures. Medial fractures of the forelimb carry the worst prognosis. POTENTIAL RELEVANCE: The determination of prognosis increases knowledge on apical sesamoid bone fractures and potential for arthroscopic restoration of the ability to race; and enables the value of yearlings for subsequent sale to be established.  相似文献

From 1985 through 1994 approximately 39,800 Thoroughbred horses competed in over 3,400 races each year conducted by the Japan Racing Association (JRA). During this time the prevalence of fractures incurred during racing was 1.99, and 0.09% during training. Of these, 0.32% of the horses during racing died or were humanely destroyed as a direct result of their injuries. During 1994, 1,837 fractures occurred. The cuboidal bones of the carpus and the distal end of the radius accounted for 33.5% of all recorded fractures, with 47.4% of these occurring during racing and 24.2% occurring during training. The most common sites of fracture within the carpus were the distal end of the radius, the third carpal bone, and the radial carpal bone accounting for 35.3, 35.2, and 29.5% of fractures, respectively. Of these, 86/7% of osteochondral fractures occurred on the dorsal aspect of the cuboidal bones. At racing speed, characteristic fractures of the carpus are relatively common in race horses.Since 1991 in Japan, arthroscopic surgery has been the treatment of choice for the removal of carpal joint osteochondral fragments. Of all horses having arthroscopic surgery (n=155), 68.4% of these horses returned to race 9 months after surgery. Of all horses having osteochondral fractures of the distal end of the radius, radial carpal, and third carpal bones treated with arthroscopic surgery in 1993 (n=97) 52.6%, 60.0%, and 55.6% respectively, returned to race and competed at the same level of competition or higher compared with pre-operative racing performance. Arthroscopic surgery, as adapted by the JRA, is a useful technique for the treatment of carpal bone fractures in race horses; however, following surgery some horses had chronic arthritic changes associated with the surgical site. This suggests that the surgical methods and post-operative training program have to be re-evaluated.The objectives of this paper are to describe the prevalence, location, post-operative complication and racing performance for horses having cuboidal carpal bone fractures in racing Throughbreds of JRA.  相似文献
