Titanium columns (Ti-6A1-4V) were treated with arc-deposition to roughen the surface enough to anchor the bone, and then coated with hydroxyapatite (HA) at a thickness of 5 micro m by the sputtering technique. Columns were implanted into dog femurs, and fixation of columns to bone due to bone-ingrowth was assessed histologically and with the push-out test. The HA-coated columns were inserted in the shafts of the right femurs of 2 dogs. As a control, columns that were only arc-deposited (non-coated columns) were inserted into the left femurs. The interfacial strength was higher for the HA-coated columns than for the non-coated columns. Coating a rough surface with an HA layer using a sputtering technique reinforces interfacial strength between bone and implants.  相似文献

A carbon dioxide laser, used in a rapidly pulsed mode, was evaluated for intra-articular use in horses. Under arthroscopic guidance, a lensed 5 mm laser probe attached directly to a hand-held carbon dioxide laser was inserted into one intercarpal joint of eight horses. In four horses, a cartilage crater 1 cm in diameter was created to the level of the subchondral bone of the articular surface of the third carpal bone. In four horses, the laser was directed perpendicular to the articular surface of the third carpal bone and activated to penetrate the cartilage and subchondral bone. The intercarpal joint of the opposite carpus in each horse was subjected to arthroscopic examination and insertion of the laser probe for an equivalent time. The laser was not activated and these joints served as sham operated controls. The horses were evaluated clinically for 8 weeks, then euthanatized, and the joints were examined radiographically, grossly, and histologically. Pulsed carbon dioxide laser vaporized cartilage readily but penetrated bone poorly. Cartilage vaporization resulted in no greater swelling, heat, pain on flexion, lameness, or synovial fluid reaction than the sham procedure. Laser drilling resulted in a shallow, charred hole with a tenacious carbon residue, and in combination with the thermal damage to deeper bone, resulted in increased swelling, mild lameness and a low-grade, but persistent synovitis. The carbon dioxide laser is a useful intra-articular instrument for removal of cartilage and has potential application in inaccessible regions of diarthrodial joints. It does not penetrate bone sufficiently to have application in subchondral drilling.  相似文献

Objective— To compare initial femoral fixation properties of a new implant, Graftgrab (GG), with 2 established methods of extra-articular femoral graft fixation, spiked washers (SW) and bone staples (ST).
Study Design— Experimental in vitro cohort study.
Methods— Canine semitendinosus–gracilis tendon grafts were passed through bone tunnels and fixed to the lateral surface of femoral condyles with spiked washers, bone staples, or new implant prototypes. The fixations were tested to failure with a single-cycle load at a rate of 50 mm/min. Failure and yield strength, stiffness, energy, and elongation were determined from load–displacement curves and failure modes were recorded.
Results— The graft failed midsubstance in 4 SW, 4 ST, and 1 GG fixations. In 3 SW, 3 ST, and 1 GG specimens, the graft slipped from the fixation. The graft ruptured at the clip (3) and the intra-articular (2) surface of the bone tunnel in the remaining GG specimens. There were no significant differences between fixation groups in femoral tunnel length, femoral width, or the mechanical properties evaluated.
Conclusions— The initial in vitro mechanical properties of the new fixation implant are comparable with those of spiked washers and bone staples.
Clinical Relevance— The initial mechanical performance of the new implant tested in this study was similar to those of comparable, established implants. The new implant is novel and may be useful for human anterior and veterinary cranial cruciate ligament graft reconstruction fixation.  相似文献

Objectives: To compare the number of cycles to failure of 4.5 mm broad dynamic compression plates (DCP), 4.5 mm broad limited‐contact dynamic compression plates (4.5‐LC‐DCP), and 5.5 mm broad limited‐contact dynamic compression plates (5.5‐LC‐DCP) having a rough (denoted by a prefix R‐) versus a standard smooth contact surface for the fixation of osteotomized equine 3rd metacarpal (MC3) bones. Study Design: Experimental. Animal Population: Fifteen pairs of adult equine cadaveric MC3 bones. Methods: Fifteen pairs of equine MC3 were divided into 3 test groups (5 pairs each) for comparison of (1) R‐DCP fixation with DCP fixation, (2) R‐4.5‐LC‐DCP fixation with 4.5‐LC‐DCP fixation, and (3) R‐5.5‐LC‐DCP fixation with 5.5‐LC‐DCP fixation to repair osteotomized equine MC3 bones under palmarodorsal 4‐point bending cyclic fatigue testing. For each group an 8‐hole plate with rough contact surface was applied to the dorsal surface of one randomly selected bone from each pair and a corresponding 8‐hole plate with smooth contact surface was applied dorsally to the contralateral bone from each pair. All plates and screws were applied using standard ASIF techniques. All MC3 bones had mid‐diaphyseal osteotomies. Mean number of cycles to failure for each method were compared using a paired t‐test within each group. Significance was set at P<.05. Results: Mean cycles to failure ± standard deviation was significantly greater for the R‐DCP fixation (230,025 ± 23,129) compared with the DCP fixation (103,451 ± 14,556), for the R‐4.5‐LC‐DCP fixation (99,237 ± 14,390) compared with the 4.5‐LC‐DCP fixation (46,464 ± 6325) and for the R‐5.5‐LC‐DCP fixation (65,113 ± 7796) compared with the 5.5‐LC‐DCP fixation (34,224 ± 3835). Conclusion: For the fixation of osteotomized MC3 bones, the constructs with plates having rough contact surface were superior to the corresponding constructs with plates having standard smooth contact surfaces in resisting cyclic fatigue under palmarodorsal 4‐point bending.  相似文献

REASONS FOR PERFORMING STUDY: Suspensory ligament (SL) desmitis is a common source of lameness. The results of this study will determine if blood-derived products stimulate SL matrix synthesis and have potential as regenerative therapies for SL desmitis OBJECTIVES: To determine if various blood-based biological products including plasma, blood, PRP, platelet poor plasma (PPP) and ABM aspirate stimulates anabolic and/or catabolic pathways in suspensory ligaments (SL). METHODS: The body of the SL was harvested from 6 horses and used to establish explant cultures. Explants were cultured in plasma, blood, PRP, PPP or ABM at concentrations of 10, 50 or 100%. Anabolic responses were assessed by use of quantitative PCR for collagens type I and III, cartilage oligomeric matrix protein (COMP) and decorin. Total DNA and collagen protein content were also measured. Catabolic reactions were measured by quantitative PCR for matrix metalloproteinases 3 and 13 (MMP-3, MMP-13). Results: Acellular bone marrow aspirate at 100% stimulated decorin and COMP mRNA synthesis more than all other treatments at all concentrations. No treatment at any concentration stimulated the catabolic gene MMP-13; only 50% ABM stimulated MMP-13 mRNA expression. CONCLUSIONS: Acellular bone marrow is indicated, and might be preferred to plasma, blood or PPP, as a blood-based biological source for SL tissue regenerative therapy. Long-term, placebo controlled case studies are indicated to determine if ABM aids in recovery from SL desmitis. POTENTIAL RELEVANCE: Bone marrow aspirate is an autogenous, readily available biological source for SL regenerative therapy where the aim is to stimulate matrix synthesis.  相似文献

Any traumatic event that produces maxillofacial and/or mandibular fractures generally results in gross and usually severe patient disfigurement and often results in the patient's inability to eat and drink. These fractures are exceptionally rewarding cases as simple techniques may be performed resulting in a successful functional outcome (ability to eat and drink) within a very short period of time (24 hours) after fracture stabilization. A markedly improved cosmetic appearance follows shortly thereafter once inflammation and edema resolve. The primary principle of fracture treatment, ie, providing stable fixation to the bone fragments, may be successfully used with wiring techniques only through an appreciation and proper application of biomechanical principles. Knowledge that bending forces (divided into their tensile and compressive components) are the primary forces to be neutralized dictates the use of the wiring techniques outlined as the "standard" to which all other methods of fixation for maxillofacial and mandibular fractures are compared. The location of the tension-band surface of the bone, the alveolar (oral) surface, dictates the most appropriate position for wire placement. Successful treatment is predicted on obtaining a cosmetically acceptable and functional result (Fig 29). Anatomic reduction and rigid fixation of fractures that can be reconstructed piece-by-piece creates optimal conditions for uncomplicated healing. Fractures in which bone loss or severe comminution exists, and which cannot be anatomically reconstructed, must be reduced using dental occlusion as the template for fracture fixation, thereby avoiding malocclusion. Excessive leverage on the bone fragments may occur secondary to malocclusion, resulting in an increased risk of complications (fragment motion, loosening of implants, infection). Some fractures with comminution or bone loss may not be suitable for wire fixation and must be treated by alternate methods (eg, external skeletal fixators, plates).  相似文献

OBJECTIVE: To investigate the effect of laser shock peening on the fatigue life and surface characteristics of 3.5-mm-diameter cortical bone screws. SAMPLE POPULATION: 32 stainless steel, 3.5-mm-diameter cortical bone screws. PROCEDURE: Screws were randomly assigned to an untreated control group or 2 power-density treatment groups, 6 gigawatts (GW)/cm2 and 8.5 GW/cm2, for laser shock peening. Number of cycles to failure and findings on scanning electron microscopy-assisted morphometric evaluation, including the mode of failure, surface debris, surface damage, and thread deformation, were compared between control and treated screws. RESULTS: The 6 GW/cm2 treated screws had a significant (11%) improvement in fatigue life, compared with untreated control screws. The 8.5 GW/cm2 treated screws had a significant (20%) decrease in fatigue life, compared with control screws. A mild but significant increase in thread deformation was evident in all treated screws, compared with control screws. The 8.5 GW/cm2 treated screws had significantly more surface irregularities (elevations and pits), compared with control or 6 GW/cm2 treated screws. CONCLUSION AND CLINICAL RELEVANCE: A modest positive increase in fatigue strength was produced by this design of laser shock peening on the midshaft of cortical bone screws. High laser shock peening power densities were detrimental, decreasing screw fatigue strength probably resulting from structural damage. Greater fatigue life of cortical bone screws can be generated with laser shock peening and could reduce screw breakage as a cause of implant failure; however, future studies will be necessary to address biocompatibility, alternative cleaning techniques, alterations in screw strength and pullout characteristics, and effects on susceptibility to corrosion.  相似文献

OBJECTIVE: To evaluate stiffness and bone-pin interface stress for a transcortical tapered-sleeve pin (TSP) that incorporates bilateral tapered sleeves over a transcortical pin. SAMPLE POPULATION: 14 third metacarpal bones (MCIII) collected from adult horses of various breeds. PROCEDURE: Each MCIII was cut in half to provide 2 test specimens. Pins (conventional and TSP) of 3 diameters (6.35, 7.94, and 9.50 mm) were inserted in specimens (3 specimens for each diameter and each type of pin). The test fixture simulated a typical sidebar-span skeletal fixation device for horses. Single cycle load-deflection tests were performed. Cyclic fatigue tests of TSP were performed to evaluate fatigue characteristics and stress conditions at the bone-pin interface. Maximum stress and strain were calculated, and results were compared with existing data on fatigue characteristics of bone. RESULTS: Significant increases in stiffness (load-deflection) and higher loads at yield point were detected for the TSP (stiffness for conventional 9.50mm pins, 4,500 N/mm; stiffness for TSP, 19,988 N/mm). Results of cyclic tests revealed a close correlation with existing data on fatigue characteristics. CONCLUSIONS AND CLINICAL RELEVANCE: The TSP described here is stiffer than conventional transcortical pins, and stress across the bone-pin interface is more evenly distributed. Use of this TSP should minimize major problems encountered during external fixation associated with the transcortical pin and bone-pin interface (ie, bone necrosis, infection of the pin track, pin loosening, and bone failure).  相似文献

OBJECTIVE: To describe a technique of contact neodymium:yttrium-aluminum-garnet (Nd:YAG) laser-assisted excision of progressive ethmoid hematoma (PEH) in horses, to determine the recurrence rate of clinical signs of PEH in horses with follow-up time of a minimum of 12 months, and to compare this result with reported results achieved by using conventional techniques. DESIGN: Retrospective study. ANIMALS: 21 horses with 26 PEH. PROCEDURE: Medical records of all horses with PEH treated by Nd:YAG laser excision from December 1986 through August 1996 were analyzed. Twenty-one horses underwent unilateral (18 surgeries) or bilateral (14) frontonasal bone flap with excision of the mass, using an Nd:YAG laser. One horse with bilateral PEH underwent a unilateral bone flap twice, 1 year apart. RESULTS: 1 horse died. Four of 20 horses with followup times of 12 months or longer had recurrence of PEH. The PEH recurrence rate was 20% (5/25 PEH). Three of 6 horses with bilateral lesions had recurrence of PEH. Recurrence rate for horses that had bilateral PEH (3/6 horses) was greater than for horses with unilateral PEH (1/14 horses). CLINICAL IMPLICATIONS: Treatment of PEH by Nd:YAG laser excision through a frontonasal bone flap results in a recurrence rate comparable with that reported for conventional techniques. Horses with bilateral lesions are more likely to have a recurrence of PEH.  相似文献

Objective— To compare monotonic biomechanical properties and fatigue life of a broad locking compression plate (LCP) fixation with a broad limited contact dynamic compression plate (LC‐DCP) fixation to repair osteotomized equine third metacarpal (MC3) bones. Study Design— In vitro biomechanical testing of paired cadaveric equine MC3 with a mid‐diaphyseal osteotomy, stabilized by 1 of 2 methods for fracture fixation. Animal Population— Cadaveric adult equine MC3 bones (n=12 pairs). Methods— MC3 were divided into 3 groups (4 pairs each) for: (1) 4‐point bending single cycle to failure testing; (2) 4‐point bending cyclic fatigue testing; and (3) torsional single cycle to failure testing. The 8‐hole, 4.5 mm LCP was applied to the dorsal surface of 1 randomly selected bone from each pair. One 8‐hole, 4.5 mm LC‐DCP) was applied dorsally to the contralateral bone from each pair. All plates and screws were applied using standard ASIF techniques. All MC3 bones had mid‐diaphyseal osteotomies. Mean test variable values for each method were compared using a paired t‐test within each group. Significance was set at P<.05. Results— Mean yield load, yield bending moment, composite rigidity, failure load and failure bending moment, under 4‐point bending, single cycle to failure, of the LCP fixation were significantly greater than those of the LC‐DCP fixation. Mean cycles to failure for 4‐point bending was significantly greater for the LCP fixation compared with LC‐DCP fixation. Mean yield load, mean composite rigidity, and mean failure load under torsional testing, single cycle to failure was significantly greater for the broad LCP fixation compared with the LC‐DCP fixation. Conclusion— The 4.5 mm LCP was superior to the 4.5 mm LC‐DCP in resisting the static overload forces (palmarodorsal 4‐point bending and torsional) and in resisting cyclic fatigue under palmarodorsal 4‐point bending. Clinical Relevance— The results of this in vitro study may provide information to aid in the selection of a biological plate for the repair of equine long bone fractures.  相似文献

This study was designed to assess the effectiveness of a modified silk ligature twisted with wire for inducing advanced periodontitis. Periodontitis was induced in five premolars and one molar of 20 healthy dogs over a 60-day period. The dogs were divided into four groups according to the ligature-inducing materials used: soft moistened food only, wire ligature (WL), silk ligature (SL) and twisted ligature with silk and wire (SWL). Periodontal indices were recorded, and dental radiographs were taken before and after 60 days of ligation. The ligatures were checked daily and the day the ligature fell out was noted. The period during which the ligatures were maintained was significantly shorter for the SL group compared to the SWL group (p < 0.05). Results of the clinical examination showed that almost all periodontal status parameters including the plaque index, gingival index, clinical attachment level, and bleeding on probing were significantly exacerbated in the SWL group compared to the other groups (p < 0.05). Radiographic evaluation demonstrated that alveolar bone levels were significantly lower in the SWL group than the other groups on day 60 (p < 0.05). These results suggested that experimental periodontitis induced by SWL could be an effective method for investigating periodontitis in canine models.  相似文献

OBJECTIVES: To compare the monotonic biomechanical properties and fatigue life of a broad, limited contact, dynamic compression plate (LC-DCP) fixation with a broad, dynamic compression plate (DCP) fixation to repair osteotomized equine 3rd metacarpal (MC3) bones. STUDY DESIGN: In vitro biomechanical testing of paired cadaveric equine MC3 with a mid-diaphyseal osteotomy, stabilized by 1 of 2 methods for fracture fixation. ANIMAL POPULATION: Twelve pairs of adult equine cadaveric MC3 bones. METHODS: Twelve pairs of equine MC3 were divided into 3 test groups (4 pairs each) for (1) 4-point bending single cycle to failure testing, (2) 4-point bending cyclic fatigue testing, and (3) torsional single cycle to failure testing. An LC-DCP (8-hole, 4.5 mm) was applied to the dorsal surface of 1 randomly selected bone from each pair. One DCP (8-hole, 4.5 mm broad) was applied dorsally to the contralateral bone from each pair. All plates and screws were applied using standard AO/ASIF techniques to MC3 bones that had mid-diaphyseal osteotomies. Mean test variable values for each method were compared using a paired t-test within each group. Significance was set at P<.05. RESULTS: The mean 4-point bending yield load, yield bending moment, composite rigidity, failure load, and failure bending moment of LC-DCP fixation were significantly greater (P<.01) than those of broad DCP fixation. Mean cycles to failure for 4-point bending was significantly (P<.001) greater for broad DCP fixation compared with broad LC-DCP fixation. Mean yield load, mean composite rigidity, and mean failure load in torsion was significantly (P<.02) greater for broad LC-DCP fixation compared with broad DCP fixation. CONCLUSION: Broad LC-DCP offers increased stability in static overload testing, however, it offers significantly less stability in cyclic fatigue testing. CLINICAL RELEVANCE: The clinical relevance of the cyclic fatigue data supports the conclusion that the broad DCP fixation is biomechanically superior to the broad LC-DCP fixation in osteotomized equine MC3 bones despite the results of the static overload testing.  相似文献

OBJECTIVE: To describe a surgical technique for treatment of biologically inactive nonunions using en bloc ostectomy and compression plate fixation and clinical outcome in 17 dogs. STUDY DESIGN: Retrospective clinical study. SAMPLE POPULATION: Seventeen dogs. METHODS: A transverse ostectomy was performed adjacent and parallel to the nonunion to eliminate nonviable tissue and provide a new, viable fracture surface with a minimum circumferential contact area of 315 degrees. With most of the bony column anatomically reconstructed, compression plate fixation was used to stabilize the fracture. Autogenous cancellous bone grafting was used if a fracture gap was present (<45 degrees of missing circumferential bone contact). Resection of bone was limited so that bone shortening was less than 20% of the overall bone length. Clinical and radiographic follow-up evaluations were obtained whenever possible. RESULTS: Complete circumferential bone contact and compression plate fixation was achieved after ostectomy in 12 dogs; cancellous bone graft was used in 5 dogs. En bloc ostectomy sites were radiographically healed in a median time of 2.5 months after surgery in 11 dogs that returned for complete in-hospital follow-up, and progressive healing was observed in 3 other dogs, where in-hospital follow-up was obtained up to 2 months after surgery. These dogs had a median follow-up time of 2 months, at which time 6 dogs had no lameness, 4 had minimal lameness, and one had moderate lameness. No complications occurred, and no implants were removed. CONCLUSIONS: En bloc ostectomy with compression plate fixation was considered successful for the treatment of biologically inactive nonunions. A good to excellent prognosis can be expected with minimal complications. CLINICAL RELEVANCE: Use of an en bloc ostectomy technique for the treatment of biologically inactive nonunions permits easy resection of nonviable tissue. Subsequent fracture stabilization with compression plate fixation resulted in rapid bone healing without complications.  相似文献

OBJECTIVES: To compare the monotonic biomechanical properties of a prototype equine third metacarpal dynamic compression plate (EM-DCP) fixation with a double broad dynamic compression plate (DCP) fixation to repair osteotomized equine third metacarpal (MC3) bones. STUDY DESIGN: In vitro biomechanical testing of paired cadaveric equine MC3 with a mid-diaphyseal osteotomy, stabilized by 1 of 2 methods for fracture fixation. POPULATION: Twelve pairs of adult equine cadaveric MC3 bones. METHODS: Twelve pairs of equine MC3 were divided into 3 test groups (4 pairs each) for (1) 4-point bending single cycle to failure testing, (2) 4-point bending cyclic fatigue testing, and (3) torsional testing. The EM-DCP (10-hole, 4.5 mm) was applied to the dorsal surface of one randomly selected bone from each pair. Two DCPs, 1 dorsally (10-hole, 4.5 mm broad) and 1 laterally (9-hole, 4.5 mm broad) were applied to the contralateral bone from each pair. All plates and screws were applied using standard AO/ASIF techniques to MC3 bones that had mid-diaphyseal osteotomies. Mean test variable values for each method were compared using a paired t-test within each group. Significance was set at P<.05. RESULTS: Mean 4-point bending yield load, yield bending moment, bending composite rigidity, failure load and failure bending moment of the EM-DCP fixation were significantly greater (P<.0001) than those of the double broad DCP fixation. Mean cycles to failure in 4-point bending of the EM-DCP fixation was significantly greater (P<.0008) than that of the double broad DCP fixation. Mean yield load, composite rigidity, and failure load in torsion of the EM-DCP fixation were significantly greater (P<.0035) than that of the double broad DCP fixation. CONCLUSION: The EM-DCP provides increased stability in both static overload testing and cyclic fatigue testing. CLINICAL RELEVANCE: Results of this in vitro study support the conclusion that the prototype EM-DCP fixation is biomechanically superior to the double broad DCP fixation for the stabilization of osteotomized equine MC3.  相似文献

Fractures of Metacarpal and Metatarsal II and IV (the splint bones) were treated in 283 horses over an 11 year period. In 21 cases the proximal portion of the fractured bone was stabilized with metallic implants. One or more cortical bone screws were used in 11 horses, and bone plates were applied in 11 horses. One horse received both treatments. Complications of screw fixation included bone failure, implant failure, radiographic lucency around the screws, and proliferative new bone at the ostectomy site. Only two of the horses treated with screw fixation returned to their intended use. Complications of plate fixation included partial fixation failure (backing out of screws), wound drainage, and proliferative bony response around the plate. Six of the 11 horses treated by plate fixation returned to their intended use. The authors recommend consideration of plate fixation techniques for repair of fractures in the proximal third of the splint bone.  相似文献

A pulsed carbon dioxide laser was used to vaporize articular cartilage in four horses, and perforate the cartilage and subchondral bone in four horses. Both intercarpal joints were examined arthroscopically and either a 1 cm cartilage crater or a series of holes was created in the third carpal bone of one joint. The contralateral carpus served as a control. After euthanasia at week 8, the treated and control joints were examined for gross changes, and samples of cartilage and subchondral bone, synovial membrane, and peripheral lymph nodes were examined histologically. Depletion of cartilage matrix glycosaminoglycan was assessed by safranin-O histochemical staining of the laser site and adjacent cartilage. Cartilage removal by laser vaporization resulted in rapid regrowth, with fibrous and fibrovascular tissue and occasional regions of fibrocartilage at week 8. The subchondral bone, synovial membrane, and draining lymph nodes appeared essentially unaffected by the laser cartilage vaporization procedure. Conversely, carbon dioxide laser drilling of subchondral bone resulted in poor penetration, extensive areas of thermal necrosis of bone, and significant secondary damage to the apposing articular surface of the radial carpal bone.  相似文献
