Private equine hospital practices (PEHPs) may optimise patient healthcare by combining veterinary and farrier services. While PEHP veterinary services are generally well known, existing PEHP farrier services require better characterisation. To document farrier services at PEHPs in the USA, 41 PEHP Diplomates of the American College of Veterinary Surgeons and/or PEHP farriers completed a multiple choice questionnaire characterising PEHP farrier service prevalence and facilities; farrier details, experience and education; and farrier service finances, clinical services, training, education and research responsibilities. Questionnaire completion rate was 82%. Twenty‐two of 41 (54%) responding PEHPs had in‐house farrier service and 8 (20%) additionally provided ambulatory farrier service. Forty‐nine of 50 (98%) PEHP farriers were male; 2 (4%) were <31 years old, 10 (20%) were >51 years old; 27 (54%) worked in‐house <6 h/week. At 19 (86%) PEHPs, the farriers were paid by the client and at 3 (13%) by the PEHP. Thirty‐five of 50 (70%) PEHP farriers had a professional certification. At 4 (18%) PEHPs, the in‐house farrier service was profitable while at 14 (64%) PEHPs it met operational costs. Sixteen (73%) PEHP farrier services provided professional education in clinical settings and 11 (50%) in lecture settings. Five (10%) PEHP farriers participated in research activities. PEHP in‐house and ambulatory farrier services in the USA are a valuable resource with the potential to enhance the quality and scope of PEHP services. PEHP farriers can contribute positively to patient care, care‐provider education, and practice productivity.  相似文献

Veterinary teaching hospitals (VTHs) are facing more and greater challenges than at any time in the past. Changes in demand, expanding information, improving technology, an evolving workforce, declining state support, and an increasingly diverse consumer base have combined to render many traditional VTH modes of operation obsolete. In pursuit of continued success in achieving their academic mission, VTHs are exploring new business models, including innovative collaborations with the private sector. This report provides details on existing models for public-private collaboration at several colleges and schools of veterinary medicine, including those at Kansas State University, Purdue University, the University of Florida, and Tufts University. Although each of these institutions' models is unique, several commonalities exist, related to expansion of the case load available for teaching, the potential positive impact on recruitment and retention of clinical faculty, and the potential for easing financial pressures on the associated VTH. These new models represent innovative approaches that work to meet many of the key emerging challenges facing VTHs today.  相似文献

This article focuses on the role of veterinary teaching hospitals (VTHs) at public and private colleges and universities in the growth and nurturing of today's veterinary clinician scientists, who will assume the ranks of tomorrow's veterinary academic faculty. Virtually all residency programs offer some level of scientific education and training concurrently or sequentially with professional clinical training; approximately half (51%) include, or at least offer, the opportunity to obtain advanced scientific degrees (MS/MSc and/or PhD). If veterinary schools and colleges are the gatekeepers for the veterinary profession, then VTHs are the gatekeepers for the profession's scientific development, defining the future role of veterinary medicine in society. Although struggling to find their place in a financially challenging and competitive environment fueled by the demand for more and better clinical services, VTHs are in a potentially strong position to capitalize on the uniqueness of their faculty and academic resources and thus make an unparalleled contribution to the scientific evolution of the veterinary profession.  相似文献

Hoof preparation and shoe fit are integral to the long term soundness and athletic ability of horses. The farrier influences the balance of the hoof by both the trimming and the shoeing procedure. The goal of this study was to investigate and quantify the influence of the farrier on hoof parameters by trimming. Forty Warmblood horses ranging in age from 6–12 years and withers height 162–172 cm were included. They were divided into six groups consisting of six or seven horses each, shod by one of six different farriers. The hoofs of the horses were radiographed twice using a standardised method before and after trimming. Measurements were taken from digitalised radiographs using a software program.Significant differences were identified between farriers for almost all parameters on the lateromedial radiographic views after two consecutive trimming procedures. Comparing the results of both trimming procedures, significant differences for several farriers were found in the lateromedial and dorsopalmar views. There were significant differences for most of the measured hoof parameters between the six farriers and between consecutive trimmings of the same farrier. The results underline the individual influence of the farrier on hoof shape and balance.  相似文献

Foot management and hoof care of foals, weanlings, and yearlings tends to be considered a specialist job. Many farriers and veterinary surgeons have little exposure to young and growing stock. Nevertheless, a variety of angular and flexural limb deformities can benefit from simple podiatry techniques, which can be performed provided that a few basic rules are followed. The combined skills of the farrier and veterinary surgeon offer many young horses with angular or flexural limb deformities a chance of a future athletic career.  相似文献

Three projects recently funded by the American Veterinary Medical Association (AVMA) through the National Commission on Veterinary Economic Issues (NCVEI) focused on the veterinary school applicant pool, leadership skills in the veterinary profession, and a veterinary teaching hospital business model, respectively. The Skills, Knowledge, Aptitude, and Attitude (SKAs) Colloquium was designed to present the results of these three projects, to discuss their importance for the future of the veterinary profession, and to develop action plans accordingly. In all, 24 veterinary colleges were represented at the workshop and a total of 72 attendees participated, achieving a broad representation of the veterinary profession ( both academic and non-academic). Through an orchestrated combination of general sessions and facilitated small group discussions, recommendations for implementation and initial action plans for next steps were developed. From these, a list of potential AAVMC follow-up activities was developed, including advocating and facilitating programs across schools to engage and educate faculty regarding the results of these projects; developing realistic information on careers in veterinary medicine; organizing an AAVMC leadership consortium; working toward further development and implementation of the veterinary teaching hospital (VTH) business model; coordinating and sponsoring a national forum on the future of the VTH; reviewing admissions processes; integrating leadership into veterinary curricula; and organizing opportunities for faculty development in leadership.  相似文献

Veterinary teaching hospitals (VTHs) are experiencing case-load trends that have negatively affected efforts to prepare students for entry-level veterinary practice, particularly in the area of technical skills training. This article examines the clinical training available to veterinary students through a variety of collaborative shelter models. Benefits and potential problems related to initiating a collaborative shelter clinical training program are reviewed. Collaborative efforts between animal shelters and veterinary schools can provide crucial opportunities for outreach teaching initiatives, particularly for teaching medical and surgical skills.  相似文献

Those involved in the early history of veterinary medicine in the United States of America provided valuable services and an excellent foundation for later development of the profession. Even though the farriers and cowleeches had little knowledge, and occasionally less ethics, most performed as best they could. Certainly, the medical profession contributed greatly to the success of the American veterinary profession. Furthermore, the influence of British veterinarians and their publications added much to improving the profession. Perhaps the greatest accomplishment for the betterment of the profession was the establishment of the United States Veterinary Medical Association. Once the practicing veterinarians in the United States became organized and concerned, the profession appeared to develop rapidly providing both educational opportunities and pride in veterinary medicine.  相似文献

Veterinarian/farrier/owner communication can be difficult in a given lameness case. A small group of veterinarians and farriers discussed the factors complicating this communication. Specific definition of roles; professional respect; differences in required education; distance and driving to horse, fees, and licensure requirements were cited as the primary complicating factors. In conclusion, the group suggested the ideal communication for a specific lameness case was for the veterinarian, farrier, and owner to be present when shoeing was being planned. A telephone call would be second in preference and a written case summary was the third choice. There are suggestions on how to formulate the written summary.  相似文献

Background: Nosocomial salmonellosis is an important problem for large animal veterinary teaching hospitals (VTHs). Objective: To describe failure of an Infection Control Program (ICP) that resulted in an outbreak of salmonellosis caused by Salmonella Newport multidrug resistant (MDR)‐AmpC at a large animal VTH. Animals: Sixty‐one animals identified with the outbreak strain of Salmonella. Methods: Retrospective study: Data collected included signalment, presenting complaint, duration of hospitalization, discharge status, and financial information. Phenotypic and genotypic characterization was performed on Salmonella isolates. Results: The outbreak occurred despite an existing ICP; the ICP was reviewed and weaknesses identified. Routine patient surveillance was not performed before or during the outbreak; fecal sampling was triggered only by a patient algorithm based on clinical signs. Sixty‐one animals were infected with the outbreak strain of S. Newport, and the majority were horses (n = 54). Case fatality rate was 36.1%. S. Newport isolates demonstrated high genetic similarity (Dice ≥ 0.96), and all had the MDR‐AmpC phenotype. Environmental persistence of the organism necessitated complete hospital closure, extensive decontamination, and remediation of the facility. A paradigm shift in the relevance of biosecurity in a VTH and the establishment of a stringent ICP were integral components of successful hospital reopening. Conclusions and Clinical Importance: An ineffective ICP resulted in a nosocomial outbreak caused by a MDR S. Newport in a VTH. Closure of a VTH affected all missions of the institution and had substantial financial impact (US$4.12 million).  相似文献

The demographic characteristics of the veterinary profession in New Zealand are examined using information supplied by registered veterinarians to the Veterinary Surgeons Board in their applications for annual practising certificates in 1985. Comparisons are made with the veterinary professions in other similar countries, and with the medical and paramedical professions in New Zealand. In 1985 1308 registered veterinarians were working in New Zealand and 304 were overseas, whereas in 1976 the equivalent figures were 748 and 182. This represents an increase of 73% in the number of registered veterinarians in the last 10 years. Eighteen percent of veterinarians were female. The mean age of veterinarians was 38 years, but females in the profession were on average much younger (32 years) than males (42 years). Clinical practice provides employment for 70% of veterinarians: of these 59% work principally with farm animals and 41% with non-farm animals. The remaining veterinarians are employed by Government (22%), University (5%) and Industry (3%). The current demographic structure of the profession has been markedly influenced by the opening of New Zealand's only veterinary school at Massey University in 1963. Differences from other medical professional populations in New Zealand and overseas principally reflect the marked change in the number and sex ratio of graduates entering the veterinary profession since that time. It will take about another twenty years before the population reaches a stable age and sex structure, assuming that current graduation patterns persist throughout that period. Because the structure of the population is changing, considerable caution is needed in predicting future employment trends from data for a single year.  相似文献

OBJECTIVE: To evaluate efficacy of 2 disinfectants as used in footbaths in veterinary hospitals for reducing bacterial contamination of footwear. DESIGN: Prospective study. SAMPLE POPULATION: Bacteria collected from the soles of rubber boots after experimental contamination and exposure to disinfectant solutions or control conditions. PROCEDURES: Investigators contaminated boots by walking through soiled straw animal bedding. Swab samples were collected from the sole of 1 boot (right or left) without treatment. The other boot was briefly immersed in a disinfectant solution (either a quaternary ammonium compound [QAC] or a peroxygen compound) or water, and samples were collected after 7 minutes. Differences associated with the experimental treatments were analyzed statistically. Veterinary teaching hospitals (VTHs) in the United States and Canada were contacted to obtain information about the use of footbaths. RESULTS: Mean bacterial concentrations from peroxygen-treated boots were 67% to 78% lower, compared with samples taken from untreated boots. In contrast, there were no statistically detectable differences in mean bacterial concentrations in samples taken from QAC- or water-treated boots, compared with control boots. Disinfectant footbaths were reportedly used in 30 of 31 VTHs. CONCLUSIONS AND CLINICAL RELEVANCE: Disinfectant solution containing peroxygen applied in a footbath reduced bacterial concentrations on rubber boots under conditions representative of those found in VTHs. Footbaths are commonly used as a method to control infectious diseases in veterinary hospitals. Disinfectant footbaths should not be expected to sterilize footwear, but they may help in reducing the risk for nosocomial infection when used with effective disinfectants.  相似文献

A World Association for the Advancement of Veterinary Parasitology tradition for its conference is to present some highlights of the country hosting the event, and with an emphasis on the history of, and research in, veterinary parasitology. A review of Canada's peoples, physiography, climate, natural resources, agriculture, animal populations, pioneers in veterinary parasitology, research accomplishments by other veterinary parasitologists, centres for research in veterinary parasitology, and major current research had been presented at a World Association for the Advancement of Veterinary Parasitology Conference in Canada in 1987, and was published. This article reviews the evolution of veterinary medicine in Mexico in order to explain the foundation of the National Council for Veterinary Medicine Education (CONEVET). This institution performs college accreditation and professional certification procedures in the country. The accreditation process for colleges of veterinary medicine in Mexico is described, and the results obtained to this point are presented. A comparative analysis of veterinary education in Mexico and the United States is also presented. The Skills, Knowledge, Aptitude, and Attitude (SKA) Subcommittee of the National Commission on Veterinary Economic Issues (NCVEI) has identified the need for veterinary teaching hospitals (VTH) to be at the forefront of progressive business management to serve as a model for both students and practitioners to emulate. To provide a foundation for developing a model, this study reviewed pertinent literature applicable to the management of a VTH. Much of the literature relevant to VTH management relates to work completed for the human side of medicine (academic health centers, or AHCs) or to the private sector. This review explores management practices in strategic planning, financial management, human resource management, marketing, pricing, operations, and legal issues. It is concluded that strategic management is important to provide the foundation for success in the VTH. In addition, periodic financial reports are recommended, as are the development and use of benchmarks for financial management. Establishing positive, motivating human resource practices is also suggested, along with development of a marketing plan based on a clear understanding of VTH core competencies and the market's specific needs.  相似文献

This paper discusses the methods which veterinarians in Australia use to calculate their fees and charges. Problems associated with these methods and the tendency to undervalue the professional component of veterinary services are discussed. The paper introduces the concept of a relative value study of veterinary professional services within Australia and recommends the development of a relative value scale and conversion factors, based on appropriate resource cost accounting methodology, as a method to quantify veterinary professional service charges. A relative value scale is a table of "weights" that defines the relative value of procedures or services one to the other. A conversion factor is used to change the relative value unit to a dollar price and thus convert a relative value scale to a fee schedule. The Australian Veterinary Association is suggested as the most appropriate organisation to initiate and co-ordinate the research necessary for the development of these recommendations on a national basis.  相似文献

Veterinary teaching hospitals (VTHs) have traditionally obtained most of their patient and client base through the referral process. This worked well until the recent explosive growth of specialty practices, which compete not only for patients but also for faculty and graduating residents. Veterinary schools have had to meet this challenge by increasing both efficiency and the quality of services provided to referring veterinarians. Practitioners refer mainly because of discomfort with a case and the belief that clients will get better treatment at a referral hospital than they themselves can provide. Practitioners choose not to refer because of geography, perceived cost, or lack of confidence in the services offered. Referring veterinarians expect regular communication about services offered, access to receiving clinicians for consultation, convenient scheduling, and efficient communication and follow-up from the receiving veterinarian. They also expect the relationship between them and their clients to be maintained and enhanced. Receiving veterinarians expect a summary letter and copies of all relevant records, including radiographs. They also expect the client to have been informed about the approximate costs of referral. VTHs can develop better relationships with referring veterinarians through education, newsletters, referral guides, practice visits, and Web sites. Inadequate communication and lack of involvement on the part of the referring veterinarian are the major impediments to efficient referrals and practice growth.  相似文献

The life and work is told of John Pottie (1832-1908), a Scotsman who graduated from Edinburgh Veterinary College in 1858. A year later, he contracted to provide veterinary care to a consignment of horses bound for Australia. Once there, he founded a firm that has survived to the present day, still marketing products that originated in his own veterinary remedies. John Pottie brought with him a European tradition of livestock care and treatment that was epitomised in Clater's title and book, Every man his own farrier. His career is of interest for several reasons. Firstly it is because he used this tradition to launch a business enterprise in a new and different market in Australia. Secondly, although his training lay in what may be termed the pre-scientific era of the profession's history, he was able to adapt old traditions successfully to the changes that came in the age of Pasteur and Koch. Finally, the manner in which he did this, besides demonstrating his business acumen, also shows how the professional veterinary qualification gained in value in a new age of science.  相似文献

ObjectiveTo evaluate the efficacy of a perineural injection of dexmedetomidine combined with ropivacaine for reducing postoperative methadone requirements in dogs after tibial plateau levelling osteotomy (TPLO).Study designA prospective, clinical, randomized and blinded trial.AnimalsA total of 58 client-owned dogs.MethodsUltrasound-guided midfemoral sciatic and inguinal femoral nerve blocks with ropivacaine (1 mg kg–1 per nerve block) combined with either dexmedetomidine (0.5 μg kg–1 per nerve block; group DEX) or the same volume of saline (group CON) were performed in dogs undergoing TPLO. Pain was assessed 30 minutes, 2 hours and then every 4 hours for 24 hours after surgery with a validated pain scale (4AVet). Meloxicam (0.15 mg kg–1) was administered intravenously (IV) at recovery. Rescue methadone (0.2 mg kg–1 IV) was administered if a score ≥ 6 (maximal score 18) was recorded and the number of postoperative doses was analysed by Fisher exact tests. The study was performed in parallel at a Veterinary Teaching Hospital (VTH) and a private Veterinary Referral Centre (VRC).ResultsDogs received a total of 22 and 31 postoperative doses of methadone in groups DEX (14 doses at VRC, eight doses at VTH) and CON (28 doses at VRC, three doses at VTH), respectively. Overall, there was no difference in the postoperative rescue analgesia requirements between groups (p = 0.244). At the VRC, dogs received less methadone (p = 0.026) in group DEX compared with group CON, whereas at the VTH, there was no difference between groups (p = 0.216).Conclusions and clinical relevancePerineural dexmedetomidine combined with ropivacaine did not reduce postoperative methadone requirements in dogs after TPLO, but results may differ from one centre to another. This discrepancy might be linked to variations in clinical practices and questions the validity of results obtained from single-centre randomized controlled trials but applied to different clinical settings.  相似文献
