Who will pay for medical education in our teaching hospitals?
A S Relman 《Science (New York, N.Y.)》1984,226(4670):20-23
Although most medical educators believe that education, research, and patient care are inseparable and essential to their academic mission, the educational component of this triad has never been given adequate, earmarked support. To fund educational programs, medical centers first relied on research grants and later on third-party payments intended for patient care. However, research money has long since ceased to be available for other purposes and recent federal cost containment measures have started to reduce payments for patient care. Teaching hospitals are threatened with loss of support not only for education, but for their capital improvements and care of the poor. Many institutions are now hoping to generate new income through business deals with for-profit health care corporations, but this effort probably will also fail and may compromise professional traditions. Teaching hospitals serve the public interest and will have to depend, at least in part, on public subsidy of their unavoidable extra costs.

Animal health is key to successful livestock production in developing countries. The development and delivery of vaccines against major epidemic diseases is one component of improving animal health. This paper presents a case study from Kenya on the production and delivery of a vaccine against Contagious Caprine Pleuropneumonia (CCPP), a major disease of goats. The vaccine, while technically a viable preventative measure against CCPP, has not been well integrated into Kenya's animal health care system. From February through November, 1992, the authors explored the organizational and policy constraints to vaccine production and distribution. Data were obtained through interviews, field visits, and available documents. Results suggest an animal health system that 1) does not have a well developed infrastructure to track, identify, and control animal diseases; 2) tends to place greater emphasis on cattle, rather than small ruminant health care; and 3) is not well linked in terms of research, development, and extension services.

Given a continuation of the current approach toward the managing and funding of research across the spectrum of disciplines basic to health, the country is headed for stultification and possible social disaster. Health care delivery should be considered as a science-based social industry and managed accordingly. Stability and adequate relation of the research effort to the industry's output should be sought. The establishment of a trust fund, derived as a percentage of health revenues, and of a public commission or a board of directors to provide expert advice on its expenditures are suggested.

Although currently there are only a limited number of genomic technologies that are applicable to health care in the developing countries, this is unlikely to be the case in the near future. If, however, the full potential of genomics for health care is to be fulfilled, there will have to be a complete change of emphasis in education and research in the richer countries toward a more global view of disease and its consequences.
