OBJECTIVE: To describe changes in the initial work experiences of veterinary graduates over the last 50 years. DESIGN: A questionnaire, sent by mail. PROCEDURE: A questionnaire seeking information on work experience as a recent graduate was sent to about 100 veterinarians who graduated in or about each of 1950, 1960, 1970, 1980, 1990 and 2000, and 68% responded. Data were entered onto an Excel spreadsheet, and analysed with the SAS System 8 for Windows. RESULTS: The first veterinary position for about half the graduates of 1950 and 1960 was in government service. Since then an increasing percentage have started in small animal practice; the rest in mixed practice. Many earlier graduates worked alone and for long hours, with little or no respite after hours. Since 1950, the average recent graduate in private practice has worked progressively fewer hours with less after hours duty, and has worked with progressively more other veterinarians. Cattle were almost half the cases seen by graduates of 1950 and 1960. Dogs and cats were about 20% of the cases seen by those graduates, but this percentage ncreased progressively and was 77% of the cases seen by the graduates of 2000. Distemper was the most common transmissible disease of dogs seen by graduates of 1950-1970, but by few since then. Graduates of 1980 saw parvovirus most commonly; those of 2000 saw parvovirus and kennel/canine cough in about equal numbers. Two other major changes in canine cases seen by recent graduates have involved skin conditions, which increased progressively between 1950 and 2000, and trauma, mainly involving vehicles, which decreased as dogs have been restrained more effectively. Fracture repair was the most common surgical procedure performed by recent graduates between 1950 and 1970, but with decreasing incidence of trauma this has been overtaken by the removal of lumps and repair of lacerations. Barbiturates were used extensively by recent graduates for both induction and maintenance of anaesthesia, but with increasing use of anaesthetic machines from 1960 onwards, halothane and more recently isoflurane have largely taken their place for the maintenance of anaesthesia. CONCLUSIONS: Over the last five decades, the average recent graduate has had progressively more opportunity for support and advice from other veterinarians, to work more sociable hours, and to work with a narrower range of species, especially dogs and cats.  相似文献

Objective To describe the location, type, support from and effects of their first position for the veterinary graduates of 2005, and to make comparisons with graduates of 1950-2000. Procedure Questionnaires were sent to veterinarians who had graduated from an Australian university in 2005, and data were analysed with SAS System 8 for Windows. Results One-sixth of the graduates left for overseas, and 55% (Murdoch) to 85% (Melbourne) of the remainder found their first veterinary position in the state where they were trained. More were in small animal (48%) and less in mixed practice (48%) than in previous decades, and they spent more time (79%) with dogs and cats, and less with horses (9%), cattle (8%) and other production animals (1%). The median income of males working full-time was $43,000 and $39,000 for equivalent females. Most were satisfied with the help and support received from their boss and workplace; approximately 20% felt that support was inadequate, and many felt overworked and underpaid. The graduates with inadequate support were more likely to suffer stress and were less likely to have enjoyed their first year or to agree that they would become a veterinarian again. The levels of perceived support, and of stress, were similar to those reported by graduates of 2000, though more favourable than in the previous 2 decades. Conclusions Graduates of 2005 were less likely than their predecessors to start in mixed practice or to work with cattle and horses. OBJECTIVE: To compare the career paths of veterinarians who had grown up on farms with animals with those of veterinarians from other backgrounds, during the first 15 years after they graduated. PROCEDURE: Questionnaires were completed by veterinary students in their first and fifth year, and after 1, 5, 10 and 15 years after they graduated. The most recent questionnaires were returned by 134 of 137 graduates, 20% of whom had grown up, or lived for at least two years, on farms where animals were a primary source of income. This paper includes findings from that questionnaire but also contains comparisons with previous ones. RESULTS: Fifteen years after they graduated, veterinarians who had grown up on farms with animals were more likely than those of other backgrounds to be living in centres with < 10,000 people (58% farm, 20% other), irrespective of the type of work they were doing. Although they were more likely to be doing no veterinary work (42% farm, 21% other), they were also more likely to be working in mixed practice (15% farm, 9% other) and with cattle and sheep generally (11% full time equivalent farm, 5% full time equivalent other). However, > 80% of those who had started their careers in mixed practice were no longer in mixed practice after 15 years. Veterinarians of farm origin who were in private practice were more likely to be sole or part owners (93% farm, 46% other) and less likely to be employees (7% farm, 54% other). CONCLUSIONS: In this study, veterinarians who had grown up on farms with animals seemed slightly more likely than those from other backgrounds to continue working in mixed practice and to work with cattle or sheep, although the numbers were small and the differences not significant. Whatever their background, the majority who started their careers in mixed practice left over the next few years, and by 15 years only 15% remained.  相似文献

OBJECTIVE: To determine the clinical skills and areas of knowledge used by veterinarians in small animal practice during their first year after graduation and the degree of assistance and supervision they received while developing these skills. DESIGN: A postal survey was sent in December 1999 to 59 veterinarians who completed their training at Murdoch University in December 1998. PROCEDURE: The first part of the survey asked for information on veterinary work patterns since leaving university. The second part consisted of a list of diagnostic and therapeutic skills of varying complexity and the graduates were asked to indicate whether they had used these skills in practice and whether they had been assisted or supervised while doing them. The respondents were also asked if they had had the opportunity to practise these skills as undergraduates at university or during extramural experience. In the third part the areas of knowledge used in practice were assessed by analysis of a series of consecutive cases. RESULTS: Forty replies were received but since three graduates had done no small animal work the analysis of the skills section is based on 37 responses. Thirty graduates supplied information on 994 canine cases and 308 feline cases. The distribution of the mean work time was dogs and cats 69%, horses 13%, farm animals 11%, birds 3% and others 4%. Skills used by over 90% of graduates included general anaesthesia, examination of the tympanic membrane, taking and interpreting an abdominal radiograph, catheterising a male cat, fine needle aspiration of a mass, neutering dogs and cats, tooth scaling and extraction and treating an aural haematoma. The survey also identified the opportunities for undergraduates to practise some of these skills during extramural experence and the extent of assistance given to new graduates during their first year in practice. The areas of knowledge used in over 10% of the cases included vaccination, anaesthesia/sedation, skin/coat problems, general advice on pet health, neutering and musculoskeletal diseases. CONCLUSION: Veterinarians, in their first year after graduation, use a wide range of complex diagnostic and therapeutic skills. Although many of these skills are acquired during the undergraduate training, a significant contribution is made by extramural practical work undertaken during the clinical years of the undergraduate course and in the first year following graduation. OBJECTIVE: To map out the career paths of veterinarians during their first 10 years after graduation, and to determine if this could have been predicted at entry to the veterinary course. DESIGN: Longitudinal study of students who started their course at The University of Queensland in 1985 and 1986, and who completed questionnaires in their first and fifth year as students, and in their second, sixth and eleventh year as veterinarians. METHODS: Data from 129 (96%) questionnaires completed during the eleventh year after graduation were coded numerically then analysed, together with data from previous questionnaires, with SAS System 7 for Windows 95. RESULTS: Ten years after they graduated, 80% were doing veterinary work, 60% were in private practice, 40% in small animal practice and 18% in mixed practice. The equivalent of 25% of the working time of all females was taken up by family duties. When part-time work was taken into account, veterinary work constituted the equivalent of 66% of the group working full-time. That 66% consisted of 52% on small animals, 7% on horses, 6% on cattle/sheep and 1% on pigs/poultry. Those who had grown up on farms with animals were twice as likely to be working with farm animals as were those from other backgrounds. Forecasts made on entry to the veterinary course were of no value in predicting who would remain in mixed practice. CONCLUSIONS: Fewer than one-fifth of graduates were in mixed practice after 10 years, but the number was higher for those who grew up on farms with animals. Forecasts that may be made at interview before entry to the course were of little value in predicting the likelihood of remaining in mixed veterinary practice.  相似文献

OBJECTIVE: To describe the career paths of veterinary graduates from the University of Queensland during the first 15 years after they graduated, paying particular attention to the fifteenth year. PROCEDURE: Longitudinal study. Questionnaires were completed by 154 first year veterinary students in 1985 and 1986, then while they were in fifth year, and after 1, 5, 10 and 15 years after they graduated. This paper describes findings from the most recent questionnaire but includes comparisons with previous ones. RESULTS: Completed questionnaires were received from 134 of the 137 graduates. They were spread widely, but 58% were in Queensland and 20% in New South Wales; 17% were overseas. Most (85%) were in private practice, and 56% of them were part or sole owners; 58% were working in cities with > 100,000 people, and 28% in centres with < 10,000 people. Overall 80% of the workload of the group was with dogs and cats, 8% with horses and 11% with farm animals. Most felt that their income was too low. Of those doing less than half-time or no veterinary work, 44% were at home with family, 8% in another business, 11% another profession, and the rest in a wide variety of occupations. Changed interests, need for greater challenges, poor remuneration, loss of interest in veterinary work, and caring for family were, in order, the top five (of 15) reasons for leaving veterinary work. Three-quarters (78%) of all those doing veterinary work, but 52% of those no longer doing veterinary work agreed that their career had lived up to their expectations. CONCLUSION: After 15 years 77% were still doing veterinary work, 85% in private practice, and 80% of their work was with dogs and cats. Almost all were either very glad or generally glad that they had done the veterinary course, though most (78%) felt that their income was too low.  相似文献

OBJECTIVE: To describe the attitudes of veterinary graduates over the last 50 years to their experiences as recent graduates DESIGN: A questionnaire, sent by mail. PROCEDURE: A questionnaire seeking information on experiences as a recent graduate was sent to about 100 veterinarians who graduated in or about 1950, 1960, 1970, 1980, 1990 and 2000, and 68% responded. Data were entered onto an Excel spreadsheet, and analysed with the SAS System 8 for Windows. RESULTS: Most veterinarians who graduated in 1970 and earlier were generally satisfied with their conditions, even though these were often taxing. In the 1970s the cattle industry faltered and rural practice suffered a downturn. Veterinary positions became scarce, and there were few opportunities for graduates to move on from an unsatisfactory workplace--as many were at that time. By 1980 many recent graduates were questioning their decision to study veterinary science. Many of them, working long hours under often stressful conditions, felt that that they were not receiving adequate help or support--or remuneration--from their employers, or respect from their clients. This also afflicted many who graduated in 1990, but by 2000 graduates were reporting an improvement. Their hours of work and the expectations placed on them were more manageable, and the level of help and support, and remuneration, were more acceptable. As a result they found their first year much more enjoyable than had those who had graduated since about 1970. Many of the factors contributing to this improvement resulted from changes in society generally. Specifically, both graduates and employers have become more conscious of what might reasonably be expected, an Award specified minimum levels, the AVA started accrediting New Graduate Friendly Practices, publishing a most informative New Graduate Guide, and helping in other ways. Remuneration, especially when compared with comparable professions, continues to be a source of discontent. Furthermore, despite improved conditions on average, many graduates still suffer under substandard conditions, and these can have an adverse effect on their health and their attitude to their career in veterinary science. CONCLUSIONS: Average conditions of employment of recent veterinary graduates have improved somewhat after a nadir in the 1970s and 1980s, but they are still substandard for some. Remuneration remains the greatest source of dissatisfaction for recent graduates.  相似文献

OBJECTIVE: To determine the number, concentration and distribution of veterinarians working in Australia in 2006, and compare with data from 1981, 1991 and 2001. Procedure Data on each veterinarian who was registered, resident and apparently working in each Australian state and territory were obtained from relevant veterinary board lists, entered into an Excel spreadsheet and analysed with SAS System for Windows 8. Other data were obtained from the Official Australian Postcode Map, and the Australian Bureau of Statistics. RESULTS: A total of 7510 veterinarians, of whom 46% were female, were working in Australia in 2006. The rate of increase (230/year) between 2001 and 2006 was greater than for the previous 20 years. The number per million people (360) was 73% higher than in 1981, and more than 30% higher than in the USA and UK. With the establishment of three new veterinary schools the number of graduates, and the total number of veterinarians, will increase further. By 2011 the number of veterinarians is likely to be three-fold greater, and the number per million people two-fold greater than in 1981. The number of veterinarians per million dogs and cats will increase at a greater rate if dog and cat numbers continue to fall. As more than 75% of Australian veterinary work involves dogs and cats this has serious implications for the profession. CONCLUSIONS: Progressively increasing numbers of veterinarians will compete for a constant or diminishing resource--the dogs and cats of Australia. It seems likely that overt signs of oversupply will appear before sufficient numbers of veterinarians enter alternative forms of employment.  相似文献

OBJECTIVE: To determine the incidence and nature of adverse reactions of dogs and cats to tick antitoxin serum and to re-evaluate the role of atropine in the treatment of tick paralysis. DESIGN: A retrospective questionnaire of veterinarians. PROCEDURE: Questionnaires were posted to 320 veterinarians in tick-endemic regions of Australia. Questions referred to dogs and cats treated for tick paralysis over a period of three years: the number treated, treatment protocols and adverse systemic reactions to tick antitoxin serum. Ninety completed questionnaires were returned and responses analysed. RESULTS: Veterinarians reported that approximately 3% of dogs exhibited adverse reactions immediately following treatment with tick antitoxin serum. Eighteen percent of these reactions were described as anaphylaxis, with the remaining 82% attributed to the Bezold-Jarisch reflex. Six percent of cats treated with tick antitoxin serum reacted adversely and the majority of reactions (63%) were ascribed to the Bezold-Jarisch reflex. Atropine was used routinely by 10% of responding veterinarians in the treatment of dogs and cats with tick paralysis. A similar number of veterinarians used atropine only in selected cases. Most veterinarians (76%) reported that they never used atropine in the treatment of tick paralysis in either dogs or cats. Within the survey population, premedication with atropine reduced the number of Bezold-Jarisch reactions following tick antitoxin administration approximately five-fold in dogs and four-fold in cats. CONCLUSIONS: Data from this pilot survey indicate that more cats than dogs have adverse systemic reactions to tick antitoxin serum and that the majority of these reactions in both dogs and cats could be related to the Bezold-Jarisch reflex. The number of reactions to tick antitoxin serum in dogs and cats could be significantly reduced by the routine use of atropine prior to administration of tick antitoxin serum.  相似文献

Objectives To determine the patterns of work undertaken by veterinarians during the first 5 years after graduation, and to study the background to career decisions. Design Longitudinal study. Population Veterinarians (119) who started the veterinary course at The University of Queensland in 1985 and 1986, and who responded to a mailed questionnaire 5 years after they graduated. Procedure Questionnaires were completed in the first and fifth year of the veterinary course, and in the second and after the fifth year after graduation. The data were coded numerically and analysed using the SAS System for Windows. Results Most respondents (61%) planned to work in mixed practice and 61% entered mixed practice initially, but only 39% of these (26% of the total) remained in mixed practice after 5 years. Those who had grown up on a farm which derived primary income from animals were more likely to enter mixed practice, but half of this group had left mixed practice after 5 years. Most decisions to change career direction involved a move away from negative factors including inadequate private time and/or rewards, issues involving other people including the practice principal and sometimes clients, or aspects of the work itself. However, some cited positive reasons, including opportunities to learn, to help others or to travel. There were no significant differences between males and females in the numbers entering mixed practice or leaving it, or entering small animal practice, or in attitudes to many issues involving veterinarians. However, more females than males found their remuneration too small, and females did receive significantly smaller incomes than males. Conclusions Most graduating veterinarians, especially those from farms with animals, seek to enter mixed practice initially. Negative experiences are mainly responsible for moves to small animal practice. Gender does not affect the distribution of young veterinarians between mixed and small animal practice.  相似文献

A survey was conducted in 2000 into the use of analgesic drugs by veterinarians in South Africa. This survey was repeated in 2005 to establish whether the use of analgesic drugs has increased and which analgesic drugs are being used for acute pain and osteoarthritis. The number of sterilisations performed and the number of new cases of osteoarthritis in dogs and cats was estimated. It is estimated that approximately 260000 cats are operated on each year in South Africa and that 150000 cats are sterilised. Five hundred thousand dogs undergo surgery, of which 242000 are sterilised. It appears that the number of surgical procedures performed in South Africa has decreased. The estimated death rate following anaesthesia has remained unchanged at 1:1004. Overall, the use of analgesics by South African veterinarians has increased significantly. Fifty-six per cent of cats and 74% of dogs were given peri-operative analgesics but this increased to 94% and 84% after including pre-anaesthetic medications with analgesic properties. The use of opioids (morphine and buprenorphine) and propofol has increased significantly. Approximately 253000 dogs and 33000 cats with osteoarthritis are seen by veterinarians in South Africa annually. The recognition by veterinarians of osteoarthritis in cats appears to be poor and is in need of attention. Carprofen and glucosamine/chondroitin are the most commonly used agents for the treatment of osteoarthritis. Details of the drugs used by veterinarians are given. Knowledge of analgesic drugs has increased significantly over the last 5 years. Continuing education is thought to have played an important role in the changes reported in this study.  相似文献

Objective To estimate the time veterinary graduates spend in private practice and in veterinary work generally, and to determine what factors influence this.
Methods Questionnaires were completed in the sixth year after graduation by 119 veterinarians who had participated in this longitudinal study since starting the veterinary course, and the data were analysed using the SAS System for Windows.
Results Of those who started the veterinary course, 90% graduated, and 96% of these then entered private practice. Five years later 94% (73% in full-time equivalents) were working as veterinarians, and 64% (50% in full-time equivalents) were still in private practice in Australia. Hours, attitudes of principals and clients, and inadequate rewards were the main reasons for leaving private practice. The likelihood of being still in private practice was greater for those who had had significant responsibility for animals before they entered the course, but it was not related to geographical origin (city vs country), age at entry or gender. Women were, however, more likely than men to be working part time as veterinarians. Predictions of veterinary working life were not affected by geographical origin or by previous experience with animals or on farms, but men expected to work longer than women.
Conclusion The average veterinary career, estimated by veterinarians who graduated 5 years earlier, is 24 years in fulltime equivalents for men, and 16 years for women. Almost all (94%, representing 73% in full-time equivalents) still work as veterinarians after 5 years, most of them (76%, representing 59% in full-time equivalents) in private practice. The likelihood of remaining in private practice is related to previous responsibility for animals.  相似文献

AIMS: To establish the incidence, frequency and type of deliberate animal abuse seen in veterinary practice in New Zealand, and ascertain veterinarians' knowledge of human abuse within the families where animal abuse was occurring. To explore attitudes of veterinarians to and knowledge about the correlation between animal abuse and human violence, and their perceived role in dealing with such issues. METHODS: A postal questionnaire was sent to practising veterinarians in New Zealand. It covered demographics, frequency and type of animal abuse cases seen, awareness of violence towards humans, within families where animal abuse was seen, the methods used for managing animal and human abuse, the incidence of psychological abuse of animals, and the attitudes of veterinarians towards and their beliefs about the link between animal abuse and human violence. RESULTS: A total of 383/1,412 (27% response rate) questionnaires with useable data were returned. Within the group of respondents, the sexes were evenly represented; 40% of respondents worked in small-animal practice, 50% in mixed practice and 10% in large-animal practice. Deliberate animal abuse had been seen by 63% of respondents in the last 5 years; 37% of these had seen such cases once a year or less, while 9% had seen abuse cases at least four times a year. Dogs were the species most commonly reported as abused, followed, in numerical order, by cats, cattle and horses. Among respondents who had seen deliberate animal abuse, 16% either knew of (4%) or suspected (12%) human abuse within the families of the abused animals. A clear majority of responding veterinarians agreed with the statement that people who abuse their animals are more likely to abuse their children (77%) or spouse (70%). CONCLUSIONS: The survey indicated that the majority of respondents had seen cases of animal abuse within the previous 5 years, and dogs were the species most often reported abused. Responding veterinarians felt a strong ethical duty to deal with cases of animal abuse, but they were less comfortable about issues of human abuse, even though the awareness of the link between abuse of animals and abuse of humans was relatively high. CLINICAL RELEVANCE: The greater the awareness of animal abuse and its linkage with human abuse, the greater the possibility that veterinarians can contribute in a meaningful way to the reduction of violence in society.  相似文献

The Tulsa Registry of Canine and Feline Neoplasms was the second animal tumor registry in the United States concerned with a defined population in a delimited geographic area. Only tumors histologically confirmed by registry pathologists were included in frequency statistics based on the annual dog and cat population presented to veterinarians. During the first registry year, about 1% of the 63,504 dogs and 0.5% of the 11,909 cats had one or more primary tumors. While the incidence rate for malignant tumors in dogs was similar to that in cats, the incidence of benign tumors of dogs was over 10 times that of cats. The most common tumors were sebaceous adenoma in dogs and lymphosarcoma in cats. Mammary cancer was the most common malignant tumor in dogs. Mammary tumors of female dogs were significantly more frequent in Pointers, Poodles and Boston Terriers, in that order, than in other breeds. A greater incidence of mammary tumors among intact compared to spayed female dogs was seen for virtually every age group except in the Pointer breed.  相似文献

Four hundred and seventeen Canadian veterinarians were surveyed to determine their postoperative use of analgesics in dogs and cats following 6 categories of surgeries, and their opinion toward pain perception and perceived complications associated with the postoperative use of potent opioid analgesics. Three hundred and seventeen (76%) returned the questionnaire. An analgesic user was defined as a veterinarian who administers analgesics to at least 50% of dogs or 50% of cats following abdominal surgery, excluding ovariohysterectomy. The veterinarians responding exhibited a bimodal distribution of analgesic use, with 49.5% being defined as analgesic users. These veterinarians tended to use analgesics in 100% of animals following abdominal surgery. Veterinarians defined as analgesic nonusers rarely used postoperative analgesics following any abdominal surgery. Pain perception was defined as the average of pain rankings (on a scale of 1 to 10) following abdominal surgery, or the value for dogs or cats if the veterinarian worked with only 1 of the 2 species. Maximum concern about the risks associated with the postoperative use of potent opioid agonists was defined as the highest ranking assigned to any of the 7 risks evaluated in either dogs or cats. Logistic regression analysis identified the pain perception score and the maximum concern regarding the use of potent opioid agonists in the postoperative period as the 2 factors that distinguished analgesic users from analgesic nonusers. This model correctly classified 68% of veterinarians as analgesic users or nonusers. Linear regression analysis identified gender and the presence of an animal health technologist in the practice as the 2 factors that influenced pain perception by veterinarians. Linear regression analysis identified working with an animal health technologist, graduation within the past 10 years, and attendance at continuing education as factors that influenced maximum concern about the postoperative use of opioid agonists.  相似文献

OBJECTIVE: To ascertain anaesthetic practices currently for dogs and cats in Australia. METHODS: A questionnaire was distributed to 4,800 veterinarians throughout Australia, seeking data on numbers of dogs and cats anaesthetised per week; drug preferences for anaesthetic premedication, induction and maintenance; use of tracheal intubation, supplemental nitrous oxide and anaesthetic antagonists; and types of vaporisers, breathing systems and anaesthetic monitoring devices used or available. Additional questions concerned proportions of different animal types seen in the practice, and the respondent's university and year of graduation. RESULTS: The response rate was 19%; 95% of respondents graduated from Australian universities, about half since 1985. Most responses (79%) came from mainly small animal practices. On average 16 dogs and 12 cats were anaesthetised each week. Premedication was used more often in dogs than cats, with acepromazine and atropine most favoured in both species. For anaesthetic induction, thiopentone was most preferred in dogs and alphaxalone/alphadolone in cats. Inhaled agents, especially halothane, were preferred for maintenance in both species. Most respondents usually employed tracheal intubation when using inhalational anaesthetic maintenance, but intubation rates were lower during injectable anaesthetic maintenance and a minority of respondents provided supplemental O2. Nitrous oxide was administered regularly by 13% of respondents. The agents most frequently used to speed recovery from anaesthesia were doxapram and yohimbine. The most widely used vaporisers were the Fluotec Mark III and the Stephens machine. Most (95%) respondents used a rebreathing circuit for large dogs and a non-rebreathing system was used for small dogs by 68% of respondents. Most respondents (93%) indicated some form of aid was available to monitor general anaesthesia: the three most mentioned were an apnoea alarm, oesophageal stethoscope and electrocardiogram. CONCLUSION: Diverse approaches were evident, but there appeared to be less variation in anaesthetising dogs: premedication was more frequent and less varied in type, while thiobarbituates dominated for induction and inhalants for maintenance. Injectable maintenance techniques had substantial use in cats, but little in dogs. Evident disparity between vaporisers available and circuits used suggested either confusion in terminology or incorrect use of some vaporisers in-circuit. While most respondents used monitoring equipment or a dedicated observer to invigilate anaesthesia, the common reliance on apnoea alarms is of concern, because of unproven reliability and accuracy.  相似文献

OBJECTIVE: To compare the attitudes of French veterinarians to pain, and their provision of analgesia to animals, with that reported from other countries. STUDY DESIGN: Epidemiological study. METHODS: In June 1999, 379 French veterinarians were surveyed to ascertain their views on pain evaluation and control in dogs and cats, and their use of analgesics in daily practice. Survey results are expressed as a percentage of responses. RESULTS: The questionnaire was returned by 189 veterinarians (49.9%). The response rate was influenced by age (younger veterinarians were more likely to respond) but not gender. A majority (99.5%) expressed moderate to extreme concern over pain in their patients. Pain evaluation was based on the animal's attitude (88.3% dogs, 82.5% cats), interaction with the caregiver, response to palpation of the painful area (66.5% dogs, 62.7% cats) and inappetence (29.3% dogs, 46.3% cats, p < 0.001). Only 14.3% of respondents considered their knowledge of pain recognition to be inadequate. Many (58.8%) considered their methods of pain quantification and control (47% dogs, 59% cats) to be inadequate. Difficulties in recognizing pain (58.3%), a lack of knowledge in the appropriate use of analgesics (41.7%) and fear of drug side effects (30%) were used to explain inadequate provision of analgesia. Only 16.1 and 8.1% used opioids in dogs and cats, respectively. This low level of use resulted from the imposition of French narcotic legislation (79.9%) and lack of knowledge of opioid pharmacology (73.7%). Nonsteroidal anti-inflammatory drugs (NSAIDs) and glucocorticoids were the most commonly used analgesics in both species (100% (dogs) and 96.7% (cats)). The most popular NSAID used in France was tolfenamic acid, followed by meloxicam (dogs), ketoprofen, nimesulide (cats) and carprofen (dogs). The type of surgery performed influenced the use of analgesics, from 17.2% for castration to 83.7% for orthopaedic procedures. Nonsurgical conditions believed to warrant analgesia included osteoarthritis (97.8%), trauma (97.3%) and bone neoplasia (93.4%). Female veterinarians were more likely than males to evaluate pain and provide analgesia. CONCLUSION: French practitioners demonstrate a level of interest in analgesia, which appears to be at least equivalent to that reported from English-speaking countries. The signs used to indicate the presence of pain do not, in general, appear to differ. Excessive confidence in their ability to recognize pain (despite a general ignorance of the subject), the minor role of animal health technicians in pain management and misconceptions about analgesics (mainly opioids) are French pecularities.  相似文献

Objective   To identify veterinarians' approaches and concerns when managing canine and feline patients with acute and chronic seizure disorders.
Design   Cross-sectional survey.
Method   A questionnaire was distributed to veterinarians to determine how many dogs and cats they were actively treating for seizures, their anticonvulsant drug (ACD) preferences for treating acute and chronic seizure disorders and whether serum anticonvulsant concentrations and/or biochemical analytes were routinely measured. Additional questions involved the respondent's year and place of graduation and identified concerns they faced when managing patients with seizure disorders.
Results   Phenobarbitone was the most commonly used ACD for managing chronic seizure disorders in both dogs and cats, with 82% of respondents using a combination of phenobarbitone and potassium bromide to manage refractory seizure disorders in dogs. Most respondents (96%) felt comfortable managing seizures in dogs, but only 63% were comfortable managing affected cats. Routine monitoring of serum ACD concentrations and of liver biochemical analytes was performed routinely by 71% and 45% of respondents, respectively. Of the respondents, 86% graduated from Australian universities and of these 53% had graduated after 1985.
Conclusion   Veterinarians identified when to commence medication, whether regular monitoring of serum ACD concentrations and liver enzyme activity was necessary, and if the cost was justified. Veterinarians also identified the need to balance dose rates and side-effects by using combination therapy, and the importance of providing accurate information to clients about what to expect in terms of seizure control for their pet.  相似文献

AIMS: To survey current anaesthesia practices for dogs and cats in small and mixed animal practices in New Zealand in order to improve anaesthesia education.

METHODS: A questionnaire was sent to 440 small and mixed animal practices, including questions regarding the type of practice, preanaesthetic examination, anaesthetic drugs and management, anaesthetic machines, monitoring and topics of interest for continuing professional development.

RESULTS: Responses were obtained from 113/440 (26%) practices, with 78 (69%) respondents from small and 35 (31%) from mixed animal practices. A preanaesthetic physical examination was carried out by >95% of respondents and premedication was usually given to dogs (112/113; 99%) and cats (95/113; 85%). Acepromazine was the preferred sedative for dogs and cats, with morphine or buprenorphine. Propofol and alfaxalone were the preferred induction agents, and isoflurane was preferred for maintenance in both dogs and cats. A venous catheter was usually placed for anaesthesia in dogs (59/113; 52%), but less so in cats (39/113; 35%). Perioperative fluid was administered at 10?mL/kg/hour by 62/110 (56%) respondents. Intubation was usually used for anaesthesia in dogs (111/112; 99%), and cats (87/112; 78%). Almost 40% of respondents usually administered supplementary oxygen if patients were not intubated. Local analgesia was used by 69/111 (88%) respondents sometimes or always if applicable. Morphine or buprenorphine, and meloxicam were common choices for post-operative analgesia after neuter surgery in dogs and cats. A semiclosed (non-rebreathing) system was used in animals weighing <10?kg, and a Mapleson E or F non-rebreathing circuit was used by 66/109 (61%) practices. Only 15/111 (14%) practices had a ventilator in their practice. A dedicated anaesthetist was usually used by 104/113 (92%) practices, and apnoea alarms, pulse oximeters, thermometers and oesophageal stethoscopes were the main monitoring devices available in practices. Loco-regional block, pain management, and anaesthetic drugs were the main topics of interest for continuing education.

CONCLUSIONS AND CLINICAL RELEVANCE: Responses by the veterinarians taking part in this survey indicated that they had a reasonably good standard of anaesthetic practice. A physical examination was carried out preanaesthesia, and premedication including analgesia was routinely administered to most patients. A dedicated anaesthetist usually monitored patients and most respondents reported they had access to basic anaesthetic monitoring equipment. Areas where changes could lead to improved anaesthetic practice were increased use of I/V catheterisation, endotracheal intubation, and supplementary oxygen, and reduced I/V fluid rates.  相似文献
