OBJECTIVE: To determine total stiffness and gap stiffness of an external fixation system in a canine mandibular fracture gap model incorporating a full interdental pin as the only point of rostral fixation in a bilateral type-I external fixator. SAMPLE POPULATION: 10 canine mandibles. PROCEDURE: Bilateral mandibular ostectomies were performed between premolars 3 and 4. A type-I external fixator incorporating a full interdental pin was placed to stabilize a 0.5-cm fracture gap. Four pin configurations (intact mandibular bodies with fixator; ostectomized mandibular bodies and complete fixator; ostectomized mandibular bodies with caudal pins of rostral fragment cut; ostectomized mandibular bodies with all pins of rostral fragment cut) were tested in dorsoventral bending 5 times on each mandible. The full interdental pin remained intact in all configurations. Total stiffness and gap stiffness were determined for each configuration on a materials testing machine. RESULTS: Total stiffness of intact mandibles was significantly greater than that of ostectomized mandibles, regardless of external fixator configuration. However, total stiffness and gap stiffness were not significantly different among different external fixator configurations applied to ostectomized mandibles. CONCLUSION AND CLINICAL RELEVANCE: External fixator configurations with only the full interdental pin engaging the rostral fragment were as stiff as configurations that had 2 or 4 additional pins in the rostral fragment for the applied loads. External fixators for rostral mandibular fractures may be rigidly secured with rostral fragment implants applied extracortically, avoiding iatrogenic trauma to teeth and tooth roots.  相似文献

OBJECTIVE: To evaluate the use of polyamide bands, manufactured for securing electrical cables, for repair of oblique femoral fractures in rabbits. STUDY DESIGN: Experimental study. ANIMALS: Twelve nulliparous, 21-25-week-old, California female rabbits. METHODS: Rabbits were divided into 3 groups (n=4) and studied for 14, 28, or 56 days. A Z-shaped mid-diaphyseal femoral osteotomy was repaired with a 2.5 mm intramedullary pin and a polyamide 6.6 cerclage band. Healing was evaluated at intervals by physical examination, limb use, femoral radiographs, and callus histology. RESULTS: Rabbits had early limb use with good wound healing. From the 1st day, movement of the hip and stifle joints was satisfactory. Radiographically, fractures were healed at 28 days. Histologically, there was no foreign body reaction and bone healing was normal. CONCLUSION: Nylon cerclage band application was accomplished easily, maintained reduction, and resulted in good healing and limb use. CLINICAL RELEVANCE: Sterilized nylon bands, manufactured for electrical use, can be used for cerclage in rabbits.  相似文献

Highly comminuted supracondylar humeral fractures were stabilised in six large-breed dogs with a modified type I external fixator using a craniomedial acrylic connecting column and an Intramedullary pin which was incorporated into the connecting frame. This construct provided sufficient stability to allow satisfactory bone healing in five of the six dogs, while premature removal of the intramedullary pin and external fixator resulted in subsequent refracture of the humerus in the remaining dog. Limb function at the time of final assessment was considered excellent in two dogs, good in three dogs and poor in one dog. The craniomedial acrylic connecting column simplified application of this modified type 1(a) configuration to the humerus by reducing the number of clamps required; the acrylic column also facilitated contouring of the cranlomedial connecting column to the brachium and was readily extended proxlmally to engage the intramedullary pin. In addition, the acrylic connecting column allowed placement of intramedullary pins of varying diameter.  相似文献

Maxillofacial miniplates and screws were used for skeletal fixation in 15 dogs and 3 cats that sustained a variety of mandibular and maxillary fractures. These implants were used as neutralization or buttress fixation in 11 caudal (junction of the ramus with the mandibular body) and 2 rostral mandibular fractures, 4 maxillary fractures, and 2 zygomatic arch fractures. All but one of the fractures healed with appropriate occlusion and excellent function. In one case of a rostral mandibular fracture, soft tissue dehiscence occurred accompanied by a loss of the fixation and subsequent distraction of the bone fragments; reasonable function was obtained by performing a rostral mandibulectomy. Plate contouring and application of the miniplates along the appropriate biomechanical lines of stress was easily performed and permitted the biomechanical principles of tension band fixation to be applied in most cases. Miniplate fixation, either used alone or in combination with other fracture fixation techniques, achieved sufficiently rigid skeletal fixation to provide uncomplicated healing and good to excellent functional and cosmetic results in 14 dogs and 3 cats.  相似文献

This study evaluated the effects of normograde and retrograde intramedullary pinning of mid-diaphyseal fractures of the feline tibia on the anatomical structures of the stifle joint. Using the paired pelvic limbs from five mature feline cadavers, a transverse, mid-diaphyseal osteotomy was performed, and each tibia was pinned in a normograde or retrograde fashion. The stifle joints were examined to determine the pin exit site and measure the distance from the exit site to pertinent anatomical structures of the stifle joint. Neither normograde nor retrograde intramedullary pinning resulted in damage to the cruciate ligaments, menisci, intermeniscal ligament, femoral condyles, or joint capsule. The patellar tendon was penetrated in all five tibias during retrograde pin insertion.  相似文献

OBJECTIVE: To develop a free vascularized tibial bone graft based on the periosteal saphenous blood supply. STUDY DESIGN: Preliminary anatomic study of medial tibial blood supply. In vivo comparison of a vascularized and avascular tibial bone graft. ANIMALS: Nine canine cadavers; 14 healthy adult dogs that weighed 25 to 32 kg. METHODS: An anatomic study of the vascular supply of the medial aspect of the tibia was performed using the Spalteholz technique. A bone graft consisting of the medial aspect of the tibia was transferred to a mandibular defect as a vascularized graft in 7 dogs and as an avascular graft in 7 dogs. Bone scans were performed to evaluate graft perfusion. Radiographic evaluation of the mandibles and tibias was performed. The dogs were killed after 60 days, five mandibles from each group were examined histologically, and two from each group were evaluated using the Spalteholz technique. RESULTS: The saphenous vascular pedicle provides vascular perfusion to the medial tibial cortex. Bone scans and radiographic evaluations were consistent with viable bone in the vascularized grafts, and nonviable bone in the avascular grafts. Histological examination revealed live, healing bone in vascular grafts and necrotic bone in avascular grafts. Spalteholz evaluation revealed many small arborizing vessels in the vascular grafts and no organized vasculature in the avascular grafts. CONCLUSIONS: The vascularized medial tibial cortical bone graft survived and proceeded to bony union in the mandibular body defect more readily than the avascular graft in this experimental model. CLINICAL RELEVANCE: A vascularized medial tibial bone graft is a suitable free graft for use in reconstructing bone defects in dogs.  相似文献

The Kirschner-Ehmer splint can be used to treat open, infected or highly comminuted fractures, gunshot fractures, nonunions, delayed unions, mandibular fractures and angular deformities in association with osteotomy, as well as to immobilize joints and as an adjunct to other fixation devices. After the fracture is reduced, 2-4 percutaneous pins are inserted with a Jacobs hand chuck through one or both cortices at 45-60 degrees to the longitudinal axis of the bone and attached to a connecting bar with clamps. Complications are minimized by not inserting the pin through large muscle masses, the fracture hematoma, large blood vessels or the incision line, avoiding encroachment of soft tissue with the clamps, and restricting the animal's activity during healing.  相似文献

OBJECTIVE: To investigate the effectiveness of intramedullary xenograft cortical bone pins compared with stainless steel Kirschner wire for the repair of a standardized avian humeral fracture. STUDY DESIGN: Prospective randomized study. SAMPLE POPULATION: Thirty mature pigeons (Columba livia). METHODS: Birds were randomly assigned to 3 groups. Transverse mid-diaphyseal humeral fractures were created in 1 humerus in each bird. Fractures were stabilized with intramedullary ostrich or canine xenograft cortical bone pins or Kirschner wire. Radiographic, histological, and biomechanical assessments were used to compare fracture healing 6 weeks after fracture stabilization. The contralateral humerus of each bird was used as a control. RESULTS: All fractures healed regardless of intramedullary pin type. There were no statistically significant biomechanical differences among groups or within groups. Xenograft cortical bone pins induced a mononuclear inflammatory reaction that did not impair bone healing. Bones stabilized with intramedullary cortical bone pins had more periosteal callus and inflammation at the fracture site than bones stabilized with stainless steel Kirschner wires. CONCLUSIONS: Intramedullary xenograft cortical bone pins, derived from mammalian or avian sources, appear to represent an alternative for the repair of avian humeral fractures. CLINICAL RELEVANCE: Intramedullary xenograft cortical bone pins are biodegradable and may reduce the need for additional surgery to remove implants after fracture healing.  相似文献

The normal blood supply to the canine mandible and mandibular teeth was determined by microangiography and correlated histology. Branches of the inferior alveolar artery supplied the cortical bone of the mandibular body. Vessels from the periosteal and endosteal surfaces supplied symphyseal cortical bone. Direct vascular anastomoses were not found to cross the fibrous mandibular symphysis. Blood supply to the mandibular teeth was via dental arteries derived from the inferior alveolar artery, with interdental and interradicular arteries supplying the alveolar bone and periodontal ligament.  相似文献

Interlocking nails have been shown to be an effective means to stabilise long bone fractures. When used in non-load sharing fracture repairs with minimal medullary canal filling, the authors noted instability of the main fracture fragments. The addition of multiple intramedullary pins in stack pin fashion eliminated the instability. ILN and ILN/SP constructs were loaded in four-point bending using an unstable osteotomy model. The gap between the ILN and ILN/SP groups was compared. Significant changes were seen with the ILN/SP constructs in the cranial to caudal plane.  相似文献

Transverse midshaft fractures of femurs from freshly euthanatized dogs were stabilized by means of 6 methods: (1) 3.5-mm bone plate and screws, (2) single intramedullary pin, (3) double intramedullary pins retrograded proximally and driven distally to the level of the femoral trochlea, (4) double intramedullary pins retrograded distally and driven proximally into the trochanteric region, (5) double intramedullary pinning in Rush pin fashion, and (6) multiple intramedullary pinning that filled the medullary cavity at the fracture site. All bones were subjected to torsional stress. The measured strain was converted to forces of torque and correlated with bone diameter to normalize the data. The forces of torque from each fixation technique were compared with each other and with the mean torque force necessary to fracture intact femurs. Torsional shear applied to plated femurs resulted in failure at a mean level of 33.8% of the calculated theoretic moment. Torsional forces were concentrated at one end of the plate and catastrophically failed at that point, whereas the fracture site remained rigidly fixed. There was no significant difference in the initial moment of torsional failure between the single intramedullary pin technique (0.05 Nm) and the double-pinning techniques (0.03 to 0.04 Nm). The multiple-pinning technique was 1.8 to 3 times as effective in resisting rotational forces, compared with the other pinning techniques, but not significantly so.  相似文献

Objectives— To compare bone healing of tibial osteotomy repaired with Nitinol wire braid and hardened steel rods (Braid system) and polymethylmethacrylate bone cement with an interlocking intramedullary (IM) nail fixation in an ovine model.
Study Design— In vitro and in vivo experimental study.
Animals— Adult female sheep (n=22).
Methods— Using sheep tibia, a middiaphyseal transverse osteotomy was performed in the right tibia, which were then randomly assigned to the Braid system group or IM nail group (n=5). The left tibia were used as controls. The torsional properties of tibial constructs were compared. The study was repeated in vivo in 12 sheep and mechanical properties and bone healing were evaluated at 12 weeks.
Results— In vitro, there was no significant difference in torsional stiffness between the groups. In vivo, operative time for the Braid system group was significantly shorter than the IM nail group. At 12 weeks, there were no significant differences in maximum torque and torsional stiffness between IM nail and Braid system groups nor were there significant radiographic or histologic differences between the groups.
Conclusions— The Braid system might decrease operative time for repair of transverse middiaphyseal tibial fractures and result in similar bone healing at 12 weeks after surgery compared with an interlocking IM nail repair.
Clinical Relevance— A Nitinol Braid system may be a treatment option for transverse midshaft tibial fractures.  相似文献

Radiologic and gross anatomic evaluation of bone healing in the dog.   总被引:1,自引:0,他引:1  
Bone healing associated with 3 techniques of midshaft femoral fracture fixation in 36 young adult Beagle-type dogs was evaluated in radiographic and gross pathoanatomic studies. A serrated transverse fracture was surgically and aseptically created on the midshaft of either the left or the right femur of each dog. The fixation devices used were intramedullary (IM) pin, IM pin and 1/2 Kirschner device, and tension bone plates. The radiographic evaluation was done in series. The first radiographs were taken during surgery. Radiographs were then taken immediately after surgery, to record the status of reduction, alignment, and fixation. Radiographs were taken at the 4th and 10th postoperative weeks, to monitor healing. All dogs were euthanatized at the 10th week. Both the normal femur and the healing femur were removed from all dogs, all soft tissue was removed from the bone, and each femur was examined grossly. Each type of fixation was associated with a distinct mode of osteogenesis. Bony union and clinical union (that stage in the healing process when fixation can be removed) were defined as a successful conclusion to each case. Malunion and nonunion were defined as unsuccessful conclusions. Delayed union was defined as neither successful nor unsuccessful. The IM pin cases had a 64.2% success rate and a 14.2% failure rate. The IM pin and 1/2 Kirschner cases had a 100% success rate. The bone plate cases were 91% successful, with 0% failure. A problem identified with IM pinning was axial rotation (6 of 14 or 42% of the cases). It was concluded that IM pins should not be used alone for the fixation of femoral fractures in adult dogs.  相似文献

A radiographic study of healing patterns of nongrafted and grafted basilar osteotomies of proximal sesamoid bones in 14 horses was performed. Osteotomies were created in one proximal sesamoid bone of each fromt leg. One was treated by an autogenous rib graft, and the other was left nongrafted. Wedge-shaped cortical bone, corticocancellous bone with multiple drill holes, and chips of cancellous hbone were used as autogenous grafts. Presurgical, surgical, and postsurgical radiographic examinations were performed. The longest follow-up period was 40 weeks. High detail radiography of 3-mm bone sections and microangiography were also perofromed. Radiographic interpretation of lack of bone healing was erroneous in approximately one-half of the cases. Lack of an external bridging callus was incorrectly interpreted as lack of bony union. If noted, periosteal new bone formation failed to develop into a pattern of bridging callus. Radiographs did not permiit detection of the osteotomy line entering the articular surface or displacement of distal fragments. From angiography obtained ten weeks after surgery, the pattern of blood supply was similar in nontreated and grafted sesamoid bones. Microangiography showed rich vascularization of the cancellous graft and callus, reflecting good healing activity, whitle vascularization of the osteotomy site was absent in the nontreated osteotomies.  相似文献

Five cats with femoral fractures caused by pneumatic gun shot wounds were treated using intramedullary pin/external skeletal fixation "tie-in" (IM/ESF tie-in) technique between 1997 and 2005. Clinical and radiographical examinations were used to assess bone healing, limb function recovery and complications. Clinical results as well as fracture healing were excellent in all five cats. The study showed that the IM/ESF tie-in technique can be used in cats for the treatment of comminuted femoral fractures caused by pneumatic bullets.  相似文献

Objectives— To describe the clinical outcome of a 4 pin lumbosacral fixation technique for lumbosacral fracture–luxations, and to refine placement technique for iliac pins based on canine cadaver studies.
Study Design— Retrospective and anatomic study.
Sample Population— Dogs (n=5) with lumbosacral fracture-luxations and 8 cadaveric canine pelvi.
Methods— Lumbosacral fracture–luxations were stabilized with a 4 pin (positive-profile threaded) and bone cement fixation. Caudal pins were inserted in the iliac body and cranial pins were inserted into the L7 or L6 pedicle and body. Follow-up examinations and radiographs were performed to assess patient outcome. Intramedullary pins were inserted into the iliac bodies of 8 cadaver pelvi. Radiographs were taken to measure pin insertion angles and define ideal insertion angles that would maximize pin purchase in the ilium.
Results— Follow-up neurologic examination was normal in 4 dogs. Radiographic healing of the fracture was evident in 5 dogs. One implant failure occurred but did not require re-operation. For cadaver iliac pins, mean craniocaudal insertion angle was 29° and mean lateromedial insertion angle was 20°.
Conclusions— Four pin and bone cement fixation effectively stabilizes lumbosacral fracture luxations. The iliac body provides ample bone stock, which can be maximized using an average craniocaudal pin trajectory of 29° and an average lateromedial pin trajectory of 20°.
Clinical Relevance— Lumbosacral fracture–luxations can be stabilized with 4 pin and bone cement fixation in the lumbar vertebrae and iliac body, using 29 and 20° as guidelines for the craniocaudal and lateromedial pin insertion angles in the ilium.  相似文献

An 8-year-old Siberian husky dog was presented for a mass involving the rostral mandible. Intraoral radiographs demonstrated diffusely irregular bone and displacement of all mandibular incisor teeth. The mass was diagnosed as a grade I multilobular tumor of bone based on incisional biopsy. A bilateral rostral mandibulectomy was performed with tumor negative margins. Oral examination at 14-months following surgery indicated normal healing with minimal side effects and no evidence of tumor recurrence.  相似文献

An 11-year-old Campolina horse was referred to the Veterinary Hospital due to the presence of an exophytic mass located in the rostral region of the mandible. Radiographic evaluation showed no evidence of tumour infiltration in the underlying mandibular bone, and the histopathology and immunohistochemical evaluations confirmed the diagnosis of fibromatous epulis. The treatment included surgical excision of the mass, followed by extensive curettage of the underlying mandibular bone. Complete healing of the wound was evidenced 60 days after the procedure; however, relapse was reported by the owner between 1 and 3 years after surgery.  相似文献

A 13-month-old female Andalusian was evaluated because of a congenital left deviation of the maxilla and nasal septum, which had resulted in a gross malocclusion of the maxillary incisor teeth. Surgical correction of a deviated nasal septum and premaxilla in a horse was first reported in 1978 and involved a pre-maxilla osteotomy and use of an autogenous rib graft, with a nasal septum osteotomy performed in a second surgery; to the authors' knowledge, no alternative surgical treatments have since been described. In this horse, a new surgical treatment by distraction osteogenesis without nasal septum osteotomy was attempted. After the procedure, the premaxilla and nasal septum deviations were corrected; however, an overjet lesion (rostral projection of maxillary arcade over the mandibular arcade) was evident, for which the owners declined treatment. Compared with previously recommended procedures, distraction osteogenesis appears to be a less invasive treatment for congenital facial deformities and may be considered an option in the treatment of congenital deviation of the nasal septum and premaxilla (wry nose), head scoliosis, brachygnathism, and prognathism in horses.  相似文献

OBJECTIVE: To compare the biomechanical properties of pantarsal arthrodesis achieved with a dorsal bone plate-alone (BPA) or pin-plate combination (PPC). SAMPLE POPULATION: 8 pairs of cadaver canine tarsi. PROCEDURE: Within a pair, 1 tarsus was arthrodesed by use of a 13-hole 3.5-mm broad dynamic compression bone plate applied to the dorsal aspect of the tarsus; the paired tarsus received an identical plate similarly applied, with the addition of an intramedullary pin filling approximately 40% of the tibial medullary canal, spanning the tibiotarsal joint. Plates were instrumented with strain gauges proximal and distal to the solid portion of the plate. Specimens were mounted on a servo-hydraulic testing machine and loaded at 20%, 40%, and 80% of body weight for 10 cycles at 1 Hz. Construct compliance, angular deformation, and plate strain were determined during the 10th cycle. RESULTS: PPC specimens were less compliant than BPA specimens at all loads and had significantly less angular deformation than BPA specimens at loads of 40% and 80% of body weight. Tibiotarsal gauge microstrain was significantly less in PPC specimens, compared with BPA specimens, regardless of loads. Maximal strains were 33.5% to 40.5% less in PPC than BPA specimens. CONCLUSIONS AND CLINICAL RELEVANCE: For pantarsal arthrodesis in dogs, our results indicate that the PPC construct is biomechanically superior to the BPA construct. By improving construct stability, addition of an intramedullary pin to the traditional BPA technique may lessen implant-related complications and improve plate fatigue life. A subsequent decrease in postoperative morbidity may occur with little addition of time or complexity to the surgical procedure.  相似文献
