OBJECTIVE: To describe cancellous architecture of the proximal portion of the femur in dogs with osteoarthritis. ANIMALS: 30 dogs with coxofemoral osteoarthritis. PROCEDURE: All dogs had femoral head and neck excision or total hip arthroplasty. Histomorphometry software was used to analyze computer images of 100-microm-thick coronal and transverse plane sections of the head and neck of the femur. Histologic preparations of coronal and transverse sections of articular cartilage were graded. RESULTS: Bone volume/total volume, trabecular thickness, trabecular number, and bone surface/total volume were significantly higher in the femoral head than femoral neck. Trabecular alignment (anisotropy) and separation were significantly higher in the femoral neck than femoral head. Anisotropy was significantly increased in the medial portion of the femoral head in the coronal plane and in the cranial portion of the femoral neck in the transverse plane, compared with healthy dogs. The medial half of femoral head cartilage that overlies the proximomedial cancellous bone region had significantly more degraded cartilage than the lateral half. Histologic grades for cranial and caudal halves of femoral head articular cartilage were similar. CONCLUSIONS AND CLINICAL RELEVANCE: Most findings were similar to those in healthy dogs. Greater trabecular alignment in the proximomedial region of the femoral head and craniolateral region of the femoral neck in dogs with osteoarthritis suggests an altered transfer of load through the coxofemoral joint. Greater cartilage degradation on the medial half of the femoral head supports an association between increased trabecular alignment and cartilage degradation.  相似文献

OBJECTIVE: To describe the cancellous bone architecture of the head and neck of the femur in healthy dogs by use of automated histomorphometry techniques in conjunction with histologic grading of articular cartilage. ANIMALS: 30 mature male dogs with healthy coxo-femoral joints PROCEDURE: Dogs were 1.5 to 4 years old and weighed 27 to 37 kg. Computer images of fine-detail radiographs of 100-microm-thick coronal and transverse plane sections of the head and neck of the femur (14 dogs) were analyzed by use of histomorphometry software. Statistical comparisons among histomorphometric indices of 4 regions were performed. Histologic preparations of coronal and transverse plane sections of femoral head articular cartilage (16 dogs) were graded. Median grades for lateral, medial, cranial, and caudal halves of the femoral head articular cartilage were determined. RESULTS: Bone volume/total volume, trabecular thickness and number, and bone surface/total volume were significantly higher in the femoral head than in the femoral neck. Anisotropy (trabecular alignment) and trabecular separation were significantly higher in the femoral neck than in the femoral head. Anisotropy was significantly higher in the caudal half of the femoral neck than in the cranial half. Cartilage had histologic grades indicating health without significant differences among lateral, medial, cranial, and caudal halves of femoral head cartilage. CONCLUSIONS AND CLINICAL RELEVANCE: A predictable cancellous architecture in the head and neck of the femur is associated with healthy cartilage.  相似文献

The proximal portion of the femur was evaluated as a source of autogenous cancellous bone in dogs. Bilateral oval cortical defects were created in the lateral subtrochanteric area of the femur in 16 dogs. Cancellous bone was removed and the weight recorded. Cancellous bone was similarly harvested from the proximal portion of the humerus in 7 of these dogs. Subtrochanteric femoral defects in 11 dogs were randomly assigned to receive cancellous bone graft obtained from the femur (n = 4) or the humerus (n = 7). Subtrochanteric defects in 5 dogs were not grafted. Radiographic assessment of subtrochanteric defects was performed at 4-week intervals, and histologic assessment at 4, 8, 16, and 24 weeks after surgery. Nongrafted donor sites healed by ingrowth of trabecular bone during the first 12 weeks after surgery. By week 24, the lateral cortical wall had reformed, but remodeling was incomplete. Donor sites grafted with cancellous bone healed similarly, but with more rapid healing and more complete remodeling evident by week 24. Although the mean weight of cancellous bone harvested from the proximal portion of the femur (0.82 +/- 0.22 g) was significantly (P less than 0.05) less than that harvested from the proximal portion of the humerus (1.38 +/- 0.29 g), there was no qualitative histologic or radiographic difference in bony healing of grafted defects. We determined that the proximal portion of the femur can be safely used to provide moderate amounts of cancellous bone, and that a second bone graft can be collected from the same subtrochanteric donor site after 12 weeks.  相似文献

Objective— To describe indirect decompression by means of cervical spine locking plate (CSLP) fixation with vertebral distraction, discectomy, and cancellous block bone grafting in large breed dogs with single caudal cervical dynamic spondylotic lesions diagnosed by myelography with linear traction to the cervical spine, and contrast-enhanced computed tomography.
Study Design— Prospective clinical study.
Animals— Dogs (n=12) with caudal cervical spondylotic myelopathy because of a single dynamic, traction-responsive lesion.
Methods— Single, traction-responsive, caudal cervical spondylotic lesions were treated by vertebral distraction, discectomy, cancellous block bone grafting, and CSLP fixation. Follow-up was obtained by sequential recheck examination by the author or referring veterinarian or by telephone inquiries.
Results— Ten dogs had neurologic improvement after surgery. Indirect decompression by maintained distraction with cancellous block grafting and CSLP fixation was readily accomplished with less risk of blood loss or iatrogenic spinal cord injury than that associated with direct (ventral) decompression. There were no complications of graft intrusion, extrusion or subsidence, implant loosening, foraminal impingement, or end-plate failure. Two dogs that had satisfactory short-term recoveries developed clinical signs associated with adjacent segment disease and were euthanatized. At long-term follow-up, 8 dogs had satisfactory function, either a normal gait or one with slight to moderate proprioceptive deficits.
Conclusions— CSLP fixation with cancellous block interbody grafting is an effective and perhaps safer method of treating single-level, traction-responsive cervical spondylosis in large breed dogs.
Clinical Relevance— CSLP fixation with interbody bone grafting is a viable alternative to other techniques for treatment of single-level, traction-responsive cervical spondylosis.  相似文献

Four one quarter inch evenly spaced circular defects were created bilaterally in the lateral femoral diaphysis of 12 clinically normal adult dogs. The defects were left unfilled (control), or were filled with one of the following: (1) plaster of Paris, (2) an equal-volume mixture of plaster of Paris and autogenous cancellous bone, and (3) autogenous cancellous bone. The degree of bone healing was evaluated radiographically and histologically at 2, 4, 6, 8, 10, and 12 weeks. Radiographically, no objective conclusions could be drawn due to the small size of the defects and limited amount of plaster of Paris implanted. Histologically, there was no inflammatory reaction to the plaster of Paris. No differences were determined in the degree of bone healing between autogenous cancellous bone, plaster of Paris, and a mixture of plaster of Paris and autogenous cancellous bone. All implants were superior to the control defect in degree of bone healing.  相似文献

Objective: To evaluate a distractable titanium cage for the treatment of disk‐associated wobbler syndrome (DAWS). Study Design: Prospective study. Animals: Dogs (n=7) with DAWS. Methods: After total discectomy of C5–C6 and C6–C7, the median part of the vertebral body of C6 was removed with preservation of the lateral walls and dorsal cortex. The removed cancellous bone was collected. The implant was placed in the bony defect of C6. After placement, the titanium cage was distracted and affixed by 4 screws. Finally, the implant was filled and covered with cancellous bone. Dogs had follow‐up examinations at 1, 3, 6, 12, and 24 months. Six months after surgery, cervical radiographs and computed tomography (CT) were performed. Results: Although no intraoperative complications occurred, correct placement of the cage was technically challenging. Revision surgery was necessary in 2 dogs because of implant loosening and aggravation of vertebral tilting. All dogs improved after discharge from the hospital. In 1 dog, recurrence of clinical signs caused by articular facet proliferation at an adjacent intervertebral disk space occurred. Radiographs at 6 months demonstrated cage subsidence in 4 dogs. In all dogs, CT was suggestive for fusion of the bone graft with the vertebral body. Conclusions: Although results are promising, technical adaptations will be necessary to make this specific surgical technique, designed for humans, suitable for routine use in dogs.  相似文献

OBJECTIVE: To evaluate femoral adaptation after unstable long-term cemented total hip arthroplasty (cTHA) in dogs. STUDY DESIGN: Clinical study. ANIMALS: Four dogs. METHODS: Paired femurs were examined from client-owned dogs that were donated to a retrieval program after death from causes unrelated to their cTHA. Mean (+/-SEM) dog age was 10.0+/-1.5 years and implant duration was 6.0+/-1.4 years. Implanted femurs had radiographic changes associated with implant loosening and gross mechanical instability at femur retrieval. Femurs were evaluated at 3 levels relative to implant length. Cortical area and medullary area were measured at each level, and cortical porosity was assessed at each level in 4 quadrants and in 3 regions. Implanted femurs were compared with the contralateral non-implanted femurs. RESULTS: Cortical area and cortical porosity were increased at all levels in femurs with unstable implants. Implanted femurs had increased porosity in all quadrants and regions at the proximal 2 levels, and increased porosity in only the cranial quadrant and mid-cortical area at the distal level, when compared with contralateral non-implanted femurs. Corresponding medullary areas were not different. CONCLUSIONS: Significant histomorphometric changes occur in femurs after unstable cTHA. The patterns of periosteal bone formation and endosteal bone resorption support mechanisms of stress shielding and wear debris-mediated osteolysis as factors that may contribute to femoral adaptation and implant loosening. CLINICAL RELEVANCE: Despite popularity and excellent return to function with cTHA, aseptic loosening remains a serious long-term complication. Substantial net bone loss and unfavorable environment with unstable cTHA may make revision surgery less successful.  相似文献

Thirty uncemented porous-coated anatomic total hip prostheses in 24 dogs were evaluated radiographically at 6 months, 12 months, or 18 months after arthroplasty. All dogs were fully weight bearing and free of lameness at the time of follow-up. Radiographs were assessed for prosthetic component alignment, percentage of femoral canal fill, resorptive and formative bony changes, and changes at the implant bone interface. No evidence of infection, loosening, or failure of component fixation was seen. Settling of the acetabular component and subsidence of the femoral stem was seen early after surgery. Cortical atrophy was seen in all femurs and was the most significant bony remodeling change. Early results of clinical and radiographic evaluation of uncemented total hip arthroplasty in dogs have been encouraging.  相似文献

OBJECTIVE: To investigate factors influencing screw loosening after triple pelvic osteotomy (TPO) and ischial wire stabilization of the acetabular segment. DESIGN: Retrospective case series. Animals-227 dogs with congenital hip dysplasia or subluxated hip joints. PROCEDURES: Medical records and radiographs of 227 dogs that underwent 332 TPO procedures were evaluated, and data pertaining to screw type, plate position, sacral screw engagement, use of ischial interfragmentary wires, and pelvic alignment were assessed for associations with screw loosening. RESULTS: Complications developed in 96 of the 332 (29%) procedures. Cancellous screws without sacral engagement were associated with the lowest frequency (6%) of loosening, compared with cancellous and cortical screws engaging the sacrum and cortical screws that did not engage the sacrum. Frequency of screw loosening increased when cortical or cancellous screws engaged the sacrum and when cortical screws were used. In dogs that had surgery bilaterally, the first limb on which TPO was performed had a higher frequency of screw loosening than the second limb. Pelvic alignment loss was greatest (5.4 degrees ) when the 3 most cranial screws were loosened. Loss of pelvic alignment was significantly different between dogs that underwent surgery and had complications and those that underwent surgery and did not have complications in association with loosening of 1, 2, and 3 screws. CONCLUSIONS AND CLINICAL RELEVANCE: TPO screw loosening was multifactorial and related to stability of the affected ilium, screw type, and screw position. Placing cancellous screws that do not engage the sacrum in pelvic osteotomy plate positions 1 through 3 may decrease the number of screws that loosen.  相似文献

Osteosarcoma involving the distal right femur was diagnosed in a nine-year-old female neutered Rottweiler seven years after total hip arthroplasty had been performed on that limb. The findings were consistent with a primary bone tumour and pathological fracture of the right femoral condyle with loosening of the orthopaedic implant and fracture of the polymethylmethacrylate at the distal aspect of the femoral component. Possible hypotheses to explain the association of osteosarcoma with total hip arthroplasty suggest that the neoplastic process was the result of some derangement of host tissue and the healing process or that the implants or their by-products were carcinogenic. Given the large number of total hip arthroplasties that are routinely performed in dogs, the development of a malignant lesion appears to be an extraordinary complication and may be completely coincidental.  相似文献

OBJECTIVE: To report the clinical, radiographic, and pathologic features of extraosseous cement granuloma (ECG), a low occurrence, long-term complication of total hip replacement (THR). STUDY DESIGN: Retrospective clinical study. Sample Population-Six client-owned dogs. METHODS: Medical records for 6 dogs that had a cemented modular THR and developed ECG were reviewed for the clinical, radiologic, and histopathologic findings of ECG. Morphologic and elemental analyses of retrieved particles were conducted in 2 dogs using scanning electron microscopy (SEM) and energy-dispersive x-ray spectroscopy (EDS). RESULTS: Three golden retrievers and 3 Labrador retrievers had progressive lameness in the operated limb 4 to 7 years after THR and developed ECG. Other clinical features included an acute deterioration to non-weight bearing associated with pathologic fracture of the proximal femur in 2 dogs, and obstipation in 1 dog. In 5 dogs, there was a large caseous mass surrounding the proximal femur. Obstipation in 1 dog was caused by a similar mass adjacent to bone cement used for fixation of the acetabular cup. Radiographic changes included a multilobular soft tissue mass, containing irregular mineral densities, in the gluteal mass surrounding the proximal femur, THR interface deterioration, osteolysis, new bone formation, and implant subsidence were consistent with chronic aseptic loosening. The histopathologic diagnosis was sterile granuloma associated with a massive heterogeneous particulate burden, which was assumed to have resulted from severe wear of all prosthesis components. CONCLUSIONS: ECG is a manifestation of severe aseptic loosening of cemented total hip replacement, with the generation of wear debris probably responsible for granuloma formation. CLINICAL RELEVANCE: Because revision can offer a good prognosis in early aseptic femoral loosening, yearly radiographic monitoring of the recipients is recommended.  相似文献

Objective— To document the prevalence of femoral medullary infarction associated with the Zurich Cementless Total Hip Replacement (ZCTHR) system in dogs.
Study Design— Case series.
Animals— Dogs (n=35) with 41 ZCTHR implants.
Methods— Medical records (February 1999–December 2002) were reviewed for dogs that had ZCTHR and at least 1 year follow-up with radiographic evaluation. Thirty-five dogs (41 ZCTHR) met the inclusion criteria. Femoral morphologic data, implant to bone relationships, and medullary infarcts were recorded. Data were analyzed for associations between infarct occurrence and morphologic details and dog characteristics.
Results— Eight of 41 femurs had radiographic evidence of infarcts (19.5%). Dogs with infarcts were significantly younger (mean [±SD] age, 18.5±5.2 months) compared with those without infarction (mean age, 44.4±5.6months; P =.027). None of the other variables were significantly different between dogs with and without infarcts. Three femurs with infarcts ultimately developed stem loosening.
Conclusion— A higher prevalence of femoral medullary infarcts was identified with ZCTHR compared with incidence reported for other total hip systems. Younger dogs were more likely to develop infarction.
Clinical Relevance— Femoral infarction appears to be associated with stem loosening. Adjustments in surgical technique or delaying surgery beyond 18 months of age may reduce incidence of infarction but needs further evaluation.  相似文献

Total hip endoprotheses are a good possibility for treatment degenerative wear and pathologic damage of the hip joint in man as well as in dogs. However, aseptic loosening of the protheses, especially in the area of the shaft, is still a problem in conventional total hip endoprotheses. The purpose of the present study was to use the finite-element-analysis (FEA), to enhance endoprothetic design and to prevent loosening of protheses. In order to simulate the femur of the dog for numerical analysis, a material law for the compacta in the femur of the dog was developed. The elastic properties of the compacta were experimentally determined by using compression tests of bone samples of euthanised dogs. The results show constant denseness and a constant axial elastic modul in the compacta.  相似文献

OBJECTIVE: To describe the histomorphometric properties of epiphyseal and metaphyseal trabecular bone of the proximal portion of the femur of dogs with moderate osteoarthritis. SAMPLE POPULATION: Proximal portions of a femour from 24 dogs. PROCEDURE: The proximal portion of a femur was obtained from each dog. Eleven and thirteen specimens were sectioned in the transverse and coronal planes, respectively. Three evenly spaced sections from each specimen were chosen, surface stained, and digitized, and the stained areas were preferentially selected. Custom software was used for histomorphometric analysis of each section. A mixed-model analysis was used to evaluate the effect of slice location and region on 6 parameters, and a Fisher protected t test was used when differences were detected. RESULTS: There was a significant difference between the femoral head and femoral neck for all parameters tested. In coronal sections, the femoral neck was significantly more anisotropic than the femoral head. In transverse sections, the craniolateral region of the femoral neck was significantly more anisotropic than the caudomedial and craniomedial regions. CONCLUSIONS AND CLINICAL RELEVANCE: There is a predictable cancellous microarchitecture in the proximal portion of femurs from dogs with moderate osteoarthritis. Trabeculae are more numerous, thicker, and closer together but more randomly arranged in the femoral head than in the femoral neck. Dogs with moderate osteoarthritis had an increase in trabecular anisotropy in the craniolateral region of the femoral neck. However, there was no corresponding increase in trabecular alignment of the proximomedial region of the femoral head. Results support an association between trabecular alignment and the progression of osteoarthritis.  相似文献

OBJECTIVES: To evaluate the effects of different cementing techniques on radiographic cement mantle grade and short-term aseptic loosening of the femoral component in canine total hip replacement (THR). STUDY DESIGN: Retrospective study. SAMPLE POPULATION: Radiographs of 284 dogs that had THR. METHODS: Immediate postoperative radiographs of 284 dogs that had cemented THR were reviewed by 4 surgeons and 1 radiologist and assigned a cement mantle grade using a 4-grade classification system. Dog age and weight at surgery, cementing technique (1st, 2nd, 3rd generation), complications (type and timing), and follow-up time were retrieved and analyzed. RESULTS: Mean cement mantle score for 2nd generation technique was significantly higher than 3rd generation technique and both were significantly higher than 1st generation technique (P<.001). Aseptic loosening was the 2nd most common complication observed with an incidence of 2.1%. Mean time from surgery to last follow-up examination was 122 days. No statistically significant difference in incidence of aseptic loosening was identified among different cement mantle grades or cementing techniques. CONCLUSIONS: Advanced cementing techniques resulted in better cement mantles based on grading of immediate postoperative radiographs, however grading did not predict short-term aseptic loosening. Cementing technique seemingly did not affect the incidence of short-term aseptic loosening of the femoral component for dogs in this study. CLINICAL RELEVANCE: Our study suggests that advances in cementing technique may result in improvements in the radiographic grade of cement mantles. With respect to aseptic loosening of the femoral component, our data only suggest that short-term (3 months-3 years) loosening cannot be predicted by immediate postoperative radiographic evaluation of cement mantle.  相似文献

Severely comminuted diaphyseal fractures in 11 dogs were repaired with standard bone plates that spanned a fracture gap filled with autogenous cancellous bone graft. Five dogs had closed injuries, 4 dogs had open fractures, and 2 dogs had infected nonunion fractures for which previous attempts at internal pin fixation had failed. A second autogenous cancellous bone graft was performed in 3 of the dogs during the healing period. The technique was successful in all dogs. The technique was considered a versatile and relatively simple alternative, compared with meticulous small fragment reconstruction and cortical bone allografts.  相似文献

The radiographic signs associated with implanted canine total hip prostheses include: (1) a 1 mm-or-less wide static lucent zone at the acetabular and femoral bone-cement interfaces, and (2) periosteal bone proliferation surrounding the stem of the femoral prosthesis. The radiographic signs associated with total hip replacement complications are dislocation of the femoral component, bone changes suggestive of infection involving the acetabular and femoral components, and loosening of the acetabular component. Arthrography is beneficial in determining the presence of a loose prosthesis with or without infection.  相似文献

Using an autogenous bone graft (obtained from the iliac crest), 4-mm cancellous bone screws, and polymethylmethacrylate, a distracted cervical spinal fusion technique was performed on 10 dogs with myelographic evidence of caudal cervical spondylomyelopathy. All dogs had evidence of dynamic soft tissue spinal cord compression, as indicated by flexion, extension, and traction myelographic views. Of the 10 dogs, 4 previously had undergone surgery by use of ventral slot or cervical disk fenestration techniques, and their neurologic status had deteriorated after the original surgery. Preoperative neurologic status of the 10 dogs included nonambulatory tetraparesis (n = 5), severe ataxia with conscious proprioceptive deficits (n = 2), and mild ambulatory ataxia with conscious proprioceptive deficits (n = 3). Five dogs had signs of various degrees of cervical pain. Clinical improvement was observed in 8 of 10 dogs--either improved neurologic status or elimination of cervical pain. Implant loosening developed in 3 dogs; 2 of them were euthanatized because of lack of neurologic improvement. Radiographic evidence of bony cervical fusion was observed during a 9- to 24-week period in 6 of the 8 surviving dogs. The distracted cervical fusion technique appears to be a valid surgical procedure to manage cervical spondylomyelopathy in those dogs in which the lesions are limited to one cervical intervertebral disk space.  相似文献

Deep-Frozen Allogeneic Cancellous Bone Grafts in 10 Dogs: A Case Series   总被引:1,自引:0,他引:1  
Deep-frozen, aseptically collected and processed allogeneic cancellous bone was implanted in eight dogs during the surgical repair of diaphyseal long bone fractures and in two dogs during arthrodeses. A combined allogeneic and autogeneic cancellous bone graft was used in two fractures with a segmental bone loss of more than 5 cm. Bone union occurred in five fractures and in both arthrodeses. Failure of fixation occurred in two dogs with nonunion fractures and in a third dog with an open, infected fracture. Biopsies from the fracture sites were obtained from these dogs following failure of their fracture fixation. The cancellous bone graft appeared to be in the process of normal incorporation in each case. Failure of fixation was attributed to technical or case management errors or both, in each of the three fractures that failed to achieve bony union. Frozen allogeneic cancellous bone grafts were effectively incorporated when used in the primary repair of fractures and arthrodeses. Combined autogenous and allogeneic cancellous bone grafts may be particularly useful in the repair of fractures with large segmental diaphyseal bone defects. The use of allogeneic cancellous bone grafts in nonunion fractures requires further investigation before it can be recommended.  相似文献
