To study communications and boundaries of the middle carpal and carpometacarpal joints of the horse, 50 forelimbs were obtained from fresh cadaver specimens. Blue latex solution (20 +/- 2.5 ml) was injected into the middle carpal joint, and the specimens were frozen in extension. Frozen specimens were cut into 1-cm sagittal sections from the middle of the radius to the middle of the metacarpus. The communications between the middle carpal and carpometacarpal joints and the presence, length, and position of the distopalmar outpouchings of the carpometacarpal joint were recorded. The middle carpal and carpometacarpal joints always communicated between os carpale III (C3) and os carpale IV (C4). An additional communication between the joints existed in 17 (34%) of the specimens, 10 on the palmar aspect of C4, and 3 on the palmar aspect of os carpale II (C2). When os carpale I (C1) was present (n = 5), communication between C1 and C2 was observed in 4 of the 5 specimens. In all specimens, medial and lateral distopalmar outpouchings of the carpometacarpal joint were observed and were located between the axial surface of os metacarpale II (MC2) and os metacarpale IV (MC4) and the abaxial surface of the suspensory ligament. There was no significant difference between the lengths of the lateral (2.3 +/- 0.54 cm) or medial (2.6 +/- 0.75 cm) distopalmar outpouchings. Small extensions from the distopalmar outpouchings were seen and extended axially into the fibers of the suspensory ligament or between the suspensory ligament and the distal accessory ligament of the deep digital flexor tendon.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

OBJECTIVE : To examine the fracture failure surfaces from Thoroughbred horses that had sustained a catastrophic condylar fracture. SAMPLE POPULATION : Bone specimens from the failure surface were obtained from 12 Thoroughbred racehorses with catastrophic injury and 2 non-racing horses with accidental long bone fracture. METHODS : Bone specimens from the failure surface of each fracture were incubated with gold microspheres to label microcracks before examination at x50 to x60,000 using scanning electron microscopy. Microcracking at the failure surface was assessed using a visual analog scale. RESULTS : Branching arrays or clusters of microcracks were seen over a range of magnifications in adapted subchondral bone in the distal end of the MC3/MT3 bone from racing Thoroughbreds with a catastrophic displaced condylar fracture. In the palmar/plantar region, microcracking was associated with the formation of an array of macroscopic cracks in the condylar groove. A different pattern of microcracking was seen in specimens of bone from distal metaphyseal and diaphyseal MC3/MT3 failure surfaces from Thoroughbred racehorses with catastrophic fracture and non-racing horses with an accidental diaphyseal long bone fracture. Few microcracks were seen and typically did not form branching arrays. CONCLUSION : These data suggest that propagation of condylar fracture in Thoroughbred racehorses is initiated by the formation of nanoscale microcracks in adapted subchondral bone that form during exercise-induced bone adaptation. CLINICAL RELEVANCE : Accumulation and coalescence of branching microcracks into arrays or clusters appears to eventually lead to the development of macroscopic subchondral cracks in the condylar groove and initiation of a condylar fracture.  相似文献

The accessory ligament of the hindlimb suspensory ligament arises from the plantar aspect of the calcaneus and fourth tarsal bones and blends with the suspensory ligament in the proximal aspect of the metatarsus. The accessory ligament of the suspensory ligament of both hindlimbs of 12 mature horses, with no history of hindlimb lameness, was assessed ultrasonographically. The ligament comprised linear parallel echoes which were consistently shorter than those of the lateral digital flexor tendon; the dorsal and plantar borders were parallel. Lameness associated with injury of the accessory ligament of the suspensory ligament was identified in 6 of 8 lame hindlimbs of 5 horses, unilaterally in 4 horses and bilaterally in one horse. In all horses there was concurrent proximal suspensory desmopathy. There was localised oedematous swelling on the distal medial aspect of the chestnut extending distally in 2 horses. Lameness was best identified when the horses were ridden. Perineural analgesia of the deep branch of the lateral plantar nerve abolished lameness in 3 horses but perineural analgesia of the tibial nerve was required to abolish lameness in the hindlimb with injury of the accessory ligament of the suspensory ligament in 2 horses. Injury of the accessory ligament of the suspensory ligament was characterised by marked decrease in echogenicity of the ligament and loss of parallel alignment of the linear echoes, which were shorter than normal in longitudinal images.  相似文献

Developmental morphology of the forelimb distal phalanges and lateral palmar processes of 9 Thoroughbred foals aged 3–32 weeks was assessed using radiography, microradiography and histology. For inclusion in the study, all distal phalanges had no pathologic radiographic abnormalities.
Vascular channels that are characteristically found in the dorsal region of the distal phalanx were not evident radiographically in the palmar process. The proximal and distal angles of the palmar processes were separated by a lucent line continuous with the incisure in foals only through 12 weeks of age. The distal phalanges were triangular-shaped in foals 3–12 weeks of age, and were oval-shaped in older foals.
The palmar aspect of the distal phalanx was the major contributor to growth of the distal phalanx in the sagittal plane, especially between 3 and 12 weeks of age. Growth of the lateral palmar process occurred through the means of endochondral ossification. The body and cortices of the lateral palmar process were composed of coarse cancellous bone. Porosity within the lateral palmar process was greater in regions sampled axial, compared to abaxial, to the parietal sulcus and did not change with age.
A fracture was identified microradiographically and/or histologically in 9 of 18 (50%) and 10 of 17 (59%), respectively, of the lateral palmar processes examined. The fracture line was consistently associated with the parietal sulcus on the dorsal cortical surface and was always immediately abaxial to the deep digital flexor tendon attachment. No age-related morphological changes of the lateral palmar processes were identified with microradiography or histological examinations.  相似文献

Three techniques for inducing analgesia of the proximal metacarpal region were evaluated for the frequency of inadvertent injection into the middle carpal and carpometacarpal joints. Using methylene blue solution as a marker dye and 30 fresh cadaver specimens each, three clinicians performed either 30 infiltrations at the origin of the suspensory ligament (method A), 30 palmar and palmar metacarpal nerve blocks at the proximal end of the metacarpus (method B), or 30 palmar and palmar metacarpal nerve blocks at the distal aspect of the accessory carpal bone (method C). The frequency of inadvertent injection into the distal carpal joints was 37, 17, and 0% for methods A, B, and C, respectively. The association between method and injection into the joints was significant (p less than 0.01). Infiltration of the distal carpal joints occurred with injection distances from the carpometacarpal joint of 1.5 to 4.5 cm. Although there was no joint injection with method C, the carpal synovial sheath was inadvertently infiltrated in 68% of the specimens. Injection into the distal carpal joints can occur when deep injections are made into the proximal palmar aspect of the metacarpus because of the distopalmar outpouchings of the carpometacarpal joint between the axial surfaces of the second and fourth metacarpal bones and the abaxial surface of the suspensory ligament.  相似文献

Between January 1977 and June 1980, 38 Standardbred and 23 Thoroughbred horses had fractured distal splint bones removed surgically. Standardbred horses were treated significantly more frequently compared with the hospital population. The incidence of distal splint bone fractures in the front legs was significantly greater (p 0.05) than in the rear legs in both breeds. Associated suspensory desmitis was present in 81% of Standardbred and 67% of Thoroughbred horses at the time of surgery.
The most common radiographic sign was fracture with associated callus. It was not possible to correlate preoperative radiographic signs with presence of associated suspensory desmitis. One Thoroughbred horse developed a large mass of amorphous bone at the surgical site which required a second surgical procedure.
Follow-up information was available on 21 of 38 Standardbred and 17 of 23 Thoroughbred horses. There was no correlation found between preoperative radiographic signs or the location and number of distal splint bone fractures with postoperative results. Twenty-five percent of Standardbreds that had preoperative suspensory desmitis eventually reached at least their previous level of performance. None of the Thoroughbred racehorses with preoperative suspensory desmitis returned to their previous level of performance. Five of 17 Thoroughbred horses were pleasure horses, and all were reported "sound for intended use" following surgery; however, only four of five were used at their previous level of performance.
The significant feature in the prognosis of distal splint bone fractures in this series was the presence of associated suspensory desmitis. Postsurgical performance was limited by the recurrence of suspensory desmitis.  相似文献

Objective— To describe a lateral approach for screw fixation in lag fashion of simple spiral medial condylar fractures of the third metacarpus/metatarsus (MC3/MT3).
Study Design— Case series.
Animals— Thoroughbred racehorses (n=9).
Methods— Nondisplaced medial MC3/MT3 condylar fractures (3 thoracic, 6 pelvic limbs), with mean length 126 mm (range, 91–151 mm) were repaired by internal fixation, under general anesthesia, using multiple 4.5 mm cortical screws inserted in lag fashion from the lateral aspect of the limb, using radiographic or fluoroscopic guidance. Horses were recovered from anesthesia in half-limb casts; 7 unassisted and 2 using a rope-recovery system. Horses had 2 months box rest, 1 month in-hand walking, and follow-up radiographic examination at 3 months.
Results— Horses recovered uneventfully from anesthesia. Five horses raced; 1 returned to training, was persistently lame, and was retired to stud; 2 were retired directly to stud; and 1 horse was lost to follow-up.
Conclusions— MC3/MT3 medial condylar fractures were successfully repaired by screws inserted n lag fashion form the lateral aspect.
Clinical Relevance— Use of a lateral approach to medial condylar MC3/MT3 fractures allows screw insertion perpendicular to the fracture plane without interference with palmar/plantar soft tissue structures or from the splint bones. Although repair was performed under general anesthesia, the technique should be adaptable to application in standing horses.  相似文献

OBJECTIVE: To measure the effects of transection of the accessory ligament of the superficial digital flexor (SDF) muscle (superior check desmotomy) on flexor tendon and suspensory ligament (SL) strain in vitro. STUDY DESIGN: In vitro experimental biomechanical investigation. ANIMALS USED: Ten equine cadaver forelimbs. METHODS: The effects of superior check desmotomy were determined using equine cadaver forelimbs secured in a servocontrolled hydraulic testing machine. Strain sensors were used to measure strain on the superficial and deep digital flexor tendons and SL, and a goniometer was used to measure joint angles when the limb was loaded at 890 N and 3,115 N before desmotomy, and at 3,115 N after desmotomy. RESULTS: Superior check desmotomy was associated with significantly increased strains on the SDF tendon and SL, and significant alterations in the angles of the metacarpophalangeal and carpal joints. CONCLUSIONS: The superior check ligament has an important role in maintaining joint angles and load distribution in the forelimb. Lengthening of the SDF musculotendinous unit after superior check desmotomy may be associated with increased strain on the SL. CLINICAL RELEVANCE: Transection of the accessory ligament of the SDF muscle may predispose horses to SL desmitis postoperatively.  相似文献

The vascular organisation of the hind limb claws from clinically normal cattle was studied using plastic corrosion casts. The proper digital artery ran dorsodistally within the interdigital space and gave off a bulbar branch to the bulb, a plantar branch to the axial aspect of the wall and sole, and a coronary artery to the coronary margin and proximal wall. The artery entered the distal phalanx at the axial foramen, coursed through the nutrient canal as the terminal arch, and emerged at the abaxial foramen to anastomose with branches of the bulbar artery. During its interosseous course, the terminal branch gave off 6-8 primary branches which provided the arterial supply to the coronary margin, axial and abaxial wall, white zone and sole. The dorsal proper digital vein and axial and abaxial proper plantar digital veins drained all venous blood from the claw. Each of these large veins arose from numerous smaller veins which joined to form the venous networks of the coronary margin, wall, sole and bulb. A large arched vein passed through the coronary cushion and formed a connection between the superficial coronary venous plexus and the intra-osseous plexus of the distal phalanx. The axial and abaxial proper plantar digital veins were connected by a vessel through the bulbar cushion. At the same level, the axial proper plantar digital veins of the lateral and medial claw were connected to each other by a large vessel through the interdigital space, situated just proximal to the distal interdigital ligaments. This resulted in the presence of a complete distal venous arch along the plantar aspects of both claws. The parietal collecting veins drained most of the venous blood from the sole, white zone and wall, and connected directly to the venous plexuses of the coronary margin and bulb. Valves occurred commonly in medium and larger calibre veins throughout the claw.  相似文献

There are a number of differences between the claws of the front and hind limbs in cattle concerning macroscopic shape, chemical and physical properties of claw horn and epidemiological data. Front claws have superior data in almost every respect. Virtually no information is available on the mechanical stability of the suspensory apparatus of the third phalanx. It was the objective of this study to develop a method to measure the maximum strength (N/mm2) of the suspensory apparatus of the bovine distal phalanx. This apparatus includes all layers of tissue such as bone, corium, corioepidermal junction and wall horn. The feet of 13 clinically healthy beef bulls were collected from the local abattoir. Testing was performed in three locations of the wall segment (dorsal, abaxial, axial) in a material testing machine. The corium and the corioepidermal junction were set under increasing tension until failure occurred. Maximum stress was recorded. The values ranged between 2.47-3.13 N/mm2 (dorsal), 4.08-4.87 N/mm2 (abaxial) and 2.27-2.66 N/mm2 (axial). No differences could be validated statistically between front and hind claws and between lateral and medial claws respectively. Abaxial ultimate stress values were significantly higher than dorsal and axial ones. Between the two a statistical difference could not be validated. Most of the specimens (57.1%) were torn apart at the corioepidermal junction, almost a quarter of the samples (27.6%) were to a degree disrupted at the corioepidermal junction and to a certain extent in the deeper layers of the corium. A few samples showed disruption within the horn (11.5%) or at the attachment of the third phalanx (3.8%). Biomechanical features of the experimental set-up are discussed. The biological significance of these findings is that the body weight and the additional dynamic loading is predominantly taken over by the abaxial part of the claw, while the dorsal and axial aspects are less loaded.  相似文献

Objective: To develop a tenoscopic approach for desmotomy of the accessory ligament of the deep digital flexor tendon (AL‐DDFT) in horses. Study Design: Experimental. Animals: Cadaveric forelimbs (n=10) and 4 forelimbs from 2 horses anesthetized for terminal teaching procedures, and 12 forelimbs of 6 experimental horses. Methods: Saline distention of the carpal flexor sheath facilitated insertion of an arthroscope into the distal medial aspect of the sheath between the AL‐DDFT and deep digital flexor tendon (DDFT). Location of an instrument portal on the lateral aspect of the metacarpus was identified with a needle. The lateral aspect of the AL‐DDFT was transected and the arthroscope and instrument were switched to transect the remaining fibers on the medial aspect. Cadaveric specimens were dissected for evaluation. Experimental horses were monitored for 30 days postoperatively. Results: Minor complications including incomplete division of the AL‐DDFT and shallow incision into the suspensory ligament were observed in some cadaver specimens. The AL‐DDFT was completely transected in all experimental horses with no suspensory ligament damage. Mean±SD surgical time (incision to skin closure) was 28.3±11.8 minutes. On ultrasonographic examination, transection of the AL‐DDFT was complete in all experimental horses. Minor DDFT fiber disruption was noted in 1 limb during ultrasound examination at day 30. Conclusions: A tenoscopic approach through the carpal flexor sheath provided adequate access for desmotomy of the AL‐DDFT.  相似文献

The current study identified 124 lateral condylar fractures of the distal metacarpus or metatarsus over a 6-year period. Thirty of 124 lateral condylar fractures were classified as displaced. Eight of 30 displaced lateral condylar fractures and 1/86 incomplete lateral condylar fractures had concurrent sagittal axial fracture of the lateral proximal sesamoid bone. A 20-degree DMPLO view is the most sensitive radiographic projection to identify sagittal axial fracture of the lateral proximal sesamoid bone. A flexed 20-degree DMPLO projection demonstrates displacement of complete sagittal axial sesamoid fractures. Radiographic identification of axial sesamoid fracture is critical to provide the most accurate prognosis for return to racing. Arthroscopic assessment of the axial sesamoid fracture and cartilage injury on the lateral proximal sesamoid bone, as well as post-operative progression, are described in four surgical cases.  相似文献

Complete displaced lateral condylar fractures of the metacarpus with triangular osteochondral fragments at the palmar articular surface occurred in two Thoroughbred horses. The condylar fractures were repaired with cortical bone screws following removal of the osetochondral fragments via a dorsal arthrotomy and distraction of the condyles. One horse raced successfully after surgery, and the other is being used successfully as an English pleasure mount.  相似文献

REASONS FOR PERFORMING STUDY: Bone strains quantify skeletal effects of specific exercise and hence assist in designing training programmes to avoid bone injury. OBJECTIVE: To test whether compressive strains increase on the lateral surface of the inside third metacarpal bone (McIII) and the medial surface of the outside McIII in a turn. METHODS: Rosette strain gauges on dorsal, medial and lateral surfaces of the midshaft of the left McIII in 2 Thoroughbred geldings were recorded simultaneously during turning at the walk on a bitumen surface. RESULTS: Medial surface: Compression peaks were larger in the outside limb. Tension peaks were larger in the inside limb and in a tighter turn. On the lateral surface compression and tension peaks were larger on the inside limb, which showed the largest recorded strains (compression of -1400 microstrains). Dorsal compression strains were larger on the outside limb and on a larger circle. Tensile strains were similar in both directions and larger on a larger circle. CONCLUSIONS: Compressive strains increased on the lateral surface of the inside McIII and medial surface of the outside McIII in a turn. POTENTIAL RELEVANCE: Slow-speed turning exercise may be sufficient to maintain bone mechanical characteristics in the inside limb lateral McIII cortex. Further work is needed to confirm these findings and to determine whether faster gaits and/or tighter turns are sufficient to cause bone modelling levels of strain in the medial and lateral McIII cortex.  相似文献

OBJECTIVE: To determine changes in the distal ends of the third metacarpal and metatarsal bones (MCIII and MTIII) of Thoroughbred racehorses that had sustained a catastrophic condylar fracture during high-speed exercise. SAMPLE POPULATION: Fractured and contralateral MCIIIs and MTIIIs from 11 Thoroughbred racehorses that sustained a displaced condylar fracture during racing, both MCIIIs from 5 Thoroughbred racehorses euthanatized because of a catastrophic injury other than a condylar fracture, and both MCIIIs from 5 horses of other breeds that had not been professionally trained or raced. PROCEDURE: Macroscopic observations were made of the distal ends of the bones before and after digestion of the articular cartilage with NaOH. RESULTS: In all 11 racehorses with a displaced condylar fracture, the fracture was associated with a branching array of cracks in the condylar groove. In this region, fracture margins were smooth, and there was loss of subchondral bone. Comminution of the dorsal cortex was also seen. Parasagittal linear wear lines in the articular cartilage, erosions in the articular cartilage of the condyles, loss of the underlying subchondral bone, and cracking of condylar grooves were all more severe in the Thoroughbred racehorses than in the horses that had not been professionally trained or raced. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that condylar fractures in horses are pathologic fatigue or stress fractures that arise from a preexisting, branching array of cracks in the condylar groove of the distal end of MCIII or MTIII.  相似文献

This study describes a detailed computed tomographic reference of the normal equine foot. Ten forefeet of five adult cadavers, without evidence of orthopaedic disease, were used. Computed tomography (CT) was performed on all feet. Two‐millimetre thick transverse slices were obtained, and sagittal and dorsal planes were reformatted. The CT images were matched with the corresponding anatomic slices. The phalanges and the distal sesamoid bone showed excellent detail. The extensor and flexor tendons (including their attachments) could be clearly evaluated. The collateral (sesamoidean) ligaments could be readily located, but were difficult to delineate at their proximal attachment. The distal digital annular ligament could only be distinguished from the deep digital flexor tendon proximal to the distal sesamoid bone, and its proximal attachment could be identified, but not its distal insertion. Small ligaments (impar ligament, chondrosesamoidean, chondrocoronal and chondrocompedal ligaments, axial and abaxial palmar ligaments of the proximal inter‐phalangeal joint) were seen with difficulty and not at all slices. The joint capsules could not be delineated from the surrounding soft tissue structures. The lateral and medial proprius palmar digital artery and vein could be visualized occasionally on some slices. The ungular cartilages, corium and hoof wall layering were seen. The nerves, the articular and fibrocartilage of the distal sesamoid bone and the chondroungular ligament could not be assessed. Computed tomography of the equine foot can be of great value when results of radiography and ultrasonography are inconclusive. Images obtained in this study may serve as reference for CT of the equine foot.  相似文献

采用大体解剖学方法,研究了成年双峰驼的8个跖趾关节,并与马、牛、羊、猪、犬等动物的对等器官进行了比较。结果发现,双峰驼跖趾关节的形态结构有以下显著特征:(1)跖骨远端关节面后部有矢状嵴,近趾节骨近端关节面后部有与之相对应的矢状沟,这种结构与跖行动物和趾行动物相类似;(2)籽骨上没有对应近趾节骨近端的关节面;(3)籽骨间韧带只连接同一趾上的2个籽骨,与牛的将2趾的4个近籽骨连在一起的结构不同;(4)轴侧侧副韧带不像牛的那样与对侧的同名韧带共同起始于跖骨远端滑车间切迹,而是分别起始于此切迹的轴侧韧带窝及韧带结节;(5)轴侧侧副韧带仅为1层,而远轴侧侧副韧带分为2层;(6)籽骨直韧带、趾间趾节骨籽骨韧带及趾间近韧带均缺如。  相似文献

Reasons for performing study: Although fractures of the metacarpal and metatarsal condyles are the most common long‐bone fractures of Thoroughbred horses in training, limited data on variations in morphology and incidence have been published. Additionally, grouped analyses of previous studies from the UK and USA would permit comparison between study groups and the creation of a substantial pool of international data. Methods: Retrospective analysis of case records of horses with fractures of the distal condyles of third metacarpal/metatarsal bones seen over the last 10 years at Newmarket Equine Hospital. The current series was compared with a prior series from the UK; pooled analyses of these, and series from the USA were also compared. Results: One hundred and seventy‐four fractures were identified in 167 racehorses. The current series contained a significantly lower proportion of medial condylar fractures than in a similar population 17 years earlier. Fractures that originated more abaxially tended to be shorter, and a significant proportion of lateral condylar fractures arose outside of the condylar groove. There was also some apparent seasonality of fractures of the lateral condyles in 2‐year‐old horses, but not so in older horses or those with medial condylar fractures. Conclusions and potential relevance: A substantial portion (approximately 50%) of fractures in the present series cannot be explained by unifying theories of aetiopathogenesis, which suggest that fractures usually arise within the condylar groove, as a focus of cumulative fatigue and failure of adaptation of bone. A separate aetiopathogenesis may be responsible for a proportion of condylar fractures encountered in racehorses, and further work is required to elucidate this. Changes in the number of medial condylar fractures encountered may reflect the introduction of artificial racing and training surfaces, and also may reflect changes in the age demographic of horses racing in the UK since the introduction of year‐round ‘all‐weather’ racing.  相似文献

Objective— To characterize donor and recipient sites for autologous osteochondral transplantation in the canine stifle joint with respect to split-line pattern and cartilage thickness.
Study Design— In vitro study.
Sample Population— Stifle joints (n=30) of dogs >20 kg.
Methods— Collagen network orientation of the hyaline cartilage coverage of the distal femur was assessed using split-line technique (n=10). Cartilage thickness was measured radiographically on osteochondral plugs harvested at the abaxial and axial surfaces of the femoropatellar joint (n=15), the most proximal abaxial aspect of the medial condyle (CO; n=5) and at both femoral condyles (n=15).
Results— Cartilage within the femoropatellar joint and both femoral condyles had mostly transversely orientated pattern of split-lines. Abaxial to the femoropatellar joint split-lines were absent. All donor sites had significantly thinner cartilage than the medial condyle (CM). Only the distal trochlea had cartilage thickness comparable to the lateral condyle (CL). The thinnest cartilage was found abaxial to the femoropatellar joint followed by CO, which was ∼2–3 times thinner than the cartilage at both condyles.
Conclusions— None of the investigated donor sites provided transplants of comparable cartilage thickness to CM. Transplants from within the femoropatellar joint should be harvested in priority as they reach cartilage morphology comparable to CL.
Clinical Relevance— Adjusting for cartilage thickness and split-line pattern may improve long-term outcome after autologous osteochondral transplantation. But surface curvature and donor-site morbidity have to be considered as well when choosing the most suitable donor site.  相似文献
